    The objectives of this survey were to assess the salt intake level of 1475 adults, 483 men and 992 women, in city B, and to observe the relationship between blood pressure (BP) and age, salt intake, urinary potassium, Quetelet's index, serum cholesterol, smoking, and alcohol consumption.
    The subjects had a high mean salt intake level, 14.5 g per day. Hypertensive subjects on anti-hypertensive drugs (22.4%) and those not receiving any therapy had the same salt intake level as normotensives.
    In male subjects, age, alcohol intake, salt intake and Quetelet's index were the main systolic BP determinant factors; diastolic blood pressure was determined mainly by alcohol intake, Quetelet's index and serum cholesterol.
    Both the systolic and diastolic BP of female subjects were mainly determined by Quetelet's index and age.
    Quetelet's index had a stronger effect on BP in subjects with a family history of hypertension. This was more evident in males.
    These results indicate that body weight, alcohol and salt intake are the major determinant factors of BP and that their effects differ depending on sex and familial disposition to hypertension.

    The author investigated developmental changes in the thickness of the mucosal gel layer and its responsiveness to prostaglandins in the stomach of developing rats. The results showed that (1) both mucus gel thickness and mucosal hexosamine content increased with age, (2) oral administration of 16, 16-dimethyl prostaglandin E2 increased mucosal prostaglandin E2 content and mucus gel thickness, and reduced ethanol-induced mucosal damage in 8-wk-old rats, none of which were observed in 1-wk-old rats, (3) oral 2'-carboxymethoxy-4, 4'-bis (3-methyl-2-butenyloxy) calchone increased mucosal prostaglandin E2 content and mucus gel thickness, and reduced ethanol-induced mucosal damage both in 1-wk-old and 8-wk-old rats, and (4) oral low-dose ethanol did not increase mucosal prostaglandin E2 content nor mucus gel thickness, but it did reduce ethanol-induced mucosal damage in both 1-wk-old and 8-wk-old rats. The author concluded that the mucosal susceptibility of the stomach of newborn rats may be due to the smaller amount of mucus gel found at that age.

    The present study was designed to investigate the usefulness of dipyridamole-thallium imaging in the diagnosis of myocardial ischemia as compared with standard exercise-thallium imaging. Neither sensitivity nor specificity for the detection of coronary artery lesions differed between dipyridamole and exercise-thallium imaging, although left ventricular load was less marked and electrocardiographic changes were less frequent with dipyridamole stress than with exercise. Depression of global and regional left ventricular contraction was also less prominent during dipyridamole stress. However, dipyridamole-thallium imaging accurately revealed associated coronary diseases in high risk patients with aortic aneurysm or arteriosclerosis obliterans. The diagnostic sensitivity and specificity of dipyridamole-thallium imaging were similar to exercise-thallium imaging in spite of less marked left ventricular loading and myocardial ischemia. Dipyridamole-thallium imaging is thought to be superior to routine exercise-thallium imaging in patients with limited exercise capability.

    Peripheral blood, regional lymphnode and spleen lymphocytes were measured by using different monoclonal antibodies and flow cytometry in 20 patients with gastric cancer. The patients were divided in two groups, patients with stage I and II disease and patients with stage III and IV disease, according to the General Rules for Gastric Cancer Study, Japanese Research Society for Gastric Cancer. Such parameters as OKT3, OKT4, OKT8, OKM1, OKT4+ 2H4+, OKT4 + 2H4-, OKT8 + Leu15+ and OKT8 + Leu15- were used as monoclonal antibodies. NK activity, interleukin-2 production and interleukin-2 receptors were also measured.
    In peripheral blood lymphocytes, IL-2 production was significantly lower in the stage III and IV group, than in the stage I and II group. The percentages of OKT4, OKT4 + 2H4- and the OKT4/OKT8 ratio were lower in stage III and IV group than in stage I and II group. Among the regional lymphnodes in the stage I and II group, the percentages of OKT4 + 2H4- and the OKT4/OKT8 ratio were higher in proximal lymphnodes than in distal nodes. In stage III and IV group, the percentages of OKT3, OKT8 and OKT8 + Leu15- were lower and the OKT4/OKT8 ratio was higher in proximal lymphnodes than in distal nodes. These results suggest that helper T-cell activity in peripheral blood and the induction of killer T cells in regional lymphnodes decrease as gastric cancer progresses.

    We experienced five cases of acute superior mesenteric artery occlusion (SMAO) over the past four years, and conducted a clinical assessment of the diagnosis, treatment and prognosis of SMAO. A case suggestive of SMAO with the symptom of abdominal angina for one year before is also presented. The mean age of the five patients was 72 years, and their chief complaint was abdominal pain of acute onset. Three patients had a past history of arterial fibrillation, and one case was associated with rupture of an abdominal aortic aneurysm. The preoperative diagnosis was peritonitis or ileus, and angiography was not performed. At surgery, a partial resection of the small bowel was carried out in two cases and wide resection of the small bowel plus right hemicolectomy in two cases. Another case was unresectable because of very poor general condition. Only two patients underwent surgery within 24 hours after the onset. Angiography should be performed on suspicion of SMAO when old patients with heart disease suddenly develop abdominal pain. Early diagnosis and emergency treatment are considered important to improving the prognosis.

    We report herein the angiographic findings of pulmonary arteries in 55 congenital heart disease cases with pulmonary atresia. Main pulmonary trunks were present in 33 cases, absent in 15. In 47 cases, left and right pulmonary arteries were connected by central pulmonary arteries, but localized stenosis was demonstrated at the patent ductus arteriosus in 5 cases. We were able to determine the PA-index in 38 of these patients. The mean diameter of the left pulmonary artery in the single ventricle group was bigger than that of the right, but in the tetralogy and pure pulmonary atresia groups the right mean diameter was bigger than that of the left. The PA-index in the tetralogy group was smaller than those in the single ventricle and pure pulmonary atresia groups.

    A 65-year-old female suffered itching and erosion in the perianal region for 18 months. A biopsy specimen of the eroded skin disclosed Paget's cells. An abdominoperineal resection (Miles' operation) accompanied by posterior wall resection of the vagina was performed. Histological examination of the resected specimen showed Paget's cells whose presence of which was limited to within the epidermis. Miles' operation with an invasive tissue resection is the treatment of choice for perianal Paget's disease. Long-term follow-up is necessary because extramammary Paget's disease sometimes causes distant metastasis and is accompanied by malignancies of other organs.

    A 63-year-old man was admitted with complaints of swelling and dull pain in the left lower extremity. Phlebography showed venous obstruction in the left iliac region due to thrombi. A femoro-femoral cross-over bypass was carried out using a spiral supported expanded polytetrafluoroethylene graft. The implanted graft was shown to be patent venography 12 days after surgery. Preoperative symptoms have improved in the 11 months since surgery.

    A 34-year-old man was admitted to our hospital with complaining of an elevation of serum transaminase level (GOT211IU/L, GPT265IU/L). He was diagnosed as myotonic dystrophy by clinical features (muscle weakness, myotonia, “hatchet face”, cataract, testis atrophy, frontal balding, etc) and electromyography. Pathological findings from liver biopsy and isoenzyme patterns of LDH, CPK, and GOT suggested that the origin of transaminase might be hepatocytes.
    To improve his myotonia, we gave him mexiletine (300mg/day, P. O, ), “class Ib” type antiarrhythmic drug. Mexiletine apparently improved his myotonia, which was monitored with “finger extension time”. Furthermore, Mexiletine was very effective to reduce the elevated level of GOT, GPT, LDH, CPK, and aldolase.
    This is the first report to demonstrate that mexiletine improved both myotonia and release of muscle muscle enzymes. This case suggests that mexiletine may be useful as a drug to improve the muscle illness in patients with myotonic dystrophy.
