    The polymerase chain reaction (PCR) was used to detect hepatitis B visus (HBV) DNA from the paraffin-embedded liver tissue of chronic hepatitis B and non-A, non-B so as to determine whether HBVDNA coexists in chronic hepatitis non-A, non-B liver. Of 11 paraffin-embedded liver tissue of chronic hepatitis B, all were found HBVDNA positive by PCR. On the other hand, of 21 paraffinembedded liver tissue of chronic hepatitis non-A, non-B, none was found HBVDNA positive by PCR. Twenty five raw liver tissues of chronic hepatitis non-A, non-B were also tested HBVDNA negative by PCR. A total of 46 were found HBVDNA negative in the liver tissues of chranic hepatitis non-A, non-B, irrespective of the presence of anti-hepatitis C virus, anti-HBs, and anti-HBc. These findings indicate that HBVDNA may not exist, or extremely small in number if exist, in the liver tissue of clinically diagnosed chronic hepatitis non-A, non-B.
    Since PCR is extremely sensitive method, especially when the sample containing a small amount of DNA such as paraffin-embedded tissue was used, this method is more prone to false-positive results than are other diagnostic tests.

    Liver biopsys were performed in thirty-six HBV-carriers with e-antigen positive and normal range of transaminase.
    The results were as follows:
    1) Histological diagnosis; There were seven cases of normal liver, six of CPH, seven of CAH2A, two of CAH2B and three of liver cirrhosis. Healthy carriers were rather few cases.
    2) There was no difference between normal liver cases and chronic hepatitis cases in serological studies. But there was significant difference liver cirrhosis cases and other cases in DVA-polymerase and platelets.
    3) In asymptomatic carriers it is important that DNA-polymerase and platelets are markers of detection of liver cirrhosis.

    Using the solid-phase enzyme immunoassay with monoclonal antibodies, hepatitis B surface antigen was subtyped in sera from 672 asymptomatic carriers who donated blood units at Gifu Red Cross Blood Center in Japan. Age distribution was different between adr carriers and adw carriers. In HBV carriers aged 16-19, adw carriers were dominant, though adr carriers changed dominant progressively with advancing age. HBe antigen was significantly more often present in adr than adw carriers (26.8% of adr carriers vs 10.4% of adw carriers: p<0.001).
    Furthermore HBs antigen was subtyped in sera from 122 HBV carriers who had been investigated the mode of HBV transmission. In 68 adr carriers, 47 cases (69.1%) had been transmitted HBV vertically, and 21 cases (30.9%) had been infected HBV horizontally. On the other hand, in 44 adw carriers, 13 cases (29.5%) had been transmitted HBV vertically, and 31 cases (70.5%) had been infected HBV horizontally. This difference was statistically significant (p<0.001).
    These differences between adr and adw carriers may reflect the differences of not simply subtypic determinants, but other parts of characters between viral strains subtyped adr and adw.

    The compound subtype was recognized to be very common during active HBV proliferation in the HBeAg positive phase: in 282 HBV carriers, there was no difference between the asymptomatic carriers and chronic liver disease patients. Findings were based on serum HBeAg positive or negative status, serum HBV-DNA levels, the HBsAg titers, patient age, sex and type of liver disease. When the compound subtype appeared in a case of chronic hepatitis which was seroconverted from HBeAg positive to anti-HBe positive in the observation period, it was seen during the HBV-DNA positive period and before the elevation of serum transaminases. It was not recognized when serum HBV-DNA was decreasing and serum transaminases was normalizing.
    It was observed clinically that the compound subtype appeared during active HBV replication. As the cases of chronic hepatitis in which the compound subtype appeared were cases with active HBV proliferation, the hepatitis was active and prognosis is worse.

    We determined the HCV antibody titer using the titer obtained by diluting the initial serum. We made a comparative study of the HCV antibody titers before and after treatment of 10 cases in which the transaminase values became normal over a long period due to interferon treatment and 10 cases in which these values did not become normal. If we look at the 10 cases in which the serum transaminase values became normal due to interferon treatment, the titers before interferon treatment ranged above 27 in 3 cases were 25 in 2 cases and 21 in 1 cases, and also from 25 to 27 in 4 cases, were 23 in 3 cases and 21 in 1 case. Also, in 2 cases with a pre-treatment titer of 23 and in 1 case with a pro-treatment titer of 21, in all these cases after administration of interferon the HCV antibody titer became negative.

    Using an autoimmune hepatitis model of A/J mice, which was prepared with immunization of syngeneic liver proteins, we adoptivery transferred spleen cells of normal mice or neonatally thymectomized mice (NTx) to NTx recipients before immunization and evaluated its influences on experimental autoimmune hepatitis. Mitogen induced transformation and Interleukin-2 (IL-2) activity were meseaured in spleen cells of NTx mice or normal mice.
    PHA or ConA induced transformation and IL-2 activity were reduced in spleen cells of NTx mice. Liver damage, serum GPT level, production of auto-antibody to liver-specific membrane lipoprotein were reduced and delayed-type hypersensitivity to purified protein derivatives was promoted by adoptive transfer of normal murine spleen cells. But the spleen cells of NTx mice showed no such effects.
    These results suggest that normal spleen cells possess the capability of suppressing autoimmune hepatitis and T-cell dysfunction by NTx contributes to the promotion of autoimmune hepatitis.

    Histopathological and immunohistochemical findings of the liver of 8 immunocompromised hosts infected with cytomegalovirus (CMV) were studied. CMV infection was diagnosed by detection of typical CMV inclusions or CMV associated antigen.
    CMV inclusions were observed in intrahepatic bile duct epithelium, hepatocyte, vascular endothelium in portal area and macropages. Most of the CMV inclusions were present in the nucleus. The cells infected with CMV showed various degenerative changes such as swelling, cytoplasmic vacuolation, nuclear atypia, and anisonucleosis without necrotic changes. The degenerative cells were not associated with inflammatory cellular reaction regardless of basic disease. Only mononuclear infiltration was observed in any of the cases. These histopathological findings were not so severe as to cause liver dysfunction. Concerning the localization of CMV infected cells, they were usually detected hepatocytes in the periportal area or along interlobular septum and in bile duct epithelium with an outer diamerter of 10-35μm.

    This study was performed to evaluate the stimulatory effect of epidermal growth factor (EGF) on liver regeneration in rat with massive hepatic necrosis induced by CCl4. Regarding number of EGF receptor and endogenous EGF level in the liver, in earlier time of liver injury both of them were significantly decreased. After administration of human EGF into those animals, great increase of exogenously given EGF was observed in liver tissues. Therefore, increase of DNA synthesis observed after injection of EGF might be due to a role of EGF incorporated into hepatocytes. In conclusion, EGF is extremely important peptide which stimulates strongly hepatic DNA synthesis and is applicable to the treatment of acute liver failure caused by massive hepatic necrosis.

    This study was undertaken to elucidate the role and cellular origin of fibronectin and laminin in fibrosis of CCl4-treated rats. The localization of type I, III, IV collagens were also examined.
    In CCl4-treated rats, fibronectin deposits increased prior to collagen deposits along the sinusoids extending from central regions into the hepatic lobule. According to the result of comparing the light microscopic localization of fibronectin with that of type I and III collagens on about the same areas using serial sections, fibronectin deposits were more prominent in the margin than type I and III collagen deposits in the fibrous septa, therefore, it is histologically suggested that fibronectin may induce collagen deposition.
    Laminin deposits were found faintly in Disse's space in normal liver.
    As fibrosis progressed, laminin deposits increased in Disse's space, resulting in sinusoidal capillarization.
    Immunoelectron microscopic examination disclosed that fibronectin was synthesized by not only hepatocytes but also Ito cells most abundantly in septal fibrosis, and that Ito cells contained laminin more abundantly in normal liver than in septal fibrosis.

    The relationship between the liver volume and DNA synthetic potency of hepatocytes from biopsied specimens was investigated in various stages of PBC using computed tomography and in vitro BrdU-anti BrdU technique.
    The mean (±SE) liver volume in the early stage of PBC (1225±40cm3) was about 1.5 times that in control subjects (835±42cm3, p<0.001).
    The mean BrdU (±SE) Labeling Index (LI) for 10 patients with early histologic stage of PBC (3.4± 0.4%) was 17 times that for 6 control subjects (0.2±0.1, p<0.001), and was significantly higher than that for 19 patients with chronic aggressive hepatitis (0.9±0.2, p<0.01), 14 control cirrhotic patients of viral origin (1.0±0.3, p<0.01).
    These results would demonstrate that liver volume has already become large in the early stage of PBC because of markedly increased DNA synthetic potency in hepatocytes.

    Twenty patients with advanced hepatocellular carcinoma (HCC) were treated by combination therapy with transcatheter arterial embolization (TAE) and percutaneous ultrasonographically guided ethanol injection therapy (PEIT) (TAE-PEIT group). Twenty-eight patients with HCC treated by TAE alone were also studied as controls (TAE group).
    Survival rate of TAE-PEIT group was significantly higher than that of TAE group. Among TAE-PEIT group, nodular typed HCC with capsule was the most effective type in this combination therapy. However, in massive typed HCC, nodular typed HCC without capsule, and multi-nodular typed HCC, this combination therapy were less effective. We conclude that combination therapy of TAE and PEIT is useful in the treatment of advanced HCC.

    Multicellular rat hepatocyte spheroids were primary cultured on a hydrophobic plastic dish and cell organization and reconstitution of cell polarity therein were studied. As culture time elapsed, hepatocytes aggregated forming spheroids in which bile canaliculi and junctional complexs reappeared. K+-dependent p-NPPase, Mg2+-ATPase and ALPase activities also showed supporting facts for reorganization of hepatocytes and reconstitution of cell polarity through appearance and disappearance of thier activities each. Lanthanum nitrate and cationized ferritin used as tracers also confirmed the reorganization of cultured hepatocytes. The results suggested that cellular reorganization and the maintenance of reconstituted cell polarity nearly as in the in vivo state were carried out better and longer in the conventional monolayer culture.

    A method for the isolation and culture of intrahepatic biliary epithelial cells from the normal rat liver was developed. The rat liver was perfused with collagenase solution, and further digested with collagenase and trypsin salutian. Cells obtained by the centrifugation with Percall gradients were cultured on collagen-caated dishes or collagen-gel. After one day in culture, mare than 80% of the cells were positive by the stain for γ-GTP that those cells were derived from biliary epithelial cells. Cultured biliary cells formed a lumen and junctional complexes between cells. Cells cultured on collagen-gel were more similar to the biliary cells in situ than cells on collagen-caated dishes.

    We presented a case of 40-year-old man diagnosed as chronic active hepatitis type B, repeatedly showing marked increases of α-fetoprotein (AFP) following 4 times acute exacerbation in his clinical course. AFP increased up to 16, 791ng/ml, with marked depression of albumin, following high elevated DNA-polymerase with acute exacerbation, After sero-conversion occurred, he had a good clinical course and there has been no rise in serum AFP. Lectin affinities of the AFP was concanavalin A-reactive and lentil lectin-non reactive. This marked increase of AFP and depression of albumin may indicate a regeneration process of hepatic necrosis following the acute exacerbation.

    A 40-year-old male of cystadenoma with an pedunculated growth pattern fram the inferior surface of the liver was presented. The patient was admitted to San-in-Rosal Hospital with chief complaints of abdominal full sensation and a palpated epigastric mass. Imaging modalities showed polycystic liver tumor arising in the right lobe of the liver. Serum antigen of CA19-9, CEA and DUPAN-II were also increased in the cystic fluids. The resected specimen was 6×15×16cm in size and 2, 150g in weight. Histologically, the cyst was arranged in billiary-type epithelium and immunahistochemical studies disclosed positive staining in CA19-9 and CEA. This case suggests that benign cystadenoma is contributed to a markedly elevation of serum CA19-9.
