We treated twenty-six patients with nasopharyngeal carcinoma who consulted our department from January 1976 to September 1986. The most frequent initial site of symptom was the neck, followed by the nose and the ear. The period between the appearance of aural symptoms and the consultation was less than 12 months. The period between the consultation and the diagnosis of nasopharydgeal carcinoma was less than 13 months in pathents with aural symptoms. Eustachian tube function exhibited ventilatory dysfunction and mucociliary dysfunction caused by nasopharyngeal carcinoma. These cases were difficult to diagnose because the initial symptoms were unclear and varied. Prognosis was poor. Finding a means to diagnose early in the disease is highly desirable.

Engel (1966) introduced 13 cases of pseudocysts of the auricle. Twenty seven cases have been reportad among Chinese, Malays, Caucasians and Puerto Rican, most of whom were male. However, there are no reports of these cysts occurring in Japanese. Two cases of pseudocysts of the auricle have been reported in this article. Etiology of this disease is not established at the present time. This cyst develops without trauma, often bilaterally. The treatment is difficult and recurrence is frequent. The first case was treated simply by complete aspiration, the second by a combination therapy of complete aspiration of the content followed by injection of tincture of iodine into the cyst and contour dressing of the auricle. The first method yielded a poor result, while the combination therapy gave very satisfactory result from an aesthetic standpoint.

Large cystic parathyroid adenoma is a rare occurrence. A 70-year-old male with a mass in his neck was suspected of a thyroid adenoma with cystic degeneration. After surgery this mass was histopathologically diagnosed as cystic parathyroid adenoma. Postoperatively, hypocalcemia appeared and developed for 7 months after surgery.

Two clinically-interesting cases of epipharingeal tuberculosis which extended to neigboring organs are reported. In the first case, a 42-year-old female complained of nasal obstruction. Examination of the nasal cavity and the epipharynx revealed a granular mass. A biopsy from the cavity showed only necrotic tissue, but a biopsy from the epipharynx showed Langerhans' cells, comfirming tuberculosis. A culture of post nasal discharge showed Mycobacterium tuberculosis. In the second case a 26-year-old female complained of ear obstruction. Examination of the epipharynx revealed a small granulation and an inflammation. Examination of the left ear revealed otitis media with effusion, and later with otorrhea. Biopsies were performed and the pathological findings were consistent with Mycobacterium tuberculosis infection. Mycobacterium were stained by Ziehl-Neelsen's Carbolfuchsin method. Purulent postnasal discharge and otorrhea showed positive test results. The sputum in both cases showed no positive results for smear and culture, and X-ray examinations of the chest revealed only old lesion from tuberculosis.

Nasal respiratory resistance was measured by oscillation using microrhinography (Chest, TUC-5000) to study the corellation between nasal obstruction and nasal respiratory resistance. This study includes 150 patients admitted to our hospital during 1985 for nasal disease.
Microrhinography showed greater nasal respiratory resistance in these patient compared to healthy adults. The differentiation of right and left nose was less. A high correlation was seen between nasal obstruction and nasal respiratory resistance, however about 10% of the cases showed an unespected difference between the subjective and objective findings. The nasal respiratory resistance was studied in chronic sinusitis and allergic rhinitis. Nasal respiratory resistance of both diseases was almost the same, but oral respiratory resistance showed a slight difference. The greater the severity of allergic rhinitis, the greater the resistance. It seems that patients with allergic rhinitis have some abnormalitis in the lower airway.

A 62-year old female with sarcoidosis localized in the left palatine tonsil representing pendular hyperplasia, and in the left cervical lymph node is reported. The patient was admitted to our clinic complaining of lump in the throat and showed atypical hyperplasia of left palatine tonsil upon examination. A left palatine tonsillectomy was performed and the tonsil was assessed by pathohistology. It revealed non-caseating epitheloid granulomas, suggesting sarcoidosis. In spite of the postive reaction in the Kveim test, the chest X-ray film showed neither bilateral hilar lymphadenopathy (BHL) nor any lesions in lung fields. The general examination suggested no sarcoid lesions except of the tonsil and cervical lymph node. At three months after the tonsillectomy, the cervical lymph node swelling subsided spontaneously. A brief review for sarcoidosis of the upper respiratory tract (SURT) of the literature is presented.

This report concerns a 47-year-old woman suffering from radiation-induced cancer at the base of the tongue. She received radiation for the treatment of tuberculosis the cervical lymph nodes prior to the diagnosis of the radiation induced cancer. This patient was treated with noncrossresistant combination chemothrapy including VMPM (a combination of VCR, MTX, PEP and MMC) and VPCP (a combination of VCR, PEP, CDDP and PEP) regimens. She showed complete response (CR) to the treatment. Interstitial pneumonia occurred as a side effect of MTX during the course of this treatment. No recurrence has been found under mirror-laryngoscopy and the complete response has been maintained for 3 years after the treatment.

Sound at a rock concert was analysed by computer. In three dimensional display, chronological change of band pass data could be observed at a glance. In Level/Time display, level change in each frequency could be observed successively. We found those frequencies which showed small level change and those frequencies which showed large level change. Eighty-one cases of head and neck cancer patients who died within the last five years were analyzed. The sites of primary tumors were as follows: 15 cases in the nasal and paranasal cavity, 13 cases in the tongue, 9 cases in the larynx, 7 cases in the thyroid, and 7 cases epipharynx, and 6 cases in the hypopharyngeal-cervical esophagus. 81.8% of the patients had stage III or IV cancer at the initial diagnosis and suffered from double cancers during the course of the disease. These patients had repeated hospitalizations and were treated surgicaly by tracheostomy, gastrostomy or ligation of carotid artery. In recent years, there has been increased interest in defining what constitutes good quality care for the terminal by ill cancer patient. Our analysis documents the complex problems these patients and their physicians face. These problems challenge the medical facilities caring for such patients to develop programs that provide the highest possible levels of care for head and neck cancer patients. Head and neck surgeon must consider the protocol of informing cancer patients. Cymprehensive care must be given after informing the patients of their condition.

Accidents which happened to patients seventy years of age or over with head and neck cancer from 1972 to 1984 were reviewed. Six patients had bone fracture and one patient had a scalp wounded due to a fall. Two patients showed abnormal behavior caused by psychoneurologic diseases. All accidents happened to patients who were hospitalized more than two months. Radiotherapy and chemotherapy have become more popular treatment for head and neck cancer. They require long hospitalization.
The increase in the number of accidents to elderly patients was predicted.

Subjective and objective assessments were made of 24 vertigenous patients recieving 300mg daily Dilazep Dihydrochloride during a 2 month period. Of the patients included in this study, 7 had vertebro-basilar insufficiency, 14 had Meniere's disease, 2 had vestibular neuronitis and 1 had labyrinthitis. There was no significant difference in improvement rate between subjective and objective assessments. The average improvement rate was 33.3%, based on the excellent and moderately improved groups. The average improvement rate was 83.3%, when the slightly improved group was included. No side effects of this drug were observed among these patients during this test period.

Analysis of LMS performed on 41 patients over 70 years old during the last five years at our clinic yielded the following results: In this study there were more male cases than females. Laryngeal cancer or precancerous conditions were found in 36 cases. Twenty-six patients had preoperative complications, but we did not find serious post operative complications. Most of the elderly cases were malignant tumors. In order to detect carcinoma at an earlier stage, we find it very important to perform LMS on the elderly with laryngeal symptoms.

Azeptin, an anti-allergic drug like DSCG, Rizaben and Ketotifen, was studied to determine its usefulness in the treatment of nasal allergy. This study also clarifies the relationship between platelet function and allergic disease. Azeptin inhibits histamine release from target cells, blocks H1-receptor, and inhibits leukotriene and lipooxygenase. It was suggested that platelet-activating factor (PAF) from basophil or eosinophil aggregated the platelets and initiated release reaction from the platelets. The effect of Azeptin (Azelastin hydrochloride) to PAF was studied
in vitro. Twenty-one patients with nasal allergy were tested for plasmin inhibitor (α2-PI) activity, antithrombin III (AT-III) activity, factor VIII (F VIII) activity, fibrinogen (Fg) contents before and after administration of Azeptin. Also, platelet function (ADP, collagen and arachidonic acid-induced aggregation) was determined. The results from this study were as follows: Azeptin most strongly inhibits PAF-induced platelet aggregation in the
in vitro study. Some parameters in the coagulation-fibrinolysis system were not significantly different before and after administration of the drug. However, in PAF-induced platelet aggregation after the administration of Azeptin, platelet function was decreased. It was suggested that Azeptin did not affect the coagulation-fibrynolysis system, but did affect PAF-induced platelet aggregation.

Clinical effects of metyl B
12 were evaluated in 30 patients with tinnitus at the ENT clinic of Okayama University Hospital. All patients suffered from subjective tinnitus. In 27 cases the tinnitus was accompanied by sensorineural hearing loss, 3 cases were without hearing loss. Methyl B12 was administrated 1,500 μg/day for 8 weeks to all patients. The patients were tested by “standard tinnitus test 1984” and pure tone audiometry, before and after 8 weeks admdinistration. 18 of 30 cases were improved. General efficacy rate was 60%. No side effects were noted in any case.

The concentration of FOM in otorrhea was determined by paper disc assay in suppurative otitis media. The following results were obtained: FOM concentration in otorrhea, ranged from 90.63 to 162.5μg/ml (mean 125.63μg/ml) at 10 minutes, 68.75 to 100.0μg/ml (mean 83.75μg/ml) at 30 minutes, and 26.57 to 71.88 jig/ml (mean 50.94μg/ml) at 60 minutes after administration of 300μg/ml FOM solution in the tympanum. On the basis of these results, it is concluded that FOM is a useful antibiotic for the treatment of suppurative otitis media.
