    The changes of activities of α-L-Iduronidase and other various glycosidases were examined in the course of rat liver fibrosis induced with carbon tetrachloride.
    4 weeks after initiation of carbon tetrachloride treatment, the hepatic fibrosis was not observed but the activity of α-L-Iduronidase was increased to 94% above controls. All of other glycosidase activities examined in the present study exhibited also an increasing tendency.
    In 6, 8 or 12 weeks of treatment by carbon tetrachloride, α-L-Iduronidase activity showed the further increase with the development of fibrosis. However, other glycosidase activities were not increased and some showed the slight decreasing tendency as compared with the group treated by carbon tetrachloride for 4 weeks but were higher than the normal values.

    When peripheral blood lymphocytes from patients with chronic active hepatitis were stimulated with liver specific lipoprotein, the lymphocyte transformation and MAF production were induced in several cases. The peritoneal exudate macrophages from guinea pigs were activated by MAF-containing lymphocyte culture supernatant and cytotoxic factor acting on the isolated hepatocytes were produced in culture fluid. By a gel filtration using Sephadex G-75 column, the cytotoxic factor was fractionated as a definite component and this fraction was separated into two active fractions by a DEAE-cellulose column chromatography. These fractionated active materials exhibiting the cytotoxic effect on the isolated hepatocytes were shown to enhance the lipid peroxide formation in the isolated hepatocytes.
    These results suggest that the activated macrophages produce the factors which exert their cytotoxic effects on the isolated hepatocytes and lipid peroxide formation may play some role in occurrence of hepatocyte injuries.

    The early clinical and prognostic differentiation of hepatic encephalopathy were studied in 34 patients who developed into grade I or II encephalopathy within ten days from the onset of acute hepatitis. The patients were divided into three groups: group A (12 cases), recovered from grade I or II encephalopathy; group B-1 (4 cases), recovered, but progressed into grade III or IV encephalopathy; group B-2 (18 cases) progressed into grade IV encephalopathy and died.
    In group A, the prominent neurological symptoms included somnolence and disorder in sleep rhythm. All patients had abnormal EEG findings. The development of hepatic encephalopathy from the onset of acute hepatitis seemed to be delayed in group A, compared to other two groups.
    NT, PT and TT were most valuable in differentiating the subsequent clinical course and prognosis of these three groups before developing grade III encephalopathy.
    Our results lead us to suggest that group A is a newly described entity which is different from so-called acute hepatitis and fulminant hepatitis. It is emphasized that the coagulation studies be repeated to predict the subsequent clinical course and prognosis at the early stage of acute hepatitis associated with encephalopathy.

    On the basic assessment of a combination of glucagon and insulin which have promotive effect of liver regeneration, we have applied clinically glucagon-insulin therapy in acute liver failure. Many cases have been relieved and recovered from hepatic coma.
    On the other hand, portal infusion of these hormones in liver failure is theoretically as more effectively, directly positive action. In this paper we aimed to examine cyclic nucleotide in liver tissures and sera of d-galactosamine induced severe hepatitis bearing rats during the portal infusion of these hormones. As the result of this basic studies, we revealed that the administered group via portal vein responsed rapidly to these hormones and higher concentrations of cyclic AMP than the group of intraperitoneal administration.

    We studied the biochemical and immunological aspects of 152 asymptomatic HBsAg carriers. By radioimmunoassay HBeAg was detected in 31 cases (20.4%), HBeAb in 107 cases (70.4%) and neither HBeAg nor HBeAb in 14 cases (9.2%). The time of occurrence of seroconversion from HBeAg to HBeAb in ASC varied with different individuals. Although routine liver function tests were within normal ranges, BSP retention was abnormal in 14 out of 17 HBeAg-positive cases and in 43 out of 66 HBeAb-positive cases. The increase of abnormal BSP retention with aging suggested the insidious development of mild liver disease even after seroconversion to HBeAb. Serum ANF was positive in 9 out of 23 HBeAg-positive cases and 11 out 23 HBeAb-positive cases. This may be associated with an altered antigenicity of the nucleus of hepatocytes with active HBV replication. The majority of asymptomatic HBsAg carriers showed no general immunological abnormalities. However, some of them had slightly increased IgG, decreased complement concentrations and diminished lymphocyte response to PHA, suggesting the possibility that slight immunological response to HBV might persist. These findings emphasize the importance of long-term follow-up surveillance of asymptomatic HBsAg carriers.

    In 16 (10.2%) out of 157 cases with persistent HBs-Ag carriers, antibody directed to a different subtypic determinant of HBs-Ag was detected. These results showed the possibility that even persistent HBs-Ag carriers had been infected with the other subtype of hepatitis B virus and raised antibody againist heterologous subtypic specificity.

    To study the modes of the transition of HBeAg and anti-HBe and their prognostic significance in HBsAg positive chronic hepatitis, serial specimens of 9 HBsAg positive patients with chronic hepatitis and 1 patient of acute hepatitis B were examined for HBeAg and antiHBe by radioimmunoassay. The modes of the transition of HBeAg and anti-HBe in chronic hepatitis are classified into two large groups and five small types; HBeAg group-"stationary" type, "fluctuating" type, and "alternating" type. And anti-HBe group-"seroconversion" type, and "stationary" type. In the HBeAg group, elevated transaminase was noted in the course. Especially in the HBeAg fluctuating and alternating types, recurrent episodes of high elevated serum transaminase were noted in the course of the disease. And in the HBeAg stationary type, serum transaminase was normal or slightly elevated. On the other hand, in the anti-HBe group, serum transaminase was within normal value in the course. In this study, it is concluded that the fluctuation of HBeAg titer seem to be related with that of the serum transaminase activity, and positive anti-HBe corresponds to stable serum transaminase.

    Forty-one medical staffs accidentally exposed to HBs-antigen positive blood were given anti-HBs human immunoglobulin (HBIG) intramuscularly within 48 hours after exposure. They were followed with tests for HBs antigen, anti-HBs and transaminases in sera at intervals of two weeks for 6 months. Anti-HBc was determined on the sera obtained just before the HBIG injection and the final sera of the follow-up. All the staffs except for one had neither HBs antigen, anti-HBc detected nor transaminases increased in sera during the follow-up periods. Anti-HBs were raised up to 1: 16 to 1: 64 with PHA titer two weeks after the injection of HBIG and disappeared two to three months later.
    One of the staffs, a 26 year-old nurse treated with HBIG, acquired acute type B hepatitis 19 weeks after exposure to HBe antigen positive blood. The HBs antigen subtype was identical with that of the source of infection. A similar case was subsequently experienced in a 32year-old female doctor belonging to another hospital. She was exposed to HBe antigen positive blood, and attacked with acute type B hepatitis 30 weeks after exposure in spite of receiving a HBIG injection. These two cases indicate that HBIG might not completely protect the occurrence of acute hepatitis after accidental exposure to HBe positive blood, and that follow-up after the HBIG injection should be continued at least for 9 months, since the incubation time of the acute type B hepatitis would be prolonged.

    Serum γ-glutamyl transpeptidase, blood estrone, estradiol, testosterone and urine estrogen were measured in 57 male patients with alcoholic liver cirrhosis.
    The mean level of serum γ-GTP activity was significantly lower in 14 patients with gynecomastia than in 43 patients without gynecomastia. In gynecomastia group, ratio of blood estradiol to testosterone (E/T) was relatively high compared to no gynecomastia group. There was slightly correlation of serum γ-GTP activity and blood testosterone (r=0.5966). There was no significant correlation of serum γ-GTP activity and E/T ratio, but a case of high E/T ratio slightly tended to show low serum γ-GTP activity.
    From these findings, it was cosidered that poor respoder of serum γ-GTP activity in patients with alcoholic liver cirrhosis was concerned with gynecomastia.

    In order to elucidate the morphological characteristics in Idiopathic portal hypertention (IPH), histological observations were carried out in autopsy and biopsy materials of IPH, and a comparison between autopsy cases of Noncirrhotic portal fibrosis (NCPF) in India and those of IPH in Japan was also done.
    The livers were atrophied with an irregular distribution of the lesions grossly. Characteristic features of the cut surface of the liver were as follows; irregular thickening of the large portal veins with thrombus, approximation of the medium-sized portal tracts to the liver surface, narrowing of the portal lumen and irregular fibrosis in parenchyma without apparent nodule formation. As to the histological features of IPH, sclerosing and narrowing of the portal vein were characteristic, and especially an occlusion of the peripheral portal vein, which is considered to play an important role in pathogenesis of portal hypertention, was frequently observed. Although a little difference between IPH and NCPF, such as nodule formation localizing in the subcapsular area of the liver, exists in some case of NCPF, there were many pathomorphological similarities between them. Thus, it would be suggested that both IPH and NCPF could be categolized in the same pathological entities.

    In order to clarify the cell kinetics of hepatocytes in liver cirrhosis and hepatoma, both the DNA synthesis by means of tritiated thymidine radioautography and the DNA content of hepatocytic nuclei by means of Feulgen-microspectrophotometry in cirrhosis and noncancerous portion of hepatoma with cirrhosis were studied.
    The incorporation of tritiated thymidine to hepatocytes in noncancerous portion of hepatoma as well as in cirrhosis, developed hepatoma 3 years later, increased markedly. The distribution of ploidy pattern as measured by microspectrophotometry in noncancerous portion of hepatoma was widely spread. Moreover, each paired nuclear DNA content of a binucleate cell had almost the same amount in normal liver in contrast to that the pair in noncancerous portion of hepatoma was as much different as in hepatoma cells. The ploidy pattern of hepatocytes in patients with liver cirrhosis, who had developed hepatoma during follow-up periods of several months to several years appears to resemble to that in noncancerous portion of hepatoma.
    These results suggest the possibility that the hepatocytes themselves in noncancerous portion of hepatoma may be in a deranged state of cell cycles and this might be one of the driving factors for developing a malignancy.

    The liver function and the histology of biopsy-liver of 87 patients with collagen diseases (SLE 23, R A 26, PM-DM 17, PSS 10, Sjö-s 7, and Aort-s 4) were examined.
    The obtained results were as follows: hepatomegaly in 17 cases (19.5%), jaundice in 8 (9.1%), HBs-Ag positive in 2 (2.8%), HBs-Ab positive in 18 (20.7%), abnormal Al-p levels in 15 (17.2%) and abnormal ICG levels in 16 (18.4%). Abnormal transaminase levels were found in 61 cases (70.1%). Among them, 36 cases had GOT values over 100Ku and 16 cases had GPT values over 100Ku. These abnormalities were found more frequently in patients with SLE and PM-DM.
    Histological findings of the 40 biopsy-liver specimens were as follows: chronic hepatitis in 6, acute hepatitis in 3, nonspecific reactive hepatitis in 13, fatty degeneration in 11, and normal findings in 7 cases. Among them, prominent histological changes were found more frequently in patients with SLE and Sjö-s.
    These histological changes were assumed to be mostly caused by similar mechanism for tissue damages to those found in each collagen diseases, though in some cases other mechanisms of liver damage such as drug allergy, obesity, virus infection or metastasis of malignancy were suspected.

    During the course of an epidemic outbreak of hepatitis A in Noboribetsu Onsencho, Hokkaido, two sibling cases with hepatitis A were found to be of dual infection superimposed upon the HBs-antigen carrier state. During the clinical courses of these two cases, both both HBs-antigen titers and HBc-antibody titers were found to undergo characteristic fluctuations. In Case No. 1, both HBs-antigen titer and HBc-antibody titer showed temporary declines in the convalescence stage of hepatitis A, followed by recoveries and subsequent stabilization of the titers. In Case No. 2, the HBc-antibody titer similarly showed a temporary decline in the convalescence, followed by a similar recovery and subsequent stabilization.
    In view of the limited literatures on HAV infection superimposed upon the HBs-antigen carrier state, the significance of the present observation was discussed, with comments on the available literatures.

    An autopsy case, 59-year old woman, of idiopathic portal hypertension with hepatic nodular hyperplasia and intra- and extrahepatic portal vein thrombosis was presented. Histology, histogenesis, etiological factors and mutual relationship of those three lesions were discussed in this paper.
    At autopsy, the liver weighed 640g, and showed atrophy of the left lobe and 'the potato liver" appearance of the right lobe. Microscopically the left lobe of the liver showed obstruction and thrombosis of small radicles of portal veins which were typical findings of idiopathic portal hypertension reported in the literatures. The right lobe showed nodular hyperplasia, in which characteristic finding was the presence of ectopic portal veins. The ectopic portal veins seemed to have played an important role in development of nodular hyperplasia.
    Angiograms and scintillation scanning of nodular hyperplasia were presented. Their findings were similar to hepatoma.
    The spleen (360g) showed remarkable fibroadenia, Gamna-Gandy body and narrowing of trabecular veins.
    Although definite etiology of idiopathic portal hypertension in this case was not revealed, the possible causes were discussed.
