    58 patients with imported hepatitis were hospitalized at the Institute of Gastroenterology, Tokyo Women's Medical College during 1978-1982. Most patients (41 cases, 71.4%) had hepatitis A. B and Non-A, Non-B hepatitis accounted for 11 cases (19.6%) and 6 cases (8.9%) respectively. The age of the patients range from 10 to 60, mostly in their 20's (28 cases) and 30's (19 cases), and 5 were female. About 90% of them had stayed in South-East Asia or North Africa for some period shortly before or while the clinical symptoms presented. HBs Ag subtypes were adr (4 cases) and adw (6 cases), and had no ayr and ayw. 10 of hepatitis B patients had history of sexual contact with the foreign residents. Two of 58 patients were foreigners, one is Indian and another Chilean. Jaundice, fatigue, and anorexia were the most frequently initial symptoms. 9 of 41 hepatitis A patients prolonged for more than 90 days.

    In order to clarify the clinical significance of circulating immune complexes (CIC), the authors investigated the incidence of CIC and the relationship between the presence of CIC and disease activity in patients with liver diseases using Raji cell assay for detection of CIC. In addition, CIC containing HBsAg (HBsAg-CIC) and HBeAg (HBeAg-CIC) in sera of patients with HBV infection have been detected using PEG-trypsinisation assay which is regarded as antigen-specific method.
    CIC were detected in sera from 5 of 17 (29.4%) patients with acute type A hepatitis, 5 of 12 (41.7%) with acute type B hepatitis, 3 of 9 (33.3%) with non A, non B hepatitis, 7 of 21 (33.3%) with chronic active hepatitis, 1 of 5 (20.0%) with chronic inactive hepatitis, 9 of 32 (28.1%) with liver cirrhosis, 4 of 12 (33.3%) with hepatocellular carcinoma, 3 of 10 (30.0%) with alcoholic liver injury, 12 of 20 (60.0%) with primary biliary cirrhosis (PBC) and 3 of 28 (10.7%) asymptomatic HBsAg carriers. The incidence of CIC was highest in sera of patients with PBC as compared with the remaining liver diseases. Detection and quantitation for CIC using Raji cell assay and PEG-trypsinisation assay were simultaneously carried out in serial sera of patients with HBV infection and the incidence of CIC by these two methods was almost the same. HBeAg-CIC were transiently detected in sera of two patients with type B acute and chronic active hepatitis. Moreover, HBsAg-CIC were more frequently detectable in HBeAg-positive sera than in HBeAg-negative sera, thus suggesting the presence of correlation between HBsAg-CIC and disease activity.

    Many evidences support the necessity of the compatibility at major loci of the histocompatibility complex between effector and target cells, i.e. HLA restriction, in the killing mechanism of virus infected cells.
    In this paper, we examined the HBs antigen specific killer T cell activity in patients with various types of hepatitis B virus infection. In this new assay system autologous lymphocytes coupled with purified HBs antigen on their cell surfaces were prepared as target cells.
    This killer activity was detected in 6 of 8 patients with type B-acute hepatitis in convolescent stage (75.0%) and in 3 of 8 with type B-chronic active hepatitis (37.5%), but in none of all HBs antibody positive and negative healthy controls.
    In this assay system HLA restriction was demonstrated for the HBs antigen specific killer T cell activity, because the activity was detected only against target cells which share major loci of the histocompatibility complex with effector cells, and OKT 8 positive T cell subset proved to be responsible for the killer activity.

    The molecular polymorphism of IgA molecules was analyzed by high speed liquid chromatography, and changes in the proportion of dimeric and monomeric IgA were investigated with regard to sera from normal controls and patients with liver cirrhosis, and to the specific IgA-type antibodies against lactalbumin (L-Alb).
    The molecular form of IgA in sera from normal controls mainly consisted of monomeric IgA with an average proportion of 82%, while its proportion of dimeric IgA was 18%. On the other hand, an increased proportion of dimeric IgA was observed in sera from patients with liver cirrhosis with an average of 32%. The IgA-type antibodies against L-Alb in sera from patients with liver cirrhosis showed 55% of the dimeric form, whose proportion was found to be significantly increased compared to that of serum IgA.
    From these results, the molecular polymorphism of IgA in sera appears to, partially, depend on the production of IgA in the gut-associated lymphoid tissues, and, at the same time, on the catabolism of IgA in the liver.

    A sensitive and spesific radioimmunoassay (RIA) was developed for the carboxyterminal extension peptide (C-peptide) of human type I procollagen.
    For this assay, antibody was prepared by immunizing rabbit with purified type I procollagen from culture medium of human lung fibloblasts, and C-peptide was isolated from the preparation of type I procollagen digest with bacterial collagenase.
    Norrnal human serum was found to contain 44±20ng/ml of C-peptide. Whereas, abnormal elevation of serum C-peptide level (cut off level, 84ng/m/) were observed in 58% of 45 patients with alcoholic live disease, in 35% of 17 patients with chronic hepatitis type B, in 40% of 48 patients with non-B chronic hepatitis, in 67% of 12 patients with liver cirrhosis, in 85% of 13 patients with Schistosomiasis Mansoni, but none of 8 patients with acute hepatitis.
    These results suggest that measurement for serum concentration of C-peptide of type I procollagen provide a new means for the diagnosis of hepatic fibrosis.

    Hepatic contents of hydroxyproline (Hyp) and hydroxylysine (Hyl) were assayed in patients with and without liver diseases. Hepatic contents of Hyp and Hyl in normal liver were 4.2±0.5 and 0.4±0.1μmol/g liver, respectively. In patients with liver diseases, hepatic contents of Hyp and Hyl were increased proportionally with the progress of hepatic fibrosis. Percentage of the interstitial collagen calculated from Hyl/Hyp ratio was about 81% in normal liver and was not so changed in fibrotic liver. These results suggest that there may be almost no differences in the proportion of the interstitial and basement membrane collagen between normal and fibrotic liver.

    In order to elucidate the effect of chronic ethanol administration on the cytoskeleton of rat hepatocyte, we examined electron microscopically rat liver perfused with 0.5% Triton X-100 from portal vein for 90 min. No significant morphological changes were noted in the intermediate filaments in the rat fed by ethanol for 3 months. However, morphometric analysis showed that microtubules significantly (p<0.001) decreased in amount in the ethanol fed rat compared with the control.

    Triglyceride (TG) & nonesterified fatty acid (NEFA) content in liver and serum, and serum enzymes originating from liver were measured in two different hypothalamic obese rat groups.
    The hypothalamic obese rats were prepared according to Hetherington method (electrolytic lesion) and Scrafani method (knife cut lesion). 4 weeks after the operation, the obesity grade was the same in the two groups. However, some differences between them were observed. Electrolytic lesion rats got more fat (TG) accumulation in the liver, and enzymes (transaminases, lactic dehydrogenase, alkaline phosphatase, r-glutamyl transpeptidase) were elevated in the sera. However, the TG & NEFA elevation in the serum was not so high. The knife cut lesion rats showed almost no liver function abnormalities, and had only modest fat deposition in the liver. But, the TG & NEFA elevation in serum was very high.
    The fatty liver of the latter model was similar to that in human simple obesity. However, liver histology of these obese rats showed periportal fat deposition. This finding was different from that in human obesity.
    Those hypothalamic obese rats can be used for the study of fatty liver formation.

    Liver biopsy was performed under laparoscopic observation of our 24 cases of sarcoidosis. Noncaseating epithelioid cell granulomas were histologically demonstrated in the liver biopsy specimens in 20 cases (83%). Ten cases were examined by electron microscope. Ultrastructurally, epithelioid cell granulomas consisted of four cell type: epithelioid cells (large and small mononuclear epithelioid cells and multinuclear giant cells), histiocyte-like cells, lymphocytes and fibroblasts. Epithelioid cells seemed to be highly active cells with abundant cytoplasmic organelles: large amounts of Golgi apparatus, dense vacuoles of varing sizes and numerous mitochondria. It was peculiar not to find lysosomes in the cytoplam. Elongated cytoplasmic processes had developed an interdigitating cytoplasmic processes. Multinuclear giant cells were assumed to be formed by the fusion of large mononuclear epithelioid cells. Because of the presence of lysosomes in the cytoplasm, histiocyte-like cells were thought to be phagocytized type of highly active cells. Intermediate cells between histiocyte-like cells and epithelioid cells were not ultrastructurally evident in epithelioid cell granulomas in the liver.

    We developed a new, sensitive and reliable radioimmunoassay for AFP using monoclonal antibodies. The method is based on a solid phase sandwich method using radioiodinated antibody and antibody-coated filter paper disc.
    AFP of serum samples from 20 healthy adults and 159 patients with various diseases were measured by the radioimmunoassay. AFP values of healthy subjects and several benign diseases were 1.43±2×0.83IU/ml (mean±2 S.D.). Elevated values were detected in hepatoma, cirrhosis and hepatitis, gastric cancer, colorectal cancer, biliary tract cancer in much higher frequencies than those obtained with previous RIA methods.

    Histological changes of seven cases of primary liver cancer after transcatheter arterial embolization (TAE) were studied. Comparison was done between early (Within one month after TAE) and late (more than three months) changes. In early phase infiltration of neutrophils, proliferation of immature connective tissue, cholestasis and various degrees of degeneration and necrosis of tumor cells were noted. In late phase complete necrosis and dense fibrous tissue with little cell infiltration were dominant. Injected gelatin sponge (Spongel) were noted even within small tumor vessels and necrotic area. Fragments of Spongel were either accompanied with fibrin fibers in one case, or not in another case.

    A 21-year-old female patient with type A fulminant hepatitis cured with plasma exchange (PE) is reported. The patient was admitted to our hospital because of hepatic coma (Adams & Foley grade III), oliguria and subcutaneous bleeding in April 1983. Her total bilirubin was 10.lmg/dl, GOT 1, 005IU, GPT 2, 075IU, blood NH3 110μg/dl, total protein 5.9g/dl, BUN 44mg/dl, creatinine 6.4mg/dl, and IgM type anti-HA was demonstrated by HAVAB-M kit. Treatment included prednisolone, glucagon-insulin, heparin, l-DOPA and lactulose. In addition, PE was started on the lst hospital day, and three PEs resulted in her waking from hepatic coma and in disappearance of DIC and renal failure.
    A case of type A fulminant hepatitis is rare and five cases in Japan are reviewed.

    A very rare case of acute hepatitis type B, complicated with severe hemolytic anemia, was reported.
    A 33-year old male was admitted with right hypochondralgia.
    Laboratory studies disclosed markedly elevated serum transaminase, positive HBsAg and IgM anti HBc, low titer in anti HBc, so we diagnosed acute hepatitis type B.
    He had a good clinical course with bed rest, but from 17th hospital day he developed high fever, splenomegaly and severe anemia. Laboratory examinations disclosed marked elevation of reticulocyte and LDH and marked depression of haptoglobin. The picture of bone marrow showed hyperplasia of erythrocyte series. We diagnosed hemolytic anemia and administered high dose predonisolone with marked effect.
    Liver function improved gradually, and HBsAg became negative.
    Laboratory examinations disclosed positive anti HBs and anti HBe at about four months after discharge.
    Some mechanisms of hemolysis were discussed.

    The patient was a 50-year-old male with a long history of chronic alcoholism and a blood transfusion at the age of 30 years. He received a daily oral dose of 70mg of cyanamide for about 3 years until 5 months before admission. He was admitted to the hospital because of persistent fatigability. There were no signs of neurological disease. Biochemical liver function tests showed moderate liver injuly. Serum HBs antigen, HBs antibody and HBc antibody were negative by radioimmunoassay. Laparoscopy performed on the 5th day of the admission revealed a mottled liver. A needle liver biopsy specimen had numerous ground-glass inclusions in hepatocytes predominantly of periportal areas. The inclusions were negative by orcein and immunoperoxidase stainning for HBs antigen and HBc antigen. Ultrastructurally, the inclusions mainly consisted of hyperplastic smooth endoplasmic reticulum with irregular figures and cisterna dilation, glycogen, and lysosomes.

    A 67-year old male with pedunculated hepatocellular carcinoma had been surveyed for 4 years and two months prior to surgery because of his refusal of operation. Fairly rapid enlargement of the tumor was noticed during the last 8 months before operation and AFP sharply increased to 190, 000ng/ml from initial 26, 500ng/ml. He was successfully treated only by partial hepatic resection, and neither distant metastasis nor direct invasion to surrounding structures except greater omentum was also found. Examination of resected specimen revealed that the tumor was constituted by a hen's egg size portion and an irregularly enlarged portion through the destructed capsule of the former to extrahepatic direction. Histological examination disclosed this hepatoma was classified as Edmondson's grade III. He has been doing well for two years since surgery and AFP is less than 0.5ng/ml so far.
