  • Part II. Comparative Histometrical Study on the Splenomegaly between

    Fourty cases of autopsied congenital biliary atresia were examined histometrically. Histopathological findings of the liver, spleen and collateral vessels of the portal system were already reported Juntendo Med. J. 26 (2). Preliminary report on the histometrical study in CBA, Banti's syndrome and normal control were briefly stated in previous report. In histometrical study, volume percent of splenic sinus (Vs), total lenght of sinuses in a unit volume of the red pulp (Ls) and mean sectional area of sinus (Ss) were measured by Suwa's method. In this report, histometrical result on the splenomegaly in CBA, so-called Banti's syndrome and normal control were compared and discussed the similarity and difference between them in detail and refered to the course of the formation of the newly splenic sinuses (sinus hyperplasia). Results : 1) Progressing splenomegaly by severe obliteration of the intrahepatic portal veins due to marked biliary fibrosis of the liver. Histopathological findings of the spleen was very similar to that of so-called Banti's syndrome. 2) Splenic sinuses in CBA become finer and smaller than that of normal infant control according to the progress of splenic changes caused by icreased portal pressure 3) Histometrical study in normal control : The older the case become, the finer Ss changes. Vs and Ls were most spacuous and the longest in the teen-aged group in normal control. 4) Histometrical study in portal hypertension : The older the case become, the finer and the smaller Ss and Vs changes, and finer and smaller than that of each normal control. The older the case become, the longer Ls changes, and longer than that of each normal control. 5) Results as described above revealed the histodathological and histometrical similarity between CBA and so-called Banti's syndrome quantatively. 6) The course of the sinus hyperplasia may be the abnormal elngation of the splenic sinuses into the widened splenic cord following the initial proliferation of argentaffin fibers of the splenic cord. It is concluded that congenital biliary atresia is able to regarded as an infant cases of the so-called Banti's syndrome.

    283 patients with acute myocardial infarction (acute MI.) who were admitted to Juntendo Univ. Hospital during 1963 to 1976 were followed up. Since 1968 the total number of patients with acute M.I. tended to increase, but the total number of acute deaths did not increase. Sex ratio of the total patients was 5.2 males to 1 female. Age ranged from 29 to 87 years old (mean 63. 1). Mean age of females was 67. 3 years old and that of males 62. 3 years old. Acute death occurred in 27% (77 cases). Sex ratio of those acute death cases was 2.7 males to 1 female. Mean age of acute death cases was higher than that of all patients. About 60% of acute deaths occurred within 24 hours after the attack. The acute death was observed at a higher rete in the patients with extensive myocardial infarction. The other 206 patients, who had been discharged alive, were followed up for 2 to 14 years. Late death cases were 26%, alive cases 41%, and undetected cases 6%. The survival rate in 35 patients who had been followed up for over 10 years was poorer than that in recent 63 patients followed up for over 5 years. Long follow-up observation yielded the result that the prognosis of older patients was poorer than that of younger patients. Causes of the late deaths were cardiac-related ones in 55% and reattack in 26%. As regards physical activities of the survivors, 62% were in NYHAI0, 28% in II0 and 10% in more than III0. A half of the survivors continued some kinds of measures for prevention of secondary attacks. In conclusion, although the prognosis of younger patients with acute M.I.was not so poor, that of older patients over 60 years of age was poor both in the acute phase and the later phase.

    Serum carcinoembryonic antigen has been used in prognostic evaluation and monitoring of patients from Apr. 1976 to Dec. 1976 at Juntendo University, our study of 96 patients, in 54 patients with various cancer and in 7 patients with no cancerous disease also 29 healthy subjects were assayed by CEA Dinabot Kit. 1. CEA level of higher than 2.5ng/ml of plasma value was estimated positive case 2. Based on the above mentioned criteria, the CEA positivity was 46% in cancer diseases, 14% in non-cancerous diseases and 0% healthy subjects. 3. Raised CEA value in 14 out of 34 patients with stomach cancer was found, and has been suggested for primary monitoring in 73% of colorectal cancer. 4. The CEA positivity in patients according to Duke's classification were as follows : Dukes A : 0%, Dukes B : 50% and Dukes C : 88%. The CEA levels were higher in large tumor cases. These data showed a correlation between plasma CEA levels and the size and extention of the tumors. 5. After curative operations the CEA levels usually decreased over, but in non-curative operation the CEA value was retained on the same levels as before operation.
