    The combination chemotherapies including cisdichlorodiamineplatinum (II) (CDDP) and 5-fluorouracil (5-FU) (CF chemotherapy) have been commonly used for patients with advanced head and neck cancer, as neo-adjuvant chemotherapy. To improve complete response (CR) rate of CF chemotherapy we had designed the modified CF chemotherapy (5-FU followed by CDDP) on the basis of the results of in vitro chemo-sensitivity tests. We had applied the modified CF chemotherapy for the patients with advanced head and neck cancer, however a significant CR rate influencing the prognosis had not been obtained. For the purpose of enhancement on cytolytic activity of CDDP plus 5-FU by cyclophosphamide (CPM), we had performed the chemosensitivity tests of CDDP, 5-FU and CPM. Furthermore modified CF chemotherapy combined with CPM was applied for 50 untreated patients with advanced head and neck cancer. However a significant CR rate was not obtained. Therefore the in vitro chemo-sensitivity tests of combined treatments of CDDP, 5-FU, Methotrexate (MTX) plus Leucovorin (LV) were examined. The cytolysis of 5-FU was increased by the addition of LV, MTX. On the basis of these results of in vitro chemo-sensitivity tests, our new clinical regimen through three biochemical modulations consisting of 5-FU followed by CDDP, MTX followed by 5-FU and LV followed by 5-FU was applied for 50 untreated patients with advanced head and neck cancer. The intravenous (i.v.) MTX (30 mg/ m2) administration (day 1) was followed 1 hour after by 120 hours (day 1-5) continuous i.v. infusion of 5 -FU (800 mg/m2/day). Then the concomitant use of LV (20 mg/m2/day) was added every day (day 1-5) from the start of 5-FU until completion of 5-FU infusion on day 5. Thereafter CDDP (60 mg/m2) was administered on day 4. Cycles were repeated every 21 days. Two cycles of this chemotherapy could be performed for 45 patients. The clinical CR rate undergoing this chemotherapy regimen was 33% and this rate was significantly higher than that of modified CF chemotherapy (33% vs 20%, p < 0. 05).

    In order to clarify the prognostic value of various gene expressions in non-Hodgkin's lymphoma (NHL), the frequency of each gene expression was investigated and analyzed statistically. The expression of Epstein-Barr encoded small RNAs (EBERs) was observed by usingin situhybridization method in 137 NHL subjects. Among them, 24 cases (17.5%) were positive for EBERs. However no significant difference in the survival rate was observed between EBERs positive and negative groups. Tumor speci-mens from 102 NHL cases ranging in age from 8 to 88 years with the average age of 59.3 years were used for immunohistochemical studies. Monoclonal antibodies used in this study were p 53, bcl-2, c-myc and mdm-2, and positive rate of each gene expression was 59.8%, 78.4%, 49.0% and 14.7%, respectively. However there was no significant correlation between survival rate and each gene expression. The relapse-free survival rate had a significant correlation with overexpression of p 53 protein and the expression of bcl-2 protein (p <0.01).
    Overexpression of p 53 is considered to be a mutant, and it may elimate the suppressor activity against the malignancy. It is thought that the mutant p 53 and expression of bcl-2 proteins play an important role in the clinical course and treatment of NHL.

    A study was made on the distribution of mast cellsand eosinophils in the nasal mucosa of humans with nasal allergy and guinea pigs with experimental nasal allergy and on the histamine content in mast cells. Distribution of IgE in human nasal mucosa was also studied. The results follow.
    1. When nasal mucosal tissue of allergic rhinitiswas compared with that of hypertrophic rhinitis, increase in the number of mast cells and eosinophils in the epithelium was larger in allergic rhinitis.
    2. Expression of IgE in mast cells was clearly increascd in allergic rhinitis, but there was no clear difference between the anterior and posterior regions of the nasal concha.
    3. For both hypertrophic and allergic rhinitis, thehistamine content in mast cells was higher in deep layers than in the epithelium.
    4. In guinea pigs with experimentally induced nasalallergy by inhalation of ovalbumin, the production of histamine by mast cells was observed to increase during the induction process. However, unlike human nasal allergy, the histamine content in mast cells did not differ between the deep layers and epithelium.
    The above findings indicate that mast cells play an active role in allergic rhinitis.

  • COMPARISON OF DIRECT VOICE AND VOICE THROUGH THE TELEPHONE LINE

    An acoustic voice evaluation system has been used for screening for laryngeal diseases by a vowel signal that has been recorded directly from a microphone (direct voice). In order to apply it to the voice signal recorded through the telephone line (telephone voice), careful examination must be made on whether or not essential degradation of the acoustic measures could take place through the telephone line in terms of screening.
    Two hundred thirteen sustained vowel samples were extarcted from a set of 240 voice data, by excluding 27 vowels that showed no obvious vocal cord vibration. Three acoustic measures that have played important roles in the evaluation system for the direct voice were subjected to the acoustic analysis : (1) period perturbation quotient (PPQ), (2) amplitude perturbation quotient (APQ) and (3) normalized noise energy in a full band (NNEa)
    The findings are : (1) The fundamental frequency (F0) does not change through the line, (2) strong correlation is observed on the three measures between the direct and telephone voices, resulting in the autocorrelation coefficients of 0.962, 0.797 and 0.865 for PPQ, APQ, and NNEa, respectively, (3) APQ and NNEa show rather larger values for the telephone voice than the direct voice, reflecting frequency characteristics of the line, and (4) a statistical measure on the separability of the three hoarsenessgrade groups (Wilks lambda) indicates no significant difference. From these findings, we conclude that the telephone voice can be used for the acoustic voice evaluation as well.

    With the recent progress in the development of antibiotics, there has been reduction in the incidence of serious complications occuring subsequent to acute sinusitis in children, and therapy of the disease mainly comprises conservative treatment ; few reports describe cases requiring surgical treatment. However, we encountered a case of acute sinusitis complicated by orbital cellulitis of a one-year-old child in whom about one-month conservative treatment alleviated symptoms but intranasal examination revealed the presence of a pururent nasal discharge in the middle meatus and distinct pyocele-like shadow as depicted by CT scan. Using an endoscope, the fontanelle of the maxillary sinus was intranasally opened (middle meatal antrostomy) to release the purulent discharge. Two months later, healing was achieved with cavitary maxillary sinus being retained.
    We believe that even in infants and children, endoscopic surgical treatment with minimum stress on nasal sinuses is effective when conservative treatment results in prolongation of the disease to make it chronic.

    Rendu-Osler-Weber disease (Hereditary Haemorrhagic Telangiectasia = HHT) often causes severe epistaxis, of which management is a perplexing problem. This report describes our management of five cases of severe epistaxis caused by HHT. Thepatients were : a 63-year-old male who had previously undergone numerous cauterizations, a 42-year-old female who experienced shock by severe epistaxis, a 67-year-old male who had first presented cerebeller abscess, a 34-year-old female who had previously undergone septal dermoplasty, and a 73-year-old male. They were all diagnosed as having HHT. They were intensively treated with tanponade, cautery, fibrin glue, or septodermoplasty. In spite of therapy, the frequency and severity of bleeding did not improve for a long time. This indicates that there is no treatment which has long-lasting effect on bleeding.

    A 23-year-old man complained of swelling in the right preauricular region. A 45 x 40 mm round tumor with a hard and irregular surface was observed.
    CT scan was useful for the preoperative diagnosis of calcifying epithelioma, because we found the tumor had calcification, and no relationship to the parotid gland. So the tumor could be removed easily.
    Histopathological examination showed that the tumor had basophilic cells and shadow cells in the tissue. It is a typical appearance of calcifying epithelioma.
    We reviewed previous 30 case reports by otorhinolaryngologist in Japan, and noticed that calcifying epithelioma is mainly found in young people, in the auricular region.
