    We herein report the first case of allergic otitis media identified by an immunological examination of both otorrhea and serum. A 44-year-old female had an 18 month history of bilateral continuous otorrhea and hearing loss. These symptoms worsened from February to April when her nasal allergy to Japanese cedar pollen (Cryptomeria japonica) appeared. Her tympanic membranes were centrally perforated with a remarkable amount of viscous secretion containing many eosinophils and the promontory mucosa was also considerably swollen. An immunological examination showed a positive finding for the CAP-RAST score for Japanese cedar pollen, not only in the serum but also in the middle ear secretion. Because these facts demonstrated type I allergy to Japanese cedar pollen due to the existence of IgE antibody in the middle ear, she was diagnosed to have a middle ear allergy to Japanese cedar pollen. Combination therapy using anti-allergic drugs and steroids (p. o.), as well as the intratympanic injection of steroids and myringoplasty resulted in a good improvement in the otorrhea symptoms and, as a result, her hearing loss also recovered.

    Relapsing polychondritis (RPC) is a disorder characterized by the inflammation and destruction of the cartilaginous structures in multiple organs. The six typical diagnostic signs are bilateral auricular chondritis, nonerosive sero-negative inflammatory polyarthritis, nasal chondritis, ocular inflammation, respiratory tract chondritis and audiovestibular damage. Auricular chondritis has been reported to be the most common presenting manifestation in those symptons. We herein report a case of bilateral auricular chondritis in a 91-year-old male due to relapsing polychondritis. An early diagnosis could be made based findings of an auricular biopsy using Damiani's criteria for diagnosing RPC. The symptoms of auricular chondritis remarkably improved following steroid therapy and no relapse of the disease has yet been recognized to date. It thus concludes that bilateral auricular chondritis may be an important sign for making an early diagnosis of RPC.

    We herein report two rare cases of gustatory sweating. One case was a 26-year-old man, while the other case was a 55-year-old man. The chief complaints of both were bilateral otorrhea which had regularly occurred after eating since childhood. There were no abnormal findings on inspection, but the iodine-starch reaction revealed sweating from the external auditory canal after gustatory stimulation. Neither patient had any past history of parotid gland surgery nor any previous injury around it. There have been several reports of cases with gustatory sweating which developed after forceps delivery, and one of the above two cases had also been delivered with the aid of forceps, while the other case was suspected to have been delivered with forceps. Although these cases do not correlate with the fact that most cases of gustatory sweating caused by forceps delivery tends to occur in children, we nevertheless diagnosed them to be cases of gustatory sweating following forceps delivery since no other apparent causes could be found. The auriculotemporal nerve in these two patients was found to be distributed to the ceruminous glands in the external auditory canal and, as a result, these cases were both considered to demonstrate auriculotemporal syndrome.

    Nine normal adults were the subjects of this study. Their horizontal eye movements were recorded with a telemeter during active and passive circular movements. Each subject made two turns to the right with their eyes closed over a circle measuring 2 meters in diameter. An active turn was then made by walking at normal speed, while a passive turn was made in a wheelchair pushed by the examiner at the same speed as the active turn. The maximum slow phase velocity and stroke of nystagmus during such turning were larger in the active movements than in the passive ones, while for after nystagmus they were larger in the passive movements. These results suggest that the vestibulo-ocular reflex thus functions more effectively during active movements.

    This study retrospectively investigated 15 cases of acoustic tumor (AT) at Saga Medical School Hospital. The results of audiometry, a head X-ray examination, the auditory brainstem response (ABR), the caloric test, magnetic resonance imaging (MRI) and patient symptoms were analyzed in this study. Abnormalities were found in 80% based on the the audiometry findings, in 87% based on the X-ray findings, in 100% based on the auditory brainstem response findings, and in 80% based on the caloric test findings. Both audiometry and the head X-ray examination are thus considered to be suitable as first-line examinations for screening AT. In addition, the ABR, the caloric test and MRI are considered to be useful for making a definitive diagnosis of AT. It is plausible that the rate of positive findings for ABR, audiometry, a head X-ray examination and the caloric test may thus show a correlation with the size of ATs.

    We devised L type ceramics (artificial ceramic ossicle) as the replacement material for the absence of the long process of the incus. The L type ceramics has a form in which two handles of the ceramic PORP are cut and connected at a with right angle. The greatest merit of this ceramics is that it can be safely and surely inserted in the end of the long process of the incus and the head of the stapes. We used the L type ceramics material in one case and obtained good results. Moreover, we summarized the previously develpoed reconstruction methods for the absence of the long process of the incus and described the merits and demerits of each of these methods while also comparing them to our newly developed L type ceramics.

    Vertebrobasilar insufficiency (VBI) is found in many cases of vertigo in patients who visit otolaryngology clinics. But its symptoms tend to be transitory, and since objective findings cannot be obtained at the first examination it is difficult to diagnose. As a result, it is tentatively diagnosed based only on the past history or the outcome of an equilibrium test. It is useful to diagnose circulation insufficiency, the stenotic position or the degree of the vessels based on the angiography findings. However, an operation is dangerous and overly invasive and therefore surgery is often not performed in many cases. We performed magnetic resonance angiography (MRA) and found it to be easy to perform, safe, it required a short examination time and was not invasive. A total of 12 cases suspected to have VBI based on the findings of an equilibrium test, MRA was performed. We could identify the stenotic or obstructive position of the vessels in the vetebrobasilar system in six of the cases. In conclusion, MRA was found to have many advantages over angiography and thus was considered to be useful in making a definite diagnosis of VBI.

    An 8-year-old male presented with multiple hemangiomas on the lower lip, left cheek, and tongue. Under general anesthesia, hemangiomas were treated with 3W of KTP laser irradiation (continuous mode). After laser exposure, the superficial hemangioma on the lower lip and left cheek rapidly regressed, thus resulting in a good postoperative course which included reduced bleeding and a very mild scar. Our findings suggest that the use of the KTP laser may therefore be effective for the treatment of superficial hemangioma.

    A statistical analysis of the Department of Otolaryngology at Hachijo Hospital was performed. The largest number of patients were under 10 years of age, while the largest number of patients presented in June based on the month. The most common diseases were otitis media with effusion, chronic sinusitis and ear wax. Fifteen patients had to be introduced to other hospital and 80% of them were referred to other hospitals in order to undergo an operation.
