現在我国で臨床応用されている植込み型心臓ペースメーカのうち, デアント型ペースメーカ7社8機種について, デマンド機能特性, すなわち周波数特性, デマンド感度, 不応期時間, 出力における負荷抵抗特性, 電池消耗に対応する電源電圧特性についての試験を行なった.
デマンド型ペースメーカにおいて臨床上問題となる誤動作, 外部干渉に対しての解決をする上で役立つ基礎的な資料を, この研究により得ることができたと思われる.

Sac型人工心臓の動脈圧波型は生理的な波型に類似しているが, 人工心室の吸水性伸展と心尖部の血栓形成が問題となった.
Diaphragm IX型は鋭いdp/dtの非生理的動脈圧波型で, 移植動物の脳・肺組織に血管周囲出血を認めた. Diaphragm XII型は生理的動脈圧波型に類似し, 耐久性に問題はなかったが, 隔膜と心室移行部に線維性血栓形成を認めた.

携帯式人工腎臓開発のため, 少量の灌流液による透析法の実験を行なった. 水分除去能を考慮して高分子高濃度灌流液を試用したが, ある程度の透析能率が得られた. 抗凝血性材料による透析器が完成し, 24時間透析が可能となれば臨床的応用も可能となるが, この点について現在検討中である.

Impedance cardiograph is an useful, noninvasive technique for the determination of cardiac output value and for the continuous monitoring of stroke volume following open heart surgery. Since April 1973, the measurements of cardiac output by this method have been carried out in 56 cases (520 occasions) after open heart surgery in our institution. A comparative study of cardiac output values by the simultaneous impedance cardiograph and dye dilution techniques in 35 cases revealed a close relationship between the two.
The coefficient of correlation was 0.83. The measurement of low cardiac index such as under 1.5 l/min/m² is inaccurate with dye dilution methods, but this seems to be more accurate with impedance technique.
Isoproterenol is often used for the treatment of LOS following open heart surgery. Cardiac index measurement in cases treated with isopreterenol for LOS following artificial valve replacement disclosed CI often drop more than the value before isoproterenol administration even with elevated blood pressure when pulse rate exceeded 100/min. On the contrary, digitalis was very effective to increase cardiac output in those patients with valve replacement. This suggest that when isoproterenol used for LOS in patients after valve replacement, pulse rate must be controlled not to exceed 100/min to obtain increased cardiac output and digitalis should be started after 24 hrs postoperatively when arrhythmia is more unlikely to occur.
Impedance cardiograph proved also its availableness for the determination of optimum pulse rate of cardiac pacing, determination of appropriate pressure for positive endexpiratory pressure breathing, early recognition of pulmonary edema and observation of patients who received artificial valve replacement in the course of follow up by means of bycycle ergometer tests. Concerning these problems we would like to present in the near future.

最近, 膜型八工膜に関する研究は, 高分子化学の進歩とともに著しい発展をみて, 開心術あるいは長期補助循環に対して臨床応用も可能となってきた. われわれはLandé-Edwards membrane oxygenatorおよびGeneral-Electric Peirce lungを入手し, 実験的に検討を加え, 臨床応用も行なって種hの知見を得たので報告する.

Recent advances in artificial kidney design would suggest the possibility of small portable and wearable types. The most attractive one is a therapeutic application of substances capable of adsorbing solutes. which are pathophysiological significance in uremia.
Our programe had been undertaken in developing newer sorbent-based type dialysis, one of which is directly perf using over column of activated charcoal.
On the other hand, REDY SYSTEM (CCI Life System, Inc.) is a type of sorbent-based equipment regenerateting dialysate, and clinicaly evaluated in USA.
This unique dialysate supplying system is operated only 5.5L of dialysate.
This paper is a clinical report of this system in maintaining long-term hemodialysis patients.
As a result, we encountered no undesirable complications or hazardous accidents in 29 dialysis. Patients were able to keep good salt-water balance, body weight and acid-base balance. But in some instances, we came across hypopotassemia in using those poorbuild.

教室での69台の植え込みペースメーカ中35台 (50.7%) を占める, R波感受性型に起因する合併症を中心とした使用経験を報告した. 高感受性の問題には, 心内心電図や心筋電極リードよりのR波の測定が必要である. また最近の低出力ペースメーカーの出現とあわせて, 閾値測定のroutine化が必要である.

人工弁置換術が弁膜疾患にたいする治療手段のひとつとして広く布及しているにもかかわらず, なお種々の合併症が臨床医の悩みの種となっている.
著者らは上下弁と傾斜弁の両者の利点を生かし, 抗血栓性, 耐久性, 血行力学的有利性を向上さすべく新しいdesignを考案しcentral strut valveと名付け, pulse duplicator内におけるpoPPetの開閉およびその周辺の流水状態を造影剤注入と連続撮影によって検討した.
