One hundred forty-eight patients with squamous cell carcinoma of the oropharynx treated at the division of the head and neck surgery, National Cancer Center Hospital from 1962 through 1979 were reviewed and treatment results of 146 cases were analysed. The primary lesion was located in the lateral wall in 86 patients, the anterior wall in 41, the superior wall in 15 and the posterior wall in 6. The overall five-year crude survival rate was 45%.
The five-year survival rate according to TNM classification (UICC 1978) was 82% in stage I (21 cases), 52% in stage II (27 cases), stage III 42% in (52 cases) and 21% in stage IV (31 cases) respectively. Remaining four cases were treated as cases of primary unkown cervical metastasis. One hundred and seventeen patients were treated with radiation therapy, 28 with surgery and one with chemotherapy. Secondary treatment for residual or recurrent tumor was performed in 36 cases and 6 patients were survived for 5 years.
Analysis of these series supports the recommendation that radiation therapy is the treatment of choice for early stage of oropharyngeal cancer, whereas surgical excision plus radiation should be employed in advanced cases.

In order to minimize the post-therapeutic functional disorders, we have used CO
2 laser in treating carcinomas of the oropharynx since 1982. We have employed three types of laser usage. Carcinomas confined to the mucosa are treated with laser excision alone. Carcinomas with a superficial muscular invasion are treated with mass reduction laser surgery followed by chemoradiotherapy. To carcinomas with a extensive muscular invasion, we first attempt an induction chemotherapy consisting of cisplatin and peplomycin. When the chemotherapy is effective, we apply mass reduction laser surgery followed by chemoradiotherapy. This paper describes the technique and preliminary results.

Since 1981 we have been studying the effect of 1-hexyl arbomoyl-5-fluorouracil (HCFU) on head and neck tumors. HCFU is converted into the active FU form, with prolonged high retention in blood and tissues. We also applied HCFU combined with radiation therapy. In the present study, we followed up patients received primary treatment at our department from 1981 to 1982, with special viewpoints of the primary effects and 5-year survival rates in a concomitant HCFU and radiation therapy.(1) Effect of primary therapy and 5-year survival rates based on stage classification.(2) Effect of primary therapy with and without after-treatment following concomitant therapy with HCFU and radiation. HUFU has a higher liposolubility than 5FU-type drugs, and is rapidly absorbed by oral administration. This maintains high concentration in the blood and in tumor tissue. These characteristics indicate that concomitant use with radiation is more effective in the treatment than other types of 5FU. It is possible that HCFU improves the result of nonsurgical therapy in head and neck cancers and plays a useful role in a combined therapy.

Carcinoma of the epipharynx is one of the most difficult head and neck cancers to be treated successfully. Treatment modality for carcinoma of the epipharynx in Kurume University was introduced. Cis-platinum-peplomycin chemotherapy is firstly done and chemoradiotherapy (FAR) was performed. After chemoradiotherapy, OK-432 was dosed every week in out-patient clinic. Ten cases of nasopharyngeal carcinoma were treated with this treatment modality from March 1984 to January 1987. Better end result of nasopharyngeal carcinoma is expected by this treatment modality.

Research was conducted on the therapeutic results of simultaneous application of radiation and UFT on 17 cases of laryngeal cancer in comparison with the cases to which only radiotherapy was applied. None of the cases were in grave condition. A few cases of side effects were observed such as an absolute decrease of leukocytes and lymph corpuscles, a rise in GOT and GPT, celialgia, intensified mucitis and dermatitis caused by the radiation. Final therapeutic effects still remain to be observed since there have been a relatively small number of cases studied during the short period of observation. However, no unfavorable data have been generated and, in some cases, even favorable progress has been reported in the patients' condition.

Seventy-three cases of carcinoma of the mesopharynx treated from 1973 to 1985 at Gunma Cancer Center were evaluated. Twenty seven cases were treated with external irradiation combined with intraarterial chemotherapy and 21 cases were treated with external irradiation alone. These two treatments were compared and the following results were obtained.
1. According to TNM classification (UICC, 1983), there were no difference between the two groups of chemotherapy combined group and radiation alone group. Intraarterial chemotherapy was used for the lesion which developed to two or more regions of the mesopharynx.
2. The treatment of external irradiation combined with intraarterial chemotherapy for the mesopharyngeal carcinoma was evaluated as a intermediate treatment between radiation alone and surgery.
3. The five year cumulative survival rate in external irradiation alone was 45%. In groups with external irradiation combined with itraarterial chemotherapy, 5 year survival rate was 37.7%.
