    In an attempt to evaluate objectively the histologic images of liver tissue, light microscopic pictures of normal liver and hepatoma tissues were taken at a X66 magnification and scanned with a spot of 170μm in diameter corresponding to 0.8μm at specimen, an gray tones were digitized to 32 levels. Then thedistribution patterns of gray tones were analyzed by seven kinds of parameters, f1-f7.
    Parameter f1 is a measure of homogeneity of an image, f2 and f5, f3 and f4 represent two kinds of theamount of local variations, gray-tone linear-dependencies and the amount of total variations, respectively.
    The results were as follows:
    Parameters such as f1, f2, f4 and f5 were shown to discriminate normal liver tissue from hepatoma, and f1, f2 and f3 were also useful to discriminate well differentiated from poorly differentiated hepatoma.
    Factor analysis and plotting of calculated factor scores were helpful to visualized the relation in histologic characters among normal liver and well or poorly differentiated hepatoma.

    The localization of the form of cytochrome P-450 that is induced by phenobarbital was studied by direct immunofluorescence in the hepatocytes of rats pretreated with phenobarbital in comparison with the saline-treated rats. Specific fluorescence was seen in the hepatocyte cytoplasm both in the salinetreated and phenobarbital-treated rats. In the saline-treated rats, fluorescence was barely discernible but slightly more intense, in the centrilobular than perilobular zones. In the phenobarbital-treated rats, fluorescence was much more intense, with a similar but much greater difference between the centrilobular and perilobular zones. The tissue distribution and induction site of this component of the cytochrome P-450 depedent microsomal system may be relevant to site of drug toxicity.

    IgM class anti-HBc was tested in 184 of chronic HBs-Ag carriers by a radioimmunoassay with application of commercially available HAVAB-MR kit (Abbott Lab.) modified as previously reported. Ninety-two of chronic HBs-Ag carriers were positive for IgM class anti-HBc. The titers in these cases, however, were significantly lower than those in acute hepatitis type B (39 cases) tested. A positive correlation was found between the titers of IgM class anti-HBc and anti-HBc titers assayed by IAHA method. The sera positive for HBe-Ag showed higher values of IgM class anti-HBc than those positive for anti-HBe. In several cases with HBs-Ag positive chronic hepatitis, the elevation of IgM class anti-HBc was found to follow shortly after the rise of transaminase levels in sequential serum specimens.

    To elucidate the mechanism of coagulopathy in liver cirrhosis, the ternover rate of fibrinogen and the surface body count in patients with liver cirrhosis were assayed using radiolabeled fibrinogen.
    The life span of fibrinogen in the patients was shorter than that in the controls and could be lengthened by treatment with heparin. These results are compatible with the mechanism in consumption coagulopathy.
    In the surface body counting method, injected fibrinogen accumulated mainly in the spleen, with considerable splenomegaly, and the liver, suggesting that the main places of fibrinogen consumption were these organs.

    The pattern of cortical bone modelling in healthy and disease populations can be derived from simple measurements taken from radiographs of tubular bones. To determine the pattern of bone modelling in chronic cholestatic, we have measured these parameters in 83 women with primary biliary cirrhosis (PBC). We have compared bone modelling profiles in PBC groups with the expected pattern of bone modelling in healthy females.
    In healthy women, cortical bone is thickest in the 6th decade, after which cortical thinning occurs, In PBC, cortical thinning occurs progressively from the 4th decade onwards and the loss of cortical thickness is generally mirrored by increasing medullary cavity width. In all forms of PBC, cortical thinning is caused by widening of the medullary cavity rather than thinning at the shaft surface.

    We studied 6 patients with fulminant hepatitis by laparoscopy during acute stage of the illness.
    Three patients were noted to have severe liver atrophy in laparoscopy and massive necrosis in liVer biopsy. All died within 9 days following the examinations.
    The remaining three had milder degree of liver atrophy. Two of them survived, but another died presumably of acute necrotic pancreatitis which was diagnosed by laparoscopy done during acute stage.
    Important laparoscopic findings for evaluating the prognosis were the degree of liver atrophy and the extent of residual parenchyma on the liver surface.
    The values of HVF (hepatocyte volume fraction) obtained from the biopsy specimens were less than 10 precent in the three dead and 45, 70 percent each in the two survived. In one patient dying with complicating pancreatitis HVF was estimated as 40 percent.
    There were no major accident during the examinations.
    In conclusion, laparoscopy during acute stage of fulminant hepatitis is a safe and valuable method in predicting the clinical course and prognosis of the illness.

    A hepatic malignant mixed tumor in a cirrhotic patient with liver cell carcinoma was reported. He, 57 years old man, received partial right hepatectomy for a primary hepatic tumor. Histology of the tumor was Edomondson's grade II liver cell carcinoma associated with post-hepatitic liver cirrhosis.
    Follow-up study revealed a upward tendency of a-fetoprotein (AFP) and carcinoembryonic antigen (CEA). CT-scan of th liver five months after the operation showed a 6-cm mass lesion which located on anteriorly to the site of the previous tumor, then he was reoperated.
    Histologically, the resected tumor was composed of four components ; spindle cell sarcoma, chondrosarcoma, squamous cell carcinoma and adenocarcinoma.
    Biochemical determination of CEA and AFP of the resected tumors was positively showed CEA only in adenocarcinoma cells by peroxidase antiperoxidase method, but not identified AFP in these two tumors.

    This 78 year old man was admitted, because of skin itching and mild jaundice. On the basis of clinical course and laboratory data, the diagnosis of primary biliary cirrhosis (PBC) was made and he has followed for up to 32 months. During these periods, serum total bilirubin remained between 1.3 and 4. 0mg/dl. Serum GOT and GPT were mildly elevated (50-90K.U.) and serum alkaline phosphatase level remained markedly elevated (50-90K.A.U.). LAP and γ-GTP also showed extremely high values. Serum total cholesterol was increased up to 340mg/dl. IgM was also increased up to 2520mg/dl, although its value varied a great deal. Anti-mitochondrial antibody (AMA) was strongly positive with the highest titer of 1: 2560. ANA and anti-DNA Ab were both negative. ASMA was negative or positive on occasions, Needle liver biopsy was carried out, which showed a well preserved lobular architecture and markedly expanded portal tract with an aggregate of plasma cell and prominent atypical ductular proliferation near its periphery, There was marked piecemeal necrosis noted. The serial sections failed to demonstrat CNSDC or granuloma. Rhodanine stain for copper showed prominent perinuclear deposits of copper granule in the periportal hepatocyte. These histological findings may suggest the mixed type of CAH and PBC.

    Thyrotoxic crisis ("crisis") is often fatal in its clinical course once it occurs. Liver injury has been discussed as one of the causes of the "crisis". We report an autopsy case of hyperthyroidism complicated with remarkable acute hepatic necrosis.
    A 71-year old female, who was pointed out hyperthyroidism five years ago althougn without treatment, was admitted to our hospital on Jan 4'82 with chief complaint of blurred consciousness level. She caught cold on the day before and took a commercial medicine for herself. She had blurred consciousness level, jaundice, exophthalmos and tachycardia (pulse rate=142) on admission. She had leukocytosis, elevated T3 uptake and liver injury on laboratory examination. She died about 17 hours later despite of the institutions of digitalis, β-blockers, steroid and so on. The autopsy revealed the findings consistent with hyperthyroidism complicated with acute hepatic necrosis. We find some reports on the hepatic changes in hyperthyroidism. Fatty degeneration, central and localized necrosis etc. are reportedly main findings but we can not find a report of hyperthyroidism complicated with prominent hepatic necrosis. The cases of hyperthyroidism with jaundice are said to be often followed by the "crisis", so we must take much care whether jaundice is present or not in these cases.

    A 54 year old man was pointed out to have an egg shell shaped mass with marginal calcification on a plain X-p. His medical history included diabetes mellitus for which he had been taking oral agent. He had never noticed any GI-symptoms and had no history of abdominal trauma. Physical examination revealed no abnormality. Various examinations revealed the mass to be a common hepatic artery aneurysm. Compression and deviation of the pancreatic duct at the pancreas head by the mass and dilatation and irregularity of the proximal main pancreatic duct at body and tail were observed on ERCP. Partial aneurysmectomy and hepatic artery ligation were performed. The changes of the pancreatic duct seemed slightly recovered after surgery.
    Hepatic artery aneurysms are rare, and 180 cases had been reported till '79 in Japan. Among these, common hepatic aneurysms are very rare and only 10 cases including the present case were reported on our survey.
