  • Analysis of the initial phase of infection

    One-day-old ducklings were inoculated with 0.5ml of Duck hepatitis B virus (DHBV) positive serum intravenously. We analysed the sequential change of viral DNA and mRNA in the liver at the initial phase of the infection, using the Southern and Northern blot analysis and spot hybridization test.
    DHBV DNA inoculated was detected as supercoiled DNA at 6 hr postinoculation, and then synthesis of single stranded DNA began around 12hr postinoculation. At 24hr, single stranded DNA of genome size was detected. DHBV DNA accumulated exponentially between day 3 and day 6, and became 170 copies/cell at 10 day of inoculation. DHBV specific poly (A)+ RNA was seen as a smear at 6hr, and bands at 3kb and 4kb were detected at 12hr. Increase of specific RNA occurred between 24hr and 3 day, reaching a level of 4400 times the amount of 6hr. Accumulation of DHBV specific RNA preceded that of DNA.

    The natural course of seroconversion from HBeAg to anti-HBe was investigated for 0.5 to 9 years (average, 3.0 years) in 36 HBeAg positive children with serial examination of s-GPT levels, HBeAg and anti-HBe. Of 29 cases initially positive for HBeAg, 19 had remained HBeAg positive, and 10 eventually lost HBeAg, 9 of whom seroconverted to anti-HBe. In 16 (44.4%) of 36 cases, elevations of s-GPT levels were noted. Of 19 children who had remained HBeAg positive, 5 showed transient, slight elevations of s-GPT levels, whereas apparent elevations of s-GPT levels (>200 Karmen units) were noted in 8 carrier children who lost HBeAg, and after anti-HBe appeared, s-GPT levels were normalized. Seroconversion rate was approximately 5% per year in carrier children.

    Interaction between lymphocytes and hepatocytes were investigated in the liver biopsy specimens taken from fifteen patients with type B chronic active hepatitis under electron microscopy. Interaction was noted between lymphocytes and hepatocytes where these two types of cells were in a close contact without intervening endothelial cells. The microvilli on the surface of the hepatocytes which were in contact with lymphocytes were degenerated and lost. In some area the cytoplasmic projections of the lymphocyte had direct contact with microvilli of the hepatic cell surface through the sinusoidal fenestrae of endothelial cells. We think that it shows the phenomenon of interaction in the early stage and later close contact is established between lymphocytes and hepatocytes without intervening endothelial cells.

    A new hepatic support system was developed in order to remove toxic substances in acute hepatic failure and proved to be safe and effective, using experimental dogs. The system is composed of closed circulation system where middle and low molecular weight substances are filtrated by polyacrylonitrile (PAN) membrane then absorbed by non coated charcoal. For this experiment, dogs with hepatic failure were prepared by dimethylnitrosamine (DMN) injection. It was shown almost all of middle and low molecular weight substances were filtrated but some amino acids and protein binding substances were scarcely filtrated. The filtrate was separated by Biogel P-4, and from the fraction pattern by optical density at 280nm, charcoal column demonstrated complete absorption. Clearance rate of drugs indicated little difference between dogs with and without hepatic failure, but clearance rate of aminopyrine and chloramphenicol indicated a little difference. Clearance rate of amino acids indicated higher value in branched chain amino acids than in aromatic amino acids.

    A series of preoperative Lp-CT (CT after transcatheter arterial embolization using Lipiodol and gelatin sponge: Lp-TAE) images, low-kilovoltage radiographs and histological findings were compared in 12 resected hepatocellular carcinomas after Lp-TAE to examine accumulation of Lipiodol in the main tumors and daughter nodules for evaluation of the effect of TAE. The LpCT images well reflected accumulation of Lipiodol in the main tumors as well as daughter nodules. Lipiodol was accumulated in high concentration within the main tumor in 4 cases of completely necrotized tumor and 5 cases of more than 95% necrotized tumor. In 2 cases, TAE did not demonstrate an anticipated effect, probably because of poor embolization. Lipiodol accumulation in the daughter nodules and the effect of TAE were confirmed. This verifies the usefulness of Lp-TAE for the diagnosis and surgical intervention in the treatment of hepatocellular carcinoma.

    α-fetoprotein (AFP) was observed by Avidin Biotin Complex (ABC) method in Bouin-fixed paraffin embedded liver tissues from 50 patients with hepatocellular carcinoma (HCC). And further, AFP was electronmicroscopically demonstrated by peroxidase-labeled Fab' fraction of anti-AFP in the PLP-fixed surgical specimens obtained from 4 patients with HCC.
    Intrahepatic AFP was observed in 21 of 50 patients. AFP was distributed granularly or diffusely in the cytoplasm of hepatoma cells. These AFP-positive cells frequently appeared in patients with high level of serum AFP and also in patients with Edmondson's grading II or III. In the electron microscopic observation of hepatoma cells, AFP was localized in the cisternae and membrane of rough endoplasmic reticulum, membrane bound ribosomes, perinuclear space and Golgi apparatus.

    Light and electron microscopic autoradiographies were used to demonstrate the hepatocellular uptake of insulin and its distribution in hepatocytes.
    125I-insulin injected into the mesenteric vein of rats was taken up by hepatocytes predominantly in the periportal region of the lobule.
    125I-insulin was taken up into hepatocytes by pinocytosis from the sinusoidal blood and packed into coated vesicles.
    Then, 125I-insulin was translocated by the vesicular transport toward the canalicular pole and excreted into the bile canaliculi.
    Some of 125I-insulin grains were distributed on the rough endoplasmic reticulum and nuclear membrane. The inhibition of microtubular function by colchicine had no effect on the hepatocellular uptake of 125I-insulin. However, the intracellular transport in hepatocytes of 125I- insulin was inhibited by colchicine.
    These results suggested that the insulin taken up by hepatocytes predominantly in the periportal region is transported by the vesicular transport to the bile canaliculi, and microtubules may play an important role in the intracellular transport.

    A model of Extrahepatic Portal Obstruction was prepared with the rat and the effect of portal vein ligation (PVL) on liver functions and the formation of hepato-petal collaterals were investigated using the mitochondrial function of the liver, hepatic blood flow, portal pressure and portography.
    Hepatic blood flow (HBF) significantly decreased until 5 days after PVL, however, due to the formation of hepato-petal collaterals, HBF gradually increased and reached the normal value at 3 weeks after PVL. Respiratory parameters of mitochondria, such as RC, S3, ATP synthesis, gradually decreased and reached the lowest level at 5 days after PVL, while they recovered to the normal value in accordance with the increasing HBF at 3 weeks after PVL.
    In Extrahepatic Portal Obstruction, although the liver is temporarily deteriorated by portal vein obstruction such as portal vein thrombosis, HBF and the liver function is to be maintained normally with the formation of hepato-petal collaterals.

    In the clinical course of drug-induced allergic hepatitis, the production of anti-liver cell membrane antibody was detected in patient's serum by an antibody-dependent cell-mediated cytotbxicity test. This allergic hepatitis was induced by administration of pamoic acid hydroxyzine and liver hedrolysate medicine (proheparum), and the sensitization to these drugs was proved by lymphocyte transformation test. Disorders of liver function were continued for a long time ever after stop of the drug administration and these were presumed to due to the production of antibody against liver specific antigen, because liver function disorders were in parallel with the production of antibody.

    A 57 year-old man with a liver tumor was referred to our hospital. Extensive clinical investigations revealed three tumors in the right posterosuperior segment and hepatocellular carcinoma of the combined type was suspected. Right posterosuperior segmentectomy was carried out and the resected specimen revealed a hepatocellular carcinoma and several nodules of cholangiocellular carcinoma separately. Primary duplex liver carcinoma is very rare and the resected case is the first as far as we have been able to refer. This case is interesting on histogenesis of primary carcinomas of the liver.

    A surgical case of hepatic angiomyolipoma is reported. This case is a 59-year-old woman whose tumor was found incidentally by an ultrasonogram. At laparotomy, the tumor was present in the lower edge of the left lobe of the liver, measured 3×2×2cm in diameter and appeared solid, well-demarcated and dark red. Histologically, it consisted of abundant blood vessels of varying caliber, mature fat cells and round, spindle or epithelioid interstitial cells. The interstitialcells were considered to be of smooth muscle origin, because myofilamentous structures were demonstrated ultrastructurally in their cytoplasm and because they were positive for anti-desmin antibody by PAP method. From these findings, the tumor was diagnosed as angiomyolipoma. Although renal angiomyolipoma is not uncommon, there are only 14 cases of hepatic angiomyolipoma in the world literature. In spite of their histological resemblance, distinctive clinicopathological differences between both tumors are recognized.

    A therapeutic trial using uncoated charcoal column (plasma perfusion) was done for a female case with primary biliary cirrhosis. It relieved her itching and fatigue, but not xanthoma. Total bilirubin decreased to 70% of the previous value soon after the procedure and increased to 80%of it in the next morning. Then the total bilirubin was prone to decrease spontaneously. This spontaneous decrease of the bilirubin suggested improvement of liver function by the procedure. The level of the bile acid decreased to 70% of the previous value soon after the procedure. As the procedure did not induce change of coagulation system it was not necessary for hepatocytes to repair loss of coagulation factors, which was seen in the case of plasma exchange with albumin solution we performed previously. Blastgenic response of lymphocyte decreased after the procedure, which suggested immunomoduratory activity of plasma perfusion.

    A case of primary sclerosing cholangitis associated with ulcerative colitis was reported. A 35-year-old female was first admitted 2 years earlier because of a 5-year history of hepatic dysfunction and marked jaundice and pruritus of sudden onset. Endoscopic retrograde cholangiogram disclosed irregular narrowing and dilatation of the intrahepatic bile ducts and upper part of extrahepatic biliary tract. Hepatic needle biopsy revealed periportal fibrosis of liver. These cholangiographic and histologic findings leaded to a diagnosis of primary sclerosing cholangitis because she had no history of biliary calculi or any surgical intervention. She was readmitted this year with the chief complaint of bloody diarrhea and appetite loss. Barium enema examination showed loss of haustral markings and presence of mucosal erosions and irregular ulcers of the colon. Diffuse multiple erosions and irregular ulcers were also endoscopically observed. Diagnosis of ulcerative colitis was made from these findings. In addition, nine cases of primary sclerosing cholangitis associated with ulcerative colitis reported in Japan were briefly reviewed.

    A case of idiopathic portal hypertension (IPH) accompanied by splenic arterial aneurysm is reported. A 58 year-old female was admitted for pancytopenia. Abdominal X-rays showed a calcified shadow in the left upper abdomen. Celiac angiography showed splenomegaly, a dilated and tortuous splenic artery and a calcified splenic arterial aneurysm in the splenic hilus. Changes in the intra-hepatic arterial branches were not remarkable. Portography revealed a huge splenorenal shunt.
    Blood flow volumes in the portal system, obtained using an ultrasonic Doppler system were: portal vein; 1301ml/min, splenic vein proximal to the spleno-renal shunt; 710ml/min. Portal venous pressure was 24 mmHg. Occlusion of the splenic artery by a balloon catheter resulted in hepatofugal splenic venous flow at 456ml/min. Portal venous pressure decreased to 19 mmHg. Liver biopsy was normal.
    The splenic arterial aneurysm was thought to result from accelerated blood circulation, due to IPH, being further accelerated by the exisistence of the outflow pathway -the spleno-renal shunt- with its low vascular resistance.

    A case of IgAκ type multiple myeloma that had been diagnosed 5 years previously was admitted to our hospital with complaint of sever right hypochondralgia. Abdominal ultrasonography and computed tomography revealed a giant subcapsular hematoma and multiple hepatic tumors in the liver. Hepatic tumors were hypoechoic in abdominal ultrasonography and were of low density area with no enhancement by drip infusion of the contrast media in abdominal computed tomography. Autopsy was done on the 99th day after admission. The subcapsular hematoma had been organized and had no involvement of myeloma cell. Hepatic tumors were consisted of poorly differentiated myeloma cells.
    The long duration of the illness and the poor differentiation of the myeloma cell were con-sidered responsible for the tumor formation of the myeloma cell in the liver. The presentinghemorragic tendency might contribute to the formation of the giant hematoma in the liver.

    A fifty three year-old man admitted to our hospital with general fatigue and many small wounds on the back of hands. He had mild abnormality in liver function tests and increased excretion of porphyrin in urine. Laparoscopic examination showed charactaristic grayish blue areas on the liver surface. The liver tissue obtained by biopsy exhibited red fluorescence when exposed to ultraviolet rays. Histological examination revealed mild lymphocyte infiltration, deposition of hemosiderin and needle-like cytoplasmic inclusion bodies. He was diagnosed as porphyria cutanea tarda. Phlebotomy was performed eleven times during the following year, in which 250-300ml of blood was removed at each time. After 3 liters of blood was removed, excretion of urinary porphyrin decreased to the normal range. Although general fatigue and small wounds disappeared, mild elevation of transaminase continued so far. Second laparoscopic examination which was performed six months after phlebotomy showed that the grayish blue areas had disappeared. Inspite of the disappearance of the red fluorescence, lymphocyte infiltration and needle-like cytoplasmic inclusion bodies remained to be unchanged. It was concluded that the phlebotomy decreased the stored porphyrin of this patient and was effective to improve clinical and laparoscopic findings.
