    3β-hydroxy-5-cholenoic acid (3β-OH-Δ5) is an unusual bile acid that could be detected little amount in the serum of healthy subjects. For the determination of this bile acid in serum, a sensitive and very specific radioimmunoassay was developed and the clinical significance of this bile acid in serum was studied by using this radioimmunoassay.
    3β-OH-Δ5-BSA compound was used as antigen and antiserum to this antigen was raised in rabbit. [3-3H]-3β-OH-Δ5 was used as radioactive tracer and radioimmunoassay was carried out by the method of Simmonds et al.
    The percentage of bound radioactivity decreased linearly with a logarithmic increase in unlabeled 3β-OH-Δ5 from 10 to 200 pmole. Antiserum showed high specificity for 3β-OH-Δ5 (free and conjugate).
    In healthy subjects, serum concentration of 3β-OH-Δ5 was 135±59 pmole/ml (Mean±SD). In various hepatobiliary diseases, an increase of serum 3β-OH-Δ5 was observed; 291±309 pmole/ml in primary hepatoma, 408±68 pmole/ml in obstructive jaundice, 342±207 pmole/ml in liver cirrhosis and 292±175 pmole/ml in acute hepatitis.
    These results suggest the presence of an altered pathway on cholesterol catabolism in pathologic liver.

    The influences of heparin and steroid on the coagulation and fibrinolytic system were investigated in the experimental acute hepatic failure of rabbits induced by D-galactosamine HCI (lg per kg). The great majority of untreated rabbits died within 60 hour (mean±SD 34.2±15.1 hour) following a typical course of fulminant hepatic failure with disseminated intravascular coagulation (DIC). Both the administrations of heparin (200IU/24 hours) and steroid (prednisolone 4mg/kg/24 hours) attenuated the elevation of transaminase to some extent but only the heparin significantly prolonged the survival (54.0±6.4 hour, p<0.005). The administration of heparin induced hypocoagulability and hyperfibrinolysis and prohibited the occurrence of DIC. On the contrary, the administration of prednisolone inhibited fibrinolysis and increased the frequency of microthrombosis. It is suggested that the steroid therapy on acute hepatic failure enhanced the risk of DIC, which may finally lead to lethal complications.

    The purposes of this study are to document the sequelae of 70 nonA, nonB post-transfusion hepatitis patients and to assess the influencing factors leading to chronic hepatitis. Sixteen patients' transaminases normalized within 3mo, and were classified as resolved group (RG), and 45 patients had prolongation of abnormal transaminases for more than 6mo, and were classified as chronic hepatitis group (CHG).
    When CHG was compared to RG, no difference was found with respect to sex, volume of blood transfusion, incubation period, presence of initial symptoms, ocurrence of jaundice, immunoglobu-line levels and initial liver function tests except transaminases. However, significant differences were found between both groups in age and transaminase levels. Operation had no effect on prognosis. All acute nonA, nonB hepatitis cases occured during pregnancy and at delivery progressed to chronic hepatitis. Liver biopsies in 13 of the 45 of CHG revealed chronic active hepatitis in 5, chronic persistent hepatitis in 2, persistent hepatitis in 5 and unresolved hepatitis in 1. None of these patients died or developed cirrhosis.
    The influencing factors on the sequelae of nonA, nonB post-transfusion hepatitis may be the quantity of infected nonA, nonB hepatitis virus (NANB-HV), ability of the proliferation of NANB-HV and the unbalanced immunological status of individual.

    The specific antibody against the liver cell membrane was coated to the isolated liver cells and liver cell damage was induced by adding the peripheral blood mononuclear cells as the effector cell soarce. When liver injury was estimated by the reduction of albumin biosynthesis in the liver cells, a marked decrease of the synthesis was observed by ADCC reaction. Moreover, cytotoxic substance which cause the diminution of liver cell function was detected in the cell culture supernatant of ADCC reaction. By a gel filtration using a Sephadex G-75 column, this active material (or materials) was fractionated as the definite component and this was further separated into two fractions by DEAE-cellulose column chromatography. These two fractions were shown to be different active sub-stance each other by isoelectric electrophoresis.
    These results suggest that some cytotoxic materials were produced in consequence of ADCC reaction and those may cause the liver injury.

    In 15 patients with unresectable hepatocellular carcinoma (HCC), the authors performed trans-catheter arterial embolization (TAE) therapy with gelfoam particles soaked in adriamycin. Clinical and histological effectiveness of the therapy was detected in 13 of 15 patients and 2 of 10 survivors have been alive for more than 10 months after TAE therapy.
    AFP values in 7 patients were decreased to the half of the pretreatment value within about one week. Ultrasonography and CTscan were performed 9 of 15 patients and the tumors were decreased in size. On the follow-up angiography performed in 11 patients, 8 of them showed obvious decrease of the tumor size and disappearance of the tumor vessels and tumor stains. Laparoscopy were per-formed in 7 patients and 4 of them revealed decrease of the tumor size. In tumor biopsy spacemen, marked necrosis of the tumor cells, increased fibrous tissues and disarrangement of tumor cells were observed.
    These results indicated that TAE therapy can be regarded as specific treatment for HCC because of non-invasiveness and safety even though in cases with severe hepatic cirrhosis. However, no indication of this treatment was directed to the cases with portal truncus obstruction and combi-nation chemotherapy was needed for these cases.

    We have investigated the effect of antimetabolic, anticancer agent, Futraful, on the nuclear DNA content of human liver cells employing Feulgen-DNA cytofluorometry. The measurements of nuclear DNA content of individual liver cells were carried out on the smear preparations of the liver tissues obtained from six cases of the autopsy. The liver cells from subjects treated with Futraful showed highly advanced polyploidization, compared to the cells from normal subjects with the corresponding ages. In two of the cases studied the polyploidization has reached to such an extent that has never been found in the normal aging process. And the correlated histopathological examinations revealed the characteristic features of nuclear atypism showing micronucleus formation. The precise mechanisms of these changes remain to be elucidated. It is clearly indicated, however, that this antimetabolic, anticancer agent, Futraful, impairs nuclear DNA structure of the liver cells leading both to marked polyploidization and micronucleus formation.

    Four cases of hepatic infarction resulted from intrahepatic portal thrombosis as a complication of operation for gastric cancer were reported. The most common causes of hepatic infarction are obstruction of the hepatic artery in the presence of disturbance of portal circulation, and obstruction of portal vein branches tends to take the form of a hemorrhagic area called Zahn's infarct. In the present four cases, however, intrahepatic portal thrombosis alone led to extensive infarct of the liver, although the hepatic artery was patent. It was believed that the thrombosis was formed in combination with endothelial alterations of the portal system and disturbances in portal blood flow such as turbulence and stasis resulted from operation for gastric cancer, as well as hypercoagulability followed by splenectomy.

    A 27 year-old male student was admitted to our hospital because of remittent fever and cough. Later on he developed tenderness in the right upper quadrant of the abdomen. A chest X-ray film showed an area of consolidation extending from the right hilum. Various kinds of treatments were fruitless. The autopsy studies revealed disseminated phycomycosis involving the liver, lungs and bronchi. Especially in the liver a large area of necrosis caused by phycomycetes were found. Little infarmation was available concerning a large area of necrosis in the liver due to phycomycetes. In this paper the clinical course and pathogenesis of phycomycosis in the liver are discussed.

    A 38-yr-old man with sporadic type A hepatitis developed acute renal failure and responded dramatically to hemodialysis.
    He developed jaundice with previous symptoms as fever, nausea and vomiting. On the 12th day since the onset, macrohematuria and oligouria were suddenly accompanied without marked disturbance of liver function tests suggestive of fulminant hepatitis. Rapidly elevated serum urea-N (168mg/dl) and serum creatinine (16.38mg/dl), and remarkable loss of serum sodium caused disturbance of consciousness and convulsion. Hemodialysis was performed twice on the 19th day and the 21st day, and the patient was relieved from acute renal failure rapidly.Liver function tests other than SGOT and SGPT became normal within 3 months, but mild elevation of transminases continued till the 10th month. The liver biopsy under peritoneoscopy taken on the 89th day showed the convalescent stage of acute hepatitis. A discussion of possible etiologies of acute renal failure is presented.

    HBV-M.G.N. caused by HBe antigen-antibody immune complex in infant, who are highly positive for HBe antigen, has been reported, but there is no report of adult cases. We experienced a 48-years-old male of HBs antigen positive chronic hepatitis associated with M.G.N. caused by HBe antigen-antibody immune complex. In the course of chronic hepatitis B, proteinuria was continuously positive (100-300mg/dl). Liver biopsy showed CAH (2A), and the finding by kidney biopsy was membranous glomerulonephritis (M.G.N.). Examinations of frozen tissue sections by fluoresceinantibody technic revealed deposition of HBe antigen, IgG and βIC on the glomerular basment membrane, but no HBs antigen. At that time, serological examination showed positive for serum HBe antigen by M.O.method, βIA in 62 mg/dl, and negative for immune complex in serum.

    A 35 year old male with typical alcoholic hepatitis presented almost negligible uptake of 99mTc on the liver scan. Electron microscopic findings disclosing decreased number of Kupffer cells and impaired blood flow in the sinusoids may elucidate extremely diminshed uptake of isotope by the liver. Transient elevation of serum α-fetoprotein up to 3200ng/ml observed during the active stage may indicate a regeneration process of hepatic necrosis occured following the acute alcoholic hepatitis.

    A 41-year-old man was admitted to hospital because of dysphagia. He was diagnosed to adenocarcinoma of esophagus and received radiation therapy as shown Fig. 2. Two weeks after the finish of the first radiation (Total dose: 4000rad against left lobe of the liver), transaminases gradually elevated (Fig. 3) and left entire lobe of the liver disappeared in scintigram (Fig. 5). After normalization of liver function tests, laparoscopic examination and liver biopsy were done. Right lobe of the liver showed smooth surface with almost normal color, but left lobe showed rough surface with dark and whitisch color (Fig. 6). Histologic findings of left lobe showed sever damage of hepatocytes, ducts and vessels (Fig. 8, 9, 10) despite of almost normal findings of right lobe. And we discussed about radiation hepatitis adding our experimental studies using dogs.

    Angiosarcoma of the liver has been said to be very rare tumor but the tumor due to thorotrast was reported previously. Recently, a large number of patients of the angiosarcoma were occurred in vinyl-chloride workers and the disease is increasing.
    We have experienced of the case of 44-year-old woman who had abdominal pain in the upper quadrant and fever for a month. During the admission, celiac angiography was carried out and hepatic abscess was suspected. Histopathological examination of the tumor resected by partial hepatectomy revealed findings of angiosarcoma.
