    Using separately obtained epithelial cells and fibroblasts from the porcine laryngeal mucosa, wein vitro successfully reconstructed the laryngeal mucosa including epithelium and lamina propria in a three-dimensional collagen gel matrix culture with an air-liquid interface. The epithelial cells appreared 5-8 cells in thickness and were well-differentiated on the reconstructed lamina propria. In this model, the fibroblasts in the collagen gel matrix and air-liquid interface treatment exert a great influence on the proliferation and differentiation of the cultured epithelial cells. Especially, the fibroblasts indicated the possibility that theyse cells may synthesize and release hepatocyte growth factor. In addition, the fibroblasts also appeared to possibly exert some influence through direct contact and/or arranging the collagen gel matrix into a physiological conformation. This culture system is thus considered to provide an appropriate physiological environment to study differentiation and disease of the larynx.

    We immunhistochemically studied cellular differentiation in laryngeal carcinoma and dysplasia, using Involucrin, a marker of human mature suprabasal squamous epithelia. Furthermore, we also compared the degree of cellular differentiation with the proliferative activity by immunohistochemically evaluating the intake of Bromodeoxyuridine (BrdU). Involucrin stainings showed different stainings patterns in carcinoma and dysplasia for normal squamous epithelia. We also performed immunohistochemical double staining for the BrdU intake and Involucrin. These two markers showed a reciprocal relationship in the normal squamous epithelium as well as in dysplasia and many areas of carcinoma cells. However, some carcinoma cells also concomitantly demonstrated positive results for both the BrdU intake and Involucrin staining. These findings suggest that cellular proliferation and differentiation therefore caused some kind of disruption in these carcinoma cells.

    The viscosity of liquid contrast media, such as barium sulfate, which is used in fluorography, is important for evaluating the pharyngeal swallowing function. We measured the viscosity of various contrast media, and thus determined that the viscosity of nonionic low osmolality contrast media is nearly thd same as that of water, which is very low, while the viscosity of nonionic low osmolality contrast media containing 2-3% sodium chondroitin sulfate is as high as that of barium (164w/v%), which is 256 cP in shear rate 20. The swallowing function is influenced by the viscosity of media, a low viscous media decreases the clearance load of hypopharynx, and quickens the pharyngeal inflow velocity. On the other hand, a high viscous media increases the clearance load, and slows down the pharyngeal inflow. Therefore, we consider the use of these different viscous contrast media for fluorographic exminations to thus be useful in detecting various types of dysphasia.

    Two patiants (2 females) with malignant melanoma of the nasal caity that were recently treated in our clinic are reported. One of them was treated with the combined therapy of surgery and immunotherapy(β interferon), while the other was treated by the combined therapy, of surgery and chemotherapy (DAV combination). The surgical margin of margin of melanoma of the nasal cavity was limited by anatomical reasons, and therefore the combined therapies of surgery, chemotherapy and immunotherapy are considered to be necessary for the treatment of malignant melanoma in the nasal cavity.

    It is uncommon for liposarcoma to develop in the maxilla, and only a few such cases have been previously reported in Japan. The prognosis of patients with round cell type liposarcoma is also very poor. We herein present a case of round cell type liposarcoma. A 50-year-old male was diagnosed as having liposarcoma after undergoing several biopsies, and therefore underwent a series of radiotherapy combined with chemotherapy. In spite of such treatment, however, he eventually died due to metastasis. This patient turned out to be a rare case who demonstrated a mixed pattern of both papilloma and liposarcoma.

    A rupture of the cavernous portion of the internal carotid artery (ICA) is difficult to treat by direct surgery due to the surrounding cavernous sinus. In such cases, endovascular embolization therapy using a detachable balloon catheter may be especially useful. A 64-year-old female presented with a ruptured cavernous portion of the left ICA after being treated for cancer of the maxillary sinus. Before the left ICA was embolized, several examinations such as the Matas test for the left ICA, angiography of the cerebral blood vessels and scintigraphy of the cerebral blood flow had all been sufficiently performed. Thanks to the use of continuous neurological monitoring during the endovascular embolization therapy, endovascular detachable balloon embolization could be performed safety and without any cerebral ischemia either during or after this therapy.

    The occurrence of Hodgkin's disease of the parotid gland is rare in Japan. We experienced such a rare case who underwent a total parotidectomy and subsequently was diagnosed as having Hodgkin's disease. She thereafter received radiotherapy and has since showed no sign of recurrence. A 70-year-old female presented complaining of an enlarged mass in the right parotid gland for the past10 years. The tumor was thought to be a malignant tumor, and so she underwent a right total parotidectomy. The histopathological findings indicated Hodgkin's disease. After the operation, she received radiotherapy totaling 46. Gy and has since showed a good recovery.

    A case of epithelial-myoepithelial carcinoma of the parotid gland is reported. This case had been presumed to be a low-grade malignant tumor since 1991, because local recurrence and/or cervical, distan metastasis had been occasionally observed. Histopathologically, the tumor consisted of two cell type: namely: epithelial and myoepithelial cells, and immunohistochemical investigations also proved to 13, useful in the final diagnosis. The tumor nodule also partly extended through the capsule. The pos operative course was uneventful, and this case has since demonstrated no evidenceof either recurrence o metastasis.

    Hemangioendothelioma originating in the salivary gland is very rare. A 76-year-old male presenting with a left parotid tumor is herein reported. The tumor was removed by a left total parotidectomy under general anesthesia and was later histopathologically diagnosed to have hemangioendothelioma. The patient has since demonstrated no recurrence for 2 years and 6 months. The clinical and histopathological features of hemangioendothelioma of the head and neck are also discussed.

    A 64-year-old man who presented with a history of an intractable ulcer in the tongue and oral mucosa is reported. An initial biopsy of the tongue demonstrated findings consistent with tuberculosis. The patient was thus administered isoniazid, streptomycin sulfate and ricampicin, however, the therapy was not effective. To reconfirm the histological diagnosis we thus resected the foci using an Nd-YAG laser. A further histological examination, however, resulted in a diagnosis of South American Blastmycosis. South American Blastmycosis, which is very rare in Japan, should thus be considered in the differential diagnosis of patients demonstrating either an ulcerative or granulomated region in the oropharyngeal mucosa.

    A case of Wegener's granulomatosis that was associated with an intractable pharyngeal ulcer is herein reported. The measurment of Cytoplasmic-Anti-Neutrophil-Cytoplasmic-Antibody (C-ANCA) was found to be useful in making the final diagnosis of Wegener's granulomatosis in this case. This disease should thus be considerd in the differential diagnosis of intractable pharyngeal ulcers. In cases of pharyngeal ulcers where a final diagnosis can not be obtained even after performing both biopsy and blood examinations, generally steroid therapy appears to be necessary in order to control the symptoms of Wegener's granulomatosis.

    We herein report 4 cases of verrucous carcinoma of the oral cavity. In these cases, the histological diagnosis of the biopsy specimens was papilloma. It is important of take sufficiently large specimens since the histopathological features of verrucous carcinoma can make the diagnosis difficult. Surgery seems to be the treatment of choice for verrucous carcinoma, and we surgically treated all 4 patients with an Nd-YAG laser.

    We report a case in which pulmonary metastasis arose from a recurrent pleomorphic adenoma in the minor salivary gland. The patient, aged 43 had been receiving surgical treatment for a locally recurrent pleomorphic adenoma and pulmonary metastasis. He eventually died of cardiac tamponade due to the invasion of recurrent pulmonary tumor 19 months later. Pleomorphic adenomas of the salivary gland are usually regarded as benign tumors. In a small percentage of such cases, so-called “malignant mixed tumors” can also arise from otherwise apparently benign lesions, and thus sometimes give rise to distant metastasis. For a resection of primary pleomorphic adenoma it is therefore very important to remove the tumor completely without rupturing its capsule at the first operation.

    A case of nasopharyngeal angioleiomyoma in an 11-year-old boy is herein reported. As far as we know only 25 cases of nasopharyngeal angioleiomyoma have been previously reported in Japan. Surgery is the best therapy for angioleiomyoma. However, it is important to reduce the intraoperative hemorrhaging and to obtain a good surgical field, particularly when performing surgery for nasopharyngeal angioleiomyoma.

    Three cases of papillary hypertrophy of the tonsils are herein reported. Case 1 was a 10-year-old boy who conplained of habitoal angina in 1986. Case 2 was a 12-year-old girl who also demonstrated habitual angina in 1986. Case 3 was a 12-year-old boy who complained of dysphagia in 1992. On thd sur face of the palatine tonsils in all cases many rice-or pea-sized papillary projections were obsbrved. A tonsillectomy was thus performed on all cases under general anes thesia. The histological findings of all cases revealed chronic tonsillitis with lymphoid hyperplasia and a proliferation of both the germinal center and the connective tissue. Thesurface of the tonsils were also covered by a thick epithelium. In case 3, papillary hypertrophy of the tonsils was also identified in other family members.

    According to statistical studies the tuberculosis patients has decreased owing to the effectiveness of chemotherapy treatment for this disease. An uncommon case of pharyngeal tuberculosis occurring in a 59-year-old woman is herein reported. Since this case first appeared to be either a subcutaneous tumor or malignant lymphoma at the initial medical examination, it was difficult to differentiate from pharyngeal tuberculosis. An excisinal biopsy was performed to make the final diagnosis, and the specimen thus revealed typical tuberculosis. The patient improved after undergoing an antituberculous chemotherapy regimen for six months.

    A case of piriform sinus fistula is presented. The patient was a 38-year-old male. His main clinical symptom was recurrent cervical swelling. This case was diagnosed based on flororadiography with barium. Prior to performing a fistulectomy, we put pioctanin in the fistula, and left a Fogarty catheter in the fistula. Subsequently, a fistulectomy with a hemithyroidectomy of the left lobe was performed with those guides. The patients neck abscess has not recurred since the operation.

    A case of adenosquamous carcinoma of the mesopharynx is reported. Adenosquamous carcinoma of the head and neck is extremely rare, especially in the pharynx. A 63-year-old man who had a tumorous lesion, 25×20mm in diameter, at the right palatoglossal arch was diagnosed with adenosquamous carcinoma by preoperative biopsy. Thus, radical surgery was performed on the patient with no induction therapy. However, two years after the operation, metastatic lymphadenopathies were exhibited, resulting in death despite the use of radiotherapy. We review earlier literature and discuss the histogenesis, histopathologic characteristics and clinical features of adenosquamous carcinoma.

    The development of a granular cell tumor in the laryngeal region is a rare disturbance, with only 7 such cases previously reported in Japan. Therefore it is considered important to report the experience of granular cell tumor developing on the vocal fold, while also evaluating the findings of previous studies. We herein report one such rare case. The patient was a 47-year-old male, who presented with a complaint of paresthesia in the laryngeal region. On admission to the hospital, a nodular lesion measuring 3-5mm in diameter was noted on the vocal fold, which had shifted slightly to the arytenoid cartilage vocal process The nodular lesion had an elastic smooth surface which was covered by a mucosal epithelium. Based or the sectional findings, neither a cystic nor a submucosal tumor was suspected. The patient thus underwen microsurgery. The mass was solid and pathologically identified to be granular cell tumor. No recurrence was noted within the 3 years and 6 months since undergoing the operation. other studies in the literature have seldom shown either recurrence or and aggregation of granular cell tumors. Therefore, a thorough follow-up of this case is called for.

    A case of laryngeal spindle cell carcinoma is reported. The patient was 54-year-old female who presented with symptoms of hoarseness and dyspnea. The histogenesis of the spindle cells was evaluated by HE, immunohistochemical staining and an electron microscopic analysis. The immunohistochemical findings revealed that the spindle cells had a positive cytokeratin and vimentin immunoreactivity. In addition, an electron microscopic study revealed that such epithelial ultrastrucctures as the desmosomes and tonofilament were found in the spindle cells.

    Two cases of laryngeal cyst in a 40-year-old male and 88-year old male are herein reported. The cysts of both cases were localized in the arytenoid region, and were resected by endlaryngeal microsurgery. One cyst was lined with a ciliated cylindroepithelium, while the other was lined with a ciliated epithelium. The pathological diagnosis of both cases was that of a retention cyst.

    The results of functional surgery of the larynx were evaluated in forty-two cases with unilateral nerve paralysis. The following surgical procedures were performed: an intrafold injection in twenty-two patients, thyroplasty type 1 in fifteen patients, and a combination method in five patients. Both the maximum phonation time and air flow rate improved after these procedures in almost all cases, but two cases showed no improvement. The reason for the poor results in these two cases was considered to possibly be due to a problem in the investigative techniques and thus they would both be good cases for a multilateral study. In the future, further examinations need to be performed regarding move various factors regarding, for example, the intensity and pitch of the voice, the degree and atrophic level of the vocal folds and so on.

    We report a case of ventricular dysphonia which improved after an excision of the ventricular folds. The patient, 51-year-old male, has suffered from hoarseness for two years after suffering from an acute inflammation of the pharynx. The hypertrophy of the ventricular folds were slight, however we could not sufficiently observe the vocal folds because of the adduction of the ventricular folds on phonation. Following the excision of the ventricular folds using a CO2 laser, the patient's voice immediately immproved. Although voice therapy or psychotherapy is usually selected for the treatment of functional ventricular dysphonia, surgical therapy was found to result in a direct improvement in our case.

    We report the results of voice therapy performed on four cases with vocal fold atrophy and hoarseness. All patients were over sixty years of age and had already retired. The methodology of the voice therapy included: 1) the establishment of a new (high) pitch, 2) the pushing method, 3) respiration training, and 4) physical exercise. After completion of the voice therapy regimen, three patients demonstrated an improvement in their voice. In line with the above findings, it is considered that the need for elderly persons to undergo such voice therapy will increase in the near future.

    A case of Wallenberg's syndrome with dysphagia, a normal complication of this syndrome, treated by a cricopharyngeal myotomy is herein reported. A 53-year-old man who had suffered a left brain infarction in 1990 had been afflicted with severe dysphagia for one year. A videofluoroscopic and manometric examination showed an impairment of the cricopharyngeal muscle relaxation with severe aspiration during swallowing. After performing a cricopharyngeal myotomy, the dysphagia improved dramatically and a manometric examination also proved tha operation to be successful. We discuss the indications for a cricopharyngeal myotomy in dysphagia due to Wallenberg's syndrome. Therefore, the combination of videofluoroscopic and manometric techniques was found to be useful for the evaluation of dysphagia.

    We report a case of Forestier's disease (diffuse idiopathic skeletal hyperostosis: DISH) with severe dysphagia, in which an excision of the cervical osteophytes and cricopharyngeal myotomy was performed. After the operation, the dysphagia substantially improved. The pathogen which caused dysphagia was considered to not only be mechanical compression by the cervical osteophytes but also a secondary functional and organic disorder of the cricopharyngeal muscle. Our findings therefore suggest that Forestier's disease with severe dysphagia may sometimes require surgical treatment.

    We herein report two cases of extra-abdominal desmoid tumor in the neck. The first patient was a 69 -year-old female who presented complaining of a right neck mass. The second patient was a 44-year-old male who demonstrated a right neck mass. Surgical treatment was performed on both cases. The pathological examination showed a proliferation of wavy collagen fiber while no cell atypia was seen.

    Hibernoma is a rare, benign, soft tissue. We herein review 108 previously reported cases of hibernoma and also report an additional case of hibernoma in the right cervical region. In this case an immunohistochemical study for thermogenin was positive. Among all these cases, 24 were found to originate in the head and neck region.

    Cervical lymphangioma is most often recognized in infants and children as a benign tumor, hence it's occurrence in adulthood has so far been scarcely reported. However, we recently experienced a rare case, in which a satisfactory outcome was obtained by performing surgical treatment. A 59-year-old female presented with the chief complaint of cervical swelling, which had been evaluated to be a relatively soft tumor in the left cervical area. An echo study indicated a cystic tumor possessing a wall inside. A CT scan showed a clear cystic tumor with a uniform margin and a sharp border. The tumor was thus extirpated and after 4 years no recurrence has since been reported. We herein report a case of cervical lymphangioma found in an adult and also review the pertinent literature.

    A pediatric case of acquired laryngeal stenosis, which occured after prolonged intubation is reported. We initally used the silicon T-tube, but we could not obtain satisfactory results. However, we were finally able to successfully treat the patient by performing a mucosal transplant and setting the coremorld, which had been made based on a pattern of the subglottic space, with a laryngofisher. Based on our findings, the best material for a coremold is therefore considered to be dental protease (EXAMIX (R)).

    A total of eight patients have been treated surgically for parathyroid adenoma in our department. They comprised six females and two males, and their ages ranged from 20 to 64 years old. All patients suffered from various symptoms due to hypercalcemia or low back pain. The total serum calcium ranged from 4.6 to 7.7mEq/l (nomal 4.4-5.1mEq/l). The paratholmone levels ranged from 880 to 3068μg/ml (normal 180-560μg/ml). Several clinical examinations, such as ultrasonic echo, CT, T1 scintigraphy and angiography were performed. Angiography and ultrasonic echo also demonstrated parathyroid tumors in seven patients. Six patients were detected by T1 scintigraphy while four patients were detected by a CT scan. In addition, hypocalcemia was observed in five patients after surgery. The serum calcium levels returned to normal in four patients, while, in addition, one patient developed parathyroid adenoma post operatively.

    We herein report a case of pseudolymphoma of the thyroid. The patient was a 44-year-old female who was pathologically diagnosed by an immunohistological examination. In rare cases, pseudolymphoma has been observed to change into malignant lymphoma and as a result, it is essential that such occurrences of pseudolymphoma be closely observed.

    Between 1982 and 1993, 168 patients were surgical treatment for thyroid disease in our department (37 males and 131 females). Ninety-seven of these patients (57.7%) were pathologically diagnosed to have benign diseases while 71 other patients (42.3%) were diagnosed to have malignant diseases. The diagnosing methodology to pathologically disinguish malignant tumors and benign ones is also discussed. We selected age at surgery, tumor size and tumor extension as the predominant factors for analyzing the prognostic factors in carcinomas. As a result, the tumor extension index demonstrated prognostic significance while age and tumor size did not.
