    We assayed IgA subclasses anti-HBc in sera and bile of patients with hepatitis B virus (HBV) infection and examined molecular characterization of IgA anti-HBc for studying the mechanism of IgA anti-HBc production. IgA anti-HBc was high titer in acute type B hepatitis. Furthermore, levels of IgA anti-HBc were elevated along with the advancement of hepatic disease in persistent HBV infection, ranging from asymptomatic carrier state to chronic active hepatitis and liver cirrohosis. In acute exacerbation of chronic liver diseases, more significant changes were observed in IgA2 anti-HBc and in sIgA anti-HBc. Analysis of molecular characterization of IgA anti-HBc showed that elevation of polymeric IgA anti-HBc in acute exacerbation phase, though increase of both monomeric and polymeric IgA anti-HBc was recongnized. On the other hand IgA anti-HBc activities, which were almost found as sIgA anti-HBc, were also detected in bile. It seems that IgA anti-HBc as a good model in considering the mechanism of humoral focal immune response under central role of the liver.

    To investigate immunological properties of the monoclonal antibody (48-1 antibody) associated with non-A, non-B (NANB) hepatitis, the peripheral blood lymphocytes from chimpanzees with various infective status of NANB hepatitis were transformed by Epstein-Barr virus (EBV) and cluture supernatants were screened by immunofluorescent staining for antibody activity against the liver sections of chimpanzees with NANB hepatitis. Several types of autoantibodies reacted with liver sections were obtained and 48-1 antibody-producing cultures were obtained from any chimpanzee regardless of NANB infection.
    The frequency of 48-1 antibodies from EBV-transformants in infected chimpanzees was significantly suppressed (0.2%) in comparison with that of uninfected (2.7%) and convalescent (1.6%) chimpanzees. But the frequency of anti-SRBC antibody-producing clone and quantity of secreting immunoglobulins were almost equal with each groups. Hence 48-1 antibody-producing clone exists in normal status and NANB infectious agents bring the expression of 48-1 antigen in hepatocytes, the results would suggest that NANB infectious agents specifically suppress the frequency of 48-1 antibody-producing clone.

    We measured (2'-5') oligoadenylate synthetase (2-5 AS) mRNA levels in peripheral blood mononuclear cells (PBMC) of 16 patients with chronic hepatitis B (CHB) and non-A, non-B (CHNANB) during interferon (IFN) treatment. 2-5 ASmRNA was measured by dot blot hybridization using 32Plabeled human 2-5 AScDNA as a probe. 2-5 ASmRNA levels in patients' PBMC, which were similar to those of normal subjects before treatment, increased from 1.2 to 2.7 8h after first dose of IFN and remained high during treatment. 2-5 AS level in PBMC decreased when incubated in vitro even in the presence of IFN, however, 2-5 ASmRNA level increased in response to IFN. This suggets that the decreased 2-5 AS level in vitro may not result from decrease in the 2-5 ASmRNA level, but may be related to events beyond 2-5 AS mRNA production. We also examined the difference in the 2-5 ASmRNA level between patietns receiving different IFN doses. Patients receiving IFN in doses of 1×106IU/d had lower level of 2-5 ASmRNA compared to those receiving IFN in doses of more than 3×106 IU/d.

    The alterations of sinusoidal endothelial fenestration (SEF) were analysed morphometrically in rat liver fed liquid diets containing 5% ethanol and in rat liver after abstinence. In control rats, the diameter of SEF in zone 1 was significantly (p<0.001) larger than that in zone 3 and the number in zone 3 was significantly (p<0.001) more compared with zone 1. The porosity of SEF was significantly (p<0.01) higher in zone 3 than in zone 1. By chronic ethanol consumption the diameter of SEF in zone 3 were enlarged significantly (p<0.001) compared with the controls. The number of SEF resulted in significant decrease in zone 1 (p<0.05) and zone 3 (p<0.01) and the porosity led to significant increase in zone 1 (p<0.05) and zone 3 (p<0.001). The differences were more remarkable in zone 3 than in zone 1. After withdrawal there was a significant difference (p<0.05) in the diameter of zone 3 compared with the controls. The alterations of SEF in ethanol fed rats might be adaptive for the liver, which may lead to fat deposition in the hepatocyte.

    In the present study, the effect of chronic ethanol consumption on hepatic heme metabolism of rats was investigated. Wistar male rats were fed nutritionally adequate liquid diets containing ethanol as 36% of total calories for 5 weeks. After an overnight fast, the livers were centrifuged to obtain mitochondrial and microsomal fractions, Chronic ethanol feeding of rats resulted in about 19% of hepatomegaly. Mitochondrial protein content was not different between two groups, but microsomal protein content was significantly increased in ethanol-treated rats. Hepatic microsomal content of cytochrome P-450 (P-450) was strikingly enhanced by chronic ethanol treatment. Hepatic microsomal contents of cytochrome b5 (b5) and heme were also increased to the lesser extent than that of P-450 in ethanol-fed animals. Following chronic ingestion of ethanol, hepatic mitochondrial δ-aminolevulinic acid (ALA) synthase activity was remarkably increased and microsomal heme oxygenase activity was slightly but significantly increased when it was expressed as g body weight. These data suggest that 1) synthesis of apo-P450 is augumented by chronic ethanol intake 2) due to increased incorporation of heme into P-450, heme depletes 3) depletion of heme induces ALA synthase by the negative feedback mechanism 4) in response to increased ALA synthase activity, heme may be produced in excess 5) to regulate the intrahepatocytic levels of heme, heme oxygenase activity may be inductively increased.

  • Comparison between the spontaneous necrosis and the necrosis caused by transcatheter arterial embolization

    Histopathological changes in the spontaneous necrosis of hepatocellular cacinoma (HCC) and the necrosis of that treated with transcatheter arterial embolization (TAE) was studied in 24 HCC cases without TAE and 40 HCC cases with TAE.
    It was mainly observed that there were small central necrosis in nodules or thick trabecular cords of HCC without TAE. These necrosis were classified two types by histological findings. In one type, coagulatively necrotic tumor cells were losing their cohaesiveness and mixed with necrobiotic tumor cells. This type was dominant necrosis in a vast majority of cases without TAE. The other type was almost identical with that of TAE.
    Histological periodical changes of the necrotic area after TAE were divided into three stages; namely progression period, organization period and scarring period. The necrotic areas after organization period were extensive, monotonous and coagulative in nature, and more massive hematoidin deposition was recognized in them of cases with TAE than without TAE.
    It was assumed that these pathological differences between these necrotic foci were based no the grade and/or the speed of developed ischemia in them.
    A quite number of functional small arteries were observed around the viable satellite HCC nodules in cases with TAE. It is conceivable that arterial blood supply to them might be maintained from intrahepatic shunt and/or collateral feeding arteries.

  • Relationship between gross type and recurrence in resected HCC smaller than 3cm in diameter

    In order to clarify the relationship between the recurrence rate and gross type of resected hepatocellular carcinoma (HCC), 59 surgically resected HCCs smaller than 3cm in diameter at Kurume University Hospital during the past five years were studied grossly and histologically. According to the new gross classification proposed by the Japan Liver Cancer Study Group, the 59 cases were classified into 5 types; single nodular (39 cases), single nodular with extranodular tumor growth (8 cases), confluent multinodular (5 cases), multinodular (6 cases) and massive (0 cases). The recurrence rate of single nodular type was lower than that of other types, but there was no statistical significance. The incidence of tumor cell invasion into the portal vein branches and/or lymphatics was 20.5% in single nodular HCCs and was significantly lower than that of other type HCCs. The recurrence rates of various sized HCCs were as follows; 14.3% in HCCs smaller than 1cm, 38.5% in HCCs ranged from 1.1 to 2.0cm, and 46.2% in HCCs ranged from 2.1 to 3.0cm. Thus, the recurrence rate run parallel with the tumor diameter. The recurrence rate was much higher in the HCCs without capsule or with extracapsular tumor growth and tumor cell invasion into the portal vein branches and/or lymphatics. The recurrence rate was not much different in HCCs with or without preoperative anticancer therapies including transcatheter arterial embolization.

    We investigated the expression of 1, 25-dihydroxyvitamin D3 receptor (VD-R) by radio-receptor assay in 7 human liver cancer cell lines including HepG2 and PLC/PRF/5, and Change liver cell (Chang). VD-R was detectd in all 7 liver cancer cell lines. These cancer cell lines, except for HepG2, had about 1/6-1/30 the number of binding sites compared with rat osteosarcoma cell line, 17/2Y/7. However, HepG2, which seems to be a highly-differentiated type, also had lower binding sites. In contrast, VD-R was not detected in Chang cells, which seem to be derived from normal adult liver. These results suggest the involvement of VD-R in hepatocarcinogenesis or differentiation of liver cancer cells.
    Moreover, VD at a concentration of 10-7M significantly suppressed the proliferation of liver cancer cell lines in which the VD-R was detected, suggesting its application to cancer therapy.

    Proliferating activities of syngeneic donor hepatocytes in the rat spleens were assessed by DNA-synthesizing S phase cells after pulse-labeling monoclonal antibody to BrdU. Furthermore, the influence of hepatotrophic factors on the intrasplenic hepatocytes were also investigated in the rat with partial hepatectomy or with portacaval shunt.
    In this studies, we confirmed that inoculated hepatocytes possessed the auto-regulated proliferating activities, and they were accelerated by the hepatectomy of the host liver but no noted change in the intrasplenic hepatocytes was observed in the rats with portacaval shunt.
    Our investigations may offer significant approaches to the evaluation of cytokinetic behaviors of donor hepatocytes and regenerating hepatocytes.

    A 35-year-old man with acute non-lymphoblastic leukemia, underwent bone marrow transplantation (BMT) and then developed the HBV carrier state. His HBV infection was thought to originate from multiple blood transfusions received in the course of BMT therapy.
    After BMT, instead of recovery of the lymphocyte count in the peripheral blood, the T4/T8 ratio remained low. Predonisolone was given for the prevention of graft versus host disease. During the tapering off of predonisolone treatment, liver function abnormalities were temporarily exacerbated, and then gradually improved. Finally HBsAg became undetectable. This suggests the possibility of the effectiveness of anti-viral therapy following development of the HBV carrier state, in a patient who underwent BMT and had decreased T cell funciton, if treatment is initiated as early as possible.

    A 22-year-old women with complaints of diarrhea and vomiting was admitted to our hospital. Her liver tumor was detected by ultrasonography (US) after 2 years' clinical observation. Liver function tests revealed no abnormality except slight elevation of γ-GTP value. US and CT scan demonstrated a dumbbel shaped huge tumor, dimension of 21cm, originated in the caudate lobe. It buldged into the left caudate lobe forming a extrahepatic tumor and into the right lobe. Angiography showed a hypervascular tumor with secondary Budd-Chiari syndrome. Hepatoccellular carcinoma (HCC) was suspected and extended right lobectomy was performed under hypothermal anesthesia. After intraoperative embolization of the feeding artery of the tumor, remarkable dilation of the retroperitoneal lymphatic channels were disappeared. Pathological examination revealed focal nodular hyperplasia (FNH) with a typical central stellate scar.
    Differentiation of FNH from HCC in preoperative imaging diagnosis is very difficult even though all kinds of imaging modalities are carried out. However, in patients with a hypervascular tumor and normal liver function tests, FNH should be considered and a series of scintigraphy is proposed.

    An adult case of Alagille's syndrome was reported. A 20-year-old woman was admitted to our hospital for the second time because of liver injury and hypercholesterolemia. The patient had histories of jaundice and heart murmur in the neonatal period and pruritus in infancy. When 10 years old, she was admitted to our hospital for the first time, and chronic inactive hepatitis, hypercholesterolemia, pulmonary stenosis, and the deformity of vertebrae were pointed out. At age of 20 years, she was readmitted and liver biopsy showed paucity of interlobular bile ducts. Besides the above-mentioned abnormal findings, characteristic face, posterior embryotoxon and diffuse narrowing of the extrahepatic biliary ducts were pointed out. Abdominal ultrasonography, computed tomography and liver scintigraphy demonstrated marked swelling in the medial segment of the left hepatic lobe. To our best knowledge, this deformity of the liver was the first report of this syndrome.

    A case of anomalous branching of the intrahepatic portal vein associated with anomalous position of the round ligament was reported.
    A 66-year-old woman was admitted to our hospital because of anorexia, vomiting, and pyrexia. She was diagnosed as acute cholecystitis. Then, a celiotomy was done. The gallbladder attached to the liver on both sides of the round ligament, that is anomalous position of the gallbladder, and the liver bed of the gallbladder was seen only on the left side of the gallbladder. Accessory liver in the gallbladder wall was also disclosed. Intraoperative portography and ultrasonography revealed anomalous branching of the intrahepatic portal vein associated with anomalous position of the round ligament.
    Anomalous branching of the intrahepatic portal vein associated with anomalous position of the round ligament is not widely recognized. Anomalous position of the gallbadder should be discussed whether anomalous branching of the intrahepatic portal vein with anomalous position of the round ligament is present or not.
