  • malignant granuloma of the nose and face

    One hundred seventy nine cases of this disease that have been treated in Japan for recent. 10 years remain the subjects of dispute. As regards extent of lesion, malignant granulomas of the nose are classified into 5 clinical types: lesion is not widespread and located near the nares in Type I, limited in the unilateral nasal cavity in Type IL located in the unilateral nasal cavity with external ulceration of the face or palate in Type III, lesion is spread in the bilateral nasal cavities in Type IV, spread in the bilateral nasal cavities with destructive ulceration of the face or palate in Type V. The prognosis of these cases, is notoriously poor. Five year survival ratio is 17% (1/6) in Type I, 37% (13/35) in Type II, 0% (0/10) in Type III, 21% (3/14) in Type IV, 0% (0/7) in Type V. Considered from the therapeutic standpoint, 5 year survival ratio of the unilateral lesion (Type I, II, III) is 16% (1/6) by treatment of cortisone and antibiotics, 45% (10/22) by combined irradiation and surgical excision ; in contrast with this, that of the bilateral lesions (Type IV, V) is 30% (2/7) by cortisone and antibiotics, 7% (1/14) by surgical excision or irradiation. It is no argument that the so-called malignant granuloma of the nose are divided into two different diseases, sarcoma and Wegener's granulomatosis. Many cases of Type I, II, III belong to the former and Type IV, V to the latter.
    Thirteen cases who came under the author's care are 6 cases of sarcoma (5 alive, 1 died) and 7 cases of Wegener's disease (3 alive, 4 died). As to the therapy of malignant granulomatosis, irradiation is the best treatment to sarcoma ; however, to Wegener's granulomatosis, ACTH, steroid hormone therapy, small dosis irradiation and cervical sympathectomy have curative or temporary curative effect in some cases but in others no effect. Author's two cases were treated by autovaccine combined with above mentioned other methods. It was found that both autovaccine and steroid hormone were the most effective to Wegener's granulomatosis. The autovaccine therapy to Wegener's disease deserve particular emphasis and attention. This paper is the first report conceinirig the effectiveness of autovaccine to Wegener's disease.

    Patients suffering from esophageal obstruction are seen with increasing frequency by the internists, esophagoscopists and thoracic surgeons. For many years these unfortunate individuals were treated with indifferent success. In recent years, however, the wonderful changes brought about through vitamin therapy in dietary diseases by the internist, the increasing use of the esophagoscope, and the revolutionary developments in thoracic surgery have established tremendous progress in the treatment of esophageal lesions.

    The authors attempted to determine if there is a statistical difference between the articulation score which were obtained using the full list and the first half list of 57 AB list in an articulation test.
    Articulation scores were divided into 10 groups with 10% increments.
    Each of these divided groups had 100 tested lists which consisted of 50 monosyllables.
    Statistical calculations prove that there are significant differences between the two scores.
    Dr. B. S. Elpern's report was discussed statistically.

    20 cases of tympanoplasty were treated with “Cepharanthin” which was offered by the Kakenshoyaku Company.
    The administration of Cepharanthin ( 3mg/day peroral or 5 mg/day intramuscular injection) inhibited dischargement of wound after the tympanoplasty. The effect of this drug was recognized in 75% of the cases.
    There is no difference of the effect between the cases by peroral prescription and intramuscular prescription. No by-effect was observed.

    Oral Decongestant tablets containing Forhistal, Antrenyl and Phenylpropanolamine were used to the patients with allergic rhinitis. Forhistal is the antihistamic agent, while Antrenyl is known as a parasympatholytic and Phenylpropanolamine as a sympathomimetic.
    Remarkable symptomatic relief was obtained in 15 out of 17 cases.
    We concluded that this drag was clinically useful for the treatment of allergic rhinitis.

    There are many reports as to multiple carcinoma. The authors report here six cases of multiple carcinoma which were found at our clinic from 1946 to 1964.
    Case I, 68 male, had undergone laryngectomy about 15 years before and been healed, but now developed malignant new growth of the esophagus.
    Case II, 64 male, had undergone removal of maxillary carcinoma on the right side and completely healed, but 14 years later grew carcinoma of the larynx.
    Case III, 53 female, and Case IV, 43 male, had undergone removal of maxillary carcinoma on one side and 8 years later newly developed one on the other side.
    Case V, 51 male, had undergone laryngectomy, and 3 years later newly grew carcinoma of the stomach.
    Case VI, 60 male, had undergone laryngectomy about 4 years before and again suffered from carcinoma of the signoid colon.
    In all cases, operation of the first carcinoma had been undergone, and after a long lapse of time, the second one newly developed.
    And the definition and condition of “Double carcinoma” were discussed.

    A case of rhinopathia gangraenosa progressiva was treated by 60Co-radiation, antibiotics and steroid hormone. Additionally, a piece of gauze, dipped in α-chymotrypsin, was applied on the gangraenous region of the nasal cavity and marked effect was observed in the local findings. The authors recommended the use of α-chymotrypsin together with 60Co-radiation and others, as an effective treatment of this disease.

    Cyclonamine, one of the new hemostatic agents, has been used 1 or 2 amples on 23 patients who underwent various E. N. T. surgeries.
    The efficacy proved satisfactory in most of the cases although further observation on more cases will be necessary to evaluate the entity and complications of this drage.

    The author experienced a case of delayed decannulation. A man 20 years old complained of dyspnea 2 weeks after tracheotomy which had been performed soon after the traffic accident in February, 1959. The glottis was filled with granulation tissue, which was removed by laryngofissure. The T-formed tube was inserted but soon followed by dyspnea.
    No treatment tried after that was successful in decannulation.
    4 years later (March, 1963), the author improved the T-formed tube, which was combined with a straight vinyl-tube, and fixed it in the trachea. The patient at present does not complain of any disorders except cough and dysphonia.

    The authors applied Enjoji's analysed developmental test to deaf children with speech disorders and obtained a developmental graph of W-type which was characteristic of deeper notch in speech development than in mental development. Physical, emotional and social developments did not retard.
    The authors think that this method can be utilized as a help to differential diagnosis by holding precisely the combination of disorders beyond deafness from the deformation of those developmental graphs and give a useful mean to the judgement of effects of therapeutic steps for deaf children by recognizing the changes in developmental graphs recorded at a regular interval.
