    The use of the speech discrimination test is indispensable in fitting a hearing aid. There have occasionally been reports of improved speech discriminatiosn cores with the use of hearing aids, but noly a few studies have evaluated the effects according to the types of consonants. In the present study we investigated changes in the articulation scores for consonantis nduced by the use of hearing aids. Three groups of consonants were used: voiced, voicelesas and nasal. The study included 37 patients with sensorineural hearing loss undergoing hearing aid testing. The results indicated that the patients with greater functional insertion gain for the high frequency area tended to have improved articulation scores for voiceless consonants. No correlation was found between the articulation scores and functional insertion gain for voiced or nasal consonants.

    Ninety-six patients with hypopharyngeal carcinoma were treated for 15 years (1974-1988) the Department of Otorhinolaryngoloy at the University of Ryukyus School of Medicine and other public hospitalsin Okinawa.We have subdivided these patients into three groups according to their place of residence: the main island region, the Miyako region and the Yaeyama region. We studied the relationship between the incidence of hypopharyngeal carcinoma and the consumption of alcohol (awamori-shochu). The results are as follows: 1. The incidence of hypopharyngeal carcinoma in Okinawa is highest in the Miyakor egion. 2. Presumed consumption of awamori-shochu per person in Okinawa is highest in the Miyakor egion. 3. A high positive correlation exists between the incidence of hypopharyngeal carcinoma and the presumed per capita consumption of awamori-shochu.

    Simpler anula is easily diagnosed by its color, location and fluctuation. Plunging ranula, however, when occurring with no oral involvement, is sometimes misdiagnosed as certain cystic diseases. A mylohyoid hiatus exists in about 20%-30% of the normal population; plunging ranula often occurs through this hiatus. Therefore, when plunging ranula is suspected, ultrasonic examination should be done. We report a case of a 42-year-old male with plunging ranula. Ultrasonic examination revealed a mylohyoid fistula through which passed part of the sublingual gland. The mass was diagnosed as a plunging ranula, and the sublingual gland was excised.     A 44-year-old man with NHL of the right maxillary sinus and eyelid had previously been treated with VEMPt herapy (VCR, CPM, 6MP and PDN). Nearly one year later, however, tumor involvement was detected in his right orbit. The patient was subsequently treated with the MEC-LV regimen (MTX, CDDP, ET and LV) as salvage therapy. This regimen was successful. This case suggests that MEC-LV therapy might be a useful combination of salvage chemotherapy for NHL that is resistant to VEMP therapy.

    Long-term observations of auditory brainstem responses(ABRs)and computed tomography(CT)of the head were made in two patients with cerebellopontine angle tumors who had cranial nerve symptoms. Case I was a patient with an accessory neurinoma extending from the cerebellopontine angle to the lower brainstem. Case 2 was a patient with a meningioma extending from the cerebellopontine angle to the upper brainstem. In case 1 the ABRs from recordings ipsilateral to the tumor showed areproducible response only with wave I, in spite of total deafness evidenced by pure-tone audiograms before the second operation. In case 2, however, no reproducibrle response was recognized in spite of relatively preserved hearing before the third operation. In both cases the ABRs from recordings ipsilateral to the tumor two and three weeks after the operation showed obscure waveforms only. The ABRs from recordings contralateral to the tumor showed no change in case 1. But changes of III-V and I-V interpeak latencies were recognized in case 2. We conclude that ABRs from recordings ipsilateral and contralateral to the affected ear are important predictors of dysfunction of the brainstem auditory pathway and the degree of tumor extension in patients with cerebellopontine angle tumors. This technique should be used in combination with clinical observation, neurological findings and diagnostic imaging methods like CT and MRI when assessing a patient.

    A report has been made of 2 lethal cases of pulmonary fibrosis attributable to PEP. The total doses administered to these cases were as low as 60 mg in case 1 and 50 mg in case 2, suggesting that a group of patients exists who is highly sensitive to pulmonary disorders caused by PEP. Intramuscular injection and intravenous drip infusions for 5 hours were the methods of administration used in these cases. Both of the patients were over 70 years old. For this reason, it it considered desirable to take every precaution in the administration of PEP to patients over 70, as when administering bleomycin (BLM), and to provide them with continuous subcutaneous infusions to avoid sharp increases in the pulmonary PEP level. The prevention and early detection of the disorder is essential since treatment of pulmonary fibrosis due to PEP is difficult once it has occurred.

    Residual hearing of juvenile unilateral profound deafness was studied in 28 cases using the audiometer Rion AA-71. Although most cases responded to some degree from 100 to 130 dBHL, the responses were suspected to be due to vibratory sensations. Accordingly, it was concluded that as a rule the hearing is totally lost in cases of juvenile unilateral profound deafness of unknown origin.

    We evaluated our treatment of 27 patients with previously untreated maxillary sinus carcinoma at our institute. We treated these patients with preoperative intraarterial infusions of adriamycin (10mg/day for a total dose of 50-150mg) and Cobalt irradiation (200 rad/day for a total of 31-45 Gy), followed by partial resection of the maxilla. The five year survival rate was 56.9%. The histopathological effective rate of preoperative chemotherapy and radiotherapy according to Shimosato's criteria (Grade III-IV) was 25.9%. With the operation the primary effective rate was 81.5%. The most frequent cause of death was local recurrence; five year survival rate of all recurrent cases was 35.5%. Results indicate that it is best to treat with more extensive methods early on, and to utilize close follow up to detect early recurrence of carcinoma.

    Four cases of middle ear tuberculosis resembling exudative otitis media are reported. In spite of pharmacotherapy using antibiotics, serous otorrhea continued and tuberculin tests revealed a strong positive reaction in all four cases. Based on the characteristics of these four cases, the essential features for the early diagnosis of middle ear tuberculosis are described.

    Postural response induced by a tilting platform in healthy subjects and patients with Meniere's disease in a quiescent phase was investigated using a NEC computer PC-9801. The following results were obtained: 1. In patients with Meniere's disease, body sway gain by tilting increased significantly more than in healthy subjects for forward, backward, right and left directions. 2. The time course of the body sway area in patients with tilting stimulation in contrast to control subjects. These results suggest that postural control is easily upset by tilting in patients with Meniere's disease in spite of a quiescent phase.

    Since parotid swelling is the most informative symptom of sialoadenosis, we examined parotid swellings with sialography as a means of diagnosing sialoadenosis. An X-ray was taken from a fixed position relative to the body, using a focus film distance (FFD) of 70 cm. To determine an “index” of parotid swelling, the distance between the submandibular bone ridge and the end of the main duct was measured from the X-ray picture. After examination of 30 normal parotids, we decided that “abnormal” swelling was anything exceeding 1.9 cm. 11 out of 17 patients suspected of as having sialoadenosis showed swelling exceeding 1.9 cm. Many patients who had been diagnosed as sialoadenosis cases by aspiration biopsies also showed swelling exceeding 1.9 cm. Our method is reproducible and we can objectively see recurrence of parotid swelling correlating with sialoadenosis. From these results, we can say if the patient has some underlying disease, and if his “index” is over 1.9, then there is a good possibility that he has sialoadenosis.

    The combined effects of Norfloxacin(NDLX)and Carbocistein(SCMC)for suppurative otitis media were examined. The patients were divided into two groups and treated for seven days. The 37 patients in the first group were prescribed NFLX 600mg/day alone. The other group (24 patients) received NFLX 600mg/day and SCMC 1500mg/day in combination. the following results were obtained; 1. The overall clinical excellent effiacy rate was 40.5% in the NFLX alone group and 45.8% in the combined group. The moderate efficacy rates were 67.6%and 75.0% in the two groups, respectively. 2. Aural discharge completely resolved in 55.6% of the NFLX alone caces adn in 69.6% if the combined cases. Moderate improvement was noted in the two guoups in 77.8% and 82.6% of the patients, respectively. The volume of aural discharge was decreased dramatically in 55.6% and 69.6% of patients in the two groups, respectively. Volume was decreased moderately in 88.9% and 97.5% of patients in the two groups, respectively. Therefore, the efficacy rate of the NFLX and SCMC combined group wass superior to that of the NFLX alone group. However, the differences between the two groups were not statistically significant. The combined effcet was observed especially in the acute otitis media cases, 3. It was concluded that these data showed the combined effcet of NFLX and SCMC in the treatment of suppurative otitis media.

    Baccidal was given to 31 patients with otits media and to 20 patients with sinusitis. It was effective in 77.4% of those with otits media and in 85.0% of those with sinusitis. The principal bacteria of otitis media were Staphylococcus aureus and Pseudomonas aerugiosa. It is of interest that the efficacy rate against P. aeruginosa was very high. No severe side iffects were noted except for nausea in one patient with otitis media. These results indicate that Baccidal is a useful antibacterial agent in the treatment of otitis media and sinusitis.

    Forty-six patients with vertigo or dizziness were administered three Sabromin Tablets (R)(each containing 25.36 mg of Brovincamine fumarate) daily, with or without other remedies for motion sickness, for 4 to 8 weeks. The following results were obtained. The therapy was markedly effective for 50% of the patients and fairly effective for 33%. There were no differences in effectiveness between central and peripheral vestibular diseases, nor between acute and chronic diseases. Acute symptoms like vertigo, nausea, vomiting and palpitation were cured very successfully with the therapy. Chronic symptoms remaining after acute attacks, like tinnitus or stiffness of the neck or shoulders, were also cured to a certain but lesser extent. We concluded that Brovincamine fumarate should be effective not only for symptoms caused by decreased cerebrovascular blood flow but also for those caused by peripheral vestibular dysfunction.

    Adetphos, ATP preparation, was given orally to 30 patients with Meniere's disease (47 ears) at a daily dose of 300 mg for 2 months for the purpose of treating their hearing loss. When summing the seven frequencies 125,250,500, 1,000, 2,000, 4,000, and 8,000 Hz (a total hearing level), 23 ears showed an improvement of 40 dB or more (49%), 20 ears showed no change within 35 dB (43%) and 4 ears showed a decline of 40 dB or more (8%). The rate of hearing loss was significantly reduced in both the 1st and 2nd months of treatment. When comparing the rate of hearing loss before and after treatment with Adetphos, the latter was found to be significantly effective (χ2 test, p<0.001). Consequently, for the purpose of suppressing the progress of deafness and possibly recovering hearing, Adetphos can be considered as clinically useful for Meniere's disease.

    Fosfomycin solutions of 3 differecnot ncentrations (1, 3and 5%) and physiological saline as a control were sprayed into one nostril of healthy adults 4 times daily for 15 days (a total of 60 applications). The influence of the solutions and saline on swelling and the mucociliary function of the nasal mucosa were then investigated by measuring the rhinomanometric value and the saccharin clearance time. Simultaneously, the drug transfer to the serum and the influences of the drug on the subjective symptoms/objective findings and on the general condition of the volunteers were examined. Fosfomycin in these concentrations did not show any adverse effects on the investigated parameters even after the 60 applications.

    This is a study of the efficacy and side effects of the regular use of the following eardrop medications over the long term: cefmenoxime (CMX), fosfomycin (FOM), and ofloxacin (OFLX). These are all recently developed antibacterial, non-ototoxic agents. The results from averaging all theree groups of drugs revealed the following: 64.1% of the microorganisms were eliminated within the first week of adminnistration, and by the end of three weeks, 74.3% were eradicated. Renormalization of the 74.3% eradecation rate as 100% efficacy yields an efficacy rate of 86.3% after one week on these medications. Changes in the minimum inhibitory concentration (MIC) were not significant. Bacterial replacement was almost completely fungal. Fungal infections occured within two weeks of administration (12.8%), and its rate of infection at four weeks was 15.4%. The efficacy, safety and usefulness of CMX, FOM and OFLX as eardrop medication seems to be extremely high, but administration should be limited to two weeks, and if further treatment is needed, it should not extednd past three weeks. In addition, it is considered necessary to conduct bacterial examinations by the second week of administration.
