    In order to investigate the effect of an earplug in experimentally-induced barotrauma, an earplug was fitted in the external ear cannal of a guinea pig. Cochlear action potentials were recorded after compression and decompression in extremely short period (2-3 seconds/1ATA-2ATA). Morphological changes were investigated by both scanning electron microscope and light microscope. A shift at high frequencies of the threshold of AP was noted in ears with an earplug. Degeneration and/or disappearance of stereocilia of outer sensory cells of the basal turn and hemorrhage in the scala tympani were observed in ears fitted with earplugs. The hemorrhage originated from veins of the scala vestibuli and collecting venules. In ears without an earplug, the cochlea was morphologically almost intact.

    In the diagnosis of the small acoustic tumor (AT), ABR and CT scan have recently been the most usefull method. We evaluated an intracanalicular acoustic tumor using ABR and Air CT cisternography. A 49-year-old woman had a sudden episode of vertigo and sensation of unsteadiness. She did not complain of hearing problem and her audiogram was normal. On ABR testing, I-V inter-peak latency (IPL) was prolonged (5.04 msec), and I-V IPL difference was abnormal (1.20 msec). A CT demonstrated a slightly widened left internal auditory canal, but no enhanced mass. ABR testing was repeated using ipsilateral and contralateral recording, and reaffirmed that the I-V IPL was prolonged. We diagnosed a small size AT. An intracanalicular tumor (5mmφ) was found on Air CT cisternography with target imaging. With the team effort of otologists and neurosurgeons, the tumor was removed, preserving the hearing. Her hearing remained at the preoperative level and the I-V IPL decreased (-0.30 msec).

    Diagnostic A-mode ultrasonography has been found to be not only a simple and reliable means to provide early diagnosis of maxillary sinus diseases, but also an improved technique in monitoring the progress of treatments. A comparison between the ultrasonography and conventional X-ray examinations was made on 74 patients.

    Two cases of periesophageal abscess were reported. The first case was examined by direct esophagoscopy and the second case by fiberscope, searching for presumed foreign bodies. No foreign body was found in either case. After endoscopy, in both cases pain started to increase and a periesophageal abscess had already been formed when they first visited our clinic. It seems that the endoscopic procedure caused esophageal perforation and/or infection through a possible injury in the esophageal wall occasioned by the foreign body. The second case suggests that even fiberscopy is dangerous in examining for a foreign body which may have injured the esophageal wall. The periesophageal abscess of the second case, which was localized in the neck, was cured by drainage under direct esophagoscopy. However, in the first case, because the abscess rapidly extended into the mediastinum, surgical treatment was required. Sufficient drainage of the mediastinal abscess was achieved by postoperative continuous suction and frequent cleaning.

    A clinicopathological study of nasal septal tumors was made on 14 cases including 2 pleomorphic adenomas, 3 hemangiomas, 3 squamous cell carcinomas, 1 transitional cell carcinoma, 1 malignant melanoma, 1 extramedullary plasmacytoma and 3 malignant lymphomas. A statistical review of the Japanese literature showed that a wide variety of primary tumors of the nasal septum have been found, but the total incidence is relatively low. A definite histological diagnosis of these tumors at the first biopsy was sometimes difficult because of mechanical damage of the specimen and difficulty in getting sufficient by large biopsy specimens. Early and accurate diagnosis is of vital im ortance to ensure timely commencement of proper treatment.

    A 71-years-old man complained of the swelling and pain of the tongue for the last 4 years. The movement of the tongue was disturbed by an infiltrated tumor. The surface of this tumor was irregular, and easily bled. The tumor was diagnosed as verrucous carcinoma. It was inverted into muscle layer and had many microabscesses. Chemotherapy was given intravenously with a total dose 180 mg of peplomycin. As a result, the tumor was reduced and become softer. Two months later, however, the tumor recurred and gradually increased dysphagia gradually. The patient was died of cachexy 14 months since the first treatment. Postmortem examination gave no evidence of metastasis. Seventeen cases of verrucous carcinoma of the oral cavity, reported in Japanese literature, were reviewed.

    A case of 57-year-old male with a large painless tumor of the submandibular gland, occupying at the right submandibular region and neck, is presented. A malignant tumor was suspected by the medical history, C. T., X-ray, R. I. with 67Ga scintigram, and echogram. The tumor was removed surgically. The tumor size was 15×11×10cm, and it weighed 540g. The tumor was covered with a thin capsule, the inside of the tumor was partially cystic and contained yellowish transparent fluid. Histopathological examination revealed a malignant pleomorphic adenoma. Pleomorphic adenoma of the head and neck region is often seen in the parotid and palate glands, but the tumor in the submandibular gland is relatively rare. The authors discussed an additional 13 cases of pleomorphic adenoma in the submandibular gland treated surgically in our deparment in the past 5 years. In Japan, malignant pleomorphic adenoma in the submandibular gland, including low grade malignancy, appears to be more common than in other countries.

    During the period December 1, 1983 to June 1, 1985, 36 patients with advanced, recurrent and/or metastatic malignant tumor in the head and neck were given cisplatin and peplomycin. The overall cumulative response to this chemotherapy was 53%. However the response rate for squamous cell carcinoma was 67%. Although it was difficult to evaluate the actual and statistical significance of this anticancer drug in comparison with other chemotherapeutic agents, cisplatin seemed to be effective in treating head and neck squamous cell carcinoma in order to prolong of the survival rate. No significant side effects were detected in this study. From our results we wonder if a higher dosage may improve the currative ratio.

    Out of 102 sudden deafness cases who were treated at Kanto Teishin Hospital, 57 cases were given intravenous drip of Methyl B12 of 2000μg per day with other drugs as steroid and ATP. Methyl B12 group and non-Methyl B12 group were subdivided into 16 classes according to the two variables that were known to influence recovery from sudden deafness. Initial audiogram was divided into 4 classes and the time since onset was also divided into 4. Wilcoxon two sample tests were conducted on the hearing improvement between MB group and non-MB group in each class at 6 frequencies. MB group showed significantly greater improvement of hearing than non-MB group at 250 and 500Hz (p<0.05).

    The transfer of Cefoperazone (CPZ) into otorrhea, and the clinical effectiveness of CPZ as a therapeutic agent were studied. 1. The levels of CPZ in otorrhea follwing intravenous injection of 2g CPZ were determined by bioassay using E. coli NIHJ as a test organism. The peak CPZ level in otorrhea was seen about 60 minutes after injection in patients who secreted relatively small amount of otorrhea, while that of in patients who secreted moderate to large amount of otorrhea was tended to be observed earlier than 60 minutes. 2. A total of 21 patients suffering from chronic otitis media (12), cholesteatoma (2) and acute otitis media (1) or recieving radical mastoidectomy (3) were treated with 1 to 4g/day CPZ during pre- and/or post-operative period. Of these 21, 17 (81.0%) had a satisfactory clinical response following CPZ therapy. 3. Clinical effectiveness in terms of major causative organisms, of 7 cases in whom P. aeruginosa was isolated, and of 4 cases in whom S. aureus was isolated, 5 (71.4%) and 3 (75.0%), respectively were effectively treated with CPZ. 4. No statistically significant correlation was observed between the levels of CPZ in otorrhea and clinical effectiveness.

    Phase II of swallowing was radiologically classified in 36 patients with laryngopharyngeal dysesthesia. Eperisone hydrochloride (Myonal®), a muscle relaxant, was administered orally to those patients to determine its effects on the subjective symptoms of laryngopharyngeal dysesthesia. The response was classified as excellent in 4 patients (11%), good in 14 (39%) or fair in 7 (19%). Briefly, an improvement in subjective symptoms of laryngopharyngeal dysesthesia was produced by eperisone hydrochloride in 69% (25/36) of patients. When the results were reviewed in relation to swallowing, the drug produced an improvement in 88% patients (21/24) with epitonic swallowing. It deserves special mention that the drug elicited a higher rate of response in elderly males over 50. Our results seem to justify the conclusion that eperisone hydrochloride is a useful drug for laryngopharyngeal dysesthesia.
