    A 43-year-old woman developed severe hearing loss in the right ear and equilibrium disturbance 3 years ago. Otoneurological examinations revealed a suspected acoustic neurinoma in the right. To confirm the diagnosis, labyrinthotomy was performed and took 10μl of perilymph and a part of the utricle through the oval window. The protein content of the perilymph was found to show marked elevation. The electron microscopic findings of the utricle were diseased vestibular hair cells and marked loss of nerve fibers. The thick collagen fibers were observed directly beneath the sensory epithelia. Infiltration of the erythrocytes to the perineural tissue was also found. A large amount of “banded structures” were seen in the connective tissue of the utricle. The banded structure was confirmed to be produced within fibrocytes in the perineural areas of the utricular nerve. Although it is already known that these banded structures were found in the labyrinth of Meniere's disease or cadavers, the quantity in this case was much higher.

    During the last ten years, 600 tympanoplasties were achieved in our clinic, and in this report the results of 253 tympanoplasties in simple otitis media were analyzed and discussed. The case was decided to be successful when the postoperative hearing loss or the air bone gap was smaller than 30 dB with no discharge. The summary is as follows:
    1. Sex and age are not responsible to the healing.
    2. Results in type I, II and III tympanoplasties, the healing ratios are all over 75%, but that in type IV tympanoplasty is only 33%.
    3. When the hearing loss is greater than 45 dB before the operation, the healing is not sufficient.
    4. The size of the drum perforation is not responsible to the healing ratio.
    5. Dry ear is desirable for good result, but the amount of the discharge is not responsible to the healing.
    6. The mucous membrane of the promontrium seemes to be conservable in the most cases, but the polypous or granulated membrane must be stripped and a mass of gelfoam should be packed in the tympanic cavity.
    7. Combination of mastoidotomy and atticotomy with tympanoplasty is important to gain a good result, Especially atticotomy is absolutely necessary to gain the correct finding on the osssicular function.
    8. The material of the tympanic graft is the best with fascia or vein.
    9. For the reconstruction of the ossicular chain in type III tympanoplasty, the interposition of the graft between the stapes and the malleus shows the best result. Therefore the malleus should be conserved.
    10. The tubal function must be good.
    11. The mixed deafness is not a disadvantage for the tympanoplasty.
    12. Because of the high healing ratio, the result between the surgeons is highly different.

    The author applied the two points discrimination test to the cases of bell's palsy for diagnosis of prognosis of the palsy using Uelenburg aesthesiometer and several reviews of anatomical and physiological aspects of facial region were performed. The normal values of DL for the two points sensation in facial region (trigeminal region) were 6 mm of medium, 0.4-0.7 mm of intra subject standard deviation and 1.1-1.7 mm of between subject standard deviation. Anatomical reviews revealed many anastomosis between facial nerve and trigeminal nerve. The results of this observation showed early recovery of DL for the two points sensation than recovery of mobility of face. The author hypothesized that recovery of DL for two points sensation in early stage of facial palsy is prognostic of the recovery of palsy.

    A patient with idiopathic neuralgia of the superior laryngeal nerve was cured by alcohol injection of the nerve. In this case, a biopsied segment of the nerve showed normal findings by the electro-microscopic examination.
    The results of examinations lead to the conclusion that idiopathic neuralgia may be caused by some difficulty of receptors of the sensory nerve in the mucous membrane of the larynx. In addition, authors reported that the vocal cords of the dog kept normal color and form after the internal branch of the superior laryngeal nerve was resected.

    A 22 year-old male with a long tracheal cannula came under the authors care. The trachea was found to be granulomatous and remarkably stenosed 3 cm long at the site 4 cm above the tracheal bifurcation. A silicon tube was inserted into the thoracic trachea over its stenosis and the upper end of the tube was fixed in the trachea just under the tracheal stoma. The patient was discharged after this procedure and lived an ordinary life for 14 months. The silicon tube gave him no disturbance in the respiration. After the tube was removed through the larynx, the tracheal wall was found smoothly epithelized without any following restenosis. This method should be recommended in such a case of granulomatous tracheal stenosis in the thoracic region in which the surgical procedure may be dangerous.

    During recent fifteen years (1957-1972) 468 ears with chronic suppurative otitis media were operated. In 14 ears out of them (3.0 %) labyrinthine fistulas were found. From the clinico-otological standpoint of the labyrinthine fistula, author's 14 cases and 25 cases in some recent domestic literatures discussed.

    The present study was designed to elucidate the role of tonsils in cell-mediated immunity by the investgation in the following points: IgA and IgG were examined for 12 children in nasopharyngeal washings before and after the tonsillectomy.
    IgA/IgG after the tonsillectomy was decreased, but was disposed to return after a while.

    A 68-year-old woman was introduced as a case of bronchial foreign body of long sojourn to my clinic by a surgeon. When she had complained of stomachache, the surgeon had taken a X-ray photograph of her chest and happened to f ind a hook staying in her right bronchus. Peroral bronchoscopical removal was attempted at f irst. The hook which was buried in granulation seemed to be moving slightly, but the procedure was unavoidably given up because of bleeding. Since there appeared the subcutaneous emphysema in the neck and face 10 minutes after the bronchoscopy, tracheotomy was performed immediately. Within a week all evidence of subcutaneous emphysema completely disappeared, and the hook was easily removed percervical bronchoscopically.
    What was extraordinary in this case was that in spite of the patient being adult a hook was inspirated unconsciously and by chance found staying in the bronchus after an erroneous treatment for some period for bronchial asthma because of stridor and cough. In the adult so rarely happens an accident of foreign body which has been unconsciously inspirated that it is apt to be overlooked. At the diagnosis of the chest disease therefore we should have it in mind that the case might be of foreign body notwithstanding a clear history of foreign body accident of air passage.
    It was considered that the subcutaneous emphysema in the first procedure was caused by the mediastinal emphysema due to the microperforation in bronchial wall by the stirring of the hook.

    The authors observed on the effects of the submucous injection of Impletol in both inferior turbinates of the patients as a treatment for nasal allergy. The subjects were 9 cases (2 male and 7 female) in which the antigens were not found out by the allergen test. among of patients who complained of the nasal allergic symptomes. Impletol was injected 1ml to each inferior turbinate after surface anesthesia by the application of 8% xylocaine.
    The results were as follows: This treatreatment brought remarkable relief of the symptomes in 3 cases, tolerable relief in 1 case and was not efficacious in 5 cases. Secondary effects by this injection therapy were nasal bleeding, cerebral anemia and intranasal exsiccation, but those were little worth consideration. This treatment is simple in technique and the pains which may give to the patients are slight. Moreover, the results are roughly satisfactory. The authors recommend to try this injection therapy as a treatment for nasal allergy.

    The therapeutic effect of Ketoprofen (2-(3-Benzoylphenyl) propionic acid) on the signs after sinus operation or tonsillectomy was tested by double blind test using Armitage's sequential analysis.
    Each patient was given Ketoprofen or placebo randomly one day before and fore days after the operations which were achieved on left and right side with same clinical condition. The postoperative signs observed in this study were spontaneous pain, pain upon pressur, pain at move, swelling and fever. The grade of these signs at left or right side operation were compared and decided in which side the medicament had been active or inactive by the chief physician.
    The decision was compared to the key and the result was plotted on the Armitage's restricted design. The excess preferences had been over the upper boundary when 15 cases were accumulated.
    As a global judgement, it was concluded that Ketorofen is a significantly effective drug for the signs after sinus or tonsill operations. From the results of analysis on each sign, it was concluded that Ketoprofen is especially effective for pains but swelling and fever.
