ジャーナル フリーAround 50 thouthand patients have died from ischemic heart disease (IHD) and 800 from idiopathic cardiomyopathy (ICM) in 1980 in Japan. Out of them 4, 000 (8% of the total mortality) of IHD and 300 (39%) of ICM were younger than 55 years of age. Among 8, 000 outpatients with heart disease in our two affiriate hospitals, 423 have suffered from myocardial infarction (MI) and 205 from ICM. According to the criteria of recipient selection in Stanford there have been 9 candidates (2 with MI and 7 with ICM) for recipient among patients of these hospitals. There were 21 brain deathes without heart disease in the Department of Traumatology in our hospital last year. There have been candidates for recipients and donors, nevertheless only one clinical heart transplantation has been done in Japan. This is based on the ehtical problem, that the concept of brain death has not been accepted by the general public. Therefore, the beating heart can not be procured for heart transplantation. Accordingly, it is very important to let the public accept the concept of brain death. Before the acceptance of the concept was obtained, a methode to utilize an arrested heart as a donor heart should be developed. ジャーナル フリーRecently, mechanical cardiac assist devices are applied for the incurable cardiogenic shock patients---that are LVAD, BVAD and TAH.
New designed cardiac assist device---sac type of Avcothane-coated PVC blood pumps and driving units controlled by microprocessor---was developed. The candidates of LVAD and BVAD are estimated approximately 2, 000 per year in Japan.
Since 1959, TAH animal experiments was started in Tokyo University and 288 days survival of TAH goat was obtained.
Until now, 11 TAH animal survived over 6 months in the world and their death are pannus formation, clotting, infection and pump repture.
The candidates of TAH are estimated about 4, 000 per year in Japan.

ジャーナル フリーFrom transient circulatory assistance to total heart replacement. Currently there are many types of cardiac prosthesis available. They are transient circulatory assist devices, interim circulatory assist devices and permanent cardiac prostheses. Left ventricular assist, biventricular assist and total replacement devices are also considered necessary. Because there are various types of prostheses with different clinical needs and different criteria for development, it is very difficult to make many different prostheses. However, a multipurpose universal cardiac prosthesis applicable to transient and to permanent cases must be developed. Due to this need, a pusher plate type prosthesis adaptable to various types of actuation modes, with only minor modifications required, was developed by the Cleveland Clinic group. A transient extracorporeal left ventricular assist device utilizing a universal plastic pump was evaluated in 2 clinical cases. A permanent, implantable left ventricular assist system utilizing a universal titanium pump was recently implanted in a calf for 40 days and demonstrated the feasibility of this approach.抄録全体を表示PDF形式でダウンロード (2221K) -

ジャーナル フリーSince Cooley proposed and proved the possibility of two-staged heart replacement in 1969 with an artificial heart as the first stage to maintain the patient's life till a donor heart becomes available, three clinical cases have been reported by his group to date. Patients were all male and their ages were 47, 21, and 36. In the second patient the heart developed stone heart after aortic and mitral valve replacement, and a left ventricular assist device was employed instead of an artificial heart. But the patient's entire circulation was maintained for five days. All pump devices were of pneumatically-driven type and their main materials were fabric-lined silicone rubber, flock-lined polyether-type polyurethane, and Avcothane, respectively. Duration of artificial heart pumping was 64 hours in the first case and 54 hours in the third case. The pump output varied from 3.5 to 5.5l/min. All patients eventually died of infection 32 hours, 14 days, and 71/2 days after cardiac allograft transplantation. When comparing merits and drawbacks of artificial heart and heart transplantation, both therapeutic means should proceed together using the artificial heart as a temporary life-saving device.抄録全体を表示PDF形式でダウンロード (2419K) -
