    A clinico-pathological analysis is presented in a series of 60 patients with malignant lymphoma involving WALDEYER'S ring referred to Kagoshima University Hospital during recent 15 years period. This is the 37.3% of all malignant tumors at the same region. The anatomical distribution of the primary growth within WALDEYER'S ring was as follows: tonsil, 47 cases; epipharynx, 8cases; base of tongue, 5 cases. Male and female retio was approximately equal, and the age group of 50's and 60's were affected most commonly.
    Grossly most of the tumors tended to have smooth surface with occational thin white plaque and expanding growth with less frequent ulceration than in squamous cell carcinoma.
    The 5 year survival rate was 54.2% in 17 patients of clinical stage I, and 30% in 43 patients with metastasis. Applying RAPPAPORT'S classification, the relationship of histology and prognosis was evaluated. The survival rate of patients with nodular growth pattern (N=21) was better than the patients with diffuse pattern (N=39)(p<0.05). All 9 patients with lymphoblastic lympoma died within 24 months.
    The surface marker of tumor cells was analyzed in 6 patients, resulting 4 cases with T-cell type and each one with B-cell and Null cell type. All T-cell lymphomas have a variable cytological feature.

    The ultrastructural characteristics of the olfactory epithelia in some animals were described three-dimentionally by scanning electron microscope (SEM). Morphological features of olfactory cilia, olfactory vesicles were characterized in each animals. In special reference to olfactory vesicle and acuity, the olfactory epithelia were classified into five different types. Energy dispersive X-ray microanalysis with SEM of specimen treated with ruthenium red revealed that a larger amount of Ruthenium, Sulfur and Silica identified in the olfactory epithelia than in the respiratory ciliated epithelia. It was presumed that the olfactory mucosa should be covered with a prominent polyanionic surface coat. From these results, the relationship between the characteristics of olfactory epithelia and their physiological roles were discussed.

    Überdies ist seine Einzelheit heute nicht gerade genug offenbar und es ist der gegenwärtige Zustand, daä die klinischphysiologischeForschung nach ihm fast nie durchgeführt wird.
    Ich dachte, ob der Teil des Geschmacks mittels “BSR” erklären werden könne urid berichtete tiber hauptsächlich die grundlegende Frage von dem und der Analysenmethode und das Merkmal der Reaktionwelle (tiber die evozierte Reaktion des Hirnstamms bei dem elektristschen Reiz der Zunge des normales Menschen).
    Dann erwähnte ich das Resultat, daä ich mit der Katze wie im Menschen über die evozierte Reaktion des Hirnstamms bei dem elektristischen Reiz der Zunge und auch des Lingualnervs studierte.

    An electrophysiological study of the blink reflex was undertaken in 150 normal subjects who composed from first to seventh decade.
    Comparative work on minimum threshold of the stimulus and latencies, amplitudes and duration of early component (R1) and late component (R2) were done in each aging.
    The normal values of each component of the reflex were determined in the subjects who were 5 to 59 years old as follows: the latency of Ri was 11.7±1.22 msec and varied little (less than 1.3 msec) between right and left sides, while R2 occurred at 34.20±4.73 msec ipsilateraly and at 34.46±4.99 msec contralateraly, and a rightleft difference was less than 5.5 msec.
    Fifty patients of facial paresis (48 BELL'S palsy ; 8 Ramsay HUNT'S syndrome) were subjected for the long term serial study and the following results were obtained.
    If the Ri was elicited within 4 weeks after the onset of the clinical recovery signs, and its latency recovered to the normal range within the following several weeks, their prognosis were found to be good in general.
    In patients with poorly recovered groups and synkinesis, Ri was elicited after 5 weeks.
    As a recovery sign of the reflex response pattern, the first appearance of R2 was the most important in the majority of cases, and a significant relationship was observed between the enhancement of the amplitude of the spike potential and the degree of functional recovery of the mimic muscles.

  • Special Reference to Analysis Physical and Chemical Properties of Nasal Secretion

    The color of nasal mucosa and the physico-chemical properties of nasal secretion may be affected in pathological change of the paranasal sinus such as chronic sinusitis.
    Therefore, analysis of physico-chemical properties of nasal secretion and objective indication of the color of nasal mucosa are important in either establishing diagnosis or deciding treatment.
    With 12 normal adults and 43 patients who were suffered from nasal allergy and chronic sinusitis, color analysis of nasal mucosa was done using a new equipment that was specially designed by me as oxymeter.
    Additionally, viscosity of nasal secretion by a falling ball viscometer, and pH value, amount of Ca, Na, K, mucopolysaccaride and protein in their nasal secretion were studied.
    The results which thus obtained were follows.
    1) Color of nasal mucosa was different in each phase of pathological situation.
    2) Viscosity was found to be much more higher in the muco-purulent secretion than the others, and it was closely correlated with amount of Ca and protein.
    3) In purulent secretion, more increased amount of K, mucopolysaccaride and protein were observed clearly than the others.
    In addition, there were a liner relationship between decrease of pH and increase of K.
    From these findings, clinical significance of physicochemical characteristics of nasal secretion and color of the nasal mucosa was briefly discussed.

    Fourteen patients were treated at Prefectural Miyazaki Hospital with diagnosis of cystic disease of paranasal sinuses from August 1976 to December 1978. Study of these cases indicates as follows.
    1) These cystic lesions are more frequently observed in the postoperative cases.
    2) Cystic lesions of postoperative paranasal sinuses are frequently revealed to be more advanced compared with those of non-operative.
    3) Occlusive change of middle meatus is more often observed in the case with postoperative cystic disease of paranasal sinuses. Therefore on examination of the postoperative case with occlusive change of middle meatus, attention should be paied to the possibility of the development of these cystic lesions.
    4) Operative procedure seems to be not unique cause of the development of these cystic lesions. For postoperative occlusive change of the ostia of paranasal sinuses seems to be so affected by the proliferative tendency of surrounding tissue.

  • A New Attempt by Computer Analysed

    The purpose of the present report is to interpret human brain evoked potentials recorded by the three-dimentional approach. We studied the wave formes of three planes and then analysed their vectors on the basis of Lissajous' figure, The vectors of brain evoked potentials were identical to the direction of Lissajous' figures of waves on three planes; frontal, horizontal and sagittal.
    This time, Vector of waves recorded by on-line technique were computer analysed. Therfore we examined various kinds of brain evoked potentials recorded on three axis in a healthy subject. Lissajous' figures showed the following facts and explain the method used in a new attempt.

    Although accumulated data on the clinical and fundamental studies of salivary calculi have been reported, their analytical works from both viewpoints of either ultrastructural findings or chemical composition have been rarely performed.
    In this study, the ultrastructural characteristics of the cracked surface of salivary calculi, renal calculi and gall stones were observed with scanning electron microscope (SEM) and concurrently the relative amounts of ionic distributions were measured by using X-ray microanalyzer attached to SEM.
    All salivary calculi were composed from concentric laminated structures surroundings a single spheloid central core. SEM findings of the central core showed rough small grained configurations. On the other hand, in a high magnification of SEM micrograph it was found that an individual laminated structure was consisted of thorn-like cristals in relatively regular arrangement and was different from each thickness. These structural appearances may be indicated certain process of growth pattern of salivary calculi, and further, non-structural layers between crystalized layers were seen.
    Predominant peaks of P, Ca, O, C, Mg, Na, Cl levels were detected by EDX and WDX analysis of ion contents. From the results of three dimentional architectures and chemical composition, the more striking similarities between salivary calculi and urinary ones rather than gall stones could be seen in our studies.

  • A case report

    Adenoid cystic carcinoma originating from the trachea of a 67 years old female who complained of dyspnoe and stridor was reported.
    The tumor was removed in en bloc without any difficulties by transtracheal approach.
    A review of literature revealed 23 cases of this tumor reported for the past 28 years in Japan.
    Statistical review of clinical feature of this tracheal neoplasm was discussed.

    A case of laryngeal angiomyoma, 57-year-old man who had complained of hoarsness for one year, was reported. The tumor could be exstirpated easily by parapharyngeal approach under the general anesthesia. Histopathological examination of the specimen revealed light-and electronmicroscopically angiomyoma (vascular leiomyoma).
    The benign smooth muscle tumor is one of the most infrequent types of tumor in the head and neck region which was composed from scattering smooth muscle.
    Previously only twenty-six cases have been reported in Japan. ; Nasal cavity 11, Tongue 5, Trachea 3, Larynx, Epipharynx, Mesopharynx, Hypopharynx, Oral Mucosa, Cheek and Neck each one case, respectively. From a viewpoint of histogenesis and clinical findings, the benign smooth muscle tumor originating in the head and neck region was briefly discussed.

    Two cases of rhinoliths in a male aged 18 years and a female aged 52 years who complained of nasal obstruction with mucopurulent and sometimes, bloodstained discharge were reported. The fresh specimens of nasal calculi removed in the former case weighed 4.5 grammes and 3.0 grammes in the latter.
    In the scanning electron micrographs the core like as nucleus with furrow and polous structure were found.
    Electron probe X-rays microanalysis showed that S element was concentrically detected in the core and furrow, while Ca, P, Mg, Si, Al and Na levels were recognized much more highly in the surrounding polous or solid parts compared with that of the nuclei.
    From these ultrastructural characteristics and chemical composition, the genesis of rhinoliths were discussed.
