    The author hypothesized that circulatory insufficiency in the brainstem and/or inner ears is the most probable mechanism whereby dizziness develops without known causes. Physical examinations of vertiginous patients for blood pressure, orthostatic regulation, and baroreceptor reflex showed that the disturbed homeostasis of blood pressure plays a critical role in the development of such dizziness. Hematological examinations showed that physical changes of blood derived from stress, such as increased serum cholesterol and enhanced blood platelet aggregation, alter the property of blood and induce circulatory insufficiency. The author believes that the imbalance in blood flow between both vertebral arteries, together with above mentioned systemic circulatory changes, leads to the onset of dizziness of unknown causes.

    Comparative study of serum protein in 14 patients with sudden deafness seen within 10 days (X; 3.9 days) after the onset, and 17 normal patients was performed using statistical techniques whose results are shown below:
    1) Student's t-test revealed that the mean of serum total protein in the sudden deafness group was significantly lower than that of normals (p < 0.05).
    2) It was concluded that albumin, α2-globulin andα1-globulin were asssociated with sudden deafness to a great extent when the two groups were analyzed by factor analysis.
    3) It was concluded that the order of the strength of association with the severity of hearing loss at the time of the first medical examination was 1; albumin, 2; α2-globulin and 3; α1-globulin as demonstrated by stepwise multiple regression analysis.
    A strong correlation was also observed betweenα1-globulin andα2-globulin using stepwise multiple regresson analysis.

    One hundred fifteen cases with Meniere's disease and sudden deafness were reviewed.
    The results were as follows.
    1) Average hearing loss of patients with sudden deafness (59.2 dB) was significantly poorer than the loss of cases with Meniere's disease (36.9 dB).
    2) Patients with Meniere's disease and atypical Meniere's disease showed more frequently C. P. or D. P. in the caloric test than those with sudden deafness.
    3) Cases with Meniere's disease more frequently noticed vertiginous attack after discharge than those with atypical Meniere's disease.
    4) There were no marked differences of the results in the caloric test between patients with Meniere's disease within 2 years and patients over 2 years.
    We might conclude that the prognosis of Meniere's disease could not be suspected from the results of caloric test and pure tone audiometry.

    It is already known that the concentration of serum amylase increases in inflammation of the salivary gland.
    Amylase-isozyme tests of urine and serum were performed initially in twenty-five cases of disease of the salivary gland observed during a period of one year (April 1979 through March 1980).
    The authors concluded that amylase-isozyme tests (chromogenic method) are valuable for differential diagnosis between inflammation and tumor of the salivary gland.

    The frequency of periesophageal abscess has decreased considerably in recent year because of the advance in antibiotic therapy. During last ten years, only one case of a typical periesophageal abscess was experienced in our clinic. The case was presented here and then anatomic, diagnostic, and therapeutic regimens of periesophageal abscess were discussed further.

    83 cases of head and neck malignant tumor were treated with synergistic combination therapy of 5 FU in dry syrup form, Vitamin A and irradiation from April 1977 to March 1980. The therapeutic effects of this combination therapy are summerized as follows after evaluating of the effects of other treatments which were performed simultaneously or successively.
    1) Oral 5 FU dry syrup is effective in the treatment combined with Vitamin A and irradiation.
    2) In T2 cases of the laryngeal cancer and regional lymphnodes metastasis there are many curable cases with this therapy only.
    3) This therapy is also effective for malignant tumor of epipharynx and maxillary cancer.
    4) This therapy is not so effective for cancers of oral cavity including tongue.

    4 cases of reticulosarcoma were reported that they have had the clinical findings of intra-cranial metastasis under their treatment.
    The treatment of case 1 (parapharyngeal space, stage 2) and 4 (ethmoid sinus, stage 1) were successful, but case 2 (nasal cavity, stage 2) and 3 (base of tonge, stage 2) were unsuccessful.
    The accurate diagnosis and active treatment for those patients were emphasized.

    An unexpected case of nasopalantine cyst and a cystic case of ameloblastoma originating from the maxilla were reported.
    The first case, 54 years old male, complained of right nasal obstruction. The cyst was removed by trans-bucco-labial approach and the progress of the patient after the surgery was good. No reccurrence was seen during the past three years. The second case, 77 years old male, had complain of painless swellung in the right side of hard palate. The tumor was extirpated by trans maxillary approach.
    The histological origin and clinical findings of the cyst in the maxillary region were discussed briefly from the base of a review of the literature.

    In treatment of peripheral facial palsy, 39 patients were treated with mecobalamin (Methycobal (R)), 21 without mecobalamin. The palsy of the patients with mecobalamin therapy returned to normal earlier than those without mecobalamin therapy. In cases of suprastapedial lesion, stapedial reflex of the patients administrated mecobalamin recovered earlier than those without mecobalamin. Mecobalamin was considered to be effective in the treatment of peripheral facial palsy.

    Hirano (1975, 1977) has proposed the concept that the vocal fold has a layer structure, consisting of multiple layers which have different mechanical properties.
    The present author studied variations of the layer structure along the length of the vocal fold and age-dependent variations of the layer structure. Futhermore, vas- cular network of the vocal fold was also investigated. The results were summarized as follows: I. Variations of the layer structure along the length of the vocal fold.
    1. The layer structure of the vocal fold varies along the length of the vocal fold.
    2. The anterior and posterior macula flava are continuations of the intermediate layer of the lamina propria.
    3. The cover is the thickest at the middle and becomes thinner towards the anterior and posterior ends of the membraneous portion of the vocal fold. The intermediate layer is very thick near the ends and is the thinnest at the middle of the membraneous portion. II. Age-dependent variations of the layer structure A) Development of the layer structure
    1. In a newborn, the entire lamina propria of the mucosa looks uniform in structure.
    2. In young children, there are more fibroblasts and less fibrous components in the lamina propria of the mucosa than in adults.
    3. Fibers in the vocal ligament are supposed to be chiefly produced by the fibroblasts in the macula f lava and partly by those in the lamina propria of the mucosa.
    4. As one grows, the fibrous components of the lamina propria increase and the layer structure of the vocal fold becomes matured after puberty.
    B) Aging of the layer structure
    1. The cover tends to become thicker with aging.
    2. The intermediate layer becomes thinner with aging.
    3. The deep layer becomes thicker with aging.III. Vascular network of the vocal fold.
    1. The vessels of the free edge of the vocal fold come in from the anterior or the posterior end of the vocal fold.
    2. Around the edge of the vocal fold, most vessels run parallel to the longitudinal axis of the vocal fold.
    3. At the middle of the membraneous portion, vessels from both ends anastomose with each other to form a network.

    Patients with otorhinopharyngolaryngological infections were treated with 750 mg of ABPC given in three divided doses, either alone or with 3 capsules of Neuer, for 4 or more days. The patients were observed for development of gastrointestinal dis- orders including loss of appetite, nausea, vomiting, gastric distress, loose stool and tarry stool.
    The overall incidence of these disorders was considerably lower in patients who were concomitantly given Neuer (11.7 e. 6 in 51) than in patients given ABPC alone (26.9%, i. e. 14 in 52)
