  • WITH CHIEF REGARDS ON CHANGES IN INTRANASAL RESISTANCE FOLLOWING DRY AIR LOAD

    To explore the cause of nasal obstruction and a feeling of nasal obstruction, dry air was administered into the intranasal cavities under various conditions. From changes in intranasalresistance associated with changes in intranasal humidity, involvement of these factors were examined. In addition, the functions of the nasal cavity in elderly group (aged 60 to 79 years) were compared with those in younger group. Moreover, the relation between a feeling of nasal obstruction and dryness of the nasal cavity was also examined.
    (1) In the younger group, dryness in the nasal cavity causes reduction in humidity and elevation in resistance of the nasal cavity and decrease in mucociliary movements, inducing subjective feeling of nasal obstruction. It was postulated that increase in friction resistance of the mucosal surface and occurrence of turbulent flow are the possible causative factors.
    (2) In the elderly group, it was confirmed that, as compared to younger subjects, intranasal humidity and resistance were decreased. Moreover, it was confirmed that with an increase in intranasal humidity, the resistance decreased. At the same time, symptoms such as a feeling of nasal obstruction were improved.
    (3) As the responsible factor for the reduction in intranasal resistance observed in the elderly group, the reduction in frictional resistance and inhibition of turbulent flow were assumed.
    (4) In the treatment of nasal obstruction and a feeling of nasal obstruction not associated with unfavorable morphological change of the nasal cavity in elderly group, it is useful to correct dryness of the nasal mucos.

    Before and after administration of azelastine hydrochloride (Azelastine) in 10 patients with perenial allergic rhinitis, serum levels of cytokines, eg IL-3, IL-4, IL-6, GM-CSF, IFN-γ were measured by enzyme-linked immunosorbant assay. After treatment with Azelastine, the serum levels of IL-6 significantly decreased in three patients disappering every allergic symptoms.
    These results suggest that the decrease of IL-6 in the serum may be one of parameters to valuate the effectiveness of Azelastine therapy for patients with allergic rhinitis.

    Deep neck infection is a severe disease, and commonly occurs is adults. The pediatric case is rare. We experienced a 2 years and 7 months old female patient with deep neck infection originated in the parapharyngeal space. It was first observed in the submandibular space and spreaded under the platisma. By means of surgical drainage and antibiotic therapy, it was cured one week after the operation. Literature of pediatric parapharyngeal abscess is reviewed. Most cases developed from retropharyngeal abscesses and there were a few cases of parapharyngeal abscesses. Parapharyngeal abscesses of pediatric cases without tonsillitis and/or retropharyngeal abscess seem to spread to the outward because of the weakness of the surrounding connective tissues. It is important to know that there are differences in etiological factors and clinical behaviors between the adult case and pediatric one.

    The patient was a 9-year-old boy. He visited a local otolaryngologist with a right intranasal tumor as the chief complaint and a biopsy was performed. Since watery nasal discharge continued thereafter, he was first examined in our department on August 7, 1993. In a CT scan, a bone defect in the right cribrifom plate of the ethmoid bone and an associated isodensity lesion occupied the nasal meatus. In MRI, a long T1 lesion with a clear margin was obtained at the same site. Based on a diagnosis of meningoencephalocele, radical operation by frontal craniotomy was performed on August 24, 1993.
    No cerebral parenchyma or blood vessels were found in the arachnoid membrane which has protruded into the nasal cavity. The postoperative course has been good to date except for a right olfactory disorder.

  • FOCUSING ON A CASE WITH NASOPHARYNGEAL ABSCESS

    Complications of otitis media are potentially serious and should be treated with appropriate operations as soon as possible. We report three cases of otitis media with complications; one of them had nasopharyngeal abscess which seemed to be an extremely rare complication of otitis media.
    Case 1 was a 7-month-old baby with postauricular swelling in the left side. His diagnonis was acute mastoiditis accompanying a subperiosteal abscess. He was successfully treated by simple mastoidectomy.
    Case 2 was a 25-year-old male complaining of headache. His diagnosis was meningitis caused by a cholesteatoma which destroyed surrounding bones. He underwent radical mastoidectomy and recovered uneventfully.
    Case 3 was a 53-year-old male, who developed neurological disorders, such as soft palate palsy, after preceding symptoms of otorrhea and headache. Serial biopsies of the nasopharynx failed to prove cancer, but led the draining of pus from the site and relieved symptoms. We suspected that an abscess at the pyramidal apex had reached to the nasopharynx to form an advanced abscess recognized as an “internal Bezold abscess”, which we seldom encounter in these days.

    A rare case of primary epipharyngeal tuberculosis with adhesive otitis media is reported. The primary epipharyngeal tuberculosis was seen in a 24-year-old female. She complained of hearing impairment and ear fullness. On histopatho logicalexamination of a granulomatous masslesion in the epipharynx, she was diagnosed as having primary epipharyngeal tuberculosis.Treatment with antituberculosis agents, SM, INH and RFP greatly reduced the granular mass after 2 months of the therapy. The character of the primary epipharyngeal tuberculosis is discussed.

    A 2-year-old boy was admitted to the Jikei University Hospital with productive cough and fever. Chest X-ray studies (atelectasis of the rightupper lobe) and physical findings suggested a foreign body in the right bronchus. Under the diagnosis of bronchial foreign body, flexible bronchoscopy was performed under general anesthesia, and the “foreign body” was extracted successfully. Unexpectedly, it was not a “e xogeneous” foreign body but a “endogeneous” mucoid plug.
    This case is so-called “Mucoid Impaction of the Bronchi”, and in infants the differential diagnosis between this disease and “exogeneousbronchial foreign body” is very difficult without bronchoscopy. We suggested that, in case of infants, the flexible bronchoscopy is the first choice to diagnose and treat a foreign body.

    The hemostatic efficiency of electrocagulation using surgical universal snare (Elman, USA) versus that of ligation of clamped vessels following tonsillectomy has been tested statistically in this study of 17 tonsillectomized patients. Electrocagulation was used in 8 patients while ligation of clamped vessels was used in 9 patients. Both technique have proved to be very efficient. The overall amount of operative blood loss was 27ml and 53ml in mean, respectively. Electrocagulation has proved to be significantlly efficient in decreasing the amount of blood loss during tonsillectomy.
