    No clear description has been found regarding what influence the lateral wall of the nose has on the development of the deviation of the nasal septum. Clinically, swelling or expansion of the middle turbinal on the concave side is seen with the so-called upper deviation, but theories on the relation between the deviation and the lateral wall of the nose are many and varied. The authors tried to clarify the relation by the X-ray examination, but the ordinary X-ray failed to give any solution. Since the tomography was adopted, however, many cases have been encountered in which the expansion of the middle turbinal must be considered to be the cause of the deviation. As the expansion of the middle turbinal is due to an unbalance of the development of the ethmoid air cells, the unbalance seems to influence more or less the development of the deviation.

    Various methods of operation for atrophic rhinitis have been reported since Lautenschlaeger who had published his method in 1916. There is, however, no method which can result in perfect cure of the disease and yet has the clinical ease and conveniences. Particularly difficult is to decide the method of operation when rhinitis is complicated with sinusitis, because there lies a reverse relationship of effect between the operative treatments of these two diseases.
    Therefore in operation of atrophic rhinitis which requires simultaneous sinuectomy there is a certain limitation of applicability of the method.
    The author developed the following technique to solve this problem. As the treatment of atrophic rhinitis complicated with sinusitis he detached the inferior concha submucously from the medial surface of the maxillary bone by oral approach and transplanted a stick of spongiosa resected from the crest of the iliac bone into the crevice between the concha and the maxillary bone. As the result, the inferior concha is mobilized medially touching slightly with the septum. And then to treat sinusitis, esp. fronto-ethmo-maxillary sinusitis, he applied the author's technique of sinuectomy, i. e. the transoral and via-eyebrow combined sinuectomy.

    A complete healing was effected on a 19-year-old male patient of trigeminal neuralgia who was suffering from chronic tonsillitis and complained of pain in the temporal, preauricular and cheek regions, by tonsillectomy on the right side. It was concluded that the pain was caused by the adhesion between n. linguaris, the third branch of the trigeminal nerve that took an anatomically abnormal run and the palatine tonsil on the same side. The authors agree with Dr. Ishikawa in laying stress on the necessity of close examination of the tonsil in the case of trigeminal neuralgia of unknown cause. During the operation of the right-side tonsillectomy, an extraordinary sweating was observed on the left half of the patient's face, and it seems to be explained by Dr. Takagi's sweating reaction theory.

    The symptom of tinnitus aurium disappeared or improved with 11 cases (19%) of 58 patients who were given chlorpromazine was not very effective for tinnitus aurium. However, chlorpromazine, because of its sympathetic blocking action was effective in some cases and there are reports on its effectiveness. It is pointed out that further researches should be performed to determine the indication and the dosage. The application, the indication, the method of use, the dosage, etc. are discussed from the author's experiences.

    Studies were made into the changes of Takeya's urine fatigue reaction figures due to operation, with 14 cases of chronic sinusitis and 5 cases of deviatio septi nasi. The review of the results led to the conclusion that at least 2 weeks' resting is necessary after the radical operation of sinusitis to prevent relapses and at least one week's resting after deviotomy to prevent such complications as haematom, abscess, bleeding, etc.
    It is stressed that the problem of fatigable constitution should be investigated into, and vitamin B1 deficiency due to the partiality for polished rice diet is pointed out as one of the causes of the fatigable constitution. since it is supposed that various diseases might be caused by this fatigue, the author stresses that movements against the partiality for polished rice should be advocated from the otorhinolaryngological circles, too.

    Two cases of frontal and ethmoidal pyoceIe converted into a common cavity, with one side communicating with another, and exposing dura mater, are reported. Without any intracranial complications, they were healed by Kilian's radical operation.
    Case 1: A woman aged 59; the size of cavity was 9.5×5.0cm.
    Case 2: A man aged 29; the size of cavity was 8.5×4.7cm.

    The authors performed two operations of extirpation of the tumor of epipharynx by the palate method (T-shape incision), and report the method and its merits.
    Case 1: 14-year-old boy; Fibroma of epipharynx that had relapsed 3 times, was extirpated successfully.
    Case 2: 57-year-old man; The inoperable cancer of epipharynx was successfully extirpated, but the cancer relapsed.

    Otorhinolaryngological Clihic, University of Nagasaki Medical School The author explains a method of anesthesia suitable for the operation of children's cleft-palate: that is, premedication with “Contomine,” basal anesthesia by rectal instillment of “Ravonal” and inhalation of ether, and the protracted anesthesia by the ether inhalation. The author reports his experience in the use of this method with ten cases of patients ranging from two to twelve years in age.
    This method is advisable from the following points:
    1) The introduction and maintenance of anesthesia is easy.
    2) The disturbance of respiration is slight.
    3) No excited state, nausea or vomiting develops at the time of introduction or awaking.
    4) The apparatuses are very simple.

    The authors report a case of 36-year-old male who suffered from incurable ulcer of the pharynx. There were doubts as to whether it was of tuberculous, syphilitic or moniliatic origin, and no clear basis was found to establish the diagnosis on. Use of all sorts of medicaments reported in literatures proved effectless, and when at last the ulcerated part was resected, it was exacerbated. According to the classification by Prof. T. Goto, the local finding was the flat, creeping, solitary and aphthous form, but it could not be diagnosed as moniliasis, because actinomyces were not proved. Vaccination and blood transfusion were somewhat effective as the therapy. It seems that in such a case some less stimulant pigments are advisable for local treatment and at the same time it is important to make efforts to recover the general conditions of the patient.

    Pathological findings by E. C. G. were observed with 35 cases of 106 patients of chronic tonsillitis treated. Most of the changes were not serious. There were 25 cases of abnormal ST. T., 5 cases of abnormal R spine, 2 cases of abnormal P spine, one case of low electric potential, and 2 cases of PQ prolongation. It took at least 12 days after operation, before E. C. G. recovered normalcy. Many of the 35 cases had deep and wide fossa tonsillaris superior filled with pus, and were generally tonsillitis atrophicans. With the group of patients of inflammation of Waldeyer's ring, 40% showed abnormal E. C. G. There were 14 cases of abnormal ST. T., 6 cases of abnormal R spine, and 3 cases of abnormal P spine. In no case the patient complained of any heart symptom as the chief complaint. The subjective symptoms were palpitation with 7 cases, general exhaustion with 17 cases, stiff shoulders with 10 cases, and generally there was very little subjective symptom related to the heart. The abnormalcy of E. C. G. disappeared with over 88% of the cases by operative elimination of the foci.

    The so-called singer's nodules are commonly encountered, but the therapy is not too easy, because the operative resection is liable to cause postoperative disturbance of voise due to injury of the vocal cords, and the conservative treatment cannot bring about satisfactory results. The author reports two cases in which injection of the placental plasma took rapid and positive effects, together with considerations on this therapy.

    Statistical observations were made on the cases of carcinoma that developed in young patients of ages of 29 and under in the authors' clinic in a period of 20 years from 1936 to 1955.
    And the results obtained were as follows:
    The total number of patients was 30 (14 male and 16 female patients), and 5 cases were in their teens and 25 were in their twenties. The youngest patient was 16 years old.
    The sites of origin were: with 11 cases in the upper jaw, with 5 in the esophagus, with 4 in the hypopharynx, with 3 in the pharynx, with 2 each in the nasopharynx and palate, and with 1 each in the nasal septum, tongue and larynx.
    Histologically, there were 4 cases of undifferentiated cell cancer, 19 cases of squamous cell carcinoma and 3 cases of adeno-carcinoma, and the rest were unidentified.
    The diagnosis was clinically so difficult that some of the cases were diagnosed wrongly. The histological examination with section tissues is considered to be the most reliable.
    The prognosis was generally very bad and only two out of 18 cases of known prognosis has been cured, after taking over two years.

    The author used recently the “Hexamethonium (Methobromin)” for 3 cases of hypertensional epistaxis and obtained marked effects. Since he experienced a shock by hypotension with one of the cases, the author refers to the points requiring special attention when using this medicine and the measures against the shock. No other by-effect than the shock was experienced.

    Iwamoto and his associates report a case of malignant neurinoma of the neck, where the growth was found to be of the size of an egg and occupying the right trigonum caroticum and mesopharynx.
    The tumor was extirpated and it was confirmed that it had developed from the vagus nerve. The diagnosis was established by the histological examination to be a malignant neurinoma, which is generally of rare occurrence.
    The patient made a successful recovery after the surgical removal of the tumor.
