    It is common sense in laryngeal clinic that position of paralytic vocal cord is a matter of great importance for hoarseness.
    For that reason, patient who complains of voice disorder due to laryngeal nerve paralysis used to accept often plastic operation for hoarseness, but position of vocal cord role in hoarseness and it's relation has not yet been elucidated, and there's, moreover, no basic science with its problem.
    In this present paper, a review of literature on voice disorder was made, and some of fundamental problem with operative treatment of hoarseness was showed.
    In this operation, we consider consequently that there's no significance in gas exchange within lunge as well as in voice disorder, and must be prepared for reconsideration to indication of plastic operation of hoarseness.

    Osteosarcoma of a cranial bone is rather rare in occurance ; it is said the proportion is 1% among all the cases of osteosarcomas.
    A case of a boy aged 11 years, who was affected with a temporal bone osteosarcoma, was reported. His chief complaints were aural discharge, hearing impairment and pain of the left ear. Radiographic findings and his clinical apearance, such as swelling in the postauricular area, facial paralysis and general malacia suggested the malignant tumor, but several histological examinations revealed no malignancy. The post mortem examination showed this case was the osteoplastic osteosarcoma of the left mastoid, invading the left temporal bone and muscle.

    A cyst which developed after a surgical operation for sinuitis maxillaris is generally reffered to as a postoperative maxillary cyst, and is treated at an un-negligible frequency in the daily otological treatment as well.
    The author has experienced a case of cyst of sinus ethmoidei, combined with neurinoma, which occurred as a delayed complication after operation on sinus ethmoidei. The patient was a 25 years old housewife, complained of a painful intumescence in the right eyelids.
    Microscopic examination of the removed tissue revealed a cyst in the right sinus ethmoidei and neurinoma in the nasal cavity.
    The author supports the Hiroto's proposition to name the similar disease as postoperative ethmoid cyste.

    As one of the antigen of allergic complaints in the field of otorhinology, various kinds of medicines are known, and as for the erethism or shock by protargol, Dr, Kubo and Dr, Kamamoto have reported already.
    Recently I have also experienced one case of erethism by protargol from which a 20 years old female had suffered. So I'd like to report it here. I surmise that the symptom was occured, being lead by the medicine, in the female body who has allergic predisposition.

    Since several years the authors have been clinicaly investigating acoustic neural mechanisms. This study was designed to solve some problems on acoustic neural mechanism through effects of TTFD on neural level. As psychoacoustic method of measurement, Temporary Threshold Shift (TTS) and Temporary Auditory Summation were used in this work, and were conducted in normal ears.
    A series of experiments carried out before and after intra-venous administration of TTFD 50mg in this study were as follows.
    (1) TTS: Each TTS recovery curves after exposures to interrupted intensive tones were observed.
    (2) Aural Reflex: Effects of the Acoustic Reflex of the Intra-Aural Muscle on the TTS recovery curves were taken into consideration.
    (3) Temporary Auditory Summation: Observing the phenomena of Temporary Auditory Summation before and after intra-venous injection of TTFD, we estimated its mechanism of effects at neural level.
    Next results were obtained.
    (1) Within about 6 hours after TTFD 50mg intra-venous injection, TTS caused by high level sound exposure was decreased.
    (2) No change was found on Acoustic Reflex of the intra-aural muscle after TTFD injection.
    (3) After injection of TTFD, hearing acuity was improved temporalily. It continued for about 6 hours. The threshold was improved about 5 dB in the frequency range higher than 2000 cps. The improvement was found to be influenced by the duration of testing stimulus, and considerable improvement was obtained at 100msec duration but slight improvement at the duration shorter than 30msec.
    (4) As the result of psychoacoustic study, this hearing improvement could be considered as a effect of TTFD on synaptic level of acoustic neural mechanism. In order to obtain more exact findings for a affected level, more careful watches should be done. This problem will be studied in the future.

    Drug therapies for neural deafness have long been studied in various manners, with no satisfactory outcomes yet. A rigorous fact about the above treatment is that the restoration of hearing functions is beyond hope after the nerve cells have undergone irreversible changes. Adequate treatment, therefore, has to be started and completed while the pathologic changes are still in the functional phase and are reversible.
    Nutrase with a structural formula of a free-form vitamin B1 which is phosphorylated has recently been administered daily for 21 consecutive days to subjects picked up at random from among the inpatients and outpatients of our Department with neural deafness and also from among the Streptomycin-deaf inmates of the National Oshima Seishoen Leprosarium. Evaluation of the drug effect mainly in terms of improvement in hearing acuteness of bone conduction, found improved hearing acuteness in 27% of the former group of the drug-recipients and in 33% of the latter.
    We have found Nutrase to be an effective drug for the treatment of neural deafness.

    The clinical efficacy of EMPYNASE, a new proteolytic enzyme was estimated as follows: ambulant patients suffering from paranasal sinusitis were administrated orally 3 EMPYNASE capsules a day and the density of roentgenogram of the paranasal region was measured by an electric exposure meter before and after the pharmacotherapy. The authors considered that its light value was a standard of the grade of this disease and an objective estimation index of this drug.

    Fifty-five patients in E. N. T. Clinic of Kyoto University Hospital, suffering from paranasal sinusitis were treated with peroral administration of EMPYNASE capsule, a new drug of antiinf lmmatory proteolytic enzyme.
    The excellent effect on roentgenographic pattern was recognized in 40.0%, no apparent effect in 54.5%, aggravation in 5.5% of the all cases was observed.
    The author considered that EMPYNASE was a new efficacious chemical for chronic paranasal sinusitis.

    A biochemical study was made of in vivo action of orally administered Pronase-P. The enzymic activities of Pronase-P was determined both with casein and L-leucyl. β. naphthylamide (LN) as substrates 1) EDTA inhibited LN hydrolysing activity of Pronase-P by 83%, while it inhibited the activity serum by only about 8 % and that in intestinal fluid by 14%.
    Thus, EDTA was found to be a good inhibitor in differentiating Pronase-P from animal leucineaminopeptidase. 2) When Pronase-P was incubated with intestinal fluid in vitro the LN hydrolysing activity was found to be 10 to 15% less than the sum of the activity of each fraction. Pronase-P solution introduced in the tied loop of small intestine showed about 47% less activity after 1.5 hours of incubation. 3) 22 to 30% increase in LN hydrolysing activity of serum was found by oral administration or by direct introduction of Pronase-P into intestinal loop of rat. 4) No significant change was observed in LN hydrolysing activity in serum of rabbits after oral administration of Pronase-P for a week. 5) No detectable effect of Pronase-P administration on casein hydrolysing activity was found in sera of rabbits with orally administered Pronas-P and those with Pronase-P directly introduced into intestinal canal. 6) No casein hydrolysing activity was demonstrated in the urine both of human and rabbit with orally administrated Pronase-P.

    About 70% of the patients with chronic sinusitis showed a big release of the symptoms by the administration of Pronase-P. The chronic inflammatory change in mucous membrane of sinus was found to be gradually decreased.
    Pronase-P activity measured both by casein and L-leucyl. β. naphthylamide hydrolysing activity in sera of human and rabbit was not increased by the oral administration of Pronase-P. (With rats, in some cases, the activity was found to increase.)
    One of the possibilities that an inhibitor present in serum may cause the enzymic activity to remain within the normal range was biochemically sought. 1) The casein hydrolysing activity of Pronase-P was strongly inhibited by the addition of serum, indicating the presence of an inhibitor in serum. 2) The inhibitory action of serum was not changed by successive oral administration or after introduction of Pronase-P into intestinal loop.
    3) The casein hydrolysing activity of Pronase-P waes not affectd by the addition of ε-aminocapronic acid. 4) The successive administration of Pronase-P was found to give no pathological change in normal organs. 5) The enzyme activity in serum obtained immediately after injection of Pronase-P was found to corespond to 8-25% of that of the calculated value by assuming the uniform distribution in serum of the injected enzyme.

    The pathological and pathohistological studies were made of the influences of Pronase-P upon the chronic inflammation of the paranasal sinusitis. And as the result it can be concluded that the recovery of chronic inflammation was promoted by oral administration of pronase-P. The several of the main findings were as follows:
    A. macroscopic findings
    1) decrease of pus and mucous.
    2) gradual improvement of inflammatous changes. (such as thicking of mucosus membrane) of sinus.
    B. histological findings
    1) improvement of edema, infiltration of rund cells, new growth of capillaries.
    2) f ibrolisis in connective tissue.
    3) increase of mucopolysaccharides in the ground substance of mucous membrane especially around capillaries.
