    To study the reliability and validity of a questionnaire surveying quality of life (QOL) of patients with retinal pigment degeneration, we analysed the answers to first and second survey (161 and 111, respectively) of the patient group (Asebi kai). The reliability was confirmed by the proportion of agreement and Cohen's κ (kappa) coefficient for comparable items in the first and second survey. The construct validity of items in the questionbnaire was comfirmed by multivariate analysis (covariance structure analysis) of the second survey, although it may be necessary to research various models of QOL.

    This experiment was conducted to elucidate the effect of intraportal Interleukin-2 (IL-2) in combination with 5-Fluorouracil (5-FU) immunochemotherapy for the inhibition of liver metastasis in colorectal cancer. Two-week-old male Donryu rats (150-200g) received intraportal inoculation of ascites hepatoma AH130C (5×105 cells) were divided into 7 groups. Group 1 : no treatment, Group 2 : 5-FU (1.6-2.0mg/kg), Group 3 : 5-FU (1.6-2.0mg/kg) +IL-2 (0.115MU), Group 4 : 5-FU (1.0-1.3mg/kg), Group 5 : 5-FU (1.0-1.3mg/kg) +IL-2 (0.115MU), Group 6 : 5-FU (0.5-0.7mg/kg), Group 7 : 5-FU (0.5-0.7mg/kg) +IL-2 (0.115MU). All animals were sacrificed on the 14th day after the beginning of the experiment. The tumor volume of liver metastasis, serum value of CD4, CD8 and Natural Killer (NK) cells were calculated in each group. The tumor volume of the immunochemotherapy groups showed significantly reduction compared with those of the other groups. Serum CD4 was higher in the immunochemotherapy groups compared with those in the other groups, significantly higher in group 7 than in group 6. There were no significant differences in CD8 levels and NK activity. In addition, animal survival was significantly prolonged in the group 7 compared with that in group 6. These results suggested that intraportal immunochemotherapy is effectiveness in inhibiting liver metastasis of colorectal cancer.

    A survey of outpatients at the Juntendo Koshigaya Hospital was carried out. Our mental hospital was established in 1968, and it came to be administered by the members of the Department of Psychiatry, School of Medicine in April 1989. We have previously reported some clinical characteristics of our hospital in an analysis of outpatient data. In this paper, we compared two patient groups, those whose treatment began before April 1989 and those whose treatment began after that date. This study revealed that, after April 1989, we began to treat a wider of variety of disorders, and we received more referrals from other facilities. That is, the clinical characteristics of our hospital became similar to those of psychiatric treatment at the university hospital. We also surveyed the duration of treatment for each patient. The average treatment term was much longer than that of psychiatric treatment at university hospitals. This seems to depend on the lower rates for our consultation compared with those of other facilities, because our hospital has 221 beds (open, semi-open, and closed psychiatric wards), so if patients needed admission, we could also provide treatment at our own hospital. The two characteristics of clinical practice, cited above allow us to offer adequate clinical service to patients, however, we may have an increase in the number of outpatients. So we are concerned that patient waiting time may increase while consultation time may become shorter. We must consider various problems in the mental health of the suburban population in the suburbs of megalopolis, where our hospital stands and we will try to propose further considerations for these problems.

  • -Special reference to apical hypertrophic type-

    To clarify apical hypertrophy (APH), I compared family history, the participation of hypertension, ECG findings, clinical course, prognosis, left ventriculogram, and endomyocardial biopsy (EMB) findings in a total of 87 patients with APH and various types of hypertrophic cardiomyopathies. The subjects consisted of 28 patients with APH only, 24 with asymmetrical septal hypertrophy (ASH only), 13 with combined APH and ASH (APH+ASH), 12 with diffuse left ventricular hypertrophy (diffuse type) and 10 with hypertrophic obstructive cardiomyopathy (HOCM). Histological findings of EMB specimens were classified as O, 1+, 2+, for myocyte hypertrophy and disarray. LV wall thickness measured on end-diastolic frames of RAO-LV gram was compared with the maximal depth of negative T waves on ECG. Results were as follows : (1) Significant abnormality on EMB was found in 46% of patients with pure APH, and in 54, 54, 58, and 60% of those with APH+ASH, pure ASH, diffuse type, and HOCM, respectively. (2) EMB sampling from the left ventricle showed a higher incidence of abnormality than that from the right ventricle. (3) Participation of hypertension was relatively low at the 20% level in every group except in the APH+ASH Group, which was at 46%. (4) Although the number of patients with APH only was limited to 18 in this series, no cardiac deaths were observed and cardiac complications were noted in only two patients. (5) Maximal negative T depth/QRS voltage correlated positively to the ratio of mid ventricular thickness/apical thickness. Conclusion : (1) Certain APH patients could be categolized as a subtype of HCM, in whom hypertrophy and disarray were limited to the apical portion. (2) It is more useful to obtain EMB from left ventricle than from the right ventricle for diagnosis of HCM. (3) Regarding complications, the prognosis of APH remains unclear. Further study of the longterm prognosis is important because cardiac complications cannot be ignored. (4) The depth of negative T waves in APH patients seemed to be related to the disparity in the distribution of thickness from the middle to apical segments of the left ventricle.

    In this report, we analyzed 46 cases of bile duct injury caused by abdominal blunt trauma. We divided the patients into two groups; those with complete division of the common bile duct (CD) group and those with perforation of the bile duct (PP) group. There were 38 cases in the CD group and 8 cases in the PP group. Traffic accident was the major cause of trauma in both groups. The intervals between time of injury and surgical treatment were significantly longer in the PP group than in the CD group (p<0.05). Jaundice was notable in 8 cases (53.4%) two or more days after injury. The laceration of bile duct location was mostly at just above the pancreas in the CD group and the external hepatic duct near the bifurcation in the PP group. Choledocho-jejunostomy for the CD cases and simple closure with bile duct drainage for the PP cases were the most frequent surgical treatments in each group. Hepatic and pancreatic injury were often combined in both groups and portal vein injury caused poor prognosis.
