    A follow-up study over 178 anti-C100-3 positive blood donors, along with examination of clinicopathology in them, disclosed that approximately a half of them had persistently normal ALT levels for over 6 months. Approximately one third of these donors with normal ALT levels were positive for HCV-RNA, thus suggesting they were clinically asymptomatic HCV carriers. On the other hand, the majority of the blood donors with abnormal ALT levels had positive results for HCV-RNA, and with their liver histology they were considered to be chronic hepatitis C. Presence/absence of anti-HCV core in anti-C100-3 positive blood donors was highly consistent with presence/absence of HCV-RNA. Histological findings of any of the asymptomatic HCV carriers showed some evidence of chronic hepatitis. Therefore, it is assumed that there are few histologically normal healthy carriers of HCV existing in anti-C100-3 positive blood donors.

    We analized clinical background, serum transaminase, anti-HCV antibody (1st, 2nd ELISA, Ortho), Chiron new four-antigen recombinant immunoblot assay test (HCV RIBA II, Ortho) and HCV. RNA in 50 spouses who were married with anti-HCV antibody-positive patients with liver disease.
    Ten of 50 spouses (20%) were positive for both 1st and 2nd ELISA, 6 (12%) were positive for only 2nd ELISA, and 34 (68%) were both negative. Most of 1st ELISA-positive 10 cases were reactive of HCV RIBA II and positive for HCV-RNA with liver dysfunction. On the other hand, subclinical HCV infection (normal serum transaminase with positive reaction of c22-3) was observed in the only 2nd ELISA-positive cases. Mean age of the only 2nd ELISA-positive cases was 4.2 years younger than 1st ELISA-positive ones. Then, it was considered that the only 2nd ELISA-positive cases with HCV RIBA II-reactive might be compatible with asymptomatic HCV carrier state.
    These results suggested that the spouses of the anti-HCV antibody-positive patients with liverdisease were high risk group of HCV transmission.

    The effects of isosorbide dinitrate (ISDN) on hepatic and systemic hemodynamics were investigated in thioacetamide induced cirrhotic rats. ISDN reduced portal pressure resulting from decreasing of portocollateral resistance. ISDN showed a significant reduction of portal pressure at the infusion rate of 1μg/kg bw/min and no significant change in the reduction of portal pressure even after increasing of ISDN dose. At the rate of 1μg/kg bw/min, ISDN had no effect on liver blood flow and peripheral artery pressure. At the rate of 100μg/kg bw/min, ISDN decreased liver blood flow as the result of increasing collateral blood flow. ISDN showed different effects between control and cirrhotic rats. ISDN reduced portal pressure in cirrhotic.

    Imaging and clinical features in 6 cirrhotic patients complicated with small hepatocellular carcinoma with a large spontaneous splenorenal shunt were studied in comparison with control patients without such a collateral. The diameter of portal vein (PV) was smaller than that of splenic vein (SV) measuring on an angiogram and the diametric ratio (PV/SV) was significantly smaller than that in control group. This fact, the diameter of PV was smaller than that of SV, brings a clue to find out a splenorenal shunt on the first examination using ultrasonography. The endoscopic findings suggested that a large splenorenal shunt may prevent development of advanced esophageal varices. The increased risk of recurrence hepatic encephalopathy in the patients with a large splenorenal shunt was indicated by clinical findings, and the therapeutic procedure as transcatheter arterial embolization therapy may induce encephalopathy as well.

    We examined 26 untreated patients with unresectable hepatoma exhibited positivity for both plasma protein induced vitamin K absence or antagonist II (PIVKA-II) and serum alphafetoprotein (AFP), and studied the weekly course of these markers from the pre-TAE period to the third week of treatment. We correlated changes in these markers with tumor necrosis rate (TNR) determined on X-ray CT images. At a week after TAE, the TNR and the time course of PIVKA-II showed a significant correlation (γ=0.7) while the correlation (γ=0.2) between the time course of AFP and TNR was insignificant. At 2 and 3 weeks after TAE, both the time course of AFP and PIVKA-II showed significant correlations with the TNR. In 16 patients with TNR of not less than 90%, the mean of the actual half-life of PIVKA-II was 3.2 days, the shortest was 44 hours, and 75 percent of all AHLs of PIVKA-II clustered from 2 days to 4 days, while the mean and shortest AHLs of AFP were 6 days and 2.98 days, respectively, exhibiting a broader distribution.

    We examined about the hepatic venous lesion in thirteen autopsy livers after bone marrow transplantation. Two cases showed pathologically typical Veno-occlusive disease, and other four cases showed various degree of hepatic veno-occlusive lesion. Some of the cases showed NRH like lesion, sinusoidal dilatation, multiple central vein, abnormal neovascularization. These findings were secondary lesions following disturbance of intrahepatic blood flow. We often find these lesions following Portal venous or hepatic arterial disorder, such as Idiopathic portal hypertention (IPH). But we find such lesions in hepatic venous disorder. So we think these secondary lesions may occur when hepatic vascular or circulatory disorder is present.

    Primary biliary cirrhosis and autoimmune chronic active "lupoid" hepatitis were occurred in a 67-year-old woman and her 27-year-old daughter. The mother was asymptomatic but PBC was suspected by the increase of the serum alkaline phosphatase level. Positive anti-mitochondrial and anti-PDC antibody, and histological findings supported the diagnosis. Her daughter was suffered from acute hepatitis at 27 years old. Autoimmune chronic active "lupoid" hepatitis was diagnosed from the findings of hypergammaglobulinemia, positive anti-nuclear and anti-smooth muscle antibodies and good response to corticosteroid therapy. A common histocompatibility antigen was HLA-DR4.
    PBC and autoimmune chronic active hepatitis are both autoimmune liver disease in which liver damages occur by the immunological disorder. However, they have different target cells in immune response. Nevertheless, a similar genetic predisposition to immunologic abnormality is suggested in both PBC and autoimmune chronic active hepatitis in these cases.

    A case of sphenoid sinus metastasis from hepatocellular carcinoma, occurred in a 60-year-old femaIe, is reported in this paper with literature review. In this case, left femoral pain and subjective symptom of tongue deviation to the right side was her chief complaint on admission. An inveterate nasal bleeding started 4 months later, and fiberscopic examination and biopsy was carried out to find out that the bleeding was from the tumor occupying upper pharynx and from the pathological findings, the tumor was metastasis from the hepatocellular carcinoma.

    We reported a 25-year-old female case of focal nodular hyperplasia (FNH) with the elevated serum DUPAN2 level. Ultrasonography revealed a well-defined isoechoic mass which contained hyperechoic multiple small nodules and linear high-echoes suggesting central scarring and its radiating septa. CT study showed a hypodense mass on the precontrast scan and the hypodense stellate-shapped central scar changed hyperdense on contrast scan. Angiography showed hypervascular mass supplied by the left gastric artery. Laparoscopy was performed to differentiate from a gastric tumor. A yellow-reddish tumor protruded from the liver was visible and the tumor was not stained after ICG infusion. The resected tumor was 9×9×6 cm in size and the cut surface disclosed multiple nodules with stellateshaped central scar and radiating fibrous septa. Histology of the specimen showed the typical feature of FNH. The high serum DUPAN2 level decreased to normal after tumor resection.
