    Anti-double stranded DNA antibody (A-dsDNA) and anti-single stranded DNA antibody (A-ssDNA) were measured by ELISA method and by Crithidia immuno-fluorescence method in various chronic liver diseases (CLD). A-dsDNA were detected only in the two categories of CLD, that is autoimmune hepatitis (AIH) and primary biliary cirrhosis (PBC). Positive percentage and titer of A-dsDNA were predominantly high in AIH. Among patients with AIH, A-dsDNA positive cases were younger, and had a higher percentage in LE and RA tests and higher values in serum γ-globulin and IgG than negative cases. A-ssDNA were positive not only in AIH and PBC but also in other categories of CLD. Histologically, A-dsDNA positive patients with AIH showed more rapid progression to liver cirrhosis than did negative cases, and clinically tended to take the course of acute exacerbation, status such as subacute hepatitis. Of the immunoglobulin classes, only IgG of A-dsDNA was positive in such cases, and IgG class of A-dsDNA was supposed to have a close association with acute exacerbation of AIH. In conclusion, A-dsDNA is highly specific for AIH and has close association with the histological progression and clinical exacerbation in AIH.

    In order to elucidate the relationship between clinical state of HBV carrier and HLA, serological as well as DNA typing was performed in 78 HBs-Ag positive HBV carriers. Antigen frequencies significantly differed in HLA-A26, B27, B51, Bw62, Cw3, DR5 and DRw8 between patients and healthy individuals. Especially high frequency of Cw3 antigen was observed in the clinical active group. In contrast, high frequency of DR5 antigen was noticed in clinical inactive group. In patients with Cw3, the linkage of HLA A2 and Cw3 had a strong correlation to the patients in clinical active stage. Furthermore, the close linkage of A31, Cw3 and DR5 was observed in the clinical inactive group. By the method of HLA-DNA typing specific RFLP (restriction fragment length polymorphism) fragments were observed with respect to DR5 haplotype.

    We investigate, with urinary, hematological, biological, immunological and histopathological examinations, the process from chronic liver disease to spontaneous liver cancers in long survived LEC rats in comparison with age-matched Long Evans Agouti (LEA) rats. We then got the following results.
    LEC rats had the high frequent incidence of anemia, abnormal renal function and immuno-suppressive status. As for liver funciton, clinicobiochemical data showed the chronic liver dysfunction type and histopathology showed the figure of chronic hepatitis with both spotty necrotic and small regenerating hepatocytes.
    Oval cells, hyperplastic foci and hyperplastic nodules were detected in the process of spontaneous hepatocarcinogenesis in LEC rats and all the LEC rats eighteen months after birth had liver cancers. From these ruslts, we speculated that the long survived LEC rats, regardless of whether acute hepatitis or not, suffered from continuous liver damage and chronic hepatitis, were worsened progressively and resulted in liver cancers.

    A study was conducted to investigate changes of neutral amino acid (NAA) transport across blood-brain barrier in hepatic encephalopathy, in paticular, employing isolated brain microvessels (BMV).
    Hepatic failure was induced in rats by either portacaval anastomosis (PCA), acute ischemic hepatic failure (AIHF) or continuous infusion of ammonium acetate (NH3-R). BMV was isolated from each model, and uptake of NAA by BMV was examined. Following results were obtained.
    1) BMV isolated from PCA as well as AIHF showed significantly higher uptake of 14C-tryptophan (Trp), phenylalanine and isoleucine (Ile) as compared with each control.
    2) In kinetic analysis of 14C-Trp uptake, a higher Vmax was found in BMV isolated from PCA and AIHF as compared with control, whereas Kd values were similar in three preparations.
    3) In BMV from NH3-R, significantly higher uptake of 14C-Trp and Ile was observed when compared to control.
    4) Glutamine (Gln)-preloaded BMV took up significantly greater amount of 14C-Trp. In conclusion, increased uptake of NAAs was clearly demonstrated in BMV isolatd from rats with hepatic failure. Such increase seems to be presumably due to elevated Vmax value. In addition, it is also suggested that elevated level of Gln in the brain has contributed to increase of NAA uptake in BMV from NH3-R and in Gln-preloaded BMV.

    Sprague-Dawley male and female rats were pair-fed with a high-fat, low-protein diet plus ethanol or isocaloric sucrose for 6 weeks. Alcohol dehydrogenase (ADH) and aldehyde dehydrogenase (ALDH) activities were higher in female control than in male control. ADH and high Km ALDK activities decreased in female alcohol compared to female control, while these enzymes increased in male alcohol compared to male control. The highest accumulation of hepatic collagen-bound hydroxyproline was found in female alcohol. The development of alcoholic liver disease due to chronic ethanol consumption may be affected by the sex difference and is more severe in female.

    We investigated the expression of γ-glutamyltranspeptidase (γ-GTP) in rat liver during carcinogenesis induced by diethylnitrosamine (DEN), using both immunohistochemical and in situ hybridization techniques. At 1 month after DEN injection, γ-GTP immuno-positive cells (parenchymal cells) were scattered in the zone I and zone II of liver lobules. And at 2 months, γ-GTP immuno-positive enzyme altered foci (EAF) were observed predominantly in the zone I and zone II. At 5 months, γ-GTP immuno-positive hyperplastic nodules (HNs), which compressed surrounding tissue, were also seen in liver lobules. γ-GTP immuno-positive structures were observed at cell membrane and bile canaliculi. Using in situ hybridization, we detected assemblies of silver grains which indicated mRNA, in both the scattered hepatocytes in the early stage of hepatocarcinogenesis and the cells in hyperplastic foci. These results suggested that the hepatocytes expressing γ-GTP in zone I and zone II might be concerned with the genesis of EAF and HNs.

    To investigate the involvement of cellular oncogenes in liver regeneration, we analyzed the expression of oncogenes and vitamin D receptor (VD·R) after rat partial hepatectomy (PH). The expression of c-myc, c-fos and c-Ha-ras oncogenes transiently increased after PH. c-myc mRNA was detected in the cytosol of hepatocyte 3hr after PH with in situ hybridization and the product of this oncogene was detected in the nucleus of hepatocyte 6hr after PH with immunohistochemical examination. On the other hand, no significant expression of c-erbA oncogene was detected after PH. The number of VD·R in the cytosol of hepatocyte was increased during liver regeneration and reached maximum 6hr after PH. All of these changes were observed before DNA synthesis of hepatocyte. These results indicate that oncogenes, especially c-myc, c-fos and c-Ha-ras, and VD·R play important roles in the initial process of liver regeneration.

    In primary rat hepatocyte culture, various gene expressions and DNA synthesis were examined by Northern blot analysis and [3H]-thymidine incorporation, respectively. After plating the cells, transcript level of c-myc, p53, c-Ha-ras and ornithine decarboxylase (ODC) gene increased until 48 hours, wheareas DNA synthesis did not occur during 72 hours, suggesting that cultured hepatocytes were in competent state (G1 stage of the cell cycle). The addition of EGF in the culture medium further increased c-Ha-ras and ODC gene expression and enhanced DNA synthesis by 5 fold, suggesting progression of the cell cycle to S stage. The addition of TPA transiently increased c-myc expression and enhanced DNA synthesis by 2 fold, suggesting possible involvement of protein kinase C in growth signal transduction. The selective induction of oncogene expression by EGF and TPA supports the existence of two independent signal transduction pathways in hepatocytes.

    Soluble fraction of GPT (sGPT) exhibits genetic polymorphism by two autosomal codominant alleles, Gpt1 and Gpt2 The catalytic activity of the Gpt1 product is 2-3 times higher than that of Gpt2.
    The existence of a silent allele (Gpt0) has also so far been predicted by several authors.
    The proband, a 47-year-old female with acute exacerbation of HBV carrier, showed a remarkably low value of serum GPT activity, comparing with the elevated value of GOT, LDH, ALP at admission.
    In red cell lysates the GPT activity was 0.82U/gHb and a very weak band in its electrophoresis corresponded with the phenotype GPT2, GPT phenotype and activity of her mother were GPT1 and 2.93U/gHb, respectively.
    Low value of GPT in red cell lysates and a discrepancy in phenotype between the mother and the proband could be explained by the existence of Gpt0 which does not participate in determining the phenotypes.

    A female patient, aged 19 years, diagnosed of acute hepatitis B because of increased serum transaminase and positive HBs antigen and IgM-anti HBc. After admission, she was observed mainly at rest in bed. On the 27th hospital day, HBs-antigen became negative and HBs-antibody positive. On the 37th day, severe anemia with 5.6g/dl hemoglobin appeared and a diagnosis of hemolytic anemia accompanied with hepatitis B was made on the basis of reticulocytosis, elevated LDH, decreased haptoglobin and erythroid hyperplasia in bone marrow. Without steroid therapy nor blood transfusion, anemia and liver damage recovered with no other complication.

    We reported a case of primary biliary cirrhosis (PBC) at the precirrhotic stage which developed hepatic encephalopathy induced by a porto-systemic shunt.
    The case was a 62-year-old woman who had been diagnosed as asymptomatic PBC 6 years before. Since 2 years ago, her speech and movement were gradually becoming sluggish and serum ammonia levels were found to be occasionally elevated. She was re-admitted to the hospital for further examination of the disease. Abdominal angiography revealed a porto-systemic shunt via the azygos and paraumbilical veins, suggesting that the hepatic encephalopathy was caused by this porto-systemic shunt.
    Histologically, there was no findings which indicated the presence of cirrhosis. However, it was noted that peripheral portal vein branches were extremely narrowed. These histological changes might have been responsible for the porto-systemic shunt.
    Although PBC is often accompanied by portal hypertension, it is rare that hepatic encephalopathy occurs before the cirrhotic stage of the disease. In the present case, it was considered that porto-systemic shunt induced by the above-mentioned histological changes caused hepatic encephalopathy even at the precirrhotic stage of PBC. This point should be taken into consideration in following up PBC patients.

    A case of the tumor-like lesion of the liver which developed diethylstilbestrol phosphate (DES) was reported. The patient was a 64-year old man. At this 54-years, age, prostate and testes were resected because of the prostate carcinoma and then, he was given DES in a dose of 300mg daily for ten years as an endocrine therapy. Ultrasonogram showed a hypoechoic lesion in the S8 of the liver and celiac arteriogram revealed a hypervascular lesion in the same place. Partial subsegmentectomy of S8 was performed because HCC was strongly suspected. Resected specimen showed a yellowish nodule but histologically, it revealed a hyperplasia of the hepatocyte without malignant change.

    We have experienced an autopsy case of acute hepatic failure caused by ingesting of a single dose (4.8gr.) of acetaminophen for the purpose of suiside. A forty-year-old-woman was admitted to our hospital in comatic condition. Her clinical course and laboratory data were those of acute hepatic failure. She died 7 days after admission in spite of the therapy of plasma exchange. At autopsy, the liver was markedly atrophic and its weight was 750gr. Microscopically, a massive liver cell necrosis was noted.
    It is well known that overdose of acetaminophen induces acute liver injury. However, in this case, the dose of acetaminophen was relatively low (4.8gr.). Concerning the cause of the marked liver injury by acetaminophen, following factors may be related; 1) the patient had been a drinker 2) acetaminophen was taken together with alcohol and caffein contained in Sedes A. These factors might enhanced acetaminophen-induced hepatotoxicity.

    Recently, there are reports that the echogenecity of a minute hepatocellular carcinoma will change from hyperechoic to hypoechoic while growing and that fatty metamorphosis of the hepatocytes would be characteristic change of a minute hepatocellular carcinoma. We report here such a case that supports these possibilities.
    A fifty-nine-year-old Japanese male was admitted to Kyushu University Hospital because of a small mass in the right lobe of the liver which was detected by ultrasonography (US). The mass appeared as a hyperechoic nodule of 8mm in diameter. Fine needle biopsy disclosed the specimen consisted of relatively large cells with fatty metamorphosis. The diameter of the mass increased to 12mm and the echogenecity turned hypoechoic one and a half year later. Then, the mass grew to 19mm four months later. Hepatic arteriography failed to demonstrate the mass. However, as computed tomography taken after the lipiodol infusion disclosed a lipiodol deposition, resection of the mass was performed. Histology revealed well-differentiated hepatocellular carcinoma (Edmondson I) with fatty metamorphosis in the periphery of the tumor.
