    We studied 56 patients with primary hepatocellular carcinoma (HCC) treated with radiofrequency ablation (RFA) in our hospital to clarify the predictive factors for intrahepatic distant recurrence. The observation period was 16.4 months. Cumulative rate of recurrence was 12.0% in a year and 53.5% in 3 years. Multivariate analysis revealed that multinodular HCC (odds ratio 4.78, p=0.01) and platelet count <10.0×104l (odds ratio 5.25, p<0.01) were considered to be useful predictive factors for recurrence. These recurrence rate and factors after RFA were similar to those after surgical resection. Therefore, patients showed uninodular HCC and whose platelet count was 10.0×104l and over before treatment might not be treated with RFA, but surgical resection if possible. However, considering remnant liver function and recurrence rate, we think patients either showed multinodular HCC or whose platelet count was less than 10.0×104l before treatment should be treated with RFA if possible.

    We report a patient with adult-onset type II citrullinemia (CTLN2) who could be followed up for a long period. The patient was a 68-year-old male. He developed adult-onset citrullinemia in his 20s and repeated episodes of transient disturbance of consciousness and generalized convulsion, and was admitted to the psychiatric department due to tremor of the fingers and disturbed consciousness at the age of 45 years. Ammoniemia and citrullinemia were observed, and a diagnosis of adult-onset citrullinemia was made. After the age of 50 years, he discontinued treatment by himself but has been surviving without major episodes for a long period. Gene examination showed abnormality in the SLC25A13 gene, confirming CTLN2. CTLN2 is considered to have a poor prognosis. However, our patient has survived for about 50 years after onset, showing a symptom-free course without treatment for more than 15 years after recovery from episodes of disturbed consciousness.

    Case 1: A 66-year-old man was found to have hepatic lesions in medical examination, which were diagnosed as hepatocellular carcinoma (HCC) with tumor thrombus in the inferior vena cava (IVC). He underwent liver resection and extirpation of the tumor in the IVC under total hepatic vascular occlusion, and it was found that the tumor in the IVC was floating with a pedicle attached to the venous wall. The IVC tumor was diagnosed as hematogenous metastasis to the IVC wall on histopathological examination. Case 2: A 73-year-old man underwent liver resection for HCC with underlying chronic hepatitis B. Five years later, bone and multiple lung metastases appeared, accompanied by a mass in the right atrium, which was found to be a metastasis to the IVC wall derived from tumor thrombus at autopsy. We report two cases of HCC with metastasis to the IVC and review the literature.
