    Through the case study about a collaboration between hospital doctors and town doctors which is a type of medical cooperation, by focusing on “mutual understandings” by communication, the mechanism of process to establish a collaboration and to propose a diagnosis which enables effective and efficient management of communication to increase the probability of establishment of collaboration. If we find mutual understandings which are required to establish a collaboration and ways to evaluate whether the mutual understandings are grown, effective and efficient management of communication can be undertaken by a manager who intends to establish a collaboration with a targeted persons of other organizations. From the case study, which focus on a collaboration between hospital doctors and town doctors in Yokohama City, the mutual understandings, which are required to establish a collaboration, are defined and validity of a conceptual model which shows the process to establish a collaboration is confirmed. Moreover, a diagnosis is proposed that “by observing act of persons, state of mutual understandings can be evaluated.”
