Forty American cases of noncirrhotic acute alcoholic hepatitis were studied. Black female patients predominated in number. Female patients were significantly younger than male patients. The characteristic clinical features included abdominal pain, fever, icterus, very large liver, splenomegaly and ascites. SGOT elevation, generally of moderate degree, was present in the all patients. SGPT elevation was not so frequent, and was mild, while in the comparison the elevations of SLDH and total serum bilirubin were higher in both incidence and degree. The centrilobular area was the site of the major pathology characterized by alcoholic hyalins, swelling and lytic necrosis of liver cells, neutrophilic infiltration, fatty metamorphosis, and in a half cases, centrilobular fibrosis. The centrilobular fibrosis was an important finding, because it accompanied severer liver-cell injury and portal hypertension, and appeared an obligatory step in the evolution into monolobular regular cirrhosis. Occasionally they simmulated clinical pictures of obstructive jaundice and acute viral hepatitis.

Fasting serum bile acids were measured in 62 alcoholics with various liver diseases and compared with histological foatures in their percutaneous liver biopsy specimens.
In 51 of 62 patients (82%), total serum bile acid concentrations were elevated more than 10μ M/L at the time of admission. Two months after abstinence, serum bile acid levels were decreased in 45 of 62 patients (73%), however 11 patients with cirrhosis still had abnormal serum bile acid values.
There was a decrease in the ratio of cholic acid to chenodeoxycholic acid in patients with alcoholic hepatitis and cirrhosis.
The data suggest that the measurement of serum bile acids in alcoholics, particularly at the time of admission and two months after abstinence, is useful for evaluation of hepatic disorders in alcoholics.

Human leucocyte interferon (HLIF) was parenterally administered to a 27 year old Japanese male who was fbund to have hepatomegaly, moderately elevated transaminase values, perslstently positive HBsAg and negative c antigen and anti-e. Biopsy-proven liver pathology was consistent with chronic active hepatitis. Total dosage of HLIF was 6.4×10
7IU. Transaminases started to fall with a corresponding decrease of liver size which occurred around 30 days after the initation of therapy. Serial measurement of blood interferon level remained less than 10IU/ml by the method of Plaque-inhibition assay. Administration of HLIF did not effect the tiers of HIBsAg (RPHA), mdanti-HBc (IAHA). But the most striking changes were observed in the HBc system. HBcAg which stained strongly in 33% of the hepatocyte nuclei befbre treatment by the peroxidase-labelled antibody technique, became positive in only 5% of the nuclei with a much lower intensity after treatment. Follow up liver hiopsy perferomed after the tcrmination of therapy revealed amuch less inflamatory activity consistent with chronic persistent hepatitis, suggesting a definiteim- provement.
Conclusions
1) Some biochemical and histological improvement was noted during and after therapy. 2) administration of HLIF appeared to have no meamrable effect on HBsAg and anti-HBc. 3) there was a definite depression by HLIF of the nucleocapsid hepatitis-B core antigen in the liver. 4)further studies will bewarranted to define the optinal regimen suppressing the virus.

I. Effects on cell-mediated immunity

The parameters for cell-mediated immunity for ten patients with chronic liver diseases (CLD), who received intermittent prednisolone administration for twelve weeks, were studied periodically.
Administration of corticosteroid (CS) brought on no changes of percentage of circulating lymphocyte subpopulations, but supernormal increase of absolute numbers of both T-and B-lymphocytes which decreased previously, with the highest mark on the eighth week after administration, and subsequent decrease.
Effect on responsiveness of circulating T-lymphocytes to phytohaemagglutinin (PHA) was suppressive for patients who had normal responsiveness previously with the highest mark on the fourth week, but potentiative for patients who had decreased responsiveness previously with the highest mark on the eighth week. PHA intracutaneous reaction, which was suppressed previously, was suppressed moreover with the highest mark on the fourth week.
CS suppressed the production of macrophage migration inhibitory factor (MIF) to HBsAg by sensitized T-lymphocytes with the highest mark on the eighth week after administration.

The parameters for humoral immunity of ten patients with chronic liver diseases (CLD), who received intermittent prednisolone administration for twelve weeks, were studied periodically.
Effect of corticosteroid (CS) on titer of serum HBsAg, which reflected the production of anti HBs in vivo, was bidirectional. For the patients whose titer of HBsAg was low or production was vigorous, CS suppressed anti HBs production. Contrary, for the patients whose titer of HBsAg was high or production of anti HBs was scant, CS enhanced anti HBs. This effect of CS was analogous to one of autoantibodies, anti smooth muscle antibodies and anti nuclear factors, and anti HBs of serum anti HBs positive cases. CS heightened their incidence and titer.
CS brought on the transient decrease of serum IgG and IgA concentration with highest mark on the eighth week after administration, but not IgM. CS brough on slight decrease of serum complement activity (CH50) and complement components (C3, C4).

The action of saikosaponin, which is effective to D-galactosamine induced hepatic injury, to intracellular microorgans of normal rat liver cells was examined quantitatively by electron microscopy.
After 24 hours of injection of saikosaponin 2mg/kg of body weight, the endoplasmic reticulum, especially smooth endoplasmic reticulum, was remarkably increased. The volume density of smooth endoplasmic reticulum to cytoplasm of saikosaponin 2mg/kg treated liver cells was increased to 7.34±1.34% in comparison with 3.77±0.41%.of normal liver cells but the volume density of mitochondria was not changed. In the rat liver treated with saikosaponin 20mg/kg, the volume densities of mitochondria and golgi complex significantly increased although the endoplasmic reticulum was not increased.

The activities of Glucose-6-phosphatase and NADPH-cytochrome c reductase of the rat liver which were treated with saikosaponin 2mgtkg or 20mg/kg of body weight were compared with that of a normal rat liver. The activities of Glucose-6-phosphatase and NADPHcytochrome c reductase of the liver homogenate of saikosaponin 2mg/kg injected rat were remarkably increased but these activities of the liver homogenate of saikosaponin 20mg/kg injected rat were not changed. In the smooth microsome isolated from the liver treated with saikosaponin 20mg/kg of body weight, NADPH-cytochrome c reductase activity was slightly increased, and the remarkable increase of Glucose-6-phosphatase activity of rough microsomes isolated form the liver treated with saikosaponin 2mg/kg of body weight was observed.

Histologic features of hepatocellular carcinoma were studied in autopsy cases of 84 patients, in relation to complication of liver cirrhosis and presence or absence of hepatitis B virus (HBV) infection.
Hepatocellular carcinoma was well divided histologically according to their pure morphological patterns, into two major categories: Common (trabecular or trabeculotubular) type and special type.
The presence or absence of infection of HBV was examined by orcein or other stains for hepatitis B surface antigen in tissue sections.
As a result, 72 (86%) cases revealed common type and 12 (14%) cases special type; 24 (29%)cases showed HBV-positivity; the incidence of HBV-positivity of common type was 32%, while that of special type 8%.
Characteristically, almost all carcinomas associated with HBV-positivity, except for one case, revealed common type. All carcinomas with special type but without cirrhosis were HBV-negative. HBV might be rather related to common type hepatocellular carcinoma than special type.

An unusual case of suggestive primary biliary cirrhosis with IgM-K type M proteinemia was reported.
A 55-year-old woman, was hospitalized for unexplained hepatic dysfunction in the absence of hepatobiliary symptoms. On admission no physical abnormalities except for slight jaundice and hepatomegaly were noted.
Laboratory studies disclosed marked elevation of serum alkaline-phosphatase activity, accerelation of erythrocyte sedimentation rate and hypergammaglobulinemia. Serum anti-mitochondrial and anti-smooth muscle antibody were detected. Quantitative immunoglobulin determination showed a marked elevation of IgM (1476-2316mg/dl) without reduction of any other immunoglobulins. Although serum electrophoretic pattern was suggestive of polyclonal gammopathy, on immunoelectrophoresis M-bow was demonstrated on anti-hum an whole sera, anti-IgM and anti-K, respectively, after 2-mercaptoethanol treatment.
Bone marrow aspiration and skeletal roentgenograms revealed no abnormalities, and Bence-Jones proteinuria and cryoglobulinemia were not detected.
On needle biopsy of the liver regeneration nodules, ductular proliferation and paucity of bile ducts were revealed but not accumulations of lymphocytes.
