The
in vitro chemosensitivity of human head and neck malignant tumors was examined and analyzed in relation to several histopathologic factors. Histologically, malignant lymphomas and squamous cell carcinomas were sensitive to each examined anticancer drug. In squamous cell carcinomas, poorly differentiated tumors were more sensitive as compared to well or moderately differentiated tumors. DNA aneuploidy was found in 71% of head and neck squamous cell carcinomas. There was a tendency that poorly differentiated squamous cell carcinomas with DNA aneuploidy were most sensitive to each examined drug. The mean number of nucleolar organizer regions (AgNORs) in 81 squamous cell carcinomas was 4.58. In tumors with poor differentiation, the mean AgNORs number was higher than tumors with well or moderate differentiation. Tumors with higher AgNORs number seemed to be more sensitive to anticancer agents.

Presented in this paper are the results of treatment of 42 secondarily treated patients with squamous cell carcinoma of the oral cavity. Five year-survival rate was 44% in 9 patients previously operated on and 53% in 28 patients previously treated by radiation. Of these 28 patients, the 5-year survival rate was 44% in 18 patients with nodal recurrences. Their survival rate was lower than T1T2pN+patients who had surgery as the primary treatment. Patients previously treated by radiation had a higher frequency of nodal metastases to mid-lower jugular site and a higher percentage of distant metastases than T1T2pN+ patients primarily operated on. This seems to be the reason for the low survival rate in patients previously treated by radiation.

Thirty five cases with head and neck carcinoma who had have treatments in other clinics were secondarily treated in our clinic for seven years from 1985 to 1991. Papillary carcinoma of the thyroid gland and adenoid cystic carcinoma of the submandibular gland were relatively frequent. Results of surgery for adenoid cystic carcinoma was not satisfactory. For carcimonas of the oral cavity, the pharynx and the larynx, conservative surgeries rarely succeeded and the result of treatments was poor. Misunderstanding of tumor extent, interruption of teatment and high malignancy of recurrent carcinoma should have been the problems. Those results suggest that the secondary case need a wide removal with a combination of radiotherapy or chemotherapy for carcinomas of the submandibular gland, oral cavity and pharyngolarynx.
