    The aim of this study was to evaluate cell proliferative activity by immunostaining for proliferating cell nuclear antigen (PCNA) and MIB-1 in biopsy specimens from various liver diseases. The staining pattern of PCNA was not constant nor clear, allowing a wide range of labeling index and confusion for judgement whether the sections are malignant or not. In contrast, the MIB-1 immunostaining showed distinct staining of malignant nuclei and its labeling index was significantly higher in hepatocellular carcinoma (HCC) [11.0±9.6%] than in non-neoplastlc liver diseases [1.2±0.9%, p<0.01]. In addition, its labeling index was well correlated with the grade of histological differentiation of HCC. In conclusion, for the small tissues from the hepatic lesions which could not be clearly defined as HCC under HE staining method, MIB-1 immunostaining was considered to be useful for the differential diagnosis of early HCC from non-neoplastic liver disease.

    Immunohistochemical expressions of HLA-DR antigen on the interlobular bile duct epithelium, degree of inflammation around the bile ducts, and bile duct damage were studied in asymptomatic PBC cases showing normal (early PBC; ePBC) and elevated (asymptomatic PBC; aPBC) serum ALP values. There was no significant difference in degrees of periductal inflammation and bile duct damage between both groups, but the immunohistochemical expression of HLA-DR antigen on interlobular bile ducts was found to be significantly lower in the ePBC group as compared to those in the aPBC group. These results indicate that aberrant expressions of HLA-DR antigen occur in the course of bile ducts damage and may be related to the progression and continuation of bile duct destruction.

    We examined fast MRI under breath holding in patients with small hepatic nodular lesion. MR imagings were obtained with Fast Spin Echo (FSE) which was T1-weighted image and T2-weighted image and Fast Mutiplnar Spoiled GRASS (FMPSPGR) which was dynamic study. Sixty-four of 78 (74%) patients with small hepatocellular carcinoma (HCC) were detected by FSE sequence. Nine of 16 (56%), nine of 26 (37%) and 29 of 29 (100%) patients with adenomatous hyperplasia, large regenerative nodule and cavernous hemangioma were detedcted by FSE sequence, respectively. Fifity-seven (74%) HCC lesions and 28 (97%) adenomatous hyplasia lesions were diagnosed by FMPSPGR sequence, respectively. Fast MRI (FSE and FMPSPGR) with breath holdlng provides diagnostically useful small nodular lesion of the liver.

    We report three cases with nail damage due to tinea unguium occuring during interferon (IFN) therapy for chronic hepatitis C. The first case is a 53 year old male with right 3rd and 4th fingernail abnormalities occurring in the 4th week of IFN therapy. Changes spread distally with increasing opacity, thicking and distortion. Damage also occurred in the right toenails, and the affected fingernails eventually fell off. The second case is a 51 year old male with right 4th and 5th fingeail changes occurring in the 8th week of IFN therapy. Changes were initially confined to the proximal nail fold, however all right fingernails were subsequently affected and the 5th fingernail eventually collapsed. Third case is a 53 year old male with right 1st toenail damage occurring in the 10th week of IFN therapy. All toenails were subsequently affected. Diagnosis was made by microscopic examination of scrapings taken from affected nails. In all three cases, initial outbreak was simular to the pattern of proximal white subungual onychomycosis. After cessation of IFN, nail damage improved without the use of antifungal agents and no subsequent reccurrence was observed. It was suggested that onset of tinea unguium induced nail damage may be caused by IFN therapy. It should be noted that onychomycosis may be as adversed effect of IFN therapy.

    A 53-year-old female with pancytopenia was admitted to our hospital in June, 1994. She was diagnosed as acute myelocytic leukemia (M2 according to the FAB classification) by specimens of bone marrow puncture, and treated with BHAC-DMP (enocitabine, daunorubicin, mercaptopurine, prednisolone) regimen. She developed spiking fever at a nadir period of granulocytes during the induction chemotherapy, and high fever persisted even after achieving complete remission despite anti-bacterial and-fungal treatment. Repeated blood cultures gave negative results, and there was no evidence of local infection, Seventeen days after the beginning of high fever, elevation of biliary tract enzymes and transaminases was observed, and subsequent abdominal ultrasonography and computed tomography revealed multiple liver abscess. Specimens of initial liver biopsy showed that the liver abscess was pyogenic, but failed to define an infectious etiology. Second trial of liver biopsy, however, gave the evidence of Aspergillus infection. Anti-fungal treatment was resumed, and the liver abscess gradually reduced with improvement of inflammatory indices probably due to her immunological betterment. To our knowledge, this is the first report of a case that developed Aspergillus liver abscess without pulmonary involvement and diagnosed by liver biopsy.

    A 57-year-old woman with decompenzated liver cirrhosis was admitted to our hospital. She had severe hypersplenism with marked splenomegaly and pancytopenia. We performed partial splenic embolization (SE) with a 90% infarction rate. The SE treatment led not only to a sustained increase in both platelet and white blood cell counts but also to a great improvement of hepatic function (Child C to A). In addition, liver volume increased from 640cm3 before SE to 780cm3 at 1.5 years after SE, while 3 aneurysms of splenic artery shrank or disappered after SE. These findings may suggest that SE performed with a high infarction rate of the spleen is a useful therapy for liver cirrhosis with hypersplenism.

    A 44-year-old woman was pointed out having an abnormality on the left diaphragma on a chest radiography. Left diaphragmatic hernia of left lobe of the liver was confirmed by CT scan, liver scintigraphy and hepatic antiography. This hernia has been unchanged for approximately 7 years. This rare adult case suggests that non-traumatic liver herniation may occur even on the left diaphragma.
