    A 72 year old man with otalgia, long standing chronic otitis media in his left ear, gait disturbance, vertigo, and vomiting was reported. Detailed examination, including CT and cerebral angiography, revealed that the vertigo resulted from the cerebellar infarction occuring simultaneously with acute mastoiditis and acute external otits. Serous otorrhea with a positive glucose detecting test in his left ear and severe contact pain limited to the second and third trigeminal area in his left ear were also detected in this patient. The former symptom was considered to be caused by acute external otitis (Psuedomonas aeruginosa infection) the latter one considered to be casued by cerebellar infarction. Antibiotic resistant acute inflammation and increase of thrombus formating trend had an important role in the occurence of the cerebellar infarction, in addition to arteriosclerotic changes of vertebulovasillar artery due to hypertension, heart disease and diabetes mellitus.

    We recently treated 13 cases of sudden deafness with a combination therapy of steroid and prostaglandin E1. In these cases, a gradually reduced dosage of a steroid (dexamethazone sodium phosphate, initial dosage 40 mg) and urokinase (60000 units/day) and low molecular dextran (250 ml/day) were given for a week. In additon, patients were intravenosly treated with prostaglandin E1 (60μg/day) in the evening for a week. This therapy resulted in complete recovery or marked improvement in the 13 cases, with an improvement rate of 76.9%. Only two of the cases showed no response to this therapy.

    A rare case of a glomus jugulare tumor with a neck lymphnode metastases was reported. The subject was an 11 year old girl, who, for six months had complained of pain in the left ear and hearing loss. A radical mastoidectomy and decompression operation was performed due to the sudden onset of severe facial nerve palsy and nystagmus. Granulomatous tissue was found in the left middle ear cavity, in which subsequent pathological examiniation disclosed a glomus tumor. Post-operative radiotherapy was done. After three years, lymphadenopathy appeared in the ipsilateral neck without local reccurence. Lymphadenopathy revealed metastases of the neck lymphnode. The means of spreading and metastasis of glomus tumors is discussed here.

    Cartiliginous tumors of the hyoid bone are extremely rare. We discovered a case of chondroma originating from the right greater come of the hyoid bone. The patient was a 40 year old man who first noticed asymptomatic swelling in the right submandibular area ten years ago. The findings of CT and sialo-CT were useful for differential diagnosis from tumors originating in the submandibular gland. The tumor, and part of the right greater come of the hyoid bone, was resected surgically. This is the fifth case of a chondroma originating from the hyoid bone in world-wide literature.

    Three cases of large vocal fold polyps, severe vocal fold polyps, and large laryngeal papilloma were successfully treated by using a snare in laryngomicrosurgery techniques. Postoperative analysis showed that excellent voice restoration was obtained in all cases. It is thought that the snare minimizes excess removal of the mucous membrane, and keeps the amount of bleeding down. In this paper, the method is demonstrated with clinical case reports and discussed from the view point of the flexibility of the vocal fold mucous membrane and surgical manipulations that may influence postoperative results.

    We have employed a newly designed self-retaining therapeutic flexible laryngofiberscope, in conjunction with a VTR system, to perform phonosurgical resection of laryngeal lesions, such as vocal fold polyps. The self-retaining system allows bimanual manipulation in which forceps can be manipulated with one hand while the tip of the scope in centered on the lesion with the other. In this paper, some details of this laryngofiberscope are presented and a discussion emphasizing the clinical application in biopsy and removal of vocal fold polyps and other laryngeal lesions.

    We have been using ceramic for both partial ossicular replacement prosthesis (PORP) and total ossicluar replacement prosthesis (TORP) since 1982. In 1987, postoperative hearing results after 6 months of follow-up were assessed according to the criteria of Clinical Otology, Japan, for the 16 patients who have undergone this replacement. The success rate was 85.7% in total, 100% for PORP, 66.7% for TORP. These results are comparable to the findings at other centers. In order to improve on the shortcomings of previous ceramic prostheses, we designed two types: Hokudai-shaped PORP, and TORP. The hearing results with these new types in 9 patients was satisfactory.

    In Japan, parasitic laryngo-pharyngitis is not a common disease. Recently we treated a case of acute laryngitis caused by a small trematode, Clinostomum complanatum. The patient was a 54 year-old female who complained of a sore throat and an abnormal sensation in her throat which felt like a foreign body, after eating raw roach fish. The parasite was motile and was attached to the right arytenoid mucosa. After removal of the parasite under direct laryngoscope, the symptoms disappeared immediately.

    Dix and Hallpike reported previously of a disease state in which vertigo occurs in association with a specific position of the head in space, and is accompanied with rotatory nystagmus of transient duration. These authors postulated that the condition is distinct from Meniere's and described it as “benign paroxys-mal positional nystagmus”(BPPN). The present author noticed similar attacks of BPPN occurring frequently during the course of Meniere's disease. The purpose of this paper is to give a brief account of these cases. Six cases of BPPN were seen during 1988. In all six cases, there first occurred a typical attack of Meniere's disease, accompanied with horizontal nystagmus andsmarked deviation for the stepping test. In 4 of the 6 cases, the rotatory nystagmus of BPPN was noted to occur as the Meniere's attack was subsiding. In the other two cases, after a period of clinical quitescence following an initial Meniere's attack, a second attack started with BPPN. The duration of the BPPN attacks ranged from 4 days to 4 months. The horizontal nystagmus of Meniere's disease and the rotatory nystagmus of BPPN generally were in opposite directions. There were no striking left-to-right differences in either audiograms or calorigrams. Gait tests performed during an episode of BPPN demonstrated forward and backward gait to deviate to the same direction in many instances. This was interpreted as indicating that probably, and mainly as a result of excitation of statoconia, there was a tendency to lateropulsion, but not to laterotorsion. During the course of BPPN there occasionally were transient reversals of the rotatory nystagmus direction with the same postion of the head. This phenomenon was arbitralily called reversed nystagmus of delayed onset. In 4 of the 6 cases, the patient's received hospital treatment. During the year of 1988, a total of 97 patients were admitted to the hospital with an attack of Meniere's disease. Thus it follows that of these, 4.1% went on to develop BPPN. BPPN is considered to be a condition occurring during Meniere's disease, a process which involves the entire inner ear, in which only the statoconia are in a state of excitement. Therefore, it should be regarded as a pathologic condition of the inner ear rather than an independent disease entity.

    Samples were taken from a 40 year old man who had a sore throat with signs of only localized ulceration in both the mesopharynx and hypopharynx. Pathological findings of the pharynx include necrotizing granulation tissue with giant cell and vasculitis with fibrinoid necrosis. ESR was elevated, positive CRP, increased serum fibrinogen, CHSO and increased IgA and IgG levels were found. The patient was only treated with corticosteroids, and is presently in complete remission. He was diagnosed as having an early stage of Wegener's granulomatosis, based on the pathological features, laboratory findings, and clinical course.

    8 children with mumps induced bilateral deafness, who had consulted 2 paedoaudiologic clinics in Fukuoka between 1970 and 1987 are evaluated. They represent 14.5% of the number of sensorineural deaf ness (SNHL) and 0.3% of the bilateral SNHL cases who visited the same clinics over this period. In two cases, both with complete hearing loss in one side, the degree of deafness in the other side changed after a first consultation. In one case, hearing gradually diminished in the better side, but recovered with treat- ment. In the other case hearing changed for the worse despite treatment. 2 cases with total deafness were subjected to a rehabilitation program immediately after the onset of deafness, and aquired language appropriate for their age group at an evaluation prior to entering primary school.

    Asymmetry of nasopharyngeal structure in CT scans, such as blunting of the lateral pharyngeal recesses (LPR, fossa of Rosenmuller) and depression of the parapharyngeal space, is regarded as an essential sign in the diagnosis of malignancies or aggressive inflammatory processes. The rate of nasopharyngeal symmetry, however, has been rarely reported so far. I examined axial CT scans of the nasopharynx of 220 patients who did not have any complaints of the nasopharynx or oropharynx. LPR, tube orifices, torus tubarius, intrapharyngeal muscles, paranasopharyngeal spaces, and deeper musculofacial planes wre examined. The asymmetry rates were 17.8%, 15.8%, 16.8%, 3.7%, 5.5%, and 8.0% respectively. The former three superficial landmarks were more often asymmetric than the latter three plane tissues. There were no differences in symmetry between patients with histories of sinus surgery or facial fracture and others. The loss of symmetry of the nasopharyngeal structures, not only the deeper ones, but the superficial ones, seems to be a useful sign in differentiating the recalcitrant pathologies.

    NAC has been performed in 13 cases of non-treated squamous cell carcinoma of the hypopharynx. According to our NAC regimen, the cases were divided into 2 groups: the VPCP group (n=7), in which VCR, CDDP and PEP were used; and the ACP group (n=6), in which ADR or THP-ADR, CDDP and PEP were used. Therapeutic effects of NAC in 13 cases were found as CR in 1 case, PR in 8 cases, NC in 3 cases and PD in 1 case, with an effectiveness rate of 69%. Cumulative survival rates using the Kaplan-Meier method in 13 cases were 75% at 12 months and 62% at 52 months. These results suggest that NAC may be incorporated into a therapeutic method for treating hypopharyngeal carcinoma. Further investigation of this method in essential to improve its long term results.

    A high frequency audiometry system (9-16 kHz) which uses a condenser headphone (STAX SR-A Professional) was used to perform audiometry in three groups of subjects; 108 healthy subjects, 18 subjects with histories of noise exposure, and 34 patients who were receiving streptomycin injections. The results were as follows. 1. The high frequency hearing sensitivity of the healthy subjects dropped with age and with increases in frequency. The median thresholds of healthy 60-69 years olds at 250Hz were only 15dB (HL) larger than for 10-19 year olds. By contrast, the difference in median thresholds at 11kHz between 10-19 year old and 60-69 year old subjects was found to be a much larger value of 60dB (SPL) The correlation coefficient of age to hearing sensitivity was higher at the high frequency than at the low frequency. 2. The high frequency hearing sensitivity of noise exposed subjects was significantly poorer than in the normal subjects. The hearing sensitivity of noise exposed subjects who showed normal values at 250kHz-8kHz were poorer for the high frequencies. 3. It was recognized in two cases that streptomycin injections noticibly worsened the hearing sensitivity for high frequencies. In conclusion, measurement of hearing sensitivity above 8kHz seems to promise both better description and differentiation of noise-induced hearing loss and drug-induced hearing loss.

    We examined a case of a 69 year old male who developed an acinic cell tumor in his left maxillary sinus. He underwent partial maxillotomy on the left side and subsequent radiotherapy at another clinic. Although the tumor disappeared, he visited our clinic thereafter for a recurrence from the medial wall of the left maxillary sinus to the left nasal cavity. The medial wall was incised with a left lateral rhinotomy. About one year and two months later, a local recurrence was observed on the superior wall of the left maxillary sinus. The tumor was scraped along the superior wall of the left maxillary sinus. When we attempted to treat recurrence of the tumor with external application of 5-FU ointment, the tumor disappeared and no reccurence has been observed for the last 9 months. We report this case here and discuss the treatment method.

    The results of a comparison study of malignant tumors of the head and neck in Thailand have been compared with research data from the Japanese National Cancer Center. In our research of 1985, the total number of cases in Thailand was 835 and in Japan, 1353. The order of frequency in Thailand is as follows; oral cavity cancer (46.8%), nasopharyngeal cancer (22.6%) and laryngeal cancer (14.9%). In Japan; oral cavity cancer (33.9%) laryngeal cancer (30.4%) and maxillary cancer (11.30%). The statistical significance were evaluated using an χ2 test (p<0.05) for both regions. By sex, there were two to three times as many male patients as female, and the incidence among Japanese males was 1.5 times higher than Thai males. Of oral cavity cancer in Japan, tongue cancer had the largest proportion, however, in Thailand, cancer of the oral membrane, gingiva and lip cancer predominated. It is conjectured that this is due to the practice people in Thailand have of chewing a tobacco-like leaf called betel. Several considerations are made about why there was a high incidence of nasopharyngeal cancer in Thailand whereas Japan had a high incidence of laryngeal cancer and maxillary cancer.

    The following four multidrug combination chemotherapy regimens were administered to 74 patients with cancers of the head and neck at the Department of Otorhinolaryngology of this hospital during the period, March 1983 to March 1988. 1. CDDP, Peplomycin (PEP) 2. adriamycin (ADR), CDDP, PEP 3. vincristine (VCR), CDDP, PEP 4. CDDP, 5-FU. The subjects were 53 men and 21 women. The age distribution was from 39 to 77 years of age. Histological analysis showed 71 cases to have squamous cell carcinoma. The response rate for all the subjects was 43; with 13CR cases and 19PR cases. The response rate for previously non-treated cases was 51% with 7CR cases and 15PR cases. For the previously treated cases, the response rate was 32% with 6CR cases and 4PR cases. The response rate breakdown by treatment was as follows: 1 43%; 2 33%; 3 56%; 4 0%. When the Kaplan-Meier method was used to examine the survival rate for the previously untreated cases with high efficacy ratings, the 5-year survival rate was 47.2%. The above results show multidrug combination chemotherapy to be very useful for the treatment of advanced cancer and recurrent cancer.
