    The IgA antibody to hepatitis B core antigen (IgA anti-HBc) was examined on 93 sera from 68 patients with hepatitis B virus (HBV) infection. The high titer of IgA anti-HBc was frequently present in the patients except healthy HBs antigen carrier and chronic inactive hepatitis, and the titer in the patients with HBe antigen carrier was significantly higher than in the antigen-free patients. The pattern of IgA anti-HBc changes was similar to that of IgM anti-HBc in the acute phase of HBV infection. In chronic infection, however, this phenomenon was not observed. In addition, the IgA anti-HBc of acute HBV infection was proved to be a polymeric IgA, while that of chronic infection was a monomeric IgA,
    From these results, the occurrence of IgA anti-HBc is assumed to reflect on the active liver disease, and the molecular form of IgA anti-HBc is a specific indicator for the differentiation of acute HBV infection from chronic HBV infection.

    Hepatitis B surface antigen (HBsAg) has been proven to possess antigen-specific and nonspecific inhibitory effects to the host-immune system. So we attempt to reduce HBsAg concentration in the serum of the patients with chronic active hepatitis type B to make immunotherapy in hepatitis B virus (HBV) infection more effective. In this paper we reported to removal efficiency of HBsAg in double filtration plasmapheresis (DFP) with Evaflux 4A which showed the selective and stable separation of HBsAg (rejection rate 97%) and DNA-P (almost 100%) compared with albumin (29%) as a secondary filter in simulation model. In the simulation study, HBsAg concentration of pooled plasma was reduced exponentially to 20-30% in 90 min operation. Furthermore with the application of dilution method, 20% increase of HBsAg reduction efficiency or 20% decrease of operation time was achieved. So DFP in order to reduce HBsAg concentration in the blood could be applied for clinical use as a supportive method for immunotherapy of chronic hepatitis B infection.

    To clarify the immune mechanisms responsible for the induction of experimental autoimmune hepatitis in C57BL/6 (B6) mice, we established a syngeneic assay system for cell-mediated cytotoxicity in which 51Cr-labelled isolated hepatocytes from normal B6 mouse and sensitized spleen cells from the hepatitis B6 mice were used as target cells and effector cells respectively. In this assay, the sensitized spleen cells from hepatitis mice showed significantly higher cytotoxic activities against target cells than those from control animals. These findings suggest that the sensitized spleen cells play an important role in the induction of autoimmune hepatitis in B6 mice. This assay system is very useful because the H-2 restriction can be disregarded in the study of cell-mediated cytotoxicity.

    Spleen cells (SPC) of F1 mice were activated through proliferation and differentiation during liver regeneration after 70% partial hepatectomy. In order to study the alteration of self-reactivity in these activated SPC, the inductive activity of anti-parental autoreactive cytotoxic T lymphocytes (autoreactive CTL), due to the F1 anti-parent mixed lymphocytes reaction (MLR), was measured.
    The inductive activity of anti-parental autoreactive CTL decreased temporarily and reached the lowest level at fourth day after hepatectomy. Such decrease of the activity was mostly caused by splenic adherent cells (SAC) and inhibited by the treatment of indomethacin.
    When untreated parental SPC were injected into F1 mouse at fourth day after operation, the inductive activity of autoreactive CTL was greatly suppressed for at least two to four weeks. This suppression was not recovered by the addition of indomethacin. F1 SPC in this tolerant condition generated CTL against third party cells to almost the normal level.

    379 patients with HCC were studied for assessing the clinical characteristics in the aged (over 70yr.) patients with HCC. All patients were divided into 3 groups by age for the comparison 1)≤59, 2) 60-69 and 3) 70yr. ≤namely.
    The HCC was less frequently associated with liver cirrhosis in the aged group. The incidence of positive HBSAg and HBCAb was lowered with aging. There was no correlation between the doubling time of volume estimated by echograms and the age of patients with HCC. The prognosis of the aged was not different from that of other group in the chemotherapy, although bone marrow suppression was more common in the aged group. The causes of death more often included rupture or termination of HCC which were different from other groups.
    Consequently our retrospective studies concluded that the aged patients had different clinical characteristics from younger group and should be positively treated properly for the aged.

    The author investigated the structure of tumor capsule of so-called encapsulated hepatocellular carcinoma (HCC) morphologically and microangiographically. Materials studied were 54 cases (39 autopsy and 15 surgical cases) of HCC in all of which fibrous capsule formation around HCC was noticed macroscopically. Histologically, the author classified the tumor capsule into two categories, namely truly encapsulated HCC and pseudo-encapsulated HCC. The former was surrounded by hyalinized fibrous tissue almost completely and the latter had no or only localized hyalinized fibrous tissue around HCC. Capsular structure in the truly encapsulated HCC associated with liver cirrhosis was consisted of three layers, and two layers were determined in cases without liver cirrhosis. The thickness of the inner-most hyalinized fibrous tissue (1st layer) was varied considerably from a portion and a portion even in the same tumor nodule. No remarkable relation between the thickness of the hyalinized fibrous capsule and the size and the histological grade of malignancy of HCC was determined. It was confirmed that the zonal structure of the tumor capsule in truly encapsulated HCC was fairly well Corresponded to the clinical evidence with diagnostic imaging-procedures. From the results obtained, the author wants to propose that the category of the encapsulated HCC should be limited to the truly encapsulated HCC.

    Operative results of tumor enucleation method as minimized surgical treatment for small liver cancer patient with liver cirrhosis is reviewed.
    Over 3 years survivor counts 7 cases, and 5 is still alive, with a 4 years 6 months of longest survivor, and cummurated 4 years survival rate is 52%.
    Postoperative recurrence is seen in 9 patients. In 6 cases of this group, recuned tumor appeared in another segment from the site which had contained the primary tumor, while in 3 cases of it, recurrence was seen in the area clore to the cutting sunface.
    His to pathological examination of there 3 marginally recuned cases revealed the nature of tumor as in complate capsel formation or extra capurale invasion which suggests as the retaining cancer cells in surrounding unresected tissue.
    This fact strongly proclaims us that operative indication should he more serious to improve the radicality of whole tumor.
    We consider that the method of tumor enucleation should her performed only in tiny lesion less than 3cm in cliameter and has obvious capsel without extra capsurale invasion or any intra hepatic metastasis.
    For accomplischment to this purpose, real time US monitoring especially intra operative ultra sonography is very useful.

    We studied the diagnostic capacity of the real-time two-dimensional Doppler echography (2-D-Doppler) in the patients of hepatoma. Four cases with interesting findings in this method are presented.
    Some specific diagnostic capacity of this method revealed are as follows.
    1) Identification and follow of any vessel became easier by colour display of the blood stream.
    2) Direction of the blood flow were easily recognized by the difference of colour. Brightness corresponded proportionally with the flow velocity, so the change of velocity was recognized without difficulty.
    3) 2-D-Doppler was also useful for recognition of both the changes of arterial blood stream after transcatheter arterial embolization (TAE) and the arterio-portal shunt.
    4) Colour display of portal flow was sometimes difficult in liver cirrhosis in which velocity was small. This technique revealed us the new information, especially, about the blood stream in the patient of hepatoma.

    A 62-year-old woman with hepatocellular carcinoma (HCC) who had survived for 10 years and 8 months was reported. She was admitted to our hospital because of increased serum alpha-fetoprotein on October, 1973. Serum HBsAg and HBeAg were both positive. On December, 1973, the tumor was successfully resected from the right posterior superior segment of the liver, and its size was 3.5cm in diameter. Histological examination of the surgical specimen showed HCC with posthepatitic liver cirrhosis. Approximately 7 years later, second operation for the recurrence of HCC was performed on November, 1980. Third operation for the recurrence was performed on September, 1981. The tumor size was 2.0cm and 1.0cm in diameter, respectively. On May, 1983, transarterial embolization for the recurrence was performed and she still survive. We suspected that the development of HCC in this case resulted from multicentric occurrence of the tumor.

    A male case of primary biliary cirrhosis (PBC) with chronic thyroiditis and chronic pancreatitis is reported.
    A 66-year-old man was admitted, because of general fatigue and hepatomegaly.
    Laboratory data showed high level of serum Al-P (28.5 K-Au) and slight liver dysfunction. Serum anti-mitochondrial antibody and anti-thyroidal antibodies were positive. Rarely-identified precipitating antibodies to mitochondrial antigens such as M-C and M-D were detected by double immunodiffusion method, and especially the latter has been first discovered in Japan in this case. Biopsy of thyroid gland showed chronic thyroiditis (lymphocytic thyroiditis) and liver wedge biopsy proved typical chronic non-suppurative destructive cholangitis. Pancreas function was moderately disturbed and O-GTT showed diabetic pattern.
    Sicca syndrome appeared later. Thus, we finally diagnosed this case as PBC with chronic thyroiditis, chronic pancreatitis and sicca syndrome.
    We presented a male case of PBC with multisystem involvement and unusual mitochondrial antibodies (M-C and M-D system) in this report.

    We reported the pathological findings of AH detected in three operated cases. The results were as follows:
    Macroscopically, AH is nodular lesion with no capsule and little variation in color. Liver cirrhosis was basic lesion in these cases.
    Portal tracts were identified in the peripheral area as well as in the center of the nodules. The nodule consisted of cells devoid atypia, but had occasional foci of hypercellularity. Characteristic cell arrangement such as trabecular pattern and pseudoglandular pattern usually seen is hepatocellular carcinoma was not recognized.
    The nodules were demarcated by fibrous stroma of liver cirrhosis, but had areas indicating expanding growth.

    A 53-year-old male patient with sclerosing hepatocellular carcinoma is reported. We encountered a case of multiple liver tumors. Computed tomography (CT) revealed multiple low density areas in the liver. Peripheral portion of these areas showed definite contrast-enhancement on CT. Under laparoscopy, the tumor was revealed to be white and have irregular surface with umbilication, and peritoneal dissemination of the tumor was found. Although these findings suggest the diagnosis of metastatic liver tumors, the needle biopsy from one of the tumors showed the presence of hepatocellular carcinoma. Dense fibrosis was arranged around the rather eosinophilic polygonal neoplastic hepatocytes. This distinct morphologic appearance is identical to that of sclerosing hepatocellular carcinoma which was previously defined by Peters. The hepatic tumor invaded the duodenum, and he died of respiratory failure due to pleural metastasis.

    A 55-year-old man with chronic abuse of alcohol was admitted to our hospital because of abnormal liver scintigram. A selective hepatic angiogram disclosed a large hypervascular tumor and multiple daughter nodules in the right lobe. Because the tumor was considered unresectable because of liver cirrhosis gelfoam embolization of the right hepatic artery was performed. The 26 days after embolization revealed no tumor vascularity and AFP value decreased from 1006ng/ml to 26ng/ml on the 46th postoperative day. Four times of embolization produced right hepatic arterial obstruction, with resulting development of collateral pathway such as left hepatic artery and right inferior phrenic artery. Especially embolization of right inferior phrenic artery was so effective that AFP value dropped from 78800ng/ml to 17.1ng/ml. But the subsequent angiogram demonstrated tumor invasion to the right adrenal gland and right kidney. Patient died to cachexy 5 years and 8 months after the first embolization. Histological finding revealed well differentiated hepatocellular carcinoma, Edmondson's grade II.

    We reported here the case of a 57 year-old male patient with liver cirrhosis but whose portal vascular resistance was not increased. The portal venous pressure was 21mmHg, the free hepatic venous pressure was 17mmHg and the inferior vena cava pressure was 19mmHg. Owing to the presence of Budd-Chiari syndrome accompanied by hepatic cell carcinoma, the venous pressures were elevated. Portal venous blood flow was calculated to be 633ml/min using ultrasonic duplex system and the portal vascular resistance was revealed to be low. Biopsy at the operation, disclosed a cirrhotic liver and dilatation of hepatic and portal veins. In liver cirrhosis, elevation of portal vascular resistance is due to narrowing and closing of the intrahepatic vascular system, especially the hepatic vein. In this case, we supposed that because of elevation of the venous pressure, narrowing and closing of the hepatic vein were not encountered, or that because of the elevation, the narrowing and closing had disappeared and, therefore, portal vascular resistance had also decreased.

    A case of lupoid hepatitis complicated with chronic thyroiditis (Hashimoto's thyroiditis) was reported in a 50-year-old man who was admitted with a chief complaint of lassitude and had diffuse enlargement of the thyroid gland and hepatomegaly. Laboratory findings were as follows; s-GOT 43; s-GPT 59; T. Chol 107mg/dl; Ch. E 0.2 ΔpH; s-Alb 3.3g/dl; γ-gl 3.1g/dl. Immunological tests were positive for rheumatoid factor, LE cell phenomenon and antinuclear antibody, with a high titer of antithyroglobulin antibodies. The liver biopsy specimen showed evidence of chronic active hepatitis with formation of lymphoid follicles and a tendency to lobular reorganization. The thyroid biopsy specimen showed microscopic features characteristic of Hashimoto's thyroiditis. The case presented clinical features of what has been called the hepato-thyroiditis syndrome; some autoimmune mechanisms might underlie the pathogenesis of the liver disorder as well as that of the thyroiditis. The patient being a heavy alcohol drinker had chronically lowered serum T. cholesterol, s-Alb and Ch. E levels and the disease continued to progress despite corticosteroid therapy. Serial liver biopsy examinations consistently showed histologic findings indicative of alcoholic liver injury. Therefore, it appeared likely that his chronic alcoholic ingestion served as a direct or indirect extrinsic triggering and loading factor.

    A direct attack on the vulnerable area of the oesophagus whether by injection sclerotherapy or one of the transection-devascularization operations is obviously less effective than a shunt procedure in preventing recurrent variceal haemorrhage. However these direct procedures do not precipitate encephalopathy or cause late deterioration of liver function and therefore allow good quality of survival. Whether chronic injection sclerotherapy or oesophageal transection will give the best longterm results in patients considered unsuitable for shunt surgery is as yet an unanswered question
