    A case of Histiocytosis X of the temporal bone was reported. The patient, a 42-year-old male, had complained of an irritable sensation and itching of the external auditory meatus bilaterally. Initially, the diagnosis of external otitis was made. Conservative treatment was instituted but the symptoms persisted. At that time, the patient was also suffering from generalized xanthomatous skin lesions and pain in the right thigh. X-ray examination showed a punched-out lesion of the right temporal bone and median vertex and ballooning of the pituitary fossa. Histological diagnosis obtained from skin lesions, bone marrow and external auditory canal lesion revealed Histiocytosis X. Radiotherapy was not effective, the patient benefitted from treatment including the administration of Predonine.

    A 52-year-old woman complained of left facial palsy, vertigo, left otorrhea and hearing loss.Her chest X-ray showed signs of pulmonary tuberculosis.Radical mastoidectomy was performed, and antituberculous chemotherapy (RFP, INH, EB) was given both pre-and postoperatively. Although her facial palsy remained unchanged, vertigo has disappeared and her left hearing level has improved slightly.Because the course of this disease is rapid and irreversible, tuberculous otitis media must be considered primary in the differential diagnosis of progressive profound deafness, facial palsy and resistance to ordinary antibiotics.

    Ten cases of delayed endolymphatic hydrops (DEH) were seen in our clinic, all of which were of the ipsilateral type. Eight of those cases presented unilateral deafness and 2 cases demonstrated bilateral deafness. In all cases, the etiology of deafness was unknown. No pattern of uniformity was discerned in the clinical courses of these patients, but all 10 exhibited a gradual deterioration of vestibular function. Owing to unbalanced vestibular function, vertigo occurred in each case. In those cases where some vestibular function remained, vertigo or dizziness occurred repeatedly. Meniere's disease was also present.

    Opsoclonus, a dramatic and chaotic abnormal eye movement, was seen in three patients. The first patient, a 56-year-old man with lung cancer, exhibited opsoclonus under the paraneoplastic process. Autopsy revealed no typical pathologic findings in the brainstem or cerebellum, suggesting cellular dysfunc-tion at the molecular level without any anatomically distinguishable changes. Two other patients, a 40-year-old woman and a 49-year-old man, both suffering from spino-cerebellar degeneration, demonstrated transient but obvious opsoclonus. Previously, it was widely accepted that because of the acute sign, degenerative disease could not be associated with opsoclonus. The two rare cases presented here, supported by precise ENG (electronystagmograph) findings, suggest, to the contrary, that opsoclonus should be associated with degenerative disease.

    A 57-year-old male with acute myeloid leukemia, considered to be in complete remission, experienced a sudden left peripheral facial palsy, followed by right facial palsy. Histopathological examination at autopsy implicated leukemia cell infiltration and hemorrhage to the temporal bone as the cause of the bilateral facial palsy. During both the active and remission phases of the leukemia, the attending otologist noted the incidence of recurrent otologic symptoms.

    Two rare cases of facial erysipelas were presented. A 62-year-old woman suffered from rubor, swelling and contact pain with vesicles from the left parotid region to the cervical region. A 28-year-old wonman suffered from rubor, swelling and contact pain from the right eyelid to the cheed and demonstrated also redness and swelling of the right palatine tonsil. One patient was treated successfully with tetracycline and the other with penicillin. Although unable to isolate the bacteria responsible, we suspect that the former case was caused by a septic scratch and the latter by acute tonsillitis.

    A62-year-old female complained of right nasal bleeding. Rhinoscopy revealed a dark red mass in the right nasal cavity. The large mass, measuring 4.0×3.5×3.0 cm and weighing 13g was removed surgically. Histopathological evaluation confirmed the initial impression that the mass was a pleomorphic adenoma arising on the septal mucosa.

    The roport is of a case of extramedullary plasmacytoma in the nasal cavity of a 58-year-old male. Cytoplasmic IgG-Kappa determinants were demonstrated immunohistochemically. Following surgical treatment and post-operative irradiation, there has been no evidence of recurrence in 1.5 years of follow up.

    Twelve cases presenting visual destrubance caused by nasal and paranasal deseases (except malignant tumor) were treated at Shimane Medical University Hospital from October 1979 to December 1989. A retrospective study was done and a brief review of the literatrue was added. We reported one case in which visual acuity recovered remarkably. The following results were obained: 1) Of twelve cases, six involved nasal and paranasal cysts, four involved acute sinusitis and two involved facial injuries. 2) Lesions were found frequently in the etmoidal sinus. 3) Surgery was the most effective therapy. 4) Prognosis of visual destrubance depended upon the time elapsed between the appearance of symptoms to treatment, prether-apeutic visual acuity and fundus examination.

    A 68-year-old man who had epipharyngeal carcinoma (T2N1M0) was admitted to our clinic. He was treated both surgically and with radiotherapy consisting of 60 Gy. Seven months later, right abducens nerve paralysis appeared. CT scan revealed a mass lesion occupying the area from the skull base to the intracranial region. Reirradiation therapy was effective and CT scan confirmed a decrease in tumor size, Following treatment with cisplatin and peplomycin, the tumor disappeared completely.

    A case of verrucous carcinoma of the tongue in an 83-year-old woman is reported with a review of the literature. The patient was treated by surgical resection. Verrucous carcinoma is a varient of well-differentiated epidermoid carcinoma. It grows slowly, tends to invade local structures, and rarely metastas-izes. Adeauate surgical resection is recommended as the treatment of choice.

    85 cases of laryngeal cancer were investigated retrospectively. The mean age of patients was 62.7 years, and the male-female ratio was 9.6: 1. There were 57 cases of the glottic type, 26 of the supraglottic type, and 2 of the subglottic type. The overall five-year survival rate was 67.6%. The five-year survival rate for the glottic and supraglottic types were 75.6% and 51.2%, respectively. Local recurrence in early stages of the disease was controlled by total laryngectomy. In 15 cases (17.6%), two types of laryngeal cancer were identified.

    The efficacy of Ga-scintigraphy as a diagnostic tool was evaluated in 83 patients with epidermoid carcinoma of the head and neck from January 1984 to December 1988 at the Department of Otolaryngology, Shimane Medical College.
    The accumulation of Ga-citrate in the primary region of head and neck cancer was 48%. The correct diagnosis of cervical metastatic lymph nodes by Ga-scintigrams was 76%, false positives 3%, false negatives 48%, sensitivity 52%, and specificity 98%. The correct diagnosis of distant metastatic lesions by Ga-scintigrams was 93%, false positives 8%, false negatives 0%, sensitivity 100%, and specificity 92%. The above results suggest that Ga-scintigraphy is a valuable method in the diagnosis of distant metastatic lesions but less useful in the detection of primary lesions and cervical metastatic lymph nodes of head and neck cancer.

    A case of hypercalcemia with lingual cancer in a 69-year-old female is reported. Among patients with head and neck cancer, the incidence of hypercalcemia has been reported to be as high as 20%. Malignancy-associated hypercalcemia tends to worsen rapidly over a period of days to weeks. Because it is a potentially lethal complication, rapid and accurate diagnosis and treatment are imperative.

    74-year-old female with pseudomembranous colitis following surgery for thyroid cancer is reported. The patient complained of watery diarrhea and high fever during post-surgical antibiotic therapy. Bacteriologi-cal examination of the stool revealed the presence of clostridium difficile, and she was treated with vancomycin and cholestryamine. Previous antibiotics were discontinued. In evaluating the effect of our therapy, CD-check Dl proved very useful.
