While I often treated as a part of my practice the patients suffering from tumors in herd and neck which are actually important in the clinic of Otorhinolaryngology and Bronchoesophagology, I always wanted to have an opportunity to observe personally the head and neck service as practised in the United States which I came to know through medical literatures.
My opportunity came last fall when I visited the United States and through the kindness of Dr. William S. MacComb, Chief of Head and Neck Service, The University of Texas, M. D. Anderson Hospital and Tumor Institute, I was invited to observe his operations.
I would like to offer here in Otologia Fukuoka the brief summary of my findings at Dr. Mac Comb's Head and Neck Clinic.
What I found unusual in comparison to my experience in Japan was that in Texas there was a large number of cases of cancer of the skin of the face. I was impressed with actual practice of Dacron arterial graft as well as pathological examinations by aspiration biopsy and frozen section during an operation. The methods in treatment which I found most enlightening are as follows:
    1) Planned combination of surgery and radiation in treatment of advanced primary head and neck cancers.
    2) Efforts to explaine and recommend an operation to the patient by a hospital expert and efforts to prevent as much as possible post-operative disorder and defects, so that the patients would easily accept the rational treatment.
    3) Strict practice of follow-up cases so that recurrence, if any, can be accurately detected. The cases of recurrence thus detected can be recorded in detail which will be a valuable data in determining the method of treatment in the future.
At the end of my address. I would like to say a few words of gratitude here from Japan to Dr. MacComb and his staff Dr. Jesse who offered me their kind help in many ways.
I also am indebted to Professor Seiichi Kawata of Kyushu University and Dr. Shinji Yoshida, Fukuoka whose kindness has made it possible for me to have a presentation about Dr. MacComb's Clinic at 276th meeting of Kyushu district of Oto-Rhino-Laryngological Society of Japan and to have this paper published in the Otologia Fukuoka.

We have so many drugs for the medical treatment in Ménièr's disease that we are troubled in clinically selecting.
The authors have investigated the autonomic nervous dysfunctions considered to be the cause of Ménièr's disease, and compared the mimetic drugs effective in their treatment with the results of the functional test of the autonomic nervous system in their cases. They found that Pilocarpin was effective in the attack of sympathicotony and Adrenalin in that of vagotony, and also that the stronger the tonus of the autonomic nervous function, the more remarkable was this tendency.

Out of 555 workers at a steel mill, the hearing disturbance for speaking and high tone level were investigated in 49: 36 of them having tympanic cicatrix and 13 having tympanic membrane perforation on one ear and the other was normal.
    1) In 44.4% of the cicatrix cases of tympanic membrane of more than 15 years of employment, less disturbance for high tone level was observed compared with normal case.
    2) In 28.6% of the perforating cases of tympanic membrane of more than 15 years of employment, less disturbance for high tone level was observed compared with normal case.
    3) In the group of less than 14 years of employment, cases of the tympanic membrane cicatrix and perforation did not show much difference from normal cases in their hearing.

In the girl, aged 17, who complained of hoarseness, nearly aphonia, and mild pain in the tongue, the primary mucous lupus consisting of ulcus, granulation and cicatrix was extensively found in nose, tongue, pharynx and larynx.
She had also the miliary tuberculosis and the lupus erythematodes in the face.
Histo-pathological examination showed not only the tuberculous granuloma, but the collagenous disease.
By the administration of INAH, PAS and SM, and the irradiation of X-ray to tongue, pharynx and larynx, this area cicarically healed the shadow in the lung almost disappeared.

The authors report a case of horizontal fracture of the maxilla. The Patlent, a man aged 19, complained of the subsidence and abnormal mobility of Proc. alveolares, after a desk had fallen on his face.
The roentgenogram and the cast model showed that the fracture accompanied bone disconnection at the sutura palatins and the premoler.
The fossa callina was revealed, the fracture was ascertained and replaced. The disconnected bone was fixated with a steel-wire driven in the maxilla.
Following Kuhn's bandage, the authors tentatively made a tractive fixing apparatus and applied it to the patient, s palate.
2 weeks after the operation, the tractive fixing apparatus was unfastened, and 3 weeks after, the steel-wire was drawn out from the fossa canina, and 26days after, the patient recovered his occlusion and could chew and talk freely without discomfort and left the hospital.
The authors considered the exterllal force which caused the fracsture and concluded that it was directed from left antero-superior toward right postero-inferior.
The reporters compared the operative reposition with the conservative one, in the fracture of maxilla and Kuhn's bandage with their tractive fixing apparatus.

The authors report three cases, young male, of hyperkeratosis of the pharynx, and a case, 77 year old male, of hyperkeratosis of the larynx. The former three cases were performed tonsillectomy on both sides, and completely healed. In the removed tonsils remarkable epithelial hyper_ keratosis and the tissue of bone and cartilage were observable. The authors concluded from the histologic figures that they were caused by inflammatory metaplasia.
In the larynx of the latter case was observable a grayish white mass of hyperkeratosis with a namber of tiny, white, spiny substances as scen in the cases of hyperkeratosis of the pharynx. A marked effect was obtained by X-ray irradiation.
It was histologically parakeratosis, and no malignant change even was revealed by a repeated biopsy.

The author additionally reports a casc of a woman, aged 29, to whom a laryngeal granuloma occurred about 40 days after the endotracheal ancsthesia.
In its aetiology, the injury to the larynx imposed by the intubation is regar ded as the main cause, and previously cxisted chronic laryngitis as well as allcrgic constitution are thought to be the prolnoting factors.
Concerning the sitc of origination, the case was a rare one in which the granuloma originated in the subglottic space.
It is advisable that the anesthesist cooperates intimately with an otorhinolaryngologist to prevcnt the complication.

The report was made on 7 year-old-girl who had congenital thyroglossal fistula. It was classified as incomplete external fistula in which the syrinx existed on the cervical median line at the middle point between the hyoid bone and the jugular fossa, attaching to the under surface of the hyoid bone and ending at the foramen coecum.
The histopathological preparation of the extirpated syrinx showed the excretory duct covered with cylindrical epithelial cells in which the nuclei of various sizes existed in comparatively low positions
