    External dental fistula is a condition where chronic suppuration of a dental root creates a fistula to the skin of the face or neck. More than 60% of patients seek consultation at a dermatology or surgical clinic because of the lack of dental disease symptoms. Some cases of external dental fistula have been mistreated with multiple incisions, repeated biopsies or prolonged treatment with antibiotics. The treatment of this condition should include extraction or other treatment of the causative tooth. Observations of three cases at different stages of diseases revealed cellulitis, abscess formation and cutaneous fistula formation after the fistula tract had penetrated the mandibular bone. The author proposes that such conditions be referred to as cellulitis of the soft tissue from external dental fistula or cellulitis from external dental fistula.

    We report a case of microscopic polyangiitis (MPA) : a 63 year-old-man presented with ear discharge and hearing loss on both sides. He was treated as a case of acute otitis media and acute mastoiditis, however, his symptoms did not improve. In addition, he complained fever, general fatigue and appitite loss. Chest CT revealed nodular opacities and pleural effusion on the left side. The serum creatinine level and MPO-ANCA titer were elevated. Renal biopy revealed findings suggestive of necrotizing angiitis and glomerulonephritis, therefore, a diagnosis of microscopic polyangiitis was made. Steroid pulse therapy and cyclophosphamide pulse therapy were initiated, with improvement of the patient's symptoms. It is important to make an early diagnosis of MPA as the prognosis of this condition is abysmal.

    A case of malignant melanoma of the nasal cavity is reported. A 60-year-old male visited our clinic with the complaint of epistaxis. While he was initially diagnosed as a case of anaplastic carcinoma of the nasal cavity from biopsy specimens, the final diagnosis made was malignant melanoma. The patient was treated by surgery and radiation therapy, followed by biochemotherapy. He is still alive, nearly three years after completion of the treatment, without any evidence of recurrence. Since malignant melanomas of the nasal cavity are usually very rapidiy growing, additional biochemotherapy should always be considered in these patients for the prevention of metastases.

    We describe the effective and safe use of a navigational device in endoscopic frontal sinus surgery. The anatomy around the frontal recess is complex, and the tip of the curved apparatus is difficult to see because of the limited flexibility of the views by 70 degrees-endoscope. To overcome these difficulties and obtain easier access during operation, we have developed a new procedure as follows ; curved forceps and microdebriders are pre-calibrated by ICM4 as navigation probes. Next, some points of interest are pre-marked by the labeled point planning function. During operation, we confirm on the navigation screen the angle and the direction at which the forceps and microdebriders hit the objective.We have labeled this method as nose-shaped navigation.
