    The clinical features were elucidated for five HBe antigen-positive cases superinfection with hepatitis B virus (HBV) and hepatitis C virus (HCV). Two of the cases had chronic hepatitis (CH) due to the superinfection, while one case developed acte hepatitis (AH) B concomtantly with CH C, and the final two cases had AH due to the superinfection. The first two cases of CH were treated with interferon (IFN). In the early period after the start of the IFN therapy, one of the cases was found to become negative for HCV-RNA, and seroconversion (SC) from HBeAg to HBeAb was achieved, After the IFN therapy was completed, this case again became positive for HCV-RNA. On the other hand, the second patient treated with IFN did not achieve SC, but a negative state for HCV-RNA was maintainted. The patient with CH C complicated by AH B did not show aggravation of the hepatitis due to the superinfection by HBV. In addition, the amount of HCV-RNA was not found to change from the most severe period of concomitant manifestation of AH B to the time after HBsAg had disappeared. The two patients with AH due to superinfection with HBV and HCV first became negative for HBsAg, and then HCVAb and HCV-RNA also became negative. These two patients showed a good course and were cured.

    Virological and clinico-pathological studies for 41 individuals with 2nd generation anti-HCV but normal liver tests demonstrated that two third of these individuals were positive for HCV RNA in sera and most of their liver histologies were chronic hepatitis. Of the 16 individuals showing AST<ALT within normal range, 15 cases had hepatitis C viremia. This indicates that AST/ALT ratio could predict hepatitis C viremia. There were no significant differences in the amounts of serum HCV RNA and the distribution of HCV genotypes between asymptomatic and symptomatic HCV carriers. One asymptomatic HCV carrier with almost normal liver histology showed persistently normal liver tests for 19 months. There were few HCV healthy carriers among asymptomatic HCV carriers.

    Chemical structures of the sugar chains of human transferrin (Tf) from patients with hepatocellular carcinoma (HCC) and from liver cirrhosis were examined by pyridylamination of their oligosaccharides in combination with reversed-phase and size-fractionation high-performance liquid chromatography, and were compared with those of human alpha-fetoprotein (AFP). Crossed immunoaffinoelectrophoresis of Tf was performed in the presence of concanavalin A (Con A) and Lens culinaris agglutinin (LCA). Two, reactive and nonreactive, and 3 peaks, reactive, weakly reactive and nonreactive, were detected on crossed immuno-affinoelectrophoresis in gels containing LCA and Con A, respectively. LCA-reactive species of Tf from HCC was higher than that from LC with statistical significance, and the carbohydrate analysis showed that the ratio of the fucosylation at the innermost N-acetylglucosamine residue on the biantennary sugar chain in HCC was higher than that in liver cirrhosis. Carbohydrate analysis of AFP in HCC also showed an increased ratio of the fucosylation at the innermost N-acetylglucosamine residue in biantennary sugar chain. However, there was no statistical significant correlation in the ratios of fucosylation on the biantennary sugar chain between Tf and AFP. On the other hand, the percentage of Con A-nonreactive plus-weakly reactive species in HCC was statistically higher than that in LC. Positive correlation was observed between the percentage of Con A-nonreactive pulse-weakly reactive species and the ratio of the fucosylation in biantennary sugar chain of Tf. Thus, these results indicate that the increment in the fucosylation and branching to form new antennae is a characteristic features of the carbohydrate chains of Tf in patients with HCC.

    46 nodules of hepatocellular carcinoma (HCC) (<3cm in diameter) were studied by DNA ploidy pattern and PCNA immunohistochemical staining. The incidence of DNA aneuploidy in well differentiated HCC was significantly lower than in moderately or poorly differentiated HCC (p<0.05). PCNA labeling index (LI) corresponded to the degree of histologic differentiation (p<0.05). We classified 4 gross appearance according to The Liver Cancer Study Group of Japan: type 1; vaguely nodular type, type 2; single nodular type, type 3; single nodular type with extranodular growth, type 4; contiguous multinodular type. The incidence of DNA aneuploidy and the average PCNA LI was increasing according to this number (p<0.05). All HCCs of vaguely nodular type were diploidy and their PCNA LI showed very low value. We concluded that vaguely nodular type might be an early HCC only in view of tumor-related factors.

    In order to clarify the pathogenesis of liver dysfunction in endotoxemia, in vitro studies were performed using co-culture of hepatocytes and Kupffer cells isolated from adult rat liver. An increase of phosphatidylcholine hydroperoxide (PCOOH), a specific membrane lipid peroxidant, in hepatocytes, as well as an increase of GOT and GPT in supernatant were noted only when endotoxin was added to the co-culture with Kupffer cells, but not in the hepatocytes alone. Hepatocytes protein synthesis was also inhibited only in the co-culture added with endotoxin. Furthermore, an increase of PCOOH induced by endotoxin was inhibited with scavengers, especially with catalase. These results suggest that Kupffer cells activated by endotoxin may release free radicals, finally inducing membrane lipid peroxidation of the hepatocytes.

    Production and localization of cellular fibronectin (cFN) during repairing process after liver damage (CCl4-treated rats) was investigated by immunohistochemical and molecular pathological techniques. cFN was produced by hepatocytes and non-parenchymal cells in normal liver. According to dot blot hybridization and in situ hybridization, cFN mRNA was strongly increased in 7 days' and 14 days' CCl4-treated livers. In addition, cFN was localized in non-parenchymal cells in fibrotic lesion and hepatocytes around it. These results suggest that cFN may play an important role in the construction of sinusodal wall and cell-to-cell interactions in normal liver and is associated with fibrosis after liver damage.

    We report a case of chronic active hepatitis C (C-CAH) associated with idiopathic thrombocytopenic purpura (ITP), who received a successful treatment with recombinant human interferon alfa 2b (rh-IFN-α 2b). A 68-year-old man was admitted to our hospital because of the liver dysfunction and decreased platelet counts. Laboratory data showed the presence of serum anti-HCV antibodies and platelet associated IgG. Liver biopsy revealed CAH, and bone marrow examination showed normocellularity with slightly elevated number of megakaryocytes. Based on these findings, the patient was diagnosed as C-CAH associated with ITP. We started rh-IFN-α 2b therapy (3 MU/day, 3 times weekly for 14 days, then 6 MU/day, 3 times weekly for 80 days), which resulted in a reduction of serum transaminase levels and recovery of thrombocytopenia. This case indicates not only a possible casual association between HCV infection and the development of ITP but also a usefulness of IFN therapy for such cases.

    Two cases of hepatocellular carcinoma with adenocarcinoma after hepatic resection following are reported. Case 1 was a 78-year-old man who was diagnosed as having two hepatocellular carcinomas after hepatic resection. PEIT was performed. Subsequent histopathologic examination disclosed cholangiolocellular carcinoma. Case 2 was a 49-year-old man who was diagnosed as having hepatocellular carcinoma after hepatic resection. PEIT was performed. Subsequent histopathologic examination disclosed the presence of progression of adenocarcinomain addition to hepatocellular carcinoma. In each of these two cases, it was recognized the adenocarcinoma in liver with the hepatocellular carcinoma. These two cases were diagnosed combined hepatocellular and cholangiocellular carcinomas. The two cases presented here are very suggestive in an influence of the operation and PEIT on the hepatocellular carcinoma.

    71 years old male hospitalized with chief complaint of disturbance of consiousness. He has performed radical transeverse colectomy recieved electric pasemaker for complete A-V block and canurated arterial infuser reservoir for hepatocellular carcinoma.
    Remarkable change is not shown on electrocardiogram. Ultrasonography showed mosaic patterned main tumor in right hepatic lobe and multiple tumors in bilateral lobe of liver. Echocardiogram revealed metastatic tumor in right ventricle. On the 16th hospital day, the patient died of hepatic failure. Autopsy finding is hepatocellular carcinoam with cardiac metastasis. It is a rare case for hepatocellular carcinoma to metastasize to the heart.
