    The ototoxicity of antiseptics applied in the middle ear cavity of the guinea pig was studied. Inner ear dysfunction was evaluated with recording endocochlear dc potential (EP) and cochlear microphonics (CM). Hibitane gluconate solution 0.5% damaged the inner ear within 30 minutes after application. Hibitane gluconate solution 0.02% and Isodine 10% solution did not make any significant changes on EP and CM within 30 minutes. Hibitane gluconate solution 0.02% and Isodine 10% solution showed a significant sterilizing effect within a few minutes. These antiseptics can be used, if their concentration and application time are carefully controlled.

    A case of malignant ear tumor, 70-year-old woman, was reported. She consulted our clinic, complaining of left aural discharge. The ulcer formations were noted on retroauricular area. Histological examination revealed cholesteatoma and no malignant findings. Radical mastoidectomy was performed, however, cholesteatoma recurred and aggresively enlarged within few weeks. Repeated histological examinations still could not reveal any malignant findings, however, we treated this patient as malignant ear tumor. Radiation and chemotherapy were effective. Final histological diagnosis was squamous cell carcinoma.

    Thirty-nine specimens of palatine tonsils macroscopi cally appearing as benign tumors were examined histologically. Histopathological investigation revealed to be squamous cell papilloma in seven cases, hemagioma in one case, squamous cell hyperplasia in 13 cases, stromal overgrowth in 14 cases, combined overgrowth of epithel and stroma in three cases, and retension cyst in one case. The results suggest that reactive and inflammatory overgrowth is more common than benign neoplastic masses in this region.

    Fifty-four patients complaining of abnormal s ensations of the pharynx and larynx received psychological screening tests and psychological interviews along with various somatic tests available in the otorhinolaryngological outpatient clinic. In 18 of these patients, in whom psychogenic and psychopathic causes were suspected, the mechanism of development was evaluated. The clinical diagnosis of neurosis was made in 8, depression in 8, senestopathia in 1 and psychosomatic disease in 1, suggesting an unexpected usefulness of psychological tests. Positive answers suggesting a hypochondric tendency were uniformly obtained, indicating certain relationships between abnormal sensation of the pharynx and hypochondric tendency. Cancer-phobia and a foreign body sensation were detected in 11 of 18 patients. A psychological etiology of these symptoms were strongly suggested. Recent changes in disease awareness, including an increase awareness of malignancy of the pharynx and larynx has possibly influenced psychologically unstable patients. The obsessive-compulsive tendency seen in many of these patients may also explain this phenomenon. The hypochondric tendency, obssesive-compulsive tendency, and cancer-phobia emphasize the importance of the present social-cultural conditions in the dynamic disease picture.

    Among 790 patients who complained of ve rtigo and dizziness and were examined neuro-otologically in the department of O. R. L. at Kobe University Hospital and Kobe Central Municipal Hospital from 1983 to 1984, 204 (25.8 96) were over 60 years of age.(1) The number of patients with vertigo due to central nervous system lesions, especially cerebrovascular disorders, increased with age.(2) In elderly patients, tinnitus and hearing loss were more frequent complaints than in younger patients.(3) In elderly patients, abnormalities of E. T. T., O. K. P., and gaze nystagmus were more frequently seen than in younger patients.(4) The number of patients who suffered from hypertension, arteriosclerosis, and ophthalmological disorders increased with age.

    Twenty-nine patients with acoustic neurinoma were clinically investigated and their past medical histories were reviewed. 1) About 70 % of the patients first visited on otorhinolaryngologist and nearly 26 % of them were diagnosed as having acoustic neurinoma at that time. 2) In total, about 80 % of the patients visited an otorhinolaryngologist at one stage of the disease, and nearly 50 % of them were correctly diagnosed by them. 3) About 20 % of the patients did not have a history of physical checkup by an otorhinolaryngologist.

    From 1972 to 1981, 253 patients with laryngeal carcino ma were treated in our department. The 5-year survival rate was 73 96 (stage I 80 %, II 74 %, III 68 %) in the glottic type, 51 % (stage I 61 %, II 73 96, III 49 %, N 4196) in the supraglottic type and 27 % in the subglottic type. For advanced laryngeal carcinoma of T3, T4, the choice of treatment on the basis of this retrospective study is as follows: Glottic T3 NO-N 1: Initially a full dose radiation therapy is indicated and if this fails, a salvage total laryngectomy should be performed. Supraglottic T 3-4 N 0-1: This type is difficult to cure with radiation therapy only. We consider it safer to treat this type with total laryngectomy after pre-irradiation.

    The amount of scattering pollen of gramineae in Osaka and the therapeutic effect of flunisolide nasal solution on pollinosis of gramineae were studied in nine male and seven female patients. In 1984, the pollen of gramineae began to scatter on April 19, peaked in amount during the end of May and the first part of June (max: 37 grains/10 cm3), began to decrease in late June and increased again during mid-August to the first part of September, indicating a biphasic pattern of scattering. Flunisolide nasal solution produced a 73.3 % improvement in sneezing and nasal discharge. The drug produced a symptomatic improvement of “Moderately improved” and “Markedly improved” in 56.3% of the patients and “Slightly improved” or better in 81.3 %. The only adverse reaction reported was one case of nasal irritation. In conclusion, flunisolide nasal solution was demonstrated to be a useful drug for the treatment of pollinosis of gramineae.

    The clinical effect of SAIBOKUTOU was studied in 28 patients with abnormal sensation in the throat. Evaluation of the efficacy was made by analyzing the patients' subjective sign and symptoms. The drug dosage was 12g of SAIBOKUTOU daily for four weeks. SAIBOKUTOU was effective in 19 of 28 patients (68%). No side effects were observed during treatment. Thus this drug seemed to be effective and safe in the treatment of abnormal sensation in the throat.

    For the purpose of evaluating objectively the effectiveness, safety and usefulness of Lenampicillin (LAPC, 250 mg, q. i. d.) for acute suppurative otitis media and acute exacerbation of chronic suppurative otitis media, a double blind controlled study has been carried out by using Talampicillin (TAPC, 250 mg, q. i. d.) as a control drug. L Although LAPC group showed a higher rate in the clinical effect for acute suppurative otitis media by the doctor in charge with such effectiveness as LAPC group 84.2 % (32/38) and TAPC group 76.2 % (32/42), significant difference was not found. The effectiveness for acute exacerbation of chronic suppurative otitis media showed no significant difference likewise with LAPC group 54.5 % (36/66) and TAPC group 53.2 % (33/62). According to the judgement based on the criteria by the Committee, the LAPC group showed a superior tendency (p<0.1) in the effectiveness of acute suppurative otitis media with 89.5 % (34/38) as against TAPC group 73.8 96 (31/42). The effectiveness for acute exacerbation of chronic suppurative otitis media, however, showed no significant difference between them, with LAPC group 57.6 % (38/66) and TAPC group 53.2 % (33/62). 2. In the bacteriological of-fect, no significant difference was recognized with such eradication rate as LAPC group 68.8 % and TAPC group 65.3 %. 3. In regard to the safety, no significant difference in the appearance of side effects was recognized between both drugs, with LAPC group 7.9 96 (10/127) and TAPC group 4.6 % (6/130). 4. As for the usefulness, in terms of the rate of usefulness consisting of “quite satisfied” and “satisfied”, no significant difference was recognized with LAPC group 65.7 % (71/108) and TAPC group 61.3 96 (65/106). From the results thus obtained above, the LAPC can be considered as highly useful drug being capable of expecting the same and/or higher result when compared with TAPC for the treatment of suppurative otitis media.

    A double blind study has been carried out for the effectiveness, safety and usefulness of Lenampicillin (LAPC, 250 mg, t. i. d.) for acute tonsillitis. 1. For all the rate of effectiveness (excellent+good) just evaluated by a physician in charge with LAPC group 91.1 % (82/90) and Cefaclor (CCL) group 83.9 % (78/93), and by Committee judgement with LAPC group 78.9 % (71/90) and CCL group 72.0 % (67/93), no significant difference was found despite LAPC group being in fact higher in effectiveness. 2. In the bacteriological effect, no significant difference was recognized with almost the same eradication rate between LAPC group 97.6 % and CCL group 98.8 %. 3. In regard to the safety, no significant difference in the cases of appearance of side effects has been recognized with LAPC group, 6 cases out of 128 (4.7 %), and CCL group, 6 cases out of 126 (4.8 96). 4. As for the usefulness, no significant difference was recognized with the rate of usefulness (markedly satisfactory +satisfactory), LAPC group 87.0 % (80/92) and CCL group 84.5 96 (82/97). From the results thus obtained above, the LAPC (250 mg, t. i. d.) can be considered as highly useful drug being capable of expecting the same result when compared with CCL (250 mg, t. i. d.) for the treatment of acute tonsillitis.

    We report our experience using beclomethasone dipropionate inhaler (BDI), in the management of non-infective laryngitis and post-operative inflammation in singers and other professional voice users. This trial comprised ten cases of persisting non-infective laryngitis resulting from long term vocal abuse because of the nature of their occupations, and five cases of persisting post-operative vocal cord inflammation following laryngomicro surgery of vocal nodules. BDI was administered by oral spray in 50μg per puff, two puffs four times daily, totalling 400μg for two to four weeks. In all cases, symptoms and vocal cord inflammation markedly improved over the course of one week, and complaints of hoarseness virtually disappeared within two to four weeks. No side effects were noted. We believe that in those cases where long term therapy employing absolute silence is impractical or unsatisfactory, BDI is a useful therapeutic adjunct in noninfective laryngitis.
