Forty-six patients with hypopharyngeal carcinoma, who were treated at the Department of Otorhinolaryngology, University of the Ryukyus, Faculty of Medicine from 1985 to 1991, are herein reviewed. The three-year crude survival rate according to the primary site was 45% at the pyriform sinus, 65% at the posterior wall and 50% at the post cricoid, while regarding the stage classification stage I was 57%, stage II was 50%, stage III was 65% and stage IV was 30%. The survival rate of stage N was statistically worse than that of the other stages. The overall 5-year cumulative survival rate was 45.7%. According to the surgical procedure, 3-year crude survival rate was better for a total resection, although no statistical difference was found. A partial resection of the pharyngeal wall with more than a two centimeter safety margin is therefore not expected to result in local recurrence.

A 36-year-old fisherman gradually developed inspiratory dyspnea, stridor and numbness of the extremities during heavy labor. Flexible fiberscopic laryngoscopy and fluoroscopy suggested a flaccid epiglottis occluding his airway due to its posterior prolapse during maximal inspilatory effort. Hyperventilation syndrome might be associated with the abnormal dynamics of the epiglottis. His symptoms disappeared following a partial epiglottectomy with a CO2 laser under an operating microscope. This flaccid epiglottis showed histologically poor staining for alcian blue thus indicating a diminished acidmucopolysaccharide-cartilage matrix.

Three rare cases of odontogenic disease in our clinic are horein reported along with a review of the pertinent literature. The first case was a 64-year-old female with periodontitis of the third molar which had first become swollen in the parotid region and was initially diagnosed in our clinic to be acute parotitis. The second case was a 47-year-old male with a radicular cyst which demonstrated tenderness in the nasal vestibule and thus was diagnosed as a vestibular cyst. The third case was a 69-year-old male with an external dental fistula which had first been diagnosed as an infectious atheroma of the sub- mandibular skin. The most important point of these bases is that odontogenic diseases should be suspected if any periodontal mucosal changes or percussion pain of the tooth is found. In addition, a pantomogram and dental X-rays are also considered to be useful in making a diagnosis.

The time intensity curves for gadolinium-diethylene triaminepentacetic acid (Gd-DTPA) enhanced magnetic resonance imaging (MRI), namely dynamic MRI, were determined for thyroid diseases, and compared with findings of histopathologic examination. Time intensity curves for solid lesions were determined, excluding cases with secondary changes such as calcification, hemorrhage, necrosis, and fibrosis. Three different patterns of time intensity curves were observed: gradual, rapid and no change. For almost malignant diseases and a few benign diseases with marked cell proliferative ability, the time intensity curve displayed the gradual attenuation pattern, in which intensity was above half-maximal value within 10 minutes after injection Gd-DTPA. Almost all benign diseases and a well differentiated carcinoma displayed a rapid attenuation pattern, in which intensity was decreased to lower than half of peak grade within 10 minutes following injection. Benign diseases with normal cell density exhibited no change. These findings suggested that the time intensity curve obtained from dynamic MRI may indicate grade of cell differentiation and proliferative ability.

The clinical evaluations of nine patients (15 sinuses) who underwent both osteoplastic maxillary surgery and endoscopic sinus surgery for chronic paranasal sinusitis are herein described. The patients' ages ranged from 12 to 60 years of age with an average of 39, and they were followed for more than one year after surgery. This osteoplastic technique was mainly indicated for the patients with severe maxillary sinusitis who could not visit for regular check-ups after surgery. Each patient rated the severity of the nasal symptoms pre-and postoperatively. The nasal symptoms except choanal discharge were improved in over 75% of the patients, although nearly 50% of the patients still complained of choanal discharge. The quantitative evaluation of X-ray opacity of the maxillary sinus using a digital disposition system revealed an excellent healing of the inflammatory changes in the maxillary sinus with good aeration. This suggest that osteoplastic maxillary surgery may thus be effective in preventing the formation of postoperative maxillary cysts.

Using laser-Doppler flowmeter, the cochlear blood flow (CoBF) in response to an interruption of the internal auditory artery (IAA) was observed in guinea pigs. In order to interrupt the IAA, the bundle of vessels and nerves in the internal auditory canal was compressed and dissected. After compression, CoBF decreased to 30%-50% of the original value with a wide individual variation. The CoBF reading even after the dissection of the IAA did not reach zero and showed the same value of compression in the same animal. The residual CoBF after the dissection of the IAA was not influenced by the dynamic change in the systemic blood pressure, and thus no other blood supply to the cochlea was considered. The residual CoBF may thus be derived from the persistent venous flow through the cochlear aqueduct. Based on the above results, we concluded that to correctly estimate the decreasing degree of the CoBF during the compression of IAA, the calibration excluding the residual CoBF after the dissection of the IAA is neces- sary.

We examined the expression of proliferating cell nuclear antigen (PCNA) and epidermal growth receptor (EGFR) in tissues of cholesteatoma and chronic otitis media using immunohistochemical technique. We determined whether or not a relationship exists between the ratio of PCNA-, EGFR-positive cells and the bone destruction of cholesteatoma. The degree of bone destruction was graded using a semiquantitative scale of 0 through 6. PCNA-positive cells were higher in the epithelium and granulation of cholesteatoma than those of chornic otitis media. The bone destruction due to cholesteatoma was significantly related to both the ratio of EGFR-positive cells (r=0.91, p< 0.01) and that of PCNA-positive cells (r=0.75, p< 0.01). In the granulation just at the edge of the areas of bone destruction, many PCNA-positive cells were found. The ratio of PCNA-positive cells was related to the ration of EGFR-positive cells (r=0.79, p < 0.01). These results suggested that the cells in cholesteatoma were actively proliferating and thus resulted in bone destruction.

Changes in positive rate of scratch test with aging were examined, in the same students who had collective physical examination and relevant scrath test in both years of 1990 and 1993 in Shiraoi-cho, Hokkaido. The first graders in the elementary school in 1990 were in the fourth grade in 1993, and those in fourth grade in 1990 were in the first year in junior high school in 1993. In both groups, the positive rate of scrath test was significantly in the years of 1993 compared with the same in 1990. The increase in the positive rate of scrath test with the aging of the students is considered to result from the duration of period during which the students were exposed to allergen.

In 1965 Soeda and Sambe extracted an antigenic substance (peptides) from the urine of allergic patients and named it MS-Antigen (R)(MS-A). This agent has an antigenic character and has no allergy provoking property. The most striking finding is that both anaphylactic shock and Arthus reaction are suppressed by this agent. Ten subjects, whose allergens had already been identified through skin testing to be derived from Japanese cedar pollen, etc., were randomly selected from the outpatients who had been regularly administered MS-A (40mg) twice a week. The marked clinical effectiveness of MS-A was thus proven without any local or systemic side-effects. Subsequently, we have since been extending our investigation to the therapeutic effects of MS-A on such allergic diseases as pollinosis as well as other diseases that are all more or less related to allergies, from both a clinical and immunological point of view. This study discusses the clinical aspect of MS-A regarding such allergic diseases as pollinosis, as well as other diseases including Behcet's disease and Pustulosis palmo-plantaris.

Fundamental and clinical studies on the efficacy of Pazufloxacin (PZFX) for tonsillitis and pharyngolarygitis were performed in a multicenter cooperative system with the following results:
1. The concentration of PZFX administered at a single oral dose of 200mg was 0.04-1.68μg/g (0.68-0.81; ration to the serum level) and 0.40-2.04μg/g (0.21-1.22) in the tonsil and submandibular gland, respectively, which thus indicated food tissue penetration.
2. The efficacy rate was 85% (29/34) for lacunar tonsillitis, 4/7 for peritonsillitis, and 5/5 for pharyngolaryngitis.
3. The bacteriological eradication rate was 100% (28/28) in monomicrobial infection and 8/9 in polymicrobial infection.
4. The MIC values of PZFX against the total clinical isolates ranged from≤0.025-6.25μg/ml, which were almost equal to those of Ofloxacin (OFLX).
5. Adverse reactions were only observed in 2 of 57 cases (1 case each of diarrhea and skin rash, respectively) and they were moderate or slight in severity. Abnormal laboratory findings were observed in 4 cases (1 case respectively of elevated GPT, eosinophilia, an elevated urinary sugar level, and an elevated level of GOT·GPT·ALP·γ-GTP and urinary protein).
These results therefore suggest that PZFX is a very useful drug for the treatment of tonsillitis and pharyngolaryngitis.

In order to evaluate the efficacy, safety of Pazufloxacin (PZFX) for otitis media and otitis externa, a general clinical trial, using a multicenter integrated protocol was performed in addition to the studies on the sensitivity distribution of isolated bacteria and the pharmacokinetics, including middle ear mucosa. The results were as follows:
1. The sensitivity distribution of 100 strains, isolated from patients with otitis media and otitis externa, was 2-4 times superior to Ofloxacin (OFLX) and almost equal to Ciprofloxacin (CPFX).
2. The concentration of PZFX in the tissue including middle ear mucosa, in two hours after administration at an oral dose, reached 1.20-3.60μg/g (2.39; ration to the serum level-average), which thus indicated good tissue penetration.
3. The efficacy rate in 66 evaluable cases was 79% (52/66) for otitis media and 91% (20/22) for otitis externa.
4. Adverse reactions were observed in 4 of 103 cases (incidence: 3.9%), which were mainly consisted of gastrointestinal disorders.
As for abnormal laboratory changes, elevated GOT and GPT levels were observed in 1 case. These results therefore suggest that PZFX is a very useful drug for the treatment of otitis media and otitis externa.

In order to evaluate the efficacy and safety of Pazufloxacin (PZFX), a new oral quinolone antibacterial agent, for sinusitis, a general clinical trial using a multicenter integrated protocol was performed in addition to a pharmacokinetic study, including the nasal mucosa. The results were as follows:
1. PZFX showed a good penetration into the maxillary sinus mucosa, maxillary sinus retention fluid, nasal polyps and ethmoid sinus mucosa.
2. The efficacy rate in 45 evaluable cases was 83% for acute sinusitis treated with 100 or 200mg of PZFX 2-3 times a day and 80% for acute exacerbation of chronic sinusitis. The overall efficacy rate was 82%.
3. The improvement rate of the X-ray findings in 37 cases was 65%.
4. The bacteriological eradication rates were 82% for gram-positive and 100% for gram-negative bacteria in the case of a monomicrobial infection. The overall eradication rate was 78% when polymicrobial infection cases were included as well.
5. No adverse reaction occurred in any examined case. As for the abnormal laboratory findings, eosinophilia and increased monocytes in 1 patient each were observed and only mild in severity. Based on these results, PZFX was thus suggested to be a very useful drug for the treatment of sinusitis at 300-600mg a day.

Two cases of foreign bodies of foods were reported. The first case was 66-year-old man suffering from necroses of a finger of the foot. He had left hemiparalysis due to cerebral thrombosis. He was admitted for the surgery of the necrosis. He was found to be choked in the morning. Emergency incubation was performed on so that salad of tuna, cabbage, onion and carrot were taken out by suction. The second case was a 50-year-old man who had have cerebral bleeding and cerebral thrombosis. He was found to be unconscious in the afternoon. His breathing and heart were stopped, but emergency care succeded. A piece of candy-type jelly was found in the oral cavity, The jelly which occluded the airway was suspected to exist in hypopharynx. He was performed on cricopharyngeal myotomy and laryngeal elevation, which made him to eat and drink more easily.

Swallowing function after hemiglossectomy in two patients with tongue cancer was evaluated using videofluorography and manometry. All patients underwent tracheotomy, laryngeal suspension, radical neck dissection and hemiglossectomy with pectorial major myocutaneous flap reconstruction. Mild or moderate aspiration of contrast medium was observed in all two cases. Retention of contrast medium in valeculla and piriform sinuses was also observed. However, almost all contrast medium was transported into esophagus smoothly. Elevation of larynx and opening of esophageal inlet were almost normal. Dec- reased swallowing pressures in the mesopharynx were observed in all patients. However, pressures in the hypopharynx and the cervical esophagus were almost normal in all patients.

It has been reported that rehabilitation training for dysphagia using a high-viscosity bolus is useful for improving the swallowing function with a lower risk of aspiration. However, the choice of the most appropriate food for each patient depending on their specific condition has yet to be established. We used Isocal® Pudding as a rehabilitation material for treating swallowing dysfunction. This material, developed as a high-caloric supplemental diet, contains alpha-starch and is easily prepared at any desired viscosity by adding the proper quantity of water. Moreover, the rehabilitation program with Isocal® Pudding is based on the fluoroscopy findings using a bolus made of alpha-starch combined with an intravascular contrast medium. In this article, we introduce this rehabilitation program while presenting a typical case. In addition, we also discuss some of the advantages and disadvantages of this training based on the findings of a patient questionnaire study.
