    Effects of adenine arabinoside (Ara-A, n=10), steroid withdrawal (n=12) and a combination of these two therapies (n=12) on serum HBeAg positivity were statistically studied in patients with chronic hepatitis type B, as compared with the patients with chronic hepatits type B treatd without any specific therapies (control, n=32). Cumulative induction curves of seronegative (SN) or seroconversion (SC) for HBeAg were simulated by Weibull analysis. Following results were obtained: 1) Annual SN rates were significantly higher in Ara-A, steroid withdrawal and combination therapy than in control (p<0.05). 2) The first induction of SN occured significantly earlier than in Ara-A, steroid withdrawal and combination therapy when compared to control (p<0.05). 3) In patients with initial GPT values above 200IU/l, SN was inducted significantly faster as well as earlier in treated group compared to control. In contrast, in patients with GPT value below 200IU/l, SN induction rate was similar between the two groups. 4) Cumulative curve of each treatment consisted of the first component with steeper slope and the second component with milder one. The transition point between the two components appeared around 6-7.5 months. The effect of each treatment was always exerted only on the first part, but not on the second component. In conclusion, those therapies should be targetted on patients with GPT above 200IU/l. In addition, patients without response to one treatment beyond 6-7.5 months will possibly be treated successfully by introducing another therapy.

    Dividing 14 cases with non-A, non-B chronic hepatitis (NANBH) into 3 groups at random, human interferon-β (HuIFN-β) of 1M, 3M and 6M IU/day was administered in each group for consecutive 8 weeks, and comparative test of its dose was prospectively carried out. S-ALT gradually decreased in 3 groups during administration of IFN-β, and low level of s-ALT was noted in the group of 6 MIU during 6 months after treatment, while reelevetion was observed in other 2 groups after treatment. Resulting from that hepatohistological findings were studied by Knodell's HAI score, decreasing tendency of total HAI scores was noted in all groups at the finish of the administration, and tendency recovering to the pre-level was observed in 1M and 3 MIU groups for 6 months after treatment, while further decreased in 6 MIU group. Moreover, serum 2-5AS activity elevated dose-dependently. From of above, it was considered that 6 MIU of HuIFN-β for consecutive 8 weeks is more effective against NANBH than small-dose units.

    Cell kinetics of uni-and binucleated hepatocytes after hepatectomy were examined in rats by measurement of DNA synthetic cells using anti BrdU monoclonal antibody and analysis of nuclear DNA contents.
    The number of binuclear cells decreased until 72 hours after partial hepatectomy. The labeling index of unicleated cells 24 hours after hepatectomy was lower than that of binucleated cells. Cytofluorometric analysis revealed polyploidization in both uni-and binucleatd hepatocytes 24 to 48 hours after hepatectomy.
    The results suggest that the cell kinetics of binucleated hepatocytes were more activated than those of uninucleated cells after partial hepatectomy.

    In order to determine the adequate amount of glucose administration after hepatic resection, IVGTT and ITT were performed at 1, 2, 3 and 7 postoperative days (POD) in normal and thioacetamide-induced cirrhotic rats subjected to 70% hepatectomy and the glucose disposal rate (GDR) was evaluated. Prospected values of GDR were calculated with the analytic index of IVGTT and ITT. Subsequently, those were compared with values measured by the hyperglycemic glucose clamp method. The following results were obtained: 1) Prospected values of GDR were closely similar to the measured values, indicating that it is possible to obtain the GDR value by means of simple and convenient measure. 2) GDR in cirrhotic rats which was lower than that in control rats decreased at 1 POD and then recovered to the level of preoperative values at 3POD. 3) An increase of GDR was not observed after resection of cirrhotic livers even with maximum doses of insulin infusion. These results suggest that adequate amount of glucose must be administered in the early postoperative period after resection of cirrhotic livers, because glucose utilization is significantly decreased mainly due to insulin resistance.

    BrdU labeling indices of biopsy specimens from 16 HCC patients were examined, using an in vitro labeling technique.
    The labeling index (L.I.) of HCC was 7.06±2.71% (Mean±SE), ranging from 2.60% to 12.70%. There was a tendency that the more undifferentiated was the histological grade of Edmondson & Steiner's classification, the larger the L.I. of BrdU became; the mean L.I. in grade III group (10.64± 2.10%) was significancy (p>0.01) larger than that (5.87±1.61%) in Grade I-II group; i.e. the DNA synthesis was markedly increased in the undifferentiated HCC.

    Ethanol injection therapy was performed for rabbit VX2 cancer of the liver, to measure the extent of ethanol infiltration, using the normal hepatic tissue as control, to observe the successive historogical findings after the ethanol injection, and to study the influence of the ethanol injection on the number of survival days with VX2 cancer of the liver.
    The necrotic area caused by ethanol showed a significant dose-dependent correlation in the range of 0.1-1.0ml in the normal liver, and a significant correlation in the range of 0.2-1.0ml also in VX2 cancer of the liver.
    Further, VX2 cancer of the liver showed a significant extention in the number of survival days in the injection group after ethanol injection, being 39.5±5.7 days in the injection group and 29.3±2.9 days in the no injection group.
    In the succesive observation of histological findings after injection, fibrosis started from the 3rd-5th day after ethanol injection in VX2 cancer of the liver, showing acceleration after the 7th day. In ethanol injection for VX2 cancer of the liver, eosinophilic infiltration was seen which was not noted in the injection group in the normal liver. From the above, since ethanol causes direct necrosis of the injected region, it is suggested that ethanol injection therapy is a potentially effective method of treatment for cancer of the liver.

    We studied clinical features of patients with drug induced allergic hepatic injury, in relation to the type of causative drug. We found many symptoms which indicated the operation of an allergic mechanism in patients with halothane-induced hepatic injury. The symptoms of ampicillin-induced hepatic injury indicated an allergic mechanism more prominently than those of a cefalexin-induced hepatic injury. The symptoms of indomethacin-induced hepatic injury and aspirin-induced hepatic injury suggested the mechanism in the cases was cholestatic. The symptoms of tiopronin-induced hepatic injury also suggested a cholestatic condition. The symptoms of methyldopa-induced hepatic injury suggested an hepatitic condition. It was particularly noticeable that in some cases of methyldopa-induced hepatic injury, tests for anti-nuclear antibodies (ANA) were positive. These results indicated that the clinical features of each drug were distinctive, and the allergic mechanism of each drug was not identical in detail.

    Experimental liver damage induced by lipopolysaccharide (LPS) administration following Propionibacterium acnes (P. acnes) injection were studied in rat.
    Multiple granulomas in liver and numerous splenic mononuclear cells (SMNC) at the margenal zone and red palp were observed 7 days after injection of P. acnes in rats (P-rat). Following injection of LPS induced massive hepatocytes necrosis.
    Splenectomy before the administration of LPS reduced the degree of liver damage and the number of the infiltrating cells in granulomas.
    The injection of the splenic adherent cells of the P-rat with LPS via portal vein induced severe liver damage in P. acnes pretreated rat.
    PHA induced blastogenic activities of SMNC was reduced significantly after the injection of P. acnes and LPS. Killer activities of SMNC could not be detected through the experimental periods.
    The culture supernatant of P-rat splenic adherent cells with LPS, showed the cytotoxic effect on normal isolated hepatocytes.
    These results indicated SMNC, especially adherent cells, played an important role in the induction of hepatocytotoxicity on this experimental model.

    space occupying lesions were indicated by abdominal ultrasonographic examination. Posterior segmentectomy of the liver revealed a yellowish tumor and a greenish tumor. Both of them had thin capsules and were classified as Edmondson-Steiner's grade II. However, the yellowish tumor exhibited remarkable fatty infiltration. The tumor cells and their nuclei were small, and negative for HBs antigen. On the other hand, the greenish tumor showed bile pigmentation over hepatocytes and bile ducts. Flow cytometric determination of the nuclear DNA content of these tumors showed different aneuploid patterns. The DNA index was 1.16 for the yellowish tumor, and 1.28 for the greenish tumor. It was suggested that these nodules occurred multicentrically. Flow cytometric method also would be useful evaluation of multicentric hepatocellular carcinoma.

    A 63-year-old female was admitted to our hospital due to right hypochondralgia in May 1988. Laboratory data showed slight dysfunction of liver function test with 90U/ml of CA19-9. Imaging studies of CT, US and direct cholangiogram revealed impacted intrahepatic bile duct stones in the right lobe but no malignant tumor was detected in the liver.
    Intraoperative histological examination of the frozen section of a lymph node in the hepatoduodenal ligament indicated the metastatic adenocarcinoma. Extended hepatic lobectomy with dissection of the regional lymph nodes was performed. An elevated lesion was revealed after removal of stones in the dilated duct wall in the anterior segment and histological study showed moderately differentiated papillary adenocarcinoma of the intrahepatic bile duct. Immunohistochemical study using a monoclonal antibody to bromodeoxyuridine showed 8.6% of labeling index in the elevated lesion and 8.8% in the non-cancerous epithelia of its neighbouring duct wall, and 0.5% in the wall of the narrowed bile duct in the hilum and 0.3% in the right hepatic duct.

  • A case report of hepatocellular carcinoma (HCC)

    A case of HCC with massive tumor thrombus in portal vein is reported.
    The patient was 53 year old man and admitted to our hospital because of abdominal fullness and darkness of urine. After admission, several examinations were performed and he was diagnosed as a diffuse type HCC with massive tumor thrombus in portal vein.
    By 2D-Doppler, many arterial flows in and around the tumor thrombus were Visualized and a part of arterial flow was connected with the continuous blood flow. FFT analysis of these blood flows showed the typical pattern of anastomosis between arterial blood flow and continuous blood flow.
    In conclusion, this new modality is expected as one of the gastroenterological unique imaging method because of its superiolity such as noninvasiveness and convenience compared with other modalities.

    An autopsy case of massive hepatocellular carcinoma (HCC) developing in a HBsAg carrier was reported. At autopsy, DNA was extracted from ten tumor nodules, one metastatic lung nodule and five noncancerous portions of the liver to investigate the presence and integration mode of hepatitis B virus (HBV) DNA by Southern blot analysis. All ten tumor nodules of the liver and one metastatic lung nodule showed the same histological features of trabecular HCC. The same restriction pattern of integrated HBV DNA was detected in eight tumor nodules and a metastatic one, but HBV DNA was not detected in two tumor nodules. In noncancerous portions a free form of HBV DNA was observed. These findings suggest that Southern blot analysis of HBV DNA might be very valuable for investigation of the genesis and post-integration DNA rearrengements of HCC.
