    The pure tone audiogram (PTA), speech intelligibility, temporal resolution and frequency selectivity were measured for the moderate sensorineural hearing loss patients. The temporal resolution was measured by the method of gap detection test in broadband noise. The frequency selectivity was measured in accordance with Zwicker's method. And then the relationship between speech intelligibility and those parameters was studied. There was a wide varied relationship between PTA and the speech intelligibility. The data in this study showed that the temporal resolution was much more important than the frequency selectivity to hear words. The temporal resolution was also detected in eight persons who wore the cochlear implants. There was good relationship between the temporal resolution and the intelligibility of consonance. The temporal resolution seemed to depend on the number of residual cochlear neurons. Lastly I refer that the PTA represents only the static auditory function and that the parameters like the temporal resolution or frequency selectivity seems to be much more important to perceive speech because those parameters represents the dynamic cochlear function. In case of fitting of hearing aids, one should recognize the PTA is a only part of cochlear function.

    The incidence of mastoiditis has increased with increasing bacterial resistance to antibiotics. We report the case of 10-month-old girl suffering from acute mastoiditis with a subperiosteal abscess. Computed tomography (CT) showed clouding of the mastoid area with mastoid cortex erosion and a subperiosteal abscess. We incised and drained the abscess, culturing penicillin-intermediate streptococcus pneumoniae (PISP) from the abscess. We administrated intravenous PISP-sensitive antibiotics for a successful cure.
    Streptococcus pneumoniae is the most common organism involving acute mastoiditis. Overuse of antibiotics, however, has led to abuse growing bacterial resistance to antibiotics such as PISP and penicillin-resistant streptococcus pneumoniae.
    Antibiotics chosen to treat patients with acute mastoiditis must thus be considered most carefully.

    Obstruction of the upper airway may cause sleep breathing disorder. We conducted lingual tonsillectomy to treat 5 cases-4 men and 1 woman having lingual tonsil hypertrophy with or without tracheotomy. Results for 4 were satisfactory but 1 still suffered from poor sleep quality and excessive daytime sleepiness.
    We now conduct this procedure using an ultrasonic scalpel without tracheotomy because it decreases bleeding and edematous change. The respiratory condition must still be meticulously followed up after this surgery.

    We report the case of a patient with mandibular megaloameloblastoma. Ameloblastoma, often seen in pediatric patients, grows by local invasion. Local recurrence is often frequent and may become malignant, necessitating radical excision, depending on patient age and tumor infiltration. The tumors great size and extensive mandibular invasion mode it extremely difficult to determine treatment and reconstruction. We hemiexcited the mandible, and used fibular flaps to reconstruct the mandibular angle. Postoperative progress was satisfactory and free of occlusal failure.

  • BASAL CELL CARCINOMA WITH A SMALL FISTULA LOCATED IN THE ALA NASI

    A 70-year-old man reported noticing a small fistula surrounded by pigmentation in the right ala nasi a month earlier. Skin biopsy led to a histological diagnosis of basal cell or trichoblastic carcinoma. The lesion was excised with an 8-9 mm surgical margin from the pigmentation and the deep margin reaching the nasal mucousa. Histological examination identified morphea-form basal cell carcinoma with normal skin at the lateral end of the specimen. Seven months after excision, 2 small local recurrences were removed with an 8 mm surgical margin. Tumor cells were not identified histologically in the surgical margins. Four months after the second excision, the patient remains well with no evidence of recurrence.
    The extent of morphea-form basal cell carcinoma cannot be clinically defined by sight or touch, and the tumor is deeply invasive into subcutaneous tissues. Residual cancer cells are likely to remain in the surgical margin. Recurrent basal cell carcinoma is frequently reported to be biologically more aggressive. The recurrence for morphea-form basal cell carcinoma is high, and its surgical margins require careful attention and postoperative observation.

    Controlling allergic rhinitis reduces dependence on medical treatment, with the patient's misery lessened by reducing the allergenic area. Practical guidelines for allergic rhinitis suggest concrete measures of creating mite-free rooms. The importance of washing is not referred to in guidelines even though major allergens such as Der p (f) 1 and Der p (f) 2 are water-soluble. Our aim was to confirm whether washing was effective in removing mite allergens.
    We used high concentrations of mite textiles of different materials, water immersion time, Der 1 dissolution, and allergen reduction ratio.
    Results showed that absorption and solubility of mite allergens depended on the type of textile. Washing after soaking for 1 hour was more effective than ordinary washing. These findings will aid doctors in advising patients suffering from allergy in their daily lives.

    Massive pollen scattering has been a cause of suffering for the past three consecutive years, i.e. since the year 2000. In Tokyo, in 2002, a multi-center study was conducted to compare the efficacy of ramatroban in patients allocated into prophylactic treatment and post-pollen scattering treatment groups. With regard to the three major complaints associated with pollinosis : sneezing, rhinorrhea and nasal obstruction, the Symptom Medication Scores revealed significant inhibition of symptoms in the prophylactic treatment group after the peak pollen scattering period. In particular, the nasal obstruction score in the prophylactic treatment group showed that even at times of peak pollen scattering, only mild symptoms were experienced. Furthermore, since no adverse events were noted during this study, ramatroban appears to be an excellent drug for prophylactic treatment of Japanese cedar pollinosis.
