    Of all tinnitus patients, 11.2% had normal hearing. Three hypothesis of the mechanism of this kind of tinnitus were presented: (1) objective tinnitus,(2) tinnitus due to some organic disorders in the auditory pathway which could not be detected by regular hearing test,(3) tinnitus due to psychological problems. The appropriateness of these hypotheses are discussed.

    Two cases of post-traumatic cerebrospinal fluid rhinorrhea caused by traffic accident were reported. The site of the fistula was the posterior ethmoid cell in the first case, and the frontal sinus in the second. For diagnosis of CSF rhinorrhea, RI counts of the pledgets in the nasal cavity, as well as sagital and coronal tomographies were available. As treatment, a transnasal approach was made, and the fistulae were successfully closed using an operating microscope. The fistulae were sealed by the fascia lata using a fibrin sealant (Tisseel®), and continuous spinal drainage was done pre-and post-operatively.

    The treatment of twenty four patients suffering from hypopharynx and cervical esophageal cancer during a six year period were reviewed. The age of these patients ranged between fourth and eighth decade; seven patients were in their eighties, six in their seventies or sixties. Thirteen patients were female, and eleven were male. The primary lesion observed in these patients were in the pyriformis sinus (7), postcricoid (5), cervical esophagus (5), lateral wall of hypopharynx (4), or posterior wall (3). The size of the tumor in primary examination were T-1 in four, T-2 in eighteen, T-3 in two patients. The regional lymphnode were palpable in thirteen patients. Radical radiation therapy were performed in eight patients and five of them are alive at present. Radiotherapy for primary lesion combined with radical neck dissection to regional lymphnodes were performed in three patients (T-2, N-2), and none of them is alive. After 50 GY radiation, extripation of tumors were performed in thirteen patients and six of them are alive. Pharyngogastrostomy is recommended as a method of hypopharynx reconstruction.

    We studied the acoustic characteristics of a spiral cavity in relation to the attenuation properties of the ear plugs with a spiral cavity (Takeda's ear plugs, UNTONE). A test was carried out with microphones placed in a model resembling an enlarged cochlea. Through the spiral air canal, pure tones (0.5, 1, 2, 3, 4, and 6 KHz) and white noise at 90 dB SPL were delivered. The output of the microphones were studied using an oscilloscope. Microphone 2 (MIC-2) was placed where the sound from the source was at a minimum incident angle in the 1 st turn. MIC-1 was placed closer to the sound source than MIC-2 by 3cm, and MIC-3 was placed 25cm further than MIC-2, in the second turn. Simultaneous mesurements were obtained. At 3 and 4 KHz, MIC-2 monitored the largest sound pressure, whereas this was not observed at 2 KHz or below or with white noise. MIC-1 monitored the largest sound pressure with a 6 KHz tone, presumably due to the fact that the shorter sound wave were more attenuated further down in the spiral canal. Depending on the structure of the air canal, resonance may influence our measurements.

    Mucosal malignant melanoma arising in the mucosa of the head and neck region is an infrequently encountered clinical entity. The only curative treatment for melanoma is surgical. Very wide excisions in order to obtain adequate margins have been advocated for melanoma but have no place in head and neck melanoma treatment because of anatomical considerations. For these reasons, radiotherapy, chemotherapy, and immunotherapy has been advocated for melanoma of the head and neck. Recently we investigated, for musocal malignant melanoma of head and neck, some combinations of chemotherapy regimens (VBM, VMBM, HAB, and VMB) and obtained good results. Four patients with mucosal malignant melanoma were entered in the study. Objective partial responses were seen of two of two patients receiving VMB, two of two receiving VMBM, zero of one receiving HAB, and one of three receiving VMB. Although there was significant toxicity, it appeared tolerable, and there was no drug death.

    An endolymphatic-mastoid shunt procedure was performed in 13 cases (14 ears) of MENIERE'S disease refractory to medical treatment. A postoperative follow-up of 1 to 5 years was done. 85% (11 cases) of these patients obtained a relief of vertigo. In 8 of these 11 cases, one to four small attacks within 6 months after shunt surgery were experienced. Hearing was improved in only 14.3% (2ears), unchanged in 28.6%, and became worse in 57.1%. In glycerol positive patients, postoperarive hearing was improved or stabilized. Tinnitus and ear fullness were unchanged in 60%. In accordance with AAOO 1972 criteria, there were 14.3% class A, 21.4% class B, 50% class C, and 14.3% class D results. Ideal result were obtained in class A and B patients (35.7%). Endolyphatic shunt surgery seems to be effective in the treatment of vertigo, and possibly improves or stabilizes hearing. The procedure is conservative and should be considered before destructive surgery for intractable cases of MENIERE'S disease.

    A 44 year-old female with a PLUMMER'S disease was reported. She complained of anteriot neck tumor, easy fatigability and weight loss etc. An elastic hard mass, 4 × 3 cm sized, was noted on the isthmus of the thyroid gland. The thyroid functional test presented a hyperthyroidism. The thyroid scintigram revealed a hot nodule on the same part of the thyroid gland. Surgical removal of the thyroid nodule was carried out and the thyroid function was slowly changing to an euthyroidism.

    The author presented a new concept in the development of the phonatory organ of vertebrates. This concept was based on his own studies and other works in comparative anatomy. The evolution of the phonatory organ is developed from the same trunk from lung fish to reptile in the evolutionary tree, and divides into two branches. One branch is the route from reptile to bird, the other branch is the route from reptile to mammal. In the first branch, the lower part of the trachea just attached to the air sac develops to create the syrinx in the bird and is called the air-bladder system. In the second branch, the upper part of the trachea developed into a preventive mechanism for aspiration, forming the vocal fold, and is called the larynx system.

    One case of nodular fasciitis was reported. The patient was a 59-year-old woman who complained of a painless nodule (2cm. in diameter) in her right lower neck. Because the nodule gradually increased, a biopsy was performed. A non-malignant muscular tissue was found on histologic examination. The lesion had not changed in size for one year, but it grew rapidly and became tender. The lesion was removed under general anesthesia. The lesion, which measured about 5cm. in its greatest diameter, was fixed to the sternocleidmastoid, omohyoid and juglar vein. There has been no recurrence nine months after surgery.

    Ultrasound in many branches of modern medicine has become a very helpful and proven diagnostic means, whereas in otorhinolaryngology this method up to now rarely has been used as a standard routine examination. Diagnostic A-mode ultrasonography is a useful diagnostic tool in the evaluation of patients with paranasal sinus diseases. It is a simple, reliable and non-dangerous method for the diagnosis and follow up of maxillary sinusitis and the early diagnosis of maxillary tumor.

    The therapeutic efficacy, safety and usefulness of Aspoxicillin (ASPC, TA-058) were objectively compared with those of Sulbenicillin (SBPC) in 187 patients with acute suppurative otitis media, acute exacerbation of chronic suppurative otitis media and chronic suppurative otitis media in a well controlled comparative study at 47 institutions in Japan. Patients were given daily 2g of ASPC or SBPC in 2 divided doses by intravenous injection for, in principle, 7 days. The results were as follows: 1. In all cases, the clinical effectiveness evaluated by the attending physicians and the evaluation committee, and the usefulness in ASPC group were significantly superior to those of SBPC group respectively. 2. In the case of acute suppurative otitis media, the ASPC group was significantly superior to the SBPC group in the clinical effectiveness evaluated by the attending physicians and the evaluation committee, and the usefulness. 3. In the case of acute exacerbation of chronic suppurative otitis media, the ASPC group was significantly superior to the SBPC group in the clinical effectiveness evaluated by the attending physicians and the evaluation committee, the bacteriological effect and the usefulness. 4. In the case of chronic suppurative otitis media, the SBPC group was significantly superior to the ASPC group in the clinical effectiveness evaluated by the evaluation committee. 5. Adverse reactions were observed in 1 of the 93 cases (1.1%) of the ASPC group and in 3 of the 90 cases (3.3%) of the SBPC group. The results stated in the above suggest that ASPC is a useful antibiotic for the treatment of suppurative otitis media infections.

    For the purpose of hyperalimentation, 17 cases of ENT were fed on Clinimeal® by way of feeding tube. Abdominal fullness, nausea, vomiting and diarrhea were complained as side effects, but could be improved by decreasing the dropping velosity. It is notable that diarrhea is rare in it's occurrence. Any severe nutritional depletion and functional disturbance of organs were not found in these cases. From these results, it is concluded that this diet is useful clinically and nutritionally, when taken for a long term as well as a short term.

    Clinical effectiveness and safety of KBT-1585 in the treatment for sinusitis was studied. 1. Thirty-two patients entered into the trial and they consisted of 10 cases with acute sinusitis, 9 with acute exacerbation of chronic sinusitis and 13 with chronic sinusitis. 2. The efficacy rate judged by the doctor in charge was 67.7% (21/31), by the standard criteria was 65.6% (21/32). 3. The bacteriological responses of patient were favorable in 90% (18/20). 4. As the side effect, itching was observed in 1 case out of 32 cases (3.1%). No abnormal laboratory findings were observed. From the above results, KBT-1585 was considered to be a useful drug in the treatment for sinusitis.

    The distribution of aspoxicillin (ASPC: TA-058) and sulbenicillin (SBPC) into middle ear effusion and petrosal mucous membrane was studied in experimental otitis media in guinea pig infected with Streptococcus pneumoniae. ASPC and SBPC were administered at a dose of 40mg per kg or 100mg per kg of body weight by intravenous or intramuscular injection. ASPC reached higher level both in the middle ear effusion and the petrosal mucous membrane at its peak than SBPC, and the level was higher than SBPC at every time in every dose. ASPC had a longer term to surpass MIC's 75 than SBPC against various clinical isolates including Staphylococcus aureus, Streptococcus pneumoniae, Streptococcus pyogenes, Haemophilus influenzae and Bacteroides fragilis.
