    A retrospective study of 16 cases with visual disturbances resulting from head injury, seen in our clinic between the years 1969-1972, is described. A proposed endnasal-trans-ethomoidal operative technique was performed in all cases, and the following results were obtained.1) The patients consisted of 14 males and 2 females.2) The most prominent cause of visual disorders of the present series was the accident of motor cycle crashes.3) Once blindness occurred completely, their visual powers have never recovered, even if the operation procedures were performed safely in these 6 cases except one case.4) In 10 cases of having imcomplete loss of visual power, some remarkable improvements were gained after the operation.5) Having compared the time of operation with their results, they have been no difference in the series. 6) An otologist should be consulted early in all cases of visual loss resulted from head injury.

    The hypnotherapy was applied to the cleft palate for speech rehabilitation. Most of adult patients with cleft palate have the highly deteriorated articulation. In such cases, a functional velopharyngeal reconstruction was performed and hypnotherapy was indicated. The purpose of this treatment was to correct their habits of deformed articulation and to get normal speech. This therapy was practiced to three adult cases who acquired intelligible articulation in short time.

    A 47 years old male who complained of hoarseness and slight swelling of the right lamina of the thyroid cartilage was examined. A marked bulging of the right ventricular fold was seen. The right vocal fold appeared to have mobility. although it was covered by the bulged ventricular fold. The initial clinical impression was a laryngocele. The tumor was removed and histopathological diagnosis was a giant cell tumor. Giant cell tumor of the larynx is extremely rare and it has not been reported in Japan.

    We experienced a case of 50-year-old man who recently complained of dyspnea and found laryngeal stenosis due to granulation tissue. The removal of the granulation tissue was performed under the diagnosis of post-traumatic laryngeal stenosis. Postoperative histological examination of the granulation revealed the existance of the cancer cell. We are taught by this case that we must keep in mind the possibility of cancer in such a case.

    Relapsing polychondritis is a rare disease of unknown etiology. The distruption of the cartilagenous matrix is the predominant change of histopathology in this disease. An aditional case was reported. A woman, aged 48, complained of hoarseness with stridor and dyspnea, sore throat, and fever. After the hospitalization deformity of the left auricle, saddle nose, pain of the knee joints, episcleritis were noted. Impaired hearing w ith fullness in the ears was also present, and Bekesy audiometry revealed Jerger' type II. The deformed auricular cartilage was investigated electron microscopically, and being compared with the finding of the auricular cartilage obtained from a patient with chronic otitis media. Loss of small dense granules and thin fibrous structure in the matrix was the predominant change in the involved cartilage. Findings of the electron microscopic observation suggested that some metabolic disorder occurred in the matrix of cartilage, thereby releasing acid mucopolysaccharide, and inflammatory reactions followed.

    A case of relapsing polychondritis in a 68-year-old japanese female is reported. There was involvement of the ear, nasal and laryngeal cartilage. She noticed no eye, inner ear and joint symptomes. Elevated ESR, pathologic electrophoresis of serum and 3+ of CRP was recorded. Biopsy of the ear showed chondritis and perichondritis. She was treated with corticosteroid for 4 weeks, and it was effective. She is in good health, but saddle-nose deformity, atrophy of external ears and hoarsness is remained.

    A 40-year-old cook developed mild discomfort and swelling in the left half of the tongue one month betore As the mass slowly increased in size despite the administration of antibiotic (Lincomycin), the tongue was partially removed including the mass, in which an abscess was encountered. Histological examination from the wall of the abscess demon-strated the presence of sulfur granules and actinomyces, although the results of bacterial and fungus cultures from pus were negative. The postoperative course was unevenful, and the patient has remained symptom free over one year after operation.

    A case of multiple facial bone fractures by automobile accident was reported. In such a case, it is necessary to diagnose locations of fracture by accurate roentgenographic examination, to approach and fix with wire appositely according to the location, and to treat to get recovery functionally and morphologically.

    Sixty-two tympanoplasties were performed in the first one year after the opening of the Kitasato University Hospital. The postoperative hearing of 52 ears of them were measured repeatedly over 3 months after the operation. The hearing acuity of 65% of them came almost steady in about one month after the operation and that of the other was apt to fluctuate until 6 months. There was correlation between pre- and postoperative hearing, and its coefficient was 0.64**. The fact suggests further possiblities of some improvements in surgical technique. Bone-conducion hearing reduces more frequently in the cases with preoperative bone-conduction hearing loss in comparison with the cases without it. The columella technique did not bring so good result as we expected.

    The effect of ethacrynic acid upon the cochlear microphonics was studied by the differential electrode technique. The following results were obtained.
    1. With intravenous doses of 1mg/kg ethacrynic acid, the cochlear microphonics caused a slight depression accompanied by a gradual recovery. CM increased rapidly 50 minutes after the injection and then decreased gradually.
    2. With intravenous doses of 10mg/kg ethacrynic acid, the cochlear microphonics caused a rapid decrease to 63.3 % of the initial value within 30 minutes, to 49.3 % within 80 minutes, and then a gradual decrease to 43.7 % within 3 hours. It did not cause any recovery within 3 hours. But one of 5 cases caused the same variation of CM that the injection of 1mg/kg ethacrynic acid produced.
    Recent experimental evidence indicates that ethacrynic acid causes the damage of the intermediate cell layers of the stria vascularis and the outer hair cells. It is also known that ethacrynic acid causes an increase in the endolymphatic sodium concentration and a decrease in the endolymphatic potassium concentration. Therefore it was concluded that the depression of the cochlear microphonics caused by ethacrynic acid would be due to the ionic changes in the endolymph which is produced by the inhibition of the active sodium transport in the stria vascularis.

    Cephalexin was clinically applied to 404 cases with ear, nose and throat infections and the following results were obtained.
    1) Excellent or good clinical response was observed in 76.1% of them.
    2) Among the disease categories studied, furuncle of the ear and of the nose, acute tonsillitis and acute pharyngolaryngitis were the most responsive to cephalexin.
    3) The effective results were obtained in the most of gram possitive cocci infections and some of gram negative bacilli infections. No clinical response to pseudomonas aeruginosa was observed.
    4) The clinical results coincided well with in vitro sensitivity of the bacteria isolated from each patient.
    5) Side effects were observed in 6.6% of this series. Most of them were gastrointestinal disorders. In a few cases exanthema, dizziness or stomatitis were noted.
