    Two cases of pleomorphic adenoma of the parotid gland combined with ipsilateral facial nerve paralysis were reported. Diabetes mellitus was considered to be responsible for the occurrence of facial palsy in case 1. In case 2, direct invasion of the tumor to the facial nerve was considered to have caused facial nerve paralysis. This second case might be a rare case of pleomorphic adenoma which gave rise to distant metastasis in the thyroid gland, as there was evidence of infiltrative growth. We believe this case was one form of a malignant mixed tumor similar to the histologically benign metastasizing mixed tumor as reported by Batsakis in 1982. Since persistent facial nerve paralysis associated with an ipsilateral parotid tumor is considered a definite criterion of malignancy, a restoration of facial nerve function is not usually attempted. However, from our experience in these two cases, we conclude that the facial nerve should be restored in every case of facial nerve paralysis associated with a parotid tumor.

    A rare case of osteosarcoma originating in the base of the skull was reported. The patient was a 38-year-old woman who had multiple cranial nerve paralysis on the right side. She showed a marked swelling of the right epipharyngeal region without palpable neck masses. Repeated histological examinations revealed chronic inflammation but did not reveal malignant tumor. Clinical findings, radiographic findings, CT scan, Ga scintigraphy and bone scintigraphy strongly indicated a malignant tumor of the epipharynx. She then received telecobalt irradiation of 6000 rads with no improvement. Final histological examination revealed the osteosarcoma of the epipharynx. She died from brain stem metastasis of the tumor. Literature related to osteosarcoma of the head and neck region are reviewed.

    This paper discusses the clinical profile of tinnitus patient where no hearing-loss is present. Tinnitus of this category can be named as non-deafness tinnitus or tinnitus without hearing loss. Ten to twenty percent of patients with tinnitus have no hearing-loss. Clinical characteristics (age, sex, timbre, pitch and loudness etc) of non-deafness tinnitus were more similar to that of tinnitus concurrent with a conductive hearing-loss than that of sensorineural hearing-loss. Nevertheless, the origin of non-deafness tinnitus can be labyrinthine or retro-labyrinthine.

    Eighty-seven patients complaining of abnormal sensation in the throat were treated with Methycobal® for 4 weeks 1500μg/day, single dose. The treatment was successful in 31 patients (35.6%). The drug was more effective for the patients of CMI region III and IV. We bypothesiged that Methycobal® acted on the disordered peripheral nerves surrounding the throat.

  • Preparation of Specimens and Estimation of Mechanical Properties of the Human Vocal Fold

    Scanning acoustic microscope (SAM) gives an image which reflects not only microstructure but also mechanical properties of the object. We have attempted to use SAM in investigating the mechanical properties of the vocal fold. This paper focuses on two aspects: preparation of specimens for SAM and hardness of the tissue components of the human vocal folds. Four fresh human vocal folds were used in this study. From each vocal fold, three kinds of specimens were prepared: a thick frozen section of 1mm in thickness, a thin frozen section of 10μm in thickness, and a paraffin-embedded section. Additional paraffin-embedded sections obtained from 5 larynges fixed in 10% formalin were also investigated. The results are as follows: 1. In order to investigate mechanical properties, such as elasticity and viscosity, of vocal fold tissue, thin and thick frozen sections are recommended. 2. Relative stiffness of different tissue components can be determined even with paraffin-embedded sections. 3. Among the tissue components investigated, the fibrous component was the hardest, the epithelium came next and the ground substance was the softest. 4. The hardness of each layer of the mucosa was in the following order: intermediate and deep layers of lamina propria>epithelium>superficial layer of lamina propria.

    The projections to the nucleus ambiguus (Amb) were examined by means of the anterograde and retrograde axonal transport of wheat germ agglutinin-horseradish peroxidase (WGA-HRP) in 89cats. The existence of direct projections from the bilateral nucleus of the tractus solitarius (nTS)(except the rostral portion), the contralateral Amb, the contralateral reticular formation around Amb, the ipsilateral nucleus parabrachialis (nPB)(mainly the nucleus parabrachialis lateralis) and the lateral and ventrolateral parts of the bilateral periaqueductal gray (PAG) to Amb was identified. On the other hand, nTS, nPB and PAG were connected to each other directly. Descending fibers from the orbital gyrus to nTS and nPB were observed. Direct neuron connections between the dorsal, ventral lateral cortical bank of the cruciate sulcus (the pericruciate cortex) and PAG were also recognized. However, no cortical fibers directly connected to Amb were determined in this study.

    Pranoprofen (225mg/day) was given per os for 7 days post operatively in the patients who received surgery in otolaryngological field (i. e. tympanoplasty, Caldwell-Luc operation, tonsillectomy etc.). Effects of this drug for pain, inflammation, fever etc. were studied. Final global inprovement rate (FGIR) was excellent effects found in 16.1% of total subjects (113 patients). Excellent to moderate effects was found in 92.9% of total subjects. Positive effects (excellent-slight) was found in 99.1%. Side effect was observed only in one patient (0.88%) who showed bronchial asthma like attack. From this study it is considered that Pranoprofen is one of the most useful drug.

    In order to evaluate the adjuvant effect of Bestatin, an immunomodulator, on cancer, a randomized controlled study was performed in 365 cases with squamous cell carcinoma of the head and neck. Patients were initially divided into 2 groups, i. e. one for curative cancer therapy alone and the other for curative cancer therapy plus adjuvant Bestatin therapy. Bestatin (30mg/day) was given for 2 years together with curative cancer therapy. The number of evaluable cases was 325 in total; 165 in the Bestatin adjuvant group and 160 in the control group. The prognoses were followed up during the period of 2 years after initiation of therapy. The percentage of absolute 2-year survivals in the Bestatin adjuvant group was 80.5%, while 70.9% in the control group. There was a tendency toward difference between 2 groups (p<0.1). The survival time and relapse free period were prolonged in the Bestatin adjuvant group when compared with the control group. Mild reversible side effects were observed in 6 patients in 181 cases of the Bestatin group (3.3%). Bestatin therapy was considered to be a useful adjuvant treatment for the head and neck cancer.

    A newly developed antibiotic preparation, 1% CMX Otic Solution was evaluated for its clinical value, with the following results obtained. The effectiveness rate of 1% CMX Otic Solution was 89.9% (98 cases out of 109), that is, 93.0% (66 cases out of 71) with acute exacerbations of chronic purulent otitis media and 72.2% (13 cases out of 18) with chronic otitis media. With respect to the microbiological effect of 1% CMX Otic Solution, it was observed that microorganisms were eliminated by 1% CMX Otic Solution in 94 out of 109 cases (86.2%), that is, 94.1% with S. aureus and 77.4% with P. aeruginosa. No side effects such as shock were observed in any case. Thus, 1% CMX Otic Solution has been judged as a remarkably useful topical otic antibiotic preparation.
