For characterization of the binding-sites (albumin receptor) on HBsAg-particles to polymerized human serum albumin (pHSA), an inhibition test of enzyme-linked immunosorbent assay (ELISA) for detection of the binding-activity of HBsAg-particles against pHSA (pHSA-BA) was developed. In inhivitory reagents, protease (0.1mg/ml) and 0.05 M 2-mercaptoethanol inhibited the activity of albumin receptor, but could not inhibit the antigenicity of HBsAg. One hundred % acetic acid also inhibited that of albumin receptor, but 100% methanol, 50% acetone and 2% paraformaldehyde solutions could not inhibit the activity. The sonicated trypsinized human red blood cells (A type) also inhibited the activity, but the presence of Ca++ activated the pHSA-BA in the present assay. In human and rat liver lectins, human crude lectin inhibited the pHSA-BA specifically. For the influence of monosaccharides, Nacetyle-D-galactosamine had more inhibitory activity than the others (α-methyl-D-mannoside, Dgalactose and L-fucose).
These results indicate that the albumin receptor on HBsAg-particles is consisted with human liver lectin-like glycoprotein different from the antigenic determinant of HBsAg.

Efficacy and safety of human IFN-α and -β were investigated in patients with DNA-polymerase (DNA-P)-positive chronic active hepatitis. IFN-α was intramuscularly injected at a total dose of 119×106IU in 10 cases and IFN-β was intravenously done at 102×106IU in 12 cases, for 4 weeks.
During administration serum DNA-P level became negative in 4 cases of IFN-α group and 7 cases of IFN-β group. Also, in one case of each group was observed type II response classified by Merigan, and the rest were type III.
Increasing tendencies in levels of serum AST and ALT were observed at the beginning of administration, but after administration both of the levels and serum albumin concentration significantly improved.
Leukocytopenia mostly due to neutrocytopenia was observed, but it rapidly recovered after terminating administration.
Transient fever was observed in all cases, but there was no severe side-effect.

Two hundred and nineteen liver biopsy specimens were examined by light microscopy to assess diagnostic significance of round cytoplasmic globules (RCG), so-called megamitochondria, for alcoholinduced hepatic damage. Under light microscopy RCG were stained light red with eosin, dark red with Masson Trichrome, and were PAS-negative.
The incidence of RCG was significantly higher in patients with high alcohol consumption than in those with low or no alcohol intake (p<0.001), former being responsible for 82% of all the specimens demonstrating RCG. Twenty-eight per cent of RCG-positive specimens were derived from those who abstained from alcohol for more than one month. No Mallory's hyaline body was observed in our series. As to histologic changes, RCG were found in fatty change, cirrhosis and chronic hepatitis, in the order of their frequency. The presence of RCG in hepatocytes was considered to be highly suggestive of alcoholinduced hepatic damage.

The aminoterminal peptide of type III procollagen in the sera of 123 patients with various liver diseases was assayed according to the immunoassy method described by Rohde. The assayed values were as follows: norrnal (15): 9.90±2.60ng/ml, acute viral hepatitis (20): 14-67±6.99ng/ml, chronic hepatitis (34): 22.48±19.67 liver cirrhosis (25): 15.45±8, 01ng/ml, alcoholic liver disease (21): 14.23±12.24mg/ml, cholelithiasis (6): 18.13±5.13ng/ml, primary and metastatic liver cancer (17): 107.11±l12.90ng/ml. Serum concentration of the peptide seems to correlate more closely with disease activity and the active biosynthesis of collagen during the disease process, than the amount of deposited collagen.
17 serum samples from the patients with liver cancer showed extraordianrily high concentrations of the peptide ranging from 26 to 400ng/ml. The assay of the peptide might be useful for the evaluation of disease activity and the effectiveness of the treatment in liver cancer.

A sensitive and specific radioimmunoassay (RIA) for the measurement of serum total chenodeoxycholic acid (CDCA) has been developed, and an accuracy of this RIA has been confirmed. Then, the fasting serum CDCA level was determined by using CDCA-RIA in 18 normal subjects and 87 patients with hepatobiliary diseases. In normal the fasting level was 0.59±0.29nmole/ml, and in all patient groups serum concentration of this bile acid increased. There was a relationship between the degree of elevation of serum CDCA and the severity of chronic hepatocellular damage. Thus, this finding suggested that the measurement of serum CDCA could be useful as a screening test to monitor hepatic dysfunction.
The serum level of primary bile acid and ursodeoxycholic acid were determined by RIA of these bile acids during 24-hour period in four normal subjects, one with chronic active hepatitis and one with liver cirrhosis. In normal the serum concentrations of CDCA and cholic acid (CA) rose postprandially to peak (the level of CDCA were constantly higher than that of CA), and the area under curve (AUC) of CDCA was 1.6 larger than that of CA. In hepatic disease an elevation of fasting serum primary bile acid and postprandial peak hight were observed, and AUCs of CDCA and CA were enhanced relating to the severity of hepatocellular damage.

This paper deals with clinicopathological aspects of hepatocellular carcinoma (HCC) in Niigata area with the discussion of high association rate of HCC and Hepatitis B Virus (HBV).
One hundred twenty seven patients with HCC hospitalized in our clinic between 1967 and 1980 were studied. They consisted of 113 men and 14 wemen ranging from 23 to 78 years old, mean age of 55.5.
The prevalence of HBsAg in patients with HCC was 50.0% and an averaged age of HBsAg positive HCC patients was 6.0 years younger than HBsAg negative patients. Anti-HBs was detected in 12.6% and anti-HBc was detected in 87.1% of HCC patients.
Thus markers of HBV replication (HBsAg positive and/or high titer of anti-HBC) existed in 66.3% and markers of past HBV infection (anti-HBs positive and/or low titer of anti-HBc) were observed in 25.7% of HCC patients. Remaining 9.9% did not have any HBV markers.
Serum alpha-fetoprotein (AFP) of more than 200ng/ml was observed in 81.2% of patients, and AFP non-producing HCC was 10.3%.
The habitual alcohol intake did not substantially promote the HCC development.
Ultrasonography and computed tomography offered coplementary or supplementary procedure to detect minute HCC.

To clarify the advantage of high frequency transducer for diagnosis of liver tumor, sixty-two cases of liver tumor were examined by ultrasound, in use of 3.5MHz and 5.0 MHz transducer, under same accoustic conditions (length of focus, S.T.C., dR) except dB. And three major advantages were recognized as follows: 1. The dectectabilily of small focal lesion has remarkable increased, with high resolution of 5.0MHz transducer, 2. posterior enhancement and posterior attenuation were expressed more clearly using 5.0MHz transducer. 3. contour of focal lesion was clearly demarcated.
In addition, marginal rim of metastatic tumor was not visible using 5.0MHz transducer with minimal increase of power (5-10 dB), which serves as differential diagnosis of metastatic tumot from encapsulated hepatocellular carcinoma. But, lateral shadow in metastatic tumor is more frequently seen using 5.0MHz transducer (23%) than using 3.5MHz transducer (10%).

A number of works had been done about the intra-hepatic anatomy using corrosion cast. Therefore the anatomy of the liver has been considered to be throughly understood.
However recent advances of diagnostic imagings and the development of the idea of subsegmentectomy necessitate full appreciation of topographic anatomy of the liver more essential than ever-Conventional corrosion cast is rigid and less suitable for studying the topographic anatomy. For that reaon liver cast has not been well appreciated for clinical usage.Authors have introduced silicon rubber as injection material of liver cast. Since liver cast with silicon rubber is flexible, it is possible to study the deeper structures by separating the hindering small branches without damages.
In this paper, we report the method of liver cast preparation with silicon rubber. Its clinical usage will be reported in a separated occasion.

The hepatic artery was embolized with Gelfoam in 28 dogs to evaluate the changes of the liver by hepatic arterial embolization treatment. Serial angiography of the hepatic artery, liver function tests and histological study of the liver were carried out for four weeks after embolization.
Results were as follows;
1) 22 dogs (78.6%) survived and 6 dogs (21.4%) died within 1 week.
2) Recanalization of the embolized hepatic artery was demonstrated by 7 days after embolization. But no collateral artery was noted after embolization in any dog.
3) In survived dogs, transient elevation of serum transaminase were found in 1 and 3 days after embolization, but they returned to normal by 3 weeks. Serum bilirubin and LDH also increased and decreased gradually.
4) In survived dogs, gross examination study of the liver showed small infarcted areas but large portion of the liver was intact. Moderate centrilobular necrosis were noted in early days but it decreased gradually after treatment.
5) In died dogs, serum transaminase was elevated more than that of survived dogs, and severe centrilobular necrosis of the liver was demonstrated by microscopic study.
These results suggest that careful observation is necessary for at least one week after transcatheter arterial embolization because liver tissue damage is observed in this period.

38-year-old female was suffered from general fatigue and appetite loss in mid April of 1982. Her serological examination of hepatitis type B virus revealed positive HBs-antigen and high titer HBcantibody, which showed she got acute hepatitis type B under HB virsu carrier state. A month and a half after that, her consciousness level was decreased with marked elevation of serum transaminase and total bilirubin.
IgM-HA antibody positive data indicated superimposed infection of HA virus. In order to GO-80 and Glucagon-Insulin therapy, consciousness level was improved, but no effect was seen about the high total bilirubin and prolonged prothrombin time. She died on 22th day of admission because of gastrointestinal bleeding. On the liver tissue of the necropsy, marked bridging necrosis was observed and a few HBs-antigen positive degenerated hepatocytes were recognized.

We have recently experienced a case of non-A non-B posttransfution hepatitis (NANB-PTH) having developed liver cirrhosis during the follow-up period of several years. Such a follow-up case is a few in the literatures.
A 69-year-old female was transfused 13 units of blood when she received hysterectomy because of carcinoma of the uterus. At that time, the liver surface of the patient was visibly smooth. Thirty-five days after the blood transfusion, serum transaminase levels began to elevate, but she was almost asymptomatic and had no jaundice. No serological evidence of infection of HAV, CMV, or EBV was obtained. Since then, serum transaminases have fluctuated with s-GPT peak levels of 300 to 500IU/L and never returned to normal levels as long as about 3 years. Three years and 4 months after the onset of the disease, the liver already showed a completely cirrhotic feature on both laparoscopy and liver biopsy.

A 39-year old woman experienced nausea, anorexia and jaundice three weeks after taking a herb drug, "MKsan". There was no history of blood transfusions or alcohol abuse. On admission, jaundice and hepatomegaly were observed. Serum HBs-Ag and IgM anti-HA were negative. The serum aspartate aminotransferase (SGOT) was 351IU per liter, the bilirubin 7.1mg per 100ml, and the alkaline phosphatase 487IU per liter. The lymphocyte transformation test showed positive when "Shohakuhi", one of the ingredients of the herb drug "MKsan" was added to the culture medium of the lymphocyte.
The liver biopsy specimen showed cholestasis and focal necrosis of liver cells. These observations indicate a drug-induced liver injury caused by a herb drug.

An autopsy case of veno-occlusive disease of the liver was reported. The patient, 39-year-old female, received mitomycin C therapy (72 mg in total) after resection of ileo-cecal tumor. Two months after beginning of mitomycin C medication, edema, intractable ascites and hypoproteinemia developed. She died of the rupture of esophageal varices four months later. Postmortem examination revealed prominent congestion of the liver (nutmeg liver) without forming thrombus in the hepatic vein. Microscopically, the many of the central veins and of the sublobular veins were stenosed or occluded by the marked fibrous thickening of the intima. These were quite similar with the veno-occlusive disease in West India. In the present case, mitomycin C was regarded as the only possible causative agent of the veno-occlusive disease, because there was no history of radiation and organ transplantation. Mitomycin C is widely used as an anti-cancerous drug, but the present case suggests much attention should be needed when mitomycin is prescribed.

Primary leiomyosarcoma of the liver is a very rare disease. A 51-year-old female with primary leiomyosarcoma of the liver, who was treated with successful resection, was reported.
She was admitted to our hospital on May 24, 1982 because of a sensation of fullness and pain in the upper abdomen. Physical and laboratory examinations revealed no abnormalities except a marked hepatomegaly. Abdominal imaging techniques including CT and angiogram demonstrated a large malignant tumor in the left hepatic lobe and a samll metastatic lesion in the right hepatic lobe. The resection of two tumors was performed on July 1, 1982. The resected tumors were about 10cm and 1.5cm in diameter respectively, and yellowish colour with partially necrotic cavities.
Histological examinations of the resected tumors showed the interlacing bundles of spindle-shaped cells with blut-ended and elongated nuclei, and leiomyosarcoma of the liver was confirmed.
There has been no evidence of the recurrence during 7 months after the operation.
