    To elucidate vestibulo-thalamo-cortical pathways, we have investigated axonal projection of posterior canal-activated excitatory vestibuloocular relay (PC) neurons to the thalamus of anesthetized cats. Nineteen of 40 PC activated excitatory neurons projected to the ventrobasal complexes. Next, we examined location and axonal projection of thalamic neurons which were activated from posterior canal. Thirty-six thalamic neurons, which were activated by nose-up head rotation and by vestibular nerve stimulation, were extracellulary recorded in the ventrobasal complexes and some in the MGB (medial geniculate body). 11 of 36 neurons induced antidromic to spikes following stimulation of the ASSS (anterior suprasylvian sulcus) or PCD (postcruciate dimple). These results suggest that the PC neuron participates, at least in part, in the vestibulocortical pathways contributing to spatial orientation.

    The stabilometric velocity vector has been evaluated qualitatively by sway pattern and/or partially quantitatively by Romberg's coefficient. We examined distributions of the statistical parameters of stabilometric velocity vectors in 89 normal subjects. Each subject had 36 vectors measured with the stabilometer, into which were divided by 10 degrees from 0° to 360°. From these 36 data, we calculated, for each subject, the parameters: coefficient of variation (C. V.), skewness and kurtosis. The skewness was subject to normal distribution. The C. V. and the kurtosis were log-normal distribution. These parameters can quantitatively represent the comprehensive characteristics of the velocity vector.

    The examination of the high frequency range of the cases of tinnitus without hearing loss was done by using the tinnitus audiometer. The average age was 33 years old. It was the same as the usual report by the record of the tinnitus inspection. Dip which wasn't recognized in the pure tone audiometry was recognized in 2 example during the cases of tinnitus without hearing loss 10 example. The average of the measurement result of the high range of the cases of tinnitus without hearing loss and standard deviation were 20.7dB±12.7 with 10kHz and 28.7dB±16.7 with 12kHz. There was an example which rise in the theshold of the high range was estimated at in the cases of tinnitus without hearing loss, and the possibility that it was one factor of the tinnitus was suggested.

    The case of bilateral psychogenic hearling loss are reported. The case was 54-year-old female, and the self was also exposed to slight carbon monoxide (CO), when the carbon monoxide intoxication accident happened in her factory of independent business, and when she helped her husband and employee. At the accident, she would had a lot of stress because of correspondence for police, fire department and mass communication. Hyperbaric oxygenation (OHP) was done for CO intoxication, but under treatment, bilateral highly sensorineural deafness developed. The subjective symptom was deafness, but there were contradictions between subjective symptom and examination results, pure tone audiogram, auditory brainstem response (ABR) and speech audiogram. Therefore, it was diagnosed as psychogenic hearling loss. Psychogenic treatment has been done, however, the improvement on the subjective symptom has not been recognized by the present.

    A 52-year-old woman complained of pulsation tinnitus for 1 year and mild dizziness for 1 month. Physical examination revealed a vascular mass in the right ear and no cranial nerve deficits. Pre-operative CT, MRI and angiography demonstrated a glomus jagulare tumor in the middle ear without severe bony destruction. The tumor was classified as class C (Fisch classification). In this case, we used the technique of infratemporal fossa approach without carotid artery exposure and preserved cochlea, vestibule and facial nerve. After extirpation of the tumor, the external auditory canal was reconstructed with bony chip and patty and the osscicular chain was rearranged using ceramic prosthesis. Tympanic membrane was damaged in the operative procedures, the reconstructed membrane was too thick to hearing ordinary conversation. Canal reconstructive methods of the glomus jagulare tumor was indeed remarkable, but was to be improved against blood insufficiency.

    The development of antibiotics has decreased the incidence of deep neck infection. We can, however, still find some severe cases with serious complications, such as mediastinitis. A 50-year-old male patient with advanced deep neck infection developed mediastinitis several days after a sore throat, so was nearly dying in ICU. Appropriate surgical drainage and antibiotics led him to recovery. The time lag between the outbreak and his consultation must have plunged him into such fatal condition. It is immediate treatment including surgical approach, that can prevent the advancement of deep neck infection.

    Two Cases of deep neck abscesses are presented. Both cases were treated with emergency tracheostomy, surgical drainage and antibiotics. Case 1 was a 46-year-old male who complained of sever dyspnea and marked bilateral neck swelling. Case 2 was a 40-year-old male who complained of sore throat and left side neck swelling, and was pointed out diabetes mellitus for the first time. Computed tomography was useful in both diagnosis and treatment.

    Nasal cycle is well known as a physiological phenomenon that unilateral nasal patency spontaneously changes over time in human beings. In order to evaluate the results of rhinomanometry accurately, we must consider the influences of nasal cycle. The aim of this study is to define the fluctuations of unilateral and total nasal airway resistance (NAR) in nasal cycle. Forty-seven healthy subjects (29 males and 18 females, mean age 24.0 years) were recruited for the study, which involved serial measurements of unilateral nasal resistance using the technique of anterior rhinomanometry with nozzle. In subjects, unilateral NAR was measured hourly over a 7 hour period. The fluctuations of unilateral NAR (maximum NAR -minimum NAR) was mean 0.43Pa/cm3/s (SD 0.31) in right nasal cavity and 0.45Pa/cm3/s (0.38) in left nasal cavity with that of total NAR 0.10Pa/cm3/s (0.07). There was a significant difference between the fluctuations of unilateral NAR and total NAR. The hourly percentage changes in unilateral NAR were 43.1% (24.1) in right nasal cavity and 41.3% (19.6) in left nasal cavity with that in total NAR 22.8% (13.2). There was a significant difference between percentage changes in unilateral NAR and total NAR. The results of this study indicate that the influences of nasal cycle should be considered in order to evaluate accurately the nasal patency using rhinomanometry.

    Stellate ganglion block (SGB) has been used in order to investigate the effect of autonomic nervous system on nasal patency. The nasal resistance (NR) on the side of SGB is recognized to be increased, whereas it is debatable whether NR on the contrarateral side is increased or decreased. The aim of this study is to investigate the relationship between the changes of unilateral NR in nasal cycle and under SGB. Nine healthy subjects (8 males and 1 female, aged 22-25 years) were recruited for the study. Unilateral NR was measured hourly over 7 hour period using the technique of anterior rhinomanometry. SGB was done with 1% lidocaine 5ml. Unilateral NR under SGB was measured at interval of 2 minutes over 60 minutes period. Unilateral NR on contralateral cavity under SGB was increased in 3 subjects, decreased in 5 subjects and unclassified in 1 subject. There was no relationship between the changes of unilateral NR in nasal cycle and under SGB. The results indicate that there would be multiple mechanisms in the change of unilateral NR under SGB.

    Chemotherapy with arterial infusion is one of the constitution of the combined therapy for maxillary cancer. However, under existing conditions, the catheter for arterial infusion used therapy in a maxillary cancer is extracted, when a patient leaves the hospital. We cured a inoperable patient of advanced maxillary cancer using a implantable drug delivery system. Using a implantable drug delivery system gave him an improvement of his quality of life.

    A 45 year-old male with rhinogenic meningitis is reported. CT examination showed the primary lesion was located in the right maxillary sinus and ethomoid sinus. Caldwell-Luc operation was carried out, in addition to high dose antibiotic therapy. Although the sinus was kept clean, the consciousness of the patient was getting worse. Third CT examination showed brain abscess, but the general condition did not allow a further surgical operation and at last he died. We surveyed 38 cases reported during the past 5 years. The mortality of the rhinogenic meningitis was getting fewer, but the number of patients given diagnoses of it was preferably increased. It was considered that headache and fever-up associated with central nervous symptom may indicate intracranial complication of sinusitis and the CT examination is most helpful for the diagnosis of it.

    The eosinophilic adenoma (so-called “oncocytoma”) mainly arises in salivary gland such as parotid gland and submandibular gland, but proportion occupied for all salivary gland neoplasm is 1% or less and very rare disease. We herein describe a 57-year-old woman who presented with a mass in the parotid gland, and after the surgical resection of the parotid gland, the prognosis is good for 16 months. Oncocytoma is benign tumor, but have been reported the malignant change in some cases, so therefore a long-term observation for recurrence is necessary.

    The ranula is the cyst in which saliva leaks out by the blockage of the sublingual duct, and the frequent occurrence position is floor of the mouth. The plunging ranula in the submandibular region is rare, and the incidence is 2-5%. Several reports of the plunging ranula described that the total resection of the sublingual gland are necessary, because of the origin is the leakage of the saliva component from the sublingual gland. We report the case of plunging ranula of the 9-year-old male which the total resection of the ranula and sublingual gland was carried out without recurrence for 4 years, and discuss the review of the literature.

    A case of pleomorphic adenoma of hard palate was reported. A 74-year-old female complained unpleasantness of oral cavity. Preoperative CT and MR imaging showed tumor in left hard palate. We removed tumor from oral cavity. The surface of tumor was smooth and covered with capsule. Histopathological diagnosis was pleomorphic adenoma. There is no evidence of tumor recurrence up to the present.

    Two cases of laryngeal foreign bodies in infant are reported. The first case was 7-month-old boy with stridor and cough who was treated by a pediatrist. Fiberscopic examination revealed a foreign body in his glottic space. A piece of a plastic seal was removea unaer local anesthesia. The second case was 8-month-old girl with sudden hoarseness on picnic who was suspected a foreign body by a pediatrist. Fiberscopic examination revealed a foreign body in her El ottic space. A piece of leaf was removed under general anesthesia.

    A case of laryngeal granuloma with respiration disorder in late pregnant woman is herein reported. This case was 27-year-old woman. The granuloma increased rapidly in the late pregnancy, and the dyspnea occurred. After the tracheostomy, the laryngomicrosurgery was enforced, and the granuloma was removed. The recurrence of the granuloma was recognized after 4 months, and it dropped out naturally.

    A case of schwannoma of the larynx is reported. Fiberscopic examination revealed a swelling of the left false cord covered with an intact mucosa. Magnetic resonance imaging showed a well-defined tumor on the left false cord. The tumor was removed under general anesthesia by endolaryngeal microsurgery. The histopathological diagnosis was scwannoma because the tumor had a Antoni-A, B mixed type appearance. The literature is reviewed. Schwannoma originated from the false cord is rare.

    We experienced a rare case: Gerhardt syndrome with high tone snore. The patient was man and 58 years old, who had no central disease. By naso-pharyngo-fiberscopy, while awaking vocal cords move with no limitation, but during sleeping fix at paramedian position.
    Especially, in inspiration vocal cords closed with upper airway negative pressure. During apnea mesopharyngeal negative pressure was seen no changes, and esopharyngeal negative pressure changed up to -23.0cm H2O. By these pressures we suspected that the stenotic point existed in the area under the mesopharynx. For the diagnosis of Gerhadt syndrome, the naso-pharyngo-fiberscopy and the measurement of mesopharyngeal and esopharyngeal pressure were very effective.

    The results of the bacterial culture for one year at Tottori Prefectural Kousei Hospital, ENT out-patient clinic. High rates of S. aureus, CNS, S. pneumoniae were noted in the acute otitis media. S. pneumoniae and H. influenzae were commonly detected in child acute otitis media. S. aureus and S. pyogenes were for the acute tosillitis. The rate of H. influenzae increased in chronic tonsillitis. Sinusitis was infected by S. aureus, CNS and H. influenzae. MRSA was detected at the rate of 4.7%. Most cases of the acute otitis media in the child were affected with PISP/PRSP.

    From April 1999 to March 2000, 483 emergent otorhinolaryngology patients came to our hospital during holiday-extrahours. The rate of otitis media acuta is the highest (40%). Forty -eight of the emergent patients were admitted. Among them, the rates of acute tonsillitis including peri-tonsillar abscess, and of foreign body in esophagus are high. Tertiary emergent patient was only one case.
