    A nine-year-old child was admitted to our hospital because of acute unilateral sensorineural hearing loss accompanied by mycoplasma pneumonia. The patient showed high mycoplasma complement fixation titer and an abnormal shadow on chest X-rays and no clinical evidence indicating the presence of otitis media or meningitis was observed. The hearing loss did not improve despite the application of several medications. Further research is thus called for to elucidate the pathophysiology of an acute hearing loss followed by mycoplasmosis.

    The open method is preferable for the prevention of cholesteatoma recurrence. However, persistent oto-rrhea due to the insufficient epithelial healing of an open mastoid wound (cavity problem) may trouble patients for a long time after surgery. In this study we removed an area of infected granulation in an open mastoid wound and transplanted a pinch graft (small islands of the epidermal layer) to achieve a dry ear. The infected area of granulation was removed to a degree that a thin layer of connective tissue was then placed over the bony surface in 6 patients. One of them, who had a widespread focus, had pinch grafts transplanted from the skin of the mastoid process after covering the site with healthy granulation. In 5 patients who were only treated with the removal of the infected granulation, a complete epithelization was achieved within 2 weeks. The other patient was also treated with pinch grafts and needed 14 days until healthy granulation developed and still another 11 days until complete healing was achieved. We therefore recommend the aggressive removal of any infected granulation to improve small lesions in the cavity, while a transplantation of pinch grafts in patients with a widespread focus is also recommended.

  • The second report-Observation of the cochlear blood flow volume in the guinea pig

    We investigated the influence of salvia miltiorrhiza (SM) injection on the cochear blood flow volume in the guinea pig. Both SM (1.5g/kg) in the test group and adenosine triphosphate disodium (ATP;8mg/kg) in the control were arterially injected. The cochlear blood flow volume and blood pressure were measured using a laser doppler flowmeter both before and after drug injection. The results are as follows: 1) In the SM group, the cochlear blood flow volume began to rise slowly just after injection, attained a peak (average 179%) after an average of 10.8min. and then slowly began to decrease and became stable (average 140%) after an average of 21.5 min. 2) In the ATP group the cochlear blood flow volume rapidly began to decrease just after injection and showed a minimum value (average 39%) after an average of 0.51 min. It thereafter started to rise rapidly and reached a peak (average 158%) after an average of 1.59min. and it thereafter descended to a normal level after an average of 3.46 min. A good relationship (r=0.63, p<0.001) was thus shown between the responses of the cochlear blood flow volume and blood pressure after the injection of ATP, which thus suggested the blood flow movement of the lateral wall of the cochlear to show blood pressure dependence. On the other hand, since no relationship between the responses of the cochlear blood flow volume and blood pressure was observed (r=0.32), the influence of SM injection on the cochlear blood flow volume is thus not considered to demonstrate blood pressure dependence.

    A case of GERD (gastroesophageal reflux disease) with laryngeal inflammatory granulation tissue is herein reported. A 59-year-old man visited our training hospital with the chief complaint of hoarseness after a cold. A mass of the left vocal cord was pointed out and laryngomicrosurgery was performed and his symptoms subsided. One month later, however, he began to have hoarseness again and two month later, a tumor lesion similar to the previous one was observed. The patient reported to have recently suffered from heart burn, a severe cough and he had also noticed an acid-like taste. Esophagofiberscopy showed two ulcerous lesions in the mucosa at the gastroesophageal junction and 24-hour pH monitoring revealed significant reflux date, i. e., 49 reflux episodes of more than 5 minutes and 543 minutes (37.7%) with a pH of less than four. The administration of proton pump inhibitors for two months reduced the laryngeal tumor and his symptoms subsided. Based on these findinges, GERD should thus also be considered in the defferential diagnosis when examining patients complaining of a lump sensation, a cough and hoarseness.

  • The effectiveness of using an internal vascular shunt tube

    A carotid body tumor is a rare neoplasm. Only about 1000 cases in the world literature and 100 cases in Japan have been previously reported. The surgical treatment of a carotid body tumor requires the preservation of the vascular structure without compromising a full resection of the tumor. A 70-year-old female was diagnosed to have a bilateral functional carotid body tumor which secreted cathecholamine based on her CT, MRI and arteriography findings. She also demonstrated an abnormally highlevel of cathecholamine in her serum and urine. We began to perform a tumor resection with a bipolar coagulator device at first, however, this procedure had to be abandoned due to excessive bleeding and the adhesion of the carotid artery and the tumor to each other. Therefore an internal vascular shunt was applied. The tumor was thereafter removed completely without a vascular reconstruction of the carotid artery. No postoperative cerebrovascular complications were observed. Based on our findings, we consider the use of an internal vascular shunt tube to be an effective method for dissecting a subadvential tumor from the carotid artery.

    A case of medullary carcinoma in the thyroid gland in a 41-year-old woman with familial thyroid disease in reported. Surgical treatment, including a total thyroidectomy and neck dissection, was performed and the postoperative course was uneventful. Some members of her family also suffered from thyroid gland tumors. Medullary carcinoma is a rare thyroid gland tumors. It can be related to familial disease, including multiple endocrine neoplasia (MEN) type2. In addition, recent molecular biological studies have shown that a mutation in the RET gene may be a causative factor of medullary carcinoma. The importance of making a genetic analysis is also emphasized.

    Although deep neck infection tend to decrease after the administration of antibiotic therapy, primary focus extends to mediastinum through the parapharyngeal space and may thus cause a fatal disease. We reported three cases of deep neck infections. Surgical treatments such as lateral cervical incision was performed in connection with antibiotic administration in all patients. A CT scan is the most effective for a diagnosis and conducting postoperative observation. Based on the findings of these cases, a lateral cervical incision in combination with antibiotic administration was considered to be safe approach for cervical infectious lesions.

    We report an extremely rare case of mucoepidermoid carcinoma of the submandibular gland in a child. The patient was a 12-year-old boy whose chief complaint was a mass in the right submandibular region. The preoperative diagnosis was an inflammatory subcutaneous tumor. After the operation was performed, it was diagnosed to be a tumor of the submandibular gland. The tumor was completely removed along with the right submandibular gland. A histopathological examination revealed the presence of mucoepidermoid carcinoma. There has been no evidence of recurrence for 3 years after the operation.

    Nodular fasciitis is a rare benign lesion which is often misdiagnosed as a malignant tumor. Nodulat fasciitis, originates from subcutaneous area of the cheek region is rare. We report a case of nodular fasciitis in the patient was a 27-year-old female who demonstrated a rapidly increasing mass in the right cheek region. The mass was surgically removed. The pathological diagnosis was made based on immunological staining reveal a proliferation of fibroblasts with a tissue reaction.

    About 10 years have passed since collagen injection was first used for the treatment of unilateral recurrent laryngeal nerve paralysis in Japan. The main problems of collagen injection have been reported to be the antigenicity of collagen and the durability of its effect. The injectable collagen currently available on the market is made from bovine dermis and is processed for the removal of teropeptide by enzymes. However, collagen demonstrates some antigenicity which may induce infections disease. Collagen has an excellent character and it can sometimes become a basement for cell proliferation. The development of autologous injectable collagen would thus be a more ideal injectable material. We therefore experimentally tried to make injectable meterials from both canine and bovine skin by acid plumping and we threrafter observed and compared the collagen fibers of these materials which were made into two types of injectable collagen (non-cross-linked and cross-linked) by electron microscirpy. As a result, the collagen produced from skin by acid plumping was thus found to be very similar to cross-linked aterocollagen.
