    The literature on therapy with Chinese traditional herbal medicine (Kampo therapy) in otolaryngological diseases, which has been published over the past six years (from 1989 to 1994) is herein summarized. The contents were abstracted regarding each respective disease. It is encouraging to find that the number of articles are increasing every year. The accurate cure rates using Kampo therapy for seven major diseases such as: serous otitis media, tinnitis, vertigo, allergic rhinitis, paranasal sinusitis, pharyngitis, and pharyngeal global sensation, are shown in the tables. The excellent cure rates of each disease were pointed out. In addition, fundamental studies supporting the clinical usefulness of Kampo drugs were also reported, Today, interest in Oriental medicine is growing, even in Western countries. Kampo therapy is considered to be useful in improving patient QOL. Therefore it is highly desirable that research on Kampo medicine continues to increase even further in the future.

    The Eustachian tubal function was compared in 54 ears of 27 skindivers and 88 ears of 44 normal adults. The active opening function was determined by a sonotubometric Eustachian tubal function test, which measured the duration of patency of the Eustachian tube while swallowing. The passive opening function was determined by tubo-tympanoaerodynamic graphy (TTAG) and Valsalva's maneuver. Both X-ray and computed tomography examinations of the temporal bones were performed on 12 ears of 6 skindivers.
    1) No significant differnce were found, regarding the duration of patency of the Eustachian tube while swallowing, between skindivers and normal adults. The opening pressure of the Eustachian tube during Valsalva's maneuver was defined as the naso-pharyngeus pressure. It was found that a higher pressure was required to open the Eustachian tube of skindivers than that of normal adults.
    The longer skindivers had been diving, the higher the opening pressure of the Eustachian tube was. From the above results, it appeared that this higher mean pressure might be the resulted of the effect of ventilation training on the Eustachian tube due to the increased pressure in a hyperbaric environment.
    2) Seven skindivers ears also revealed the presence of an underdeveloped pneumatization in their temporal bones. Eight skindiver ears also showed hypertrophy of the mastoid cell septa. These findings indicated that the osseous tissue surrounding the middle ear appeared to be a reaction to such stimuli as high pressure which is continually exerted in deep water.

    The efficacy of a continuous epidural block was evaluated in patients with sudden deafness. The patients were divided into two groups which ten patients being treated with steroid therapy and a cervicothoracic continuous epidural block instead of a stellate ganglion block (group A), while another 10 patients were treated with steroid therapy and a stellate ganglion block (group B). The continuous epidural block was done by infusing 1% mepivacaine at the speed of 1ml/hr through an epidural catheter while using a disposable infusor. The effective rate was 70% in group A and 30% in group B. There was no significant difference in the factors affecting the prognosis of sudden deafness including; vertigo, hearing level, age at onset, etc., between group A and B. These findings suggested that a continuous epidural block using a disposable infusor may therefore be useful in the treatment of sudden deafness.

    Between 1983 and 1992, fifty-eight patients with carcinoma of the oropharynx were treated at Kagoshima University Hospital. The study group consisted of 56 men and 2 women (median age 61.2 years), and 56 cases of squamous cell carcinoma, 1 case of adenoid cystic carcinoma and 1 case of undifferentiated carcinoma. The local tumor control rates at 5 years according to the T stage were as follows; T1: 84%, T2: 89%, T3: 74% and T4: 0%. The neck tumor control rates at 5 years according to the N stage were NO: 95%, N1: 78%, and N2-3: 59%. Distant metastasis developed in 14% of the patients. Second primary cancer developed in 9% (5 cases) of the patients including 4 cases of esophageal cancer. In addition, the five-year cumulative survival rate was 51%.

    A 72-year old male with extramedullary plasmacytoma in the left nasal cavity is reported. He was treated with a combination surgical operation, irradiation and chemotherapy. After the extirpation of the tumor, IgA and the λ. light chain was demonstrated in the cytoplasma of the tumor cells by using an immunohistochemical method (the PAP method). It is most important to distinguish extramedullary plasmacytoma from multiple myeloma because the prognosis of the two entity is quite different. In addition, we should never forget the fact that extramedullary plasmacytoma might later change into multiple myeloma. Therefore, the detection of the tumor producing immunoglobulin either in the serum or urine is considered to be valuable in following up this disease.

    A case of papillary carcinoma arising in a thyroglossal duct cyst is reported. A 28-year-old woman presented with an asymptomatic mass in the submental midline region. A neck X-ray, ultrasonogram and a CT scan suggested the mass to be a thyroglossal duct cyst with some calcification. Surgery using the Sistrunk procedure was performed. At operaition, the thyroid gland was found to be normal and no palpable cervical lymphadenopathy was observed. A histological examination revealed the existence of papillary carcinoma in a part of the cyst wall. Thyroid scintiscans and a cervical ultrasonogram done after the operation showed no abnormalities. We examined 149 reported cases in the world literature and concluded that for this case a local excision using the Sistrunk procedure would be sufficient and no further treatment was needed. Although rare, 1-2% of thyroglossal duct cysts may contain cancer foci. In treating this disease a careful examination is therefore called for.

    From 1982 through 1992, 19 patients with malignant parotid gland tumors were treated at the Department of Otorhinolaryngology, University of the Ryukus, Faculty of Medicine. Twenty-six percent of these cases consisted of squamous cell carcinoma, while 21% were adenoid cystic carcinoma, 16% adenocarcinoma and 16% mucoepidermoid tumors and 11% acinic cell tumors. Six patients (32%) had recurrent tumors, and all were cases with high grade malignant tumors, while 5 of them were advance cases and presented with either stage III or IV at the first visit. It was suggested that the survival of patients with malignant parotid gland tumors improved with combination therapy including surgery, irradiation and chemotheapy.

    Osteosarcoma is one of most common malignant tumors of bone, and is only rarely encountered in the temporal bone. A 19-year-old woman suffering from osteosarcoma of the left temporal bone visited our clinic complaining of otorrhea and hearing difficulty in the left ear. A pure tone test and audiogram revealed a combined deafness of 70.0 dB in the left ear. A CT scan demonstorated a tumor in the area of the mastoid and middle ear with an involvement of the carotid canal and jaw joint. Chemotherapy was invalid, with HD-MTX, CDDP, ADR, CBDCA and IFX using the venous route. In additon, a local introduction of chemotherapy consisting of CDDP (120mg) and THP-ADR (75mg) to the superficial temporal artery was tried and found to be effective, resulted a sclerotic change in the tumor.

    The time course of the quality of life (QOL) for patients with head and neck cancer was evaluated by the use of a QOL check list made by the Japanese Ministry of Health and Welfare. This QOL check list is considered to be appropriate for the time course observation of QOL factors which thus makes it possible to compare each factor separately. Such information may also help us in selecting the optimum treatment to improve the QOL for patients with cancer. The assessment of the QOL should therefore be performed while taking the time course observation into consideration.

    A 32-year-old female suffered from a continuous nasal obstruction. The middle turbinate of her right nose was dark red and showed marked swelling. A coronal CT scan of the paranasal sinuses showed pansinusitis on both sides. After resection of the lateral lamella of the right middle turbinate, which containd white muco-purulent contents, a coronal CT scan showed that the pathological findings of both paranasal sinuses disappeared. The authors also discuss the origin of concha bullosa based on previous reports in the literatures.

    In order to examine whether or not laryngeal allergies can induce pharyngolaryngeal abnormal sensations, we analyzed the result of scratch tests against house dust, mites, and ceder pollen during an annual student collective physical examination in Shiraoi-cho, Hokkaido, in 1989, 1990, and 1991, while also examining the correlation between the pharyngolaryngeal symptoms and the positive rate of the scratch tests. The subjects consisted of both elementary and junior high school students in the city, and included 2, 677 cases in all. The pharyngeal symptoms were first detected by a questionnaire sent out one month before the collective examination. Correlation were then examined by X2test in each combination of the following; (1) the positive scratch test result against one or more of the three allergens,(2) the presence of pharygeal symptoms, and (3) the presence of rhinostenosis. A correlation was found between the positive scratch test result and the presence of rhinostenosis. Although we cannot deny the possibility that the pharyngeal symptoms of such students were the direct result of an allergy, it does seem likely that such pharyngeal symptoms were probably induced by allergic rhinostenosis and oral breathing. However, it should be understood that allergic factors may only play an indirect role in the cause of pharyngolaryngeal abnormal sensations.

    The Clinical effects of Celtect (R) on cedar pollinosis were investigated for 3 years in various areas of Hiroshima Prefecture.
    1) Pollen scattering was observed vary from one area to another. In addition, the total pollen scattering in 1992 was less than that seen in 1991 and 1993.
    2) The response rate in the treatment group was 75% in 1991, 82% in 1992, and 75% in 1993 respectively. Thus, the pollen amount was considered to have little influence on these rates. However, the response rates in the preventive treatment group and the initial treatment group were 67% and 92% in 1991, 100% and 85% in 1992, and 71% and 30% in 1993, respectively. A sufficient response was obtained from both the preventive treatment and the initial treatment groups administered this drug alone in 1992 when there was less pollen scattering.
    3) An adverse drug reaction occurred in 30 cases (4.7%) over the 3-years-period which mostly consisted of drowsiness.

    The clinical effect of terfenadine was investigated in 42 patients with perennial nasal allergy. In addition, the concentration of ECP in the serum was measured before and after treatment in 32 patients. A moderate and more than moderate improvement was observed in 56% of the patients while a slight or greater improvement was seen in 85%. No adverse effect was seen in any of the patients. The serum ECP levels significantly decreased according to the treatment from 16.7±10.4ng/ml to 11.8±8.9ng/ml. This tendency was the most significant in the patients showing a remarkable improvement. The ECP levels in the serum were associated with the severity of the clinical symptoms of this disease. The ECP levels in the serum significantly correlated to the eosinophil count number in the peripheral blood. It thus seems that terfenadine is useful in the treatment of patients with nasal allergy and may thus help reduce the serum ECP level, which seems to be a useful clinical parameter to estimate the severity of this disease.

    A platelet function test was performed on patients with vertigo who were suspected of having circulatory disorders in the head and neck based on the initial clinical findings, and 22 of these patients, who were found to have exacerbated platelet aggregation, were used as subjects in a study on the efficacy of Beraprost sodium (Dorner (R)). The following results were obtained:
    1. The improvement of the subjective symptoms was excellent in 12 patients, good in seven, fair in two and unchanged in one patient for an improvement rate of 86%, which thus indicated a high efficacy.
    2. The improvememt in 12 patients with positional nystagmus confirmed before treatment was excel- lent in four, good in five, fair in two and unchanged in one patient for an improvement rate of 75%.
    3. In 12 patients with stepwise ataxic following movement in a target following the movement test before treatment, the improvement was excellent in one, good in five, fair in two and unchanged in four patients for an improvement rate of 50%.
    4. Platelet aggregation was significantly inhibited after treatment.
    5. No subjective or objective adverse reaction were observed. Based on these results, Beraprost sodium was considered to be useful in improving vertigo symptoms in vertogo patients with exacerbated platelet aggregation.

    The clinical effects of nasal hyperthermotherapy in patients with nasal hyperresponsiveness were studied. The subjects consisted of 44 perennial nasal allergy cases and 4 cases of vasomotor rhinitis. The patients all inhaled saturated hot air (43°) daily at home for either two or three 10 minute sessions using the Skainar-Steam (EISAI) for 4 weeks. The nasal symptoms and signs were examined before treatment and at 2 and 4 weeks after the start of treatment. The rate of effectiveness (excellent and good) after 2 and 4 weeks were 39% and 56%, respectively. No adverse effects were noted. Regarding the symptoms, this treatment was the most effective for nasal obstruction. One possible mode of action might be related to an inhibition of degranulation from the mast cells. These results thus suggest that local hyperthermotherapy may thus have a positive effect in inhibiting nasal symptoms and signs and may therefore become a useful modality in the treatment of nasal hyperresponsiveness.
